Landmark-based analysis of speech differentiates conversational from clear speech in speakers with muscle tension dysphonia.

This study evaluated the feasibility of differentiating conversational and clear speech produced by individuals with muscle tension dysphonia (MTD) using landmark-based analysis of speech (LMBAS). Thirty-four adult speakers with MTD recorded conversational and clear speech, with 27 of them able to produce clear speech. The recordings of these individuals were analyzed with the open-source LMBAS program, SpeechMark®, matlab Toolbox version 1.1.2. The results indicated that glottal landmarks, burst onset landmarks, and the duration between glottal landmarks differentiated conversational speech f

Accent method of voice therapy for treatment of severe muscle tension dysphonia.

Nonorganic dysphonia may present a challenging diagnosis, and management. Here, we present a severe form of nonorganic dysphonia, which we termed as arytenoidal dysphonia. It was a severe form of muscle tension dysphonia, which was described earlier in literature although with different nomenclature. The outcome of the accent method of voice therapy was also presented. We concluded that accent method of voice therapy is proven to be an effective treatment modality of arytenoidal dystonia.

The neuronal mechanism underlying parkinsonism and dyskinesia, and differential roles of the putamen and caudate nucleus.

The basal ganglia were manipulated pharmacologically by using GABA antagonists and also 1-methyl-14-phenyl-1,2,3,6-tetrahydropyridine (MPTP) in rats, cats, and monkeys, and neuronal activities were recorded from the basal ganglia. It was concluded that (a) in parkinsonism, neuronal activities of the caudate nucleus decreased, those of the pars reticulata of the substantia nigra and internal segment of the globus pallidus increased, and those of the ventromedial nucleus of the thalamus depressed markedly; (b) in dyskinesia, periodical discharges of the caudate-putamen complex were the primary e

The neuropharmacology of upper airway motor control in the awake and asleep states: implications for obstructive sleep apnoea.

Obstructive sleep apnoea is a common and serious breathing problem that is caused by effects of sleep on pharyngeal muscle tone in individuals with narrow upper airways. There has been increasing focus on delineating the brain mechanisms that modulate pharyngeal muscle activity in the awake and asleep states in order to understand the pathogenesis of obstructive apnoeas and to develop novel neurochemical treatments. Although initial clinical studies have met with only limited success, it is proposed that more rational and realistic approaches may be devised for neurochemical modulation of phar

Muscle tension dysphonia and spasmodic dysphonia: the role of manual laryngeal tension reduction in diagnosis and management.

Excessive activity of the extralaryngeal muscles affects laryngeal function and contributes to a spectrum of interrelated symptoms and syndromes including muscle tension dysphonia and spasmodic dysphonia. Recognition of the role of extralaryngeal tension is helpful in ensuring proper diagnosis and selection of appropriate treatment. This report demonstrates the application of manual laryngeal musculoskeletal tension reduction techniques in the diagnosis and management of laryngeal hyperfunction syndromes. The manual technique consists of focal palpation to determine 1) extent of laryngeal elev

Adaptation in the function of pharyngeal constrictor muscles.

The palatopharyngeus and pharyngeal constrictor muscles were studied by electromyography (EMG) and by direct observation with a flexible fiberoptic scope in the anesthetized as well as in the alert rhesus monkey. The muscles were monitored to determine the change in their discharge with nasal obstruction, head posture, head extension, and swallowing. The results indicated that certain regions of the middle and inferior pharyngeal constrictors never discharged during deglutition. Extending the head could induce a tonic discharge in fibers of the middle pharyngeal constrictor for the duration of

The Regulatory Effects of Low-Intensity Pulsed Ultrasound Combined With Stretching on Muscle Tension Relief: Focusing on the Microstructural Mechanism.

