Salivary Therapeutic Monitoring of Buprenorphine in Neonates After Maternal Sublingual Dosing Guided by Physiologically Based Pharmacokinetic Modeling.

BACKGROUND: Opioid use disorder (OUD) during pregnancy is associated with high mortality rates and neonatal opioid withdrawal syndrome (NOWS). Buprenorphine, an opioid, is used to treat OUD and NOWS. Buprenorphine active metabolite (norbuprenorphine) can cross the placenta and cause neonatal respiratory depression (EC 50 = 35 ng/mL) at high brain extracellular fluid (bECF) levels. Neonatal therapeutic drug monitoring using saliva decreases the likelihood of distress and infections associated with frequent blood sampling. METHODS: An adult physiologically based pharmacokinetic model for bupren

Tracking of the beat-to-beat blood pressure changes by the Caretaker physiological monitor against invasive central aortic measurement.

OBJECTIVE: There is an unmet need for noninvasive continuous blood pressure (BP) monitoring technologies in various clinical settings. Continuous and noninvasive central aortic BP monitoring is technically not feasible currently, but if realized, would provide more accurate and real-time global hemodynamic information than any form of peripheral arterial BP monitoring in an acute care setting. As part of our efforts to develop such, herein we examined the tracking correlation between noninvasively-derived peripheral arterial BP by Caretaker device against invasively measured central aortic BP.

Duration of Pupillary Unresponsiveness to Light: A Physiological Adjunct to Electroencephalography and Motor Seizure Duration Monitoring During Electroconvulsive Therapy.

BACKGROUND: During electroconvulsive therapy (ECT) sessions, we observed that the time taken for the return of pupillary response to light (ROPL) outlasted both the electroencephalography (EEG) and the motor seizure duration after the delivery of the electrical stimulus to produce convulsions. OBJECTIVE: The objective of this study was to investigate whether ROPL can be used as a marker of cessation of seizure activity in the brain after ECT and also to study the effect of atropine premedication on seizure activity during ECT. METHODS: Forty-one patients underwent 82 sessions of ECT in a cro

Ethnic differences in biological monitoring of several organic solvents. II. A simulation study with a physiologically based pharmacokinetic model.

To improve the reliability of biological monitoring and the development of biological limit values, ethnic differences in the biological monitoring of several organic solvents were studied in Orientals and Caucasians. Six Caucasian and six Oriental volunteers were exposed to each organic solvent in an exposure chamber for 6 h at rest. The exposure concentrations were 50 ppm for perchloroethylene, 50 ppm for styrene, and 100 ppm for m-xylene, respectively. Experimental results were compared with simulation results of a physiologically based pharmacokinetic (PB-PK) model. Differences between Ori

Wireless versus routine physiologic monitoring after cesarean delivery to reduce maternal morbidity and mortality in a resource-limited setting: protocol of type 2 hybrid effectiveness-implementation study.

BACKGROUND: Women in sub-Saharan Africa have the highest rates of morbidity and mortality during childbirth globally. Despite increases in facility-based childbirth, gaps in quality of care at facilities have limited reductions in maternal deaths. Infrequent physiologic monitoring of women around childbirth is a major gap in care that leads to delays in life-saving interventions for women experiencing complications. METHODS: We will conduct a type-2 hybrid effectiveness-implementation study over 12 months to evaluate using a wireless physiologic monitoring system to detect and alert clinician

Performance of artificial intelligence models for monitoring psychological stress using noncontact physiological signals: a systematic review and meta-analysis.

BACKGROUND: Psychological stress is a common and dynamic mental health concern, but conventional assessment methods often rely on self-report questionnaires, clinical interviews, or contact-based physiological sensors, which may limit continuous and unobtrusive monitoring in everyday settings. Noncontact sensing technologies combined with artificial intelligence (AI) provide a potential approach for low-burden assessment by capturing physiological or physiology-related information without direct body contact. OBJECTIVE: This systematic review and meta-analysis aimed to evaluate the performanc

Electrical Impedance Tomography for Real-Time PEEP Monitoring and Atelectasis During Mask Ventilation: A Randomized Controlled Physiological Trial.

BACKGROUND: The combination of high inspired oxygen fraction (FiO2) and positive-pressure mask ventilation at induction can lead to atelectasis, increasing the risk of perioperative pulmonary complications. Positive end-expiratory pressure (PEEP) may reduce atelectasis, but prior studies relied on imaging performed before or after induction rather than during ongoing ventilation. We used electrical impedance tomography (EIT) as a bedside, real-time imaging tool. We hypothesized that higher PEEP levels would improve end-expiratory lung aeration and homogenize ventilation distribution without cl

Intracranial compliance monitoring using pulse shape index in traumatic brain injury: relation to cerebral physiology and clinical outcome.

BACKGROUND: The intracranial pressure (ICP) pulse waveform reflects intracranial compliance. The pulse shape index (PSI), an artificial intelligence (AI)-based metric ranging from 1 (normal) to 4 (disturbed), quantifies morphological waveform pathologies. Early findings in smaller cohorts indicate that elevated PSI is associated with mass lesions, aging, higher ICP, and worse outcome. This study examined how PSI relates to other markers of intracranial compliance, the risk of ICP crisis, and clinical outcome in a large traumatic brain injury (TBI) cohort. METHODS: This retrospective study inc

Assessing the physiology of weight loss based on real-time weight monitoring: The ADAPT behavioral weight loss study protocol.

