Bone marrow-derived mesenchymal stem cell transplantation does not improve quality of muscle reinnervation or recovery of motor function after facial nerve transection in rats.

Recently, we devised and validated a novel strategy in rats to improve the outcome of facial nerve reconstruction by daily manual stimulation of the target muscles. The treatment resulted in full recovery of facial movements (whisking), which was achieved by reducing the proportion of pathologically polyinnervated motor endplates. Here, we posed whether manual stimulation could also be beneficial after a surgical procedure potentially useful for treatment of large peripheral nerve defects, i.e., entubulation of the transected facial nerve in a conduit filled with suspension of isogeneic bone m

Stimulated single-fiber electromyography in Lambert-Eaton myasthenic syndrome before and after 3,4-diaminopyridine.

A patient with LEMS unrelated to cancer was studied by stimulated single-fiber electromyography (SFEMG) before and 3 months after the onset of therapy with 3,4-diaminopyridine. All end-plates showed a progressive reduction in blockings and jitter with the increase in stimulation rate. Treatment significantly corrected this feature, but the overall pattern of frequency-improved jitter remained. Such widespread finding is rare but diagnostic for Lambert-Eaton myasthenic syndrome. Stimulated SFEMG can be used to monitor therapy in such patients.

[Muscle cramps: mechanism , etiology and current treatment].

Spasms of skeletal muscles occur in healthy individuals or in the course of functional disorders of the peripheral motor neuron or its axons. Like fasciculations, they are a motor manifestation, but a painful one. Cramps evoke the notion of ephapsis and of immature terminal axons. There are, however, also other responsible regulatory systems, mostly the sensible and spinal ones. Three personal observations are used to illustrate the various triggering mechanisms. A review of the literature on this subject is added. Symptomatic treatment has not changed over past decades: Quinidine remains the

Block of acetylcholine-activated ion channels by an uncharged local anaesthetic.

It is now thought that amine local anaesthetic compounds (procaine, lignocaine and related molecules) depress electrical activity in nerve and muscle cells by binding to sites within ion channels and blocking current flow. Such mechanisms have been proposed to account for the effects of these local anaesthetics on both the voltage-dependent sodium current and the postsynaptic actylcholine (ACh)-activated ionic current. Recently, strong evidence for block of ion channels by cationic drug molecules has been obtained by recording current from single ACh-activated channels in the presence of perma

[Electromyographic and histopathological study of the Lambert-Eaton myasthenia syndrome. Apropos of a case].

In this case of Eaton-Lambert myasthenic syndrome the electrophysiological study demonstrated some modifications of latency of the muscle action potentials evoked by repetitive nerve stimulation at 30 c/sec. specially after administration of edrophonium chloride. This data suggest as possible some fluctuations in the mechanism of the neuromuscular transmission defect. The muscle biopsy showed changes secondary to a peripheral motor nerve involvement; electronmicroscopic study of the motor end-plates revealed a highly developped subneural apparatus.

Fluoxetine is neuroprotective in slow-channel congenital myasthenic syndrome.

The slow-channel congenital myasthenic syndrome (SCS) is an inherited neurodegenerative disease that caused mutations in the acetylcholine receptor (AChR) affecting neuromuscular transmission. Leaky AChRs lead to Ca(2+) overload and degeneration of the neuromuscular junction (NMJ) attributed to activation of cysteine proteases and apoptotic changes of synaptic nuclei. Here we use transgenic mouse models expressing two different mutations found in SCS to demonstrate that inhibition of prolonged opening of mutant AChRs using fluoxetine not only improves motor performance and neuromuscular transm

A presynaptic congenital myasthenic syndrome attributed to a homozygous sequence variant in LAMA5.

We report a severe defect of neuromuscular transmission in a consanguineous patient with a homozygous variant in the laminin α5 subunit gene (LAMA5). The variant c.8046C > T (p.Arg2659Trp) is rare and has a predicted deleterious effect. The affected individual, who also carries a rare homozygous sequence variant in LAMA1, had normal cognitive function, but magnetic resonance brain imaging showed mild volume loss and periventricular T2 prolongation. Repetitive nerve stimulation at 2 Hz showed 50% decrement of compound muscle action potential amplitudes but 250% facilitation immediately after ex

Miller Fisher anti-GQ1b antibodies: alpha-latrotoxin-like effects on motor end plates.

In the Miller Fisher syndrome (MFS) variant of the Guillain-Barré syndrome, weakness is restricted to extraocular muscles and occasionally other craniobulbar muscles. Most MFS patients have serum antibodies against ganglioside type GQ1b of which the pathophysiological relevance is unclear. We examined the in vitro effects of MFS sera, MFS IgG, and a human monoclonal anti-GQ1b IgM antibody on mouse neuromuscular junctions (NMJs). It was found that anti-GQ1b antibodies bind at NMJs where they induce massive quantal release of acetylcholine from nerve terminals and eventually block neuromuscular

Stannous ion-induced tetanic contraction of the frog sartorius muscle.

Present study was conducted to elucidate the mechanism of twitch augmentation by stannous ion. Stannous ion increased the amplitude of the endplate potential and moreover, generated repetitive firing of the muscle fiber. These findings suggest that the twitch augmentation by stannous ion may be due not only to recruitment of the muscle fibers but also to a brief tetanic contraction of the individual fibers.

