Different physical therapies for cervicalgia are described, as well as their efficacy, adverse effects, degree of evidence and recommendations. Several guidelines recommend active exercise, patient education and various treatments described here. According to the strict criteria of evidence-based medicine, active exercise, dry needling and probably laser therapy are effective as well as acupuncture. The use of a collar-neck brace is not recommended as first intention and should be limited in duration. Despite their clinical benefit and the inclusion of many therapies in several high-level scie

A 32-year-old Chinese woman sought care from our family medicine clinic because she had a headache, neck pain, and an intermittent cough that had produced white sputum for 7 days. She described the headache as severe and pressure-like, and said that it had progressively worsened over the previous 3 weeks, coinciding with her first trip outside of China to the United States. The patient indicated that she also had occasional vomiting, dizziness, a low-grade fever, chills, night sweats, and increasing fatigue.

BACKGROUND: Neck disorders are common, disabling and costly. Botulinum toxin (BoNT) intramuscular injections are often used with the intention of treating neck pain.
OBJECTIVES: To systematically evaluate the literature on the treatment effectiveness of BoNT for neck pain, disability, global perceived effect and quality of life in adults with neck pain with or without associated cervicogenic headache, but excluding cervical radiculopathy and whiplash associated disorder.
SEARCH STRATEGY: We searched CENTRAL, MEDLINE, AMED, Index to Chiropractic Literature, CINAHL, LILACS, and EMBASE from the

Through the myriad of abnormalities encountered by spine surgeons, neck pain is one of the most perplexing. The nature, onset, and location of the pain all provide information as to what the potential pain generator may be. By synthesizing data garnered from the physical examination, imaging studies, and history, a spine surgeon must formulate a differential diagnosis and treatment plan. The surgeon must determine whether the patient has cervical radiculopathy, myelopathy, or simply cervical spondylosis because the treatment of each of these is vastly different.

Patients with spontaneous neck pain, headache, dizziness and/or pain to the upper limbs are frequently observed. Common cervicodynia, due to the involvement of arthromuscular structures of the cervical spine, was diagnosed in the patients with these symptoms in the absence of trauma or neurologic signs. The authors investigated the clinical-radiologic correlation in a series of 130 symptomatic patients and considered it a metameric disorder. The frequent association of many radiographic signs at the same level often prevents single radiographic signs from being assessed individually; therefore

BACKGROUND Cervical spine imbalance can lead to neck pain and abnormal posture. However, the relationship between global cervical sagittal balance and different neck pain subtypes remains unclear. Therefore, this retrospective study aimed to compare radiographic cervical sagittal alignment parameters, including the sagittal vertical axis (SVA), spino-cranial angle (SCA), and intervertebral disc height, among 25 patients with nonspecific neck pain, 20 patients with degenerative neck pain, and 25 asymptomatic healthy controls. MATERIAL AND METHODS This retrospective observational study included

This study examined the differences in neck muscle activity at various neck flexion angles in smartphone users with and without neck pain. Forty-four participants performed texting tasks for 1 min and 30 s. Neck muscle activity and pain in the neck were measured at different neck flexion angles. There was a difference in neck muscle activity for each of the neck flexion angles; the Cervical Erector Spinae (CES) muscle activity increased while the Upper Trapezius (UT) muscle activity decreased when at increased neck flexion angles. At neck flexion angle of 0°-15°, the activity of both CES and U

OBJECTIVES: Back pain is often attributed to increased tension in the back muscles, regardless of whether the tension is primary or related to a disc/facet pathology. We hypothesized that when either lower back pain or neck pain is unilateral, the muscle tension would be more pronounced on the painful side and could be detected by palpation alone (i.e., without the need to apply pain-triggering manoeuvres).
METHODS: Patients with unilateral neck or lower back pain were enrolled in the study. Participants with scoliosis, obesity, a history of spinal surgery or pain radiating below the knee or