While often associated with neurological conditions, muscle dysfunction may also result from primary myopathies and other non-neurological causes, contributing significantly to motor impairment and reduced quality of life. Conventional therapeutic approaches, including electrotherapy, cryotherapy and pharmacological interventions, exhibit limited efficacy and potential adverse effects. By comparison, low-intensity pulsed ultrasound (LIPUS) has emerged as a promising research focus due to its non-invasive nature, safety profile and therapeutic efficiency. This study presents a novel approach fo

Changes in tone of canine gallbladder measured by an electronic pneumatic barostat.

We developed an electronic pneumatic barostat to measure changes in gallbladder muscle tone in vivo independent of biliary flow dynamics. Dogs were prepared with a gallbladder cannula for insertion of barostat balloon and an inflatable balloon-occluder around the cystic duct. The barostat was validated to maintain a constant pressure of 2 +/- 0.4 (SE) mmHg within a plastic balloon of "infinite" compliance that filled the gallbladder lumen; changes in balloon volume reflected changes in wall tone because intraluminal pressure was kept constant. Gallbladder tone, as measured by the barostat ball

[Neuroleptic malignant syndrome in a patient with polyneuropathy: mechanism of muscle rigidity and elevated serum creatine kinase levels].

A 49-year-old man was admitted with the chief complaints of muscle weakness and gait disturbance. His neurological examination was compatible with peripheral neuropathy, and laboratory tests revealed IgA monoclonal gammopathy, increased protein content in the cerebrospinal fluid (CSF) without pleocytosis, and slow motor and sensory nerve conduction velocity. He was diagnosed as having chronic inflammatory demyelinating polyneuropathy with IgA monoclonal gammopathy of undetermined significance. The patient was treated with steroid, and plasmapheresis. He became so restless that antidepressant a

Facial Hemiplegia Treated with Botulinum Toxin: A Case Report.

Facial hemiplegia happens when the seventh cranial nerve is inflamed, causing a dysfunction of the facial nerve in specific regions. This case report brings a complex case of facial hemiplegia, a non-temporary lesion caused by a traumatic accident, which had a more conservative approach, treating the patient with botulinum toxin. After explanation of treatment outcomes, the patient favored treatment on a unilateral side with botulinum toxin applied locally to the muscles. It was applied on her left side, in order to change the muscles tonus and improve the esthetic. The patient adhered to imme

Muscle Tone Assessment by Machine Learning Using Surface Electromyography.

Muscle tone is defined as the resistance to passive stretch, but this definition is often criticized for its ambiguity since some suggest it is related to a state of preparation for movement. Muscle tone is primarily regulated by the central nervous system, and individuals with neurological disorders may lose the ability to control normal tone and can exhibit abnormalities. Currently, these abnormalities are mostly evaluated using subjective scales, highlighting a lack of objective assessment methods in the literature. This study aimed to use surface electromyography (sEMG) and machine learnin

Effect of breathing, pressure and posture on palatoglossal and genioglossal tone.

1. Patency of the upper airway is critical to respiration. Although about half of patients with the sleep apnoea/hypopnoea syndrome obstruct their upper airway at the retropalatal level, the respiratory actions of the palatal muscles have been little studied. We have therefore tested the hypothesis that the nasopharyngeal dilator muscle palatoglossus is activated during inspiration and by negative pressure. 2. Using intramuscular wire electrodes inserted perorally, we have compared the response of palatoglossus and genioglossus to breathing, posture change and airway negative pressure in 10 no

The respiratory activity of human levator costae muscles and its modification by posture.

The activation of the levator costae, a set of axial muscles inserted between each rib and the vertebra immediately rostral, has been studied electromyographically in a human subject, standing upright, with head erect ('neutral' posture). Caudal levator costae muscles (T9, and T10) are active during eupnoea. Periodic activity in phase with inspiration occurs in admixture with different levels of tonic activity that is strongly influenced by posture. Postural changes and various voluntary respiratory manoeuvres which should lengthen or shorten the levator costae muscles increase and decrease, r
