This study investigates the characteristics of intraocular pressure (IOP) fluctuations in Chinese patients with pigment dispersion syndrome (PDS) after drug-induced mydriasis. In this prospective cohort study, 26 PDS patients were identified and instilled with 0.5% tropicamide and phenylephrine twice randomly in one of the 2 eyes, after which IOP and pupil diameter (PD) were measured hourly for 8 hours. A slit-lamp examination was performed after each measurement. Post-mydriatic IOPs and PDs were compared with baseline. The average IOP increased to a maximum of 18.3 ± 4.9 mm Hg (vs 16.7 ± 4.5

Perioperative hypothermia represents a major complication in pediatric anesthesia, particularly in neonates. We report the case of a newborn who underwent general anesthesia for esophageal fistula repair (following primary repair of esophageal atresia) and developed severe hypothermia manifested by the triad of refractory severe bradycardia, bilateral areflexic mydriasis, and chest wall rigidity. Management combining active rewarming, alkalinization, and cardiovascular support resulted in a complete recovery with a progressive normalization of clinical parameters. We discuss the clinical signs

A 10-year-old male neutered crossbreed dog and an 8-year-old female neutered greyhound presented after peracute onset of cerebellar dysfunction. The crossbreed dog had anisocoria with the left pupil being mydriatic, spontaneous conjugate rotatory nystagmus with fast phase to the left and delayed postural reactions on the left side. The greyhound had anisocoria with the left pupil being mydriatic, right positional ventral strabismus, absent menace response in the left eye and postural reaction deficits on the left side. For both dogs, the neuroanatomical localization was left cerebellum with pa

Nuclear shape alteration in ocular tissues, which can be used as a metric for overall cell deformation, may also lead to changes in gene expression and protein synthesis that could affect the biomechanics of the tissue extracellular matrix. The biomechanics of iris tissue is of particular interest in the study of primary angle-closure glaucoma. As the first step towards understanding the mutual role of the biomechanics and deformation of the iris on the activity of its constituent stromal cells, we conducted an ex-vivo study in freshly excised porcine eyes. Iris deformation was achieved by act

BACKGROUND: Laser therapy has been the gold standard treatment for retinopathy of prematurity (ROP), while intravitreal bevacizumab (IVB) is reported to be of significant benefit for zone I ROP. A problem with laser therapy is that it is difficult to administer in ROP patients with severely dilated tunica vasculosa lentis and poor mydriasis. However, although IVB treatment has been performed in such severe ROP cases, only 1 report has discussed its usefulness.
CASE 1: A male infant was born with a birth weight of 382 g at 23 weeks' gestation. As visualization was poor and laser therapy could

The maintenance of mydriasis and the control of postoperative pain and inflammation are critical to the safety and success of cataract and intraocular lens replacement surgery. Appropriate mydriasis is usually achieved by topical and/or intracameral administration of anticholinergic agents, sympathomimetic agents, or both, with the most commonly used being cyclopentolate, tropicamide, and phenylephrine. Ocular inflammation is common after cataract surgery. Topical steroids and nonsteroidal anti-inflammatory drugs are widely used because they have been proved effective to control postsurgical i

BACKGROUND Micropulse cyclophotocoagulation (MPCPC) is a commonly used procedure to reduce intraocular pressure (IOP) in glaucoma patients. While it is generally considered safer than traditional transscleral cyclophotocoagulation (TSCPC), complications such as transient mydriasis and reduced accommodation due to short ciliary nerve damage have been reported. However, isolated loss of accommodation without pupillary involvement has not been previously documented. We report a case of reduced accommodation without pupil dilation in a young patient after MPCPC. CASE REPORT A 26-year-old woman wit

This study aims to report a rare complication, anisocoria (mydriasis), in a patient with cleft lip and palate in the immediate postoperative period after orthognathic surgery and discuss the literature surrounding this event. Mydriasis after orthognathic surgery may be related to the separation of the pterygoid plate from the sphenoid bone, which can injure the cranial nerves. Therefore, knowledge of possible anatomical variations of the posterior region of the maxilla becomes extremely important to minimize complications in cleft patients. The reported case highlights the management of anisoc

Four patients with traumatic mydriasis and aphakia following blunt injury of the eye globe were evaluated. Patients with severe glare and photophobia due to wide pupil diameter from 6.0 to 9.0 mm were managed by combined iris cerclage pupilloplasty and retropupillar iris-claw lens implantation. The postoperative anatomic results, visual acuity, subjective degree of glare, photophobia, as well as intraoperative and postoperative complications were evaluated. The mean follow-up time was 32.6 months. Best-corrected visual acuity improved in all patients from preoperative 20/60, 20/30, 20/25, 20/2

Miller-Fisher syndrome (MFS) is characterized by variable ophthalmoplegia, ataxia, and tendon areflexia. It seems to be a variant of Guillain-Barré syndrome (GBS), but unlike in GBS, there is a primitive involvement of the ocular motor nerves, and in some cases there is brainstem or cerebellum direct damage. The unusual case of MFS in the current study started with a bilateral areflexical mydriasis and a slight failure of accommodative-convergence. Ocular-movement abnormalities developed progressively with a palsy of the upward gaze and a bilateral internuclear ophthalmoplegia to a complete op

Phenylephrine, a potent sympathomimetic, induces mydriasis via iris dilator muscle contraction. Intracameral (IC) phenylephrine has been successfully used in cataract surgery for initial mydriasis, maintaining mydriasis, and management of intraoperative floppy-iris syndrome. Serious systemic adverse events (mainly cardiovascular) have been described with topical phenylephrine drops, but we found very little evidence of such adverse events associated with IC phenylephrine use. However, we suspect under-reporting of such adverse events, as they may instead be ascribed to anxiety, positioning, an