Acute toxicity of pyridostigmine in rats: histological findings.

To assess the morphological changes due to pyridostigmine (Pyr), sublethal doses of Pyr (20 and 40 mg/kg body weight) were given to adult rats through a gastric tube. Within 24 h, acute focal necroses, leukocytic infiltrates and marked changes in the motor endplates appeared in the skeletal muscle. The changes were more evident in the diaphragm than in the quadriceps muscle. The acetylcholinesterase activity in the whole blood and in the erythrocytes was reduced to considerably less than one-half of the normal value. The histological changes induced by Pyr have not been reported to date, but a

[Terminal innervation in 3 cases of leprous neuropathy (author's transl)].

Three cases of leprosy are reported. In all patients, the diagnosis was made by finding Hansen bacilli in skin lesions, nasal secretions and ears, and the immunological typing of leprosy was established. Conduction velocity of motor and sensory nerves was measured and muscle biopsies were obtained. Histochemical typing of muscle fibers and measurement of the terminal innervation ratio of motor axon vitally stained with methylene blue were performed. A subclinical involvement of peripheral motor nerves was demonstrated by these investigations.

Rapid increase in intradiscal pressure in porcine cervical spine units negatively impacts annulus fibrosus strength.

Vertebral endplate fracture is generally accepted to occur as a result of excessive, typically rapid, pressure developed within the intervertebral disc. Clinical evidence of disc disorders later in life following endplate fracture suggests that the disc is also impacted by the fracture event. There is an abundance of evidence to indicate that loss of disc pressure at the time of fracture alters annulus fibrosus loading which could impact disc health long-term. However, little is known regarding the isolated impact of the rapid disc pressurization during the fracture event. This study sought to

Is there a nitrergic modulation of the rat external anal sphincter?

Nitric oxide is known to relax the internal anal sphincter, but its effect on the external anal sphincter (EAS) is unknown. The aim of this study was to investigate whether there is a nitrergic nerve plexus that modulates the EAS, similar to that found in oesophageal striated muscle. An in vitro ring preparation of rat anal canal was used to evaluate the effects of the nitric oxide synthase inhibitor N(ω)-nitro-L-arginine (L-NNA) and the NO donor sodium nitroprusside (SNP) on the EAS in conditions of neuromuscular blockade and the effect of SNP on nerve-evoked contractions. The effect of aging on the morphological nerve changes during muscle reinnervation after nerve crush.

The extent of the muscle endplate reinnervation that followed crush injury of the sciatic nerve was compared between young adult (4 and 5 months old) and aged (24 months old) animals. The time course of regeneration in the muscular nerve bundle, its ramification, and the nerve terminal was immunohistochemically estimated using an antibody against the neuron specific enolase (NSE), a neuronal marker. During early phases of regeneration (7, 21 and 28 days post-crush) in the young adult animal, there were tortuosity, vacuolation and/or unfasciculation in the nerve bundle and its ramification, alo

Segmental distribution of the motor neuron columns that supply the rat hindlimb: A muscle/motor neuron tract-tracing analysis targeting the motor end plates.

Spinal cord injury (SCI) that disrupts input from higher brain centers to the lumbar region of the spinal cord results in paraplegia, one of the most debilitating conditions affecting locomotion. Non-human primates have long been considered to be the most appropriate animal to model lower limb dysfunction. More recently, however, there has been a wealth of scientific information gathered in the rat regarding the central control of locomotion. Moreover, rodent models of SCI at lumbar levels have been widely used to validate therapeutic scenarios aimed at the restoration of locomotor activities.

Targeting the motor end plates in the mouse hindlimb gives access to a greater number of spinal cord motor neurons: an approach to maximize retrograde transport.

Lower motor neuron dysfunction is one of the most debilitating neurological conditions and, as such, significantly impacts on the quality of life of affected individuals. Within the last decade, the engineering of mouse models of lower motor neuron diseases has facilitated the development of new therapeutic scenarios aimed at delaying or reversing the progression of these conditions. In this context, motor end plates (MEPs) are highly specialized regions on the skeletal musculature that offer minimally invasive access to the pre-synaptic nerve terminals, henceforth to the spinal cord motor neu

Diversity and homogeneity within endplates associated with physiologically identified static gamma-axons in cat tenuissimus muscle.

The terminations of static gamma-axons on chain fibres may be associated with a muscle surface contour thrown into complex folds at one extreme, ranging through lesser degrees of folding to being apparently uninfluenced by the presence of the terminals. The folding is not necessarily confined to the postsynaptic membrane. As a quantitative indicator of the degree of folding seen in cross-sections, the perimeters of the two halves of the intrafusal fibre with, and without the ending were compared. Both complexity of endplate structure and an undifferentiated appearance could co-exist within ind

End-plate acetylcholine receptor: structure, mechanism, pharmacology, and disease.

The synapse is a localized neurohumoral contact between a neuron and an effector cell and may be considered the quantum of fast intercellular communication. Analogously, the postsynaptic neurotransmitter receptor may be considered the quantum of fast chemical to electrical transduction. Our understanding of postsynaptic receptors began to develop about a hundred years ago with the demonstration that electrical stimulation of the vagus nerve released acetylcholine and slowed the heart beat. During the past 50 years, advances in understanding postsynaptic receptors increased at a rapid pace, owi
