11. Fractures and minor head injuries: minor injuries in children II.

Fractures in children are common, but the plasticity of children's bones means that they may be incomplete. If a child has deformity, swelling or bony point tenderness in a limb after a fall, it is likely to be fractured. A fractured limb that appears deformed will most probably need to be reduced. Effective splinting, using whatever means is readily available, and early, adequate analgesia, can ameliorate the severe pain associated with a fracture. In young children with open growth plates, Salter-Harris type I injuries of the distal fibula are more common than ligament injuries of the ankle.

Protein-truncating variants in APBA1 in minors with severe, early-onset obesity: implications for genetic testing.

BACKGROUND: Severe, early-onset obesity is a monogenic disorder in a subset of patients. Recently, rare protein-truncating variants (PTVs) in BSN and APBA1 have been implicated as monogenic causes of obesity. Truncating variants in these two genes have been associated with adult-onset monogenic obesity. Here, we aimed to investigate the occurrence of rare PTVs in APBA1 and BSN in minors with severe obesity manifesting in early childhood or adolescence. METHODS: Previously obtained exome data of 209 children and adolescents (0–18 years) with severe obesity manifesting in early childhood or adol

Does the Netherlands comply with national law and Article 2 of human rights concerning deceased minors?

BACKGROUND: In the Netherlands, reporting the death of a minor to a forensic physician has been legally required since 2010. Additionally, the Postmortem Evaluation of Sudden Death in Youth (PESUDY) procedure can be initiated for unexplained natural deaths. OBJECTIVE: This study assesses the reporting rate of deceased minors to forensic physicians, evaluates PESUDY submission rates, and explores reasons for declining PESUDY, under the European Convention on Human Rights (ECHR) and the Convention on the Rights of the Child (CRC) obligations. PARTICIPANTS AND SETTING: This observational, retro

Where are Minors Initially Contacted for Sexual Exploitation Grooming in North America? Implications for Prevention Programming.

Baird and Conolly's systematic review on North American domestic minor sexual exploitation claimed that the literature most frequently identifies the internet as the initial site of contact. However, in my analysis of their sources, only three of the seven identified studies indicated that the internet was the site of initial recruitment, three studies did not have enough information to make a determination, and one study did not identify the internet as the site of initial recruitment. For the papers that did cite the internet as the initial site of recruitment, most identified the internet a

Are autopsies on minors a taboo?: The experience of Milan in a 19-year retrospective study.

Forensic autopsy is an important tool for the proper management of non-natural deaths in minors. However, it seems that autopsy in minors is a practice which may not be performed routinely. In this framework, we conducted a study analyzing autopsies of minors (under 18 years of age in Italy) performed at the Institute of Forensic Medicine in Milan in the period 2001-2019. For the period 2015-2019, we extrapolated all deaths due to non-natural causes in minors to investigate how many and which of these deaths were not subjected to forensic autopsy. Of the total, 344 minors (235 males and 109 fe

Acceptance of Sexual Interest in Minors in Self-Referred Individuals Under Treatment - An Exploratory Pilot Study.

Some therapists/scientists argue that "acceptance" of sexual interest in minors (SIM), i.e., the integration of the sexual preference into the individual self-concept, is a prerequisite for dealing with SIM in a responsible way. However, if one assumes that - even in some persons - SIM might change over time, "acceptance" could also run counter to therapeutic targets because the motivation to change as well as the specific self-efficacy for modifying SIM might be reduced. This exploratory pilot study analyzes the relationship between acceptance of SIM and (1) dynamic risk for contact sexual re

Longitudinal and Reciprocal Relationships of Depression Among Minors with Online Sexual Solicitations and Interactions with Adults.

The purpose of this study was to analyze the temporal and reciprocal relationships between depression and online child sexual victimization, including both online sexual solicitations and interactions of minors with adults. Gender differences in these relationships were also examined. A total of 1,504 adolescents (52.4 percent girls; mean age = 13.11; SD = 0.79) completed measures at T1 and at T2, 1 year apart. The relationship among variables was analyzed using structural equation modeling. The prevalence for sexual solicitation and interactions was 7.1 percent and 3.6 percent at T1 and 15.1

The Cost-Effectiveness of Emergency Hormonal Contraception with Ulipristal Acetate versus Levonorgestrel for Minors in France.

OBJECTIVE: To evaluate the cost-effectiveness of ulipristal acetate and levonorgestrel in minors in France, and analyze whether it is worthwhile to provide ulipristal acetate to minors free of charge. METHODS: The cost-effectiveness of two emergency contraceptive methods was compared based on a decision-analytical model. Pregnancy rates, outcomes of unintended pregnancies, and resource utilization were derived from the literature. Resources and their costs were considered until termination or a few days after delivery. Deterministic and probabilistic sensitivity analyses were performed. RESU

Adolescents in the Dunkelfeld: A Study of Help Seeking Minors Who Use Child Sexual Abuse Materials.

