A 19-year-old woman presented with severe hypertension, hypokalemia, and clinical signs of Cushing syndrome (CS). Computed tomography imaging revealed a large left adrenal mass encasing a major vessel, a left renal lesion, a right ovarian teratoma, and multiple pulmonary micronodules. 18F-fluorodeoxyglucose positron emission tomography (18-FDG-PET) showed hypermetabolism in the adrenal mass, lung nodules, and teratoma. Biochemical work-up confirmed adrenocorticotropin-independent CS and elevated adrenal androgens, consistent with metastatic, unresectable adrenocortical carcinoma. The patient w

PURPOSE OF REVIEW: In this article, we focus on the current and future treatment options for adrenocortical carcinoma (ACC).
RECENT FINDINGS: Radical surgery remains the only curative treatment for ACC. Recent reports showed a longer overall survival (OS) in patients with high risk of recurrence treated with adjuvant mitotane; the time in target range (14-20 mg/l) is related to low risk of relapse both in adjuvant and in palliative setting. In patients who experience disease progression after etoposide, doxorubicin, cisplatin with mitotane (EDP-M), gemcitabine and metronomic capecitabine, or

OBJECTIVES: To report a rare case of a metastatic adrenocortical carcinoma (ACC) that achieve a complete and a long-term remission.
CASE PRESENTATION: AAC is a rare and aggressive tumor, with a high risk of recurrence and that present metastases in 21% of cases at diagnosis. Treatment of advanced ACC is challenging, mitotane is the only available adrenolytic treatment, with modest and unpredictable responses. Response rates to systemic chemotherapy are not encouraging. We describe the case of a 39-year-old woman with a metastatic ACC, that achieve a complete and long-term remission after chem

Adrenocortical carcinoma (ACC) is a rare and aggressive endocrine malignancy, typically presenting at an advanced stage with distant metastases. We report a unique case of a 49-year-old female with metastatic ACC in the liver who exhibited an abscopal effect following pulse electric field (PEF) ablation. The patient initially presented with hypertension, male-patterned hair loss, voice hoarseness, and weight gain and was subsequently diagnosed with a large left adrenal mass along with liver and pulmonary metastases. Following surgical interventions-including left salpingo-oophorectomy, hepatic

Adrenocortical carcinoma (ACC) is a very rare endocrine cancer and is associated with a poor prognosis. There is a paucity of randomized clinical trials for this rare disease. We aimed to perform a systematic review of the literature on systemic therapy options in different stages of ACC. A systematic review was performed using Pubmed and Embase databases according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. A total of 24 trials of systemic therapy in the treatment of ACC were identified and included in this review. Only one clinical trial in t

Adrenocortical carcinoma (ACC) is a rare endocrine malignancy of the adrenal gland with an unfavorable prognosis. It is rare in the pediatric population, with an incidence of 0.2-0.3 patients per million in patients under 20 years old. It is primarily associated with Li-Fraumeni and Beckwith-Wiedemann tumor predisposition syndromes in children. The incidence of pediatric ACC is 10-15fold higher in southern Brazil due to a higher prevalence of TP53 mutation associated with Li-Fraumeni syndrome in that population. Current treatment protocols are derived from adult ACC and consist of surgery and/

Information about epidemiology, natural history and prognostic factors of adrenocortical carcinoma in Italy is extremely scarce. We report here 35 patients of adult age who were referred to our institution in the last two decades. Nine patients had non functioning, and 26 had functioning tumors. In non-functional tumors initial symptoms were abdominal pain in 90% of cases, fever, weakness, malaise, weight loss in 30%. Only one patient was asymptomatic. Of patients with functioning tumors, 18 presented with Cushing's syndrome, 6 with Cushing's syndrome and virilization, 1 with Cushing's syndrom

Advanced adrenocortical carcinoma (ACC) is an aggressive disease with poor prognosis, and the current therapeutic options, such as mitotane or platinum-based chemotherapy regimens, often offer limited efficacy. Here, we present the first report, to the author's knowledge, of metastatic ACC with positive octreoscan scintigraphy that was successfully treated with octreotide long-acting release (LAR). A patient with metastatic ACC who showed poor tolerance to mitotane received octreotide LAR because of positive octreoscan scintigraphy. She obtained major partial response to the somatostatin analo

BACKGROUND/OBJECTIVES: Adrenocortical carcinoma (ACC) is a rare tumor. Diagnosis relies on combined histopathological criteria that, in a multifactorial scoring system, may suggest malignancy. The recent ADIUVO trial, a multicenter trial randomizing ACC patients between mitotane treatment or surveillance only, demonstrated that patients with localized, low-grade ACC (Ki67 labeling index (LI) ≤ 10%) have a better prognosis than historically anticipated, questioning the routine use of adjuvant mitotane. This post hoc analysis aimed to investigate whether automated, centralized Ki67 LI assessment

Adrenocortical carcinoma (ACC) accounts for a minority of all malignant tumors in adults. Surgery remains the most important therapeutic option for non-metastatic ACC. Whether a subset of patients with small ACC may benefit from minimally invasive surgery remains a debated issue, but we believe that surgeon's expertise is more important than surgical technique to determine outcome. However, even a state-of-the-art surgery cannot prevent disease recurrence that is determined mainly by specific tumor characteristics. We consider that the concomitant presence of the following features characteriz