Improving health outcomes via sustained weight loss and maintenance requires advancing understanding of the metabolic, appetitive, and neurological alterations that counteract-and eventually halt-weight loss. Prior studies have demonstrated increased appetite as well as metabolic adaptations, such as increased energy efficiency, in response to weight loss. However, previous study designs utilized experimentally determined weight loss plateaus and did not investigate spontaneously occurring plateaus nor study participants in their natural, free-living, environments during weight loss. The Asses

Task-specific physiological signatures of perceptual and cognitive workload under cockpit noise: implications for pilot state monitoring and training.

Cockpit noise is a common operational stressor that can affect pilots' mental workload and performance. This study examined whether multimodal physiological measures could show task-demand-dependent physiological signatures under cockpit-noise interference, and whether these responses are moderated by flight experience. Eighty-five valid participants from an initial sample of 100 pilots performed a Stroop task and a logical reasoning task under no-noise and 85-90 dB(A) noise conditions. Eye-tracking measures and ECG-derived heart rate were recorded. Noise exposure was associated with increased

A Wrist System for Daily Stress Monitoring Using Mid-Level Physiological Fusion and Late Fusion with Survey-Based Labels.

UNLABELLED: Multi-sensor fusion can improve daily stress monitoring. METHODS: A wrist-worn device includes a system of the galvanic skin response (GSR), PPG-derived heart rate variability (HRV), skin temperature, and SpO2, paired with self-reported questionnaires. The device streams data to a mobile app over Bluetooth Low Energy and updates the UI within 1-2 s. The physiological features are captured within a fixed window around each questionnaire time and undergo a mid-level fusion; late fusion is also evaluated using self-reports. RESULTS: Against a commercial reference device, the propose

Cerebral autoregulation monitoring in neonates and infants after cardiac surgery with cardiopulmonary bypass - comparison of single ventricle and biventricular physiology.

INTRODUCTION: Cardiopulmonary bypass surgery can lead to impaired cerebral autoregulation with the risk for ischemia, hemorrhage and delirium. In particular, infants with single ventricle physiology have altered hemodynamics with persistent veno-arterial shunting, cyanosis and diastolic run-off after surgery, which may have negative effects on cerebral autoregulation. METHODS: Cerebral autoregulation was prospectively monitored in 83 neonates and infants after open heart surgery with cardiopulmonary bypass after admission to the pediatric intensive care unit. Autoregulation indices were deter

Implementation of advanced vascular access, physiological monitoring and goal-directed resuscitation during OHCA in a helicopter emergency medical service.

Outcomes after out-of-hospital cardiac arrest (OHCA) remain poor in the UK. In order to increase the chances of successful resuscitation, international society guidelines on cardiopulmonary resuscitation quality have recommended titration of chest compression parameters and vasopressor administration to arterial diastolic blood pressure if invasive catheters are in situ at the time of cardiac arrest. However, prehospital initiation of arterial and central venous catheterisation is seldom undertaken due to the risks and significant technical challenges in the context of ongoing resuscitation in

Insights into the problem of alarm fatigue with physiologic monitor devices: a comprehensive observational study of consecutive intensive care unit patients.

PURPOSE: Physiologic monitors are plagued with alarms that create a cacophony of sounds and visual alerts causing "alarm fatigue" which creates an unsafe patient environment because a life-threatening event may be missed in this milieu of sensory overload. Using a state-of-the-art technology acquisition infrastructure, all monitor data including 7 ECG leads, all pressure, SpO(2), and respiration waveforms as well as user settings and alarms were stored on 461 adults treated in intensive care units. Using a well-defined alarm annotation protocol, nurse scientists with 95% inter-rater reliabilit

Pulse oximetry home monitoring in infants with single-ventricle physiology and a surgical shunt as the only source of pulmonary blood flow.

BACKGROUND: Shunt occlusion is a major cause of death in children with single ventricle. We evaluated whether one daily measurement of oxygen saturation at home could detect life-threatening shunt dysfunction. METHODS: A total of 28 infants were included. Parents were instructed to measure saturation once daily and if less than or equal to 70% repeat the measurement. Home monitoring was defined as positive when a patient was admitted to Queen Silvia Children's Hospital because of saturation less than or equal to 70% on repeated measurement at home. A shunt complication was defined as arterial

Real-time monitoring of melittin-induced pore and tubule formation from supported lipid bilayers and its physiological relevance.

The temporal evolution of effects of antimicrobial peptide melittin on supported phospholipid bilayers (SPBs) containing negatively charged phospholipids was monitored by ellipsometry and laser scanning microscopy together with measurements of lipid mobility by Z-scan fluorescence correlation spectroscopy. Under all conditions used in our study, we observed reproducibly two effects. The first one is formation of pores in the SPB, which occupy approximately 40% of the bilayer. The formation of pores was accompanied by a decrease in lateral diffusion coefficient of the lipids to approximately 60