BACKGROUND: Learning to predict threatening events enables an organism to engage in protective behavior and prevent harm. Failure to differentiate between cues that truly predict danger and those that do not, however, may lead to indiscriminate fear and avoidance behaviors, which in turn may contribute to disability in people with persistent pain. We aimed to test whether people with persistent neck pain exhibit contingency learning deficits in predicting pain relative to pain-free, gender-and age-matched controls.
METHOD: We developed a differential predictive learning task with a neck pain-

BACKGROUND: Although the reliability of pain drawings (PDs) has been confirmed in people with chronic pain, there is a lack of evidence about the validity of the PD, that is, does the PD accurately represent the pain experience of the patient?
OBJECTIVES: We investigate whether people with chronic neck pain (CNP) can recognize their own PD to support the validity of the PD in reporting the experience of pain. Moreover, we examined the association between their ability to recognize their own PD with their levels of pain intensity and disability and extent of psychosocial and somatic features.

OBJECTIVES: The objectives of this study were the following: to determine if fatigue is present in chronic low back pain (LBP) and chronic neck pain patients to a greater extent than in controls (nonpatients); to determine which variables are associated with the presence of fatigue; and to determine which of the above chronic pain patient (CPP) groups is more fatigued. To the authors' knowledge, this is the first such study in the literature.
DESIGN: Totals of 175 chronic LBP and 33 chronic neck pain patients completed the Multidimensional Fatigue Inventory (MFI), Neuropathic Pain Scale (NPS)

BACKGROUND: Increasing evidence suggests that people with chronic neck pain (CNP) may display altered biomechanics beyond the cervical spine. However, whether spinal alignment and mobility are associated with neck pain is not clarified.
OBJECTIVES: To investigate whether there is a significant association between neck pain intensity and sagittal spinal alignment and mobility in people with CNP, and to examine whether sagittal spinal alignment and mobility differ according to pain intensity.
DESIGN: A cross-sectional study.
METHOD: Forty-four women with CNP were included. The neck pain inten

The authors undertook a case-control study of chronic neck pain and whiplash injuries in nine states in the United States to determine whether whiplash injuries contributed significantly to the population of individuals with chronic neck and other spine pain. Four hundred nineteen patients and 246 controls were randomly enrolled. Patients were defined as individuals with chronic neck pain, and controls as those with chronic back pain. The two groups were surveyed for cause of chronic pain as well as demographic information. The two groups were compared using an exposure-odds ratio. Forty-five

OBJECTIVE: This study compares the sensitivity to change of the Neck Disability Index (NDI) and the Neck Bournemouth Questionnaire (NBQ) in patients with chronic uncomplicated neck pain.
METHODS: This prospective longitudinal study was completed in an outpatient physical therapy clinic. Subjects, with uncomplicated neck pain (no concurrent shoulder pain or nerve root symptoms) for more than a 3-month duration, participated in a 4-week course of therapy that included moist heat, neck exercises, and either mobilization or massage. Outcome measures included standardized response means (sensitivi

BACKGROUND: Neck pain is reported in more than 50% of migraine patients during migraine attacks and may be an important source to migraine pain.
OBJECTIVES: To investigate phenotypical differences between migraine patients with and without ictal neck pain in the interictal phase. Additionally, to prospectively examine the association between pericranial muscle tenderness and the impending migraine attack.
METHODS: Migraine patients (n = 100) and controls (n = 46) underwent a semi-structured interview and sensory testing interictally. Pericranial muscle tenderness was determined using total t

Given the challenges of chronic musculoskeletal pain and disability, establishing a clear prognosis in the acute stage has become increasingly recognized as a valuable approach to mitigate chronic problems. Neck pain represents a condition that is common, potentially disabling, and has a high rate of transition to chronic or persistent problems. As a field of research, prognosis in neck pain has stimulated several empirical primary research papers, and a number of systematic reviews. As part of the International Consensus on Neck (ICON) project, we sought to establish the general state of know
