Quantifying the Ototoxicity of Mitomycin: Before versus after Myringotomy.

OBJECTIVES: Recent research has focused on mitomycin C (MMC) application as a means to circumvent complications that arise when using ventilation tubes during myringotomy. This study has two aims: (1) to synergize the current literature to create a standardized clinical approach for using MMC, and (2) to determine at which point during the myringotomy the application of MMC proves the safest (i.e., before or after incision). METHODS: We measured the auditory brainstem response (ABR) and distortion product otoacoustic emissions (DPOAE) in 9 female chinchillas to determine whether applying MMC

Mitomycin-C for mucous membrane overgrowth in OOKP eyes.

PURPOSE: The aim of this study was to report the use of mitomycin-C (MMC) in the treatment of mucous membrane overgrowth in eyes with osteo-odonto-keratoprosthesis (OOKP). METHODS: Records of 4 cases with mucous membrane overgrowth after stage 2 OOKP surgery were reviewed. RESULTS: All the patients had undergone a mucous membrane trimming procedure followed by MMC application. None of the patients had any episode of recurrence of the mucous membrane overgrowth after a single application of MMC in the follow-up period that ranged from 1 to 11 years. CONCLUSIONS: MMC can successfully arrest m

Postoperative application of mitomycin-C in endocanalicular laser dacryocystorhinostomy.

PURPOSE: Cavernous malformations (hemangioma) of the orbit, when symptomatic, can often be treated successfully with complete surgical excision. However, when they involve local structures in their capsule, are situated in the orbital apex, or extend through the superior orbital fissure, the risks of surgery increase significantly. In such cases, alternative treatment modalities can be explored. In this study, the authors report on the use of fractionated stereotactic radiotherapy (SFRT) for the treatment of surgically complicated cavernous malformations. METHODS: In this retrospective cohort

Histological changes due to intravesical instillation of mitomycin C.

OBJECTIVE: Transurethral resection (TUR) is highly effective in the local control of superficial bladder cancer. However, the recurrence rate can reach 80% of the cases. Adjuvant intravesical chemotherapy may decrease significantly tumor recurrence. We describe a bladder adverse reaction to mitomycin C as adjuvant therapy for non-invasive bladder cancer METHODS: Three patients with diagnosis of pTa G1 urothelial carcinoma were treated by TUR plus an instillation of 40 mg. of mitomicin C. A month later, the patients were attended for dysuria and hematuria. Cystoscopy and bladder biopsy were pe

Safe application of mitomycin C within the nasal cavity.

OBJECTIVE: To describe a novel technique of delivering mitomycin C safely via endoscopic sinus surgery. CASE REPORT: Mitomycin C was applied in the median frontal drainage pathway of a 44-year-old woman suffering from recalcitrant chronic frontal sinusitis. The mitomycin C was soaked onto a neurosurgical patty, which was delivered through a nasopharyngeal tube trimmed to ensure delivery directly to the desired area, thus sparing adjacent mucosa. CONCLUSIONS: Mitomycin C has been suggested to have a useful role in reducing post-operative scarring after endonasal surgery. The long term safety

Comeoscleral perforation after pterygium excision and intraoperative mitomycin C.

A 28-year-old man underwent pterygium excision with a single intraoperative application of mitomycin C (0.02%) for 3 minutes. One week after the surgery, the patient developed a corneoscleral perforation with iris prolapse. To the best of our knowledge, corneoscleral melting in the first postoperative week after a single intraoperative application of mitomycin C has not been reported. Moreover, the perforation occurred despite using a minimum concentration of the drug, risk factors were absent, and the surgery was performed by an experienced surgeon. Although a single case, this report questio

Hyperbaric oxygen therapy for mitomycin C-induced scleral necrosis.

Intraoperative or postoperative use of mitomycin C is one of the treatment options to reduce the recurrence of pterygia. Scleral necrosis, a potentially blinding complication after mitomycin C use, has been among great concern in ophthalmic practice. A patient is presented in whom scleral necrosis developed 20 days after a pterygiectomy with bare sclera technique was performed, followed by the application of 0.4 mg/mL of topical mitomycin C eye drops for 3 weeks. After conjunctival flap failures the patient was treated with hyperbaric oxygen therapy. The sclera revascularized and returned to n

Topical mitomycin-C for pterygia: is single application appropriate?

BACKGROUND AND OBJECTIVE: Mitomycin-C has been shown to dramatically reduce pterygium recurrence rates; however, its use has been associated with serious complications, especially in higher cumulative doses. The authors investigated the safety and efficacy of progressively reducing dosages of mitomycin-C as an adjunct to pterygium surgery. PATIENTS AND METHODS: Four hundred eighty-one consecutive primary and recurrent pterygia were surgically excised in a prospective nonrandomized trial using five different mitomycin-C dosages. Recurrence was defined as fibrovascular tissue over the corneoscl

Intraoperative application of topical mitomycin C for pterygium surgery.

BACKGROUND: Postoperative recurrence of pterygium occurs in many patients. The authors studied the recurrence rate of pterygium after administration of a single intraoperative dosage of topcial mitomycin C at the completion of pterygium excision. METHODS AND PATIENTS: Eighty-one patients underwent excision of the pterygium, leaving the sclera bare. The first 60 patients were randomized into two treatment groups of 30 patients each. Their remaining 21 patients were offered mitomycin C. Group 1 included 49 patients (30 randomized and 19 of the remaining 21 patients) who received an intraoperati