This study examines the characteristics of help-seeking minors (n = 117) who have consumed child sexual abuse material (CSAM) and who voluntarily sought support. 97% of the respondents were in Dunkelfeld at the time of data collection. About half of the respondents reported a pedohebephile sexual preference. The study found two distinct groups based on cluster analysis, primarily distinguished by their mental well-being. The results indicate that 60.7% of the participants had used CSAM in the last two weeks, with the most common content being medium severity on the COPINE scale (levels 6-9). T

Applying an Antiracist Approach to Promoting Health Equity and Psychological Well-Being in Unaccompanied Immigrant Minors.

Unaccompanied immigrant minors (UIMs) are a fast-growing demographic in the United States, doubling in population since 2014.1 According to the Office of Refugee Resettlement, a UIM is someone under the age of 18 years who enters the United States without lawful status and an accompanying guardian.2 Most UIMs in the United States originate from the Central American northern triangle (ie, El Salvador, Guatemala, Honduras), with violence, extreme poverty, and family re-unification as the top 3 reasons for migration.1,3 Repeated exposure to stressful and/or traumatic events at home, during migrat

Impact of Coronavirus Pandemic on Tuberculosis and Other Imported Diseases Screening among Migrant Minors in Spain.

BACKGROUND: In recent decades, the increase in population movements has turned the focus to imported diseases. The COVID-19 pandemic has negatively impacted the access to health care systems, especially in highly vulnerable populations. We address the effects of the pandemic on the health screening of migrant unaccompanied minors (UM) in Spain. METHOD: Retrospective cross-sectional study including UM screened for imported diseases with a unified protocol at a pediatric reference unit for tropical and infectious diseases in Madrid, Spain. We compared the pre-pandemic (2018-2019) and post-pande

Post-traumatic stress disorder in refugee minors in an outpatient care center: prevalence and associated factors.

Due to their likelihood for experiencing a number of traumatic events, refugee minors have an increased risk of developing post-traumatic stress disorder (PTSD). However, the prevalence of PTSD in refugee children varies widely between studies, and it remains somewhat unclear what factors increase children's risk of PTSD. This study aimed to assess the prevalence of PTSD in a clinical outpatient sample of refugee minors, and to evaluate the association of different risk factors with a PTSD diagnosis. N = 417 refugee minors were recruited from an outpatient clinical center in Hamburg, Germany.

The health status of newly arrived asylum-seeking minors in Denmark: a nationwide register-based study.

OBJECTIVES: Asylum-seeking minors are known to be at increased risk of physical and mental diseases compared to both native children and adult asylum seekers. We present a nationwide register-based study based on the health assessment of 7210 newly arrived minors in Danish asylum centres from 1 January 2011 to 31 December 2015. METHODS: We describe socio-demographic characteristics, trauma history and symptoms of physical and mental health. To associate the reporting of traumatic events and signs of anxiety or depression, we performed logistic regression analysis. RESULTS: We found a high, a

Collective narrative practice with unaccompanied refugee minors: "The Tree of Life" as a response to hardship.

During the summer of 2016, in the midst of one of the biggest refugee crises in centuries, the author of this article joined the team from a shelter for unaccompanied refugee minors on Samos, Greece, in an effort to provide young boys seeking asylum in Europe with an opportunity to engage in speaking about the difficulties they have encountered in life. The team used a collective narrative methodology called the "Tree of Life", originally developed by Ncube-Mlilo and Denborough. In addition to traditional psychological treatments, collective narrative practice creates a secure foundation for a

Explaining behavioral health differences in urban and rural sexual minority adolescents: A longitudinal investigation of minority stress in a diverse national sample of sexual minority adolescents: A longitudinal investigation of minority stress in a diverse national sample of sexual minority adolescents.

PURPOSE: Experiences of sexuality-based discrimination (ie, minority stressors) against youth who identify as nonheterosexual (ie, sexual minority) have been associated with increased symptoms of anxiety, depression, and post-traumatic stress disorder (PTSD) for sexual minority adolescents (SMA; ages 14-17). However, little is known about the experiences of SMA living in rural communities across the United States. Thus, the present study sought to examine differences in mental health patterns between urban and rural dwelling SMA, and to see whether these differences are, at least in part, expl

Minority stress mediates associations of sexual minority state policies and tobacco use among US sexual minority young adults.

INTRODUCTION: State policies surrounding sexual minority (SM) rights are associated with tobacco use among SM individuals. Research is scant regarding the role of distinct SM policy categories on SM young adults' (SMYAs) tobacco use and mechanisms explaining these associations. METHODS: We analyzed 2023 survey data from 1100 SMYAs (ages 18-34; 14.2 % gender minority; 66.1 % bisexual+, 29.1 % monosexual; 53.2 % racial/ethnic minority) with representation across 45 US states and DC. Procedures such as central access and tube thoracostomy are integral in the care of the injured patient. However, both increasing life span and patient complexity of comorbidities can hinder procedural success. Careful forethought should be completed before, simply, charging ahead with a procedure. This article covers the details needed to be successful in carrying out these 2 procedural "staples" in trauma. From anatomy to pain control to postprocedural management, this article will be the building block for technical success. Understanding what you are doing and careful planning ahead are no