Mitotane causes hypercholesterolemia in patients with adrenocortical carcinoma (ACC). We suppose that cholesterol increases within the tumor and can be used to activate proliferative pathways. In this study, we used statins to decrease intratumor cholesterol and investigated the effects on ACC growth related to estrogen receptor α (ERα) action at the nuclear and mitochondrial levels. We first used microarray to investigate mitotane effect on genes involved in cholesterol homeostasis and evaluated their relationship with patients' survival in ACC TCGA. We then blocked cholesterol synthesis with

Adrenocortical carcinoma (ACC) has a dismal prognosis in advanced stages. Despite treatment with the adrenal toxicant mitotane and/or aggressive chemotherapy, tumor control is often short-lived. Here, we examine trofosfamide as a salvage treatment of ACC in an observational cohort study within the German ACC registry. Response defined as progression-free survival (PFS) at first tumor evaluation was assessed by RECIST 1.1 or clinically, and PFS and overall survival (OS) were estimated by the Kaplan-Meier method. Twenty-seven patients (11 males; median age 46.9 years) progressing after mitotane

BACKGROUND: Adrenocortical carcinoma (ACC) is a rare, aggressive malignancy with limited treatment options. Mitotane, the only approved drug, has modest efficacy and frequent adverse effects, underscoring the urgent need for new therapeutic approaches. Celastrol, a triterpene compound derived from traditional Chinese medicine, exhibits anti-inflammatory activity and broad anti-tumor effects, including sensitization of cancer cells to chemo- and radiotherapy.
PURPOSE: In this study, we investigated for the first time the anti-cancer potential of celastrol in the ACC H295R cell line.
METHODS:

CONTEXT: Adrenocortical carcinoma (ACC) is a rare, aggressive malignancy with a high recurrence and mortality rate even after complete resection. Therefore, intensification of adjuvant therapies is crucial, although their effectiveness remains controversial.
OBJECTIVE: This study aimed to determine the efficacy and safety of available treatments by considering the prognostic factors affecting disease outcomes.
METHODS: The search was conducted across 3 databases (PubMed, Embase, and CENTRAL) in October 2024. Eligible studies compared overall survival (OS) and recurrence-free survival (RFS) i

Adrenocortical carcinoma (ACC) represents a very rare (an incidence of 1-2 cases per million) and aggressive malignancy (an overall 5-year survival of 15%). Less than 15-20% of cases involve a single metastatic disease. In this presentation, we documented a most exceptional survival span in a case of ACC, between the female patient's age of 50 and the current age of 66. The subject underwent an open right adrenalectomy three times (at first diagnosis, after recurrence 2 years later, and, most recently, 15 years after the initial diagnosis amid a multi-visceral resection). She initially display

Adrenocortical carcinoma (ACC) is an orphan cancer with 35% five-year survival that has been unchanged for last five decades. Patients often present with severe hypercortisolism or with mass effects. The only Food and Drug Administration (FDA)-approved drug for ACC is mitotane, an insecticide derivative, which provides only limited additional months of survival, but with toxicities. Little progress in the field has occurred due to a lack of preclinical models. We recently developed new human ACC in vitro and in vivo research models. We produced the first two new ACC cell lines for the field, C

Standard chemotherapy for adrenocortical cancer currently is under evaluation in the context of the recently completed FIRM-ACT evaluating the combination of mitotane with either streptozocin or etoposide, cisplatin, and doxorubicin. New agents are eagerly sought by the ACC community that hopes to make progress against this deadly disease. Investigators have begun to dissect the molecular and genomic context of ACC with a goal of identifying potential novel therapeutic agents. One gene consistently overexpressed in ACC is insulin growth factor type 2. Targeting its receptor IGF1R has shown enc

Adrenocortical carcinoma (ACC) is a rare and in most cases steroid hormone-producing tumor with variable prognosis. The purpose of these guidelines is to provide clinicians with best possible evidence-based recommendations for clinical management of patients with ACC based on the GRADE (Grading of Recommendations Assessment, Development and Evaluation) system. We predefined four main clinical questions, which we judged as particularly important for the management of ACC patients and performed systematic literature searches: (A) What is needed to diagnose an ACC by histopathology? (B) Which are

Almost one decade ago, etoposide, doxorubicin, cisplatin and mitotane (EDP-M) has been established as first-line systemic therapy of metastatic adrenocortical carcinoma (ACC). Although heterogeneous, the prognosis of advanced stage ACC is still poor and novel treatments are urgently needed. This article provides a short summary of current systemic ACC treatment and provides a comprehensive overview of new therapeutic approaches that have been investigated in the past years, including drugs targeting the IGF pathway, tyrosine kinase inhibitors, radionuclide treatment, and immunotherapy. Adrenocortical carcinoma (ACC) is a rare and aggressive form of cancer. This article presents a multiperspective narrative describing the experience of a patient treated for ACC, the clinical insights of an endocrinologist, and the advocacy perspective of "Let's Cure ACC", a patient association. The patient describes his journey from diagnosis to treatment, disease recurrence, and, ultimately, a disease-free state. Diagnosed at age 45 with locally invasive ACC in 2017, he underwent surgery, chemotherapy, and long-term mitotane therapy. He experienced multiple disease recurrences, repeated surg
