Ambiguous and Incomplete: Natural Language Processing Reveals Problematic Reporting Styles in Thyroid Ultrasound Reports.

OBJECTIVE: Natural language processing (NLP) systems convert unstructured text into analyzable data. Here, we describe the performance measures of NLP to capture granular details on nodules from thyroid ultrasound (US) reports and reveal critical issues with reporting language. METHODS: We iteratively developed NLP tools using clinical Text Analysis and Knowledge Extraction System (cTAKES) and thyroid US reports from 2007 to 2013. We incorporated nine nodule features for NLP extraction. Next, we evaluated the precision, recall, and accuracy of our NLP tools using a separate set of US reports

Using natural language processing to identify opioid use disorder in electronic health record data.

BACKGROUND: As opioid prescriptions have risen, there has also been an increase in opioid use disorder (OUD) and its adverse outcomes. Accurate and complete epidemiologic surveillance of OUD, to inform prevention strategies, presents challenges. The objective of this study was to ascertain prevalence of OUD using two methods to identify OUD in electronic health records (EHR): applying natural language processing (NLP) for text mining of unstructured clinical notes and using ICD-10-CM diagnostic codes. METHODS: Data were drawn from EHR records for hospital and emergency department patient visi

Moonstone: a novel natural language processing system for inferring social risk from clinical narratives.

BACKGROUND: Social risk factors are important dimensions of health and are linked to access to care, quality of life, health outcomes and life expectancy. However, in the Electronic Health Record, data related to many social risk factors are primarily recorded in free-text clinical notes, rather than as more readily computable structured data, and hence cannot currently be easily incorporated into automated assessments of health. In this paper, we present Moonstone, a new, highly configurable rule-based clinical natural language processing system designed to automatically extract information t

Natural language processing improves identification of colorectal cancer testing in the electronic medical record.

BACKGROUND: Difficulty identifying patients in need of colorectal cancer (CRC) screening contributes to low screening rates. OBJECTIVE: To use Electronic Health Record (EHR) data to identify patients with prior CRC testing. DESIGN: A clinical natural language processing (NLP) system was modified to identify 4 CRC tests (colonoscopy, flexible sigmoidoscopy, fecal occult blood testing, and double contrast barium enema) within electronic clinical documentation. Text phrases in clinical notes referencing CRC tests were interpreted by the system to determine whether testing was planned or complet

Mapping Clinical Intent in Internet Hospitals: Identifying and Profiling Patient Demand via Natural Language Processing.

Patient-generated text in internet hospitals represents a massive but unstructured source of clinical demand. Misalignment between rigid administrative menus and actual patient intent leads to triage inefficiency and resource mismatches. We developed a hybrid artificial intelligence framework to decode patient needs and optimize service allocation using real-world data from a tertiary internet hospital, with external validation on an independent multihospital dataset (N = 497). In this study, we analyzed 28,851 inquiries from a tertiary internet hospital using a 2-stage approach: (a) unsupervi

Differentiating Radiotherapy-Specific Distress From General Cancer Distress: Natural Language Processing Analysis of Patient Narratives.

BACKGROUND: Psychological distress is common among patients with cancer, and it negatively impacts treatment adherence and quality of life. Radiotherapy, with its unique procedures, such as daily sessions and physical immobilization, may induce distress distinct from general cancer anxiety. However, existing screening tools cannot differentiate these distress sources. This study leverages online patient narratives and natural language processing to distinguish radiotherapy-specific distress from general cancer distress. OBJECTIVE: This study aimed to systematically identify, differentiate, an

Characterizing thunderstorm asthma emergency department presentations using natural language processing for improved real-time surveillance.

Thunderstorm asthma events can overwhelm emergency departments within hours, making rapid detection critical for public health preparedness and patient safety. We examined whether thunderstorm asthma presentations show distinctive patterns compared to routine asthma cases in emergency department data, with an aim of enhancing the detection of thunderstorm asthma incidence. Using Victoria's SynSurv surveillance system, we analyzed triage notes from three thunderstorm asthma events occurring between 2022 and 2024 in Victoria, Australia, comparing 687 thunderstorm-related presentations with 687 p

Using natural language processing to assess proxy measures of therapeutic alliance across suicide risk tiers.

Therapeutic alliance is a core predictor of psychotherapeutic outcome, in general, and suicide prevention, specifically. Although clinical literature suggests that maintaining positive alliance with high-suicide-risk patients can be particularly challenging, few studies evaluate this phenomenon, primarily due to alliance measurement difficulties. Our study derives proxy alliance metrics using natural language processing and models differences across classified suicide risk strata and suicide decedent status. We first identified all United States Department of Veterans Affairs (VA) psychotherap

Natural language processing of biomedical text to map and prioritize protein-disease associations in HFpEF.

The validation of promising clinical biomarkers, molecular mechanisms, and novel drug targets in cardiovascular disease (CVD) is hindered by a vast and fragmented biomedical literature, which now exceeds 38 million publications indexed in PubMed. To address the central challenge of navigating and synthesizing a huge fragmented biomedical literature base, we applied our validated machine learning-based text-mining algorithm containing natural language processing (NLP) and incorporated this into a ValIdated Text-mining using Advanced Language model (VITAL) as a complementary framework. Unhealthy alcohol use detection in electronic health records: A comparative study using natural language processing.

BACKGROUND: Unhealthy alcohol use, including risky alcohol use and alcohol use disorder (AUD), are under-identified in primary care settings. Natural Language Processing (NLP) is a promising approach that could identify unhealthy alcohol use from clinical notes even when structured data (SD) indicators are lacking. This study prospectively evaluated the performance of SD and NLP in identifying unhealthy alcohol use in primary care patients. METHODS: We extracted electronic health record (EHR) data of primary care patients at a large Midwestern Health System (N = 133,144) and applied two ident

In patients' words: natural language processing of reports from patients experiencing orofacial pain and dysfunction.

BACKGROUND: Provision of value-based, patient-centered care requires careful appraisal of patients’ symptoms. Understanding subjective experiences, needs, and feelings is crucial for shared decision-making, especially when clinical findings differ from individual perceptions. Natural language processing (NLP) offers new ways to understand patients’ perspectives. This exploratory pilot study aimed to exemplify the use of three popular NLP methods to analyze open-ended textual self-reports from individuals experiencing orofacial pain and dysfunction for a more comprehensive understanding of thei
