A common occurrence in the neonatal intensive care unit (NICU) is metabolic acidosis. Sodium bicarbonate (SB) has been widely used, but there is insufficient evidence on how SB affects neonates in NICUs with metabolic acidosis. The worsening of intracellular acidosis, the impairment of myocardial function, fluctuations in cerebral blood flow, and intracranial hemorrhage are some of the unfavorable effects of SB treatment in neonates that have been documented in the literature. This study aimed to explore neonatologists' practices for using intravenous SB (ISB) in NICUs. A multi-country survey

PURPOSE: To survey current oxygen management, screening criteria, and methods for treating retinopathy of prematurity (ROP) in sub-Saharan Africa.
METHODS: An online survey was sent to ophthalmologists and neonatologists practicing in sub-Saharan Africa.
RESULTS: Ophthalmologists and neonatologists from 15 of 49 countries (31%) in sub-Saharan Africa responded. Neonatologists reported treating a median of 15 infants with supplemental oxygen per week but only had a median of 3.5 oxygen-measuring devices and a median of zero oxygen blenders in their units. Ophthalmologists reported screening a

OBJECTIVE: We undertook a survey of all practicing neonatologists in New England to determine their attitudes and practices regarding prenatal consultations for infants at the border of viability.
METHODS: A self-administered anonymous survey, mailed to every practicing neonatologist in the 6 Northeast states of Connecticut, Maine, Massachusetts, New Hampshire, Rhode Island, and Vermont, explored respondent attitudes and practices with respect to a hypothetical clinical scenario of a prenatal consultation for an infant at the border of viability.
RESULTS: Our final sample included 149 survey

Counseling a family confronted with the birth of a periviable neonate is one of the most difficult tasks that a neonatologist must perform. The neonatologist's goal is to facilitate an informed, collaborative decision about whether life-sustaining therapies are in the best interest of this baby. Neonatologists are trained to provide families with a detailed account of the morbidity and mortality data they believe are necessary to facilitate a truly informed decision. Yet these complicated and intensely emotional conversations require advanced communication and counseling skills that our curren

BACKGROUND: Centrally inserted central catheters (CICCs) are increasingly used in neonatal care. CICCs have garnered attention and adoption owing to their advantageous features. Therefore, achieving clinical competence in ultrasound-guided CICC insertion in term and preterm infants is of paramount importance for neonatologists. A safe clinical training program should include theoretical teaching and clinical practice, simulation and supervised CICC insertions.
METHODS: We planned a training program for neonatologists for ultrasound-guided CICCs placement at our level III neonatal intensive ca

BACKGROUND: CK care depends on pathways that connect risk recognition, evaluation, follow-up, and specialist input. Although CKD is often recognized later in life, CKD risk may originate at birth in infants born preterm or with low birth weight. These infants may encounter the first gap in the life-course CKD care pathway at neonatal intensive care unit (NICU) discharge, when early-life kidney risk should be linked to longitudinal follow-up. We aimed to describe post-discharge kidney follow-up practices for these infants in Japan and identify gaps among routine care, current evidence, and rece

The objective of this study was to determine the differences between neonatologists and pediatric cardiologists with regards to the initial assessment of neonatal heart murmur and to evaluate the role of echocardiography in this group of patients. During a period of 1 year, all neonates with heart murmur seen in pediatric cardiology consultation from neonatal intensive care units at the Children Hospital of Eastern Ontario and Ottawa General Hospital were included in this study. Neonates with heart murmur were initially evaluated clinically by a neonatologist and the most likely clinical diagn

Heart transplantation in infancy has become the fastest growing area in pediatric heart transplantation. Infant heart transplantation presents a profoundly complex decision-making situation for parents. There is a relationship between physician beliefs and their manner of presenting heart transplantation options to parents. The purpose of this study was (1) to determine the beliefs of physicians about neonatal heart transplantation, (2) to define their perceptions about their role in counseling parents about heart transplantation, and (3) to evaluate the differences of beliefs by physician spe

BACKGROUND: Despite evidence demonstrating clinical and cost benefits of transanastomotic tubes (TATs), following repair of congenital duodenal obstruction they are used in a minority of infants in the UK. Most infants are fed using parenteral nutrition (PN) (sometimes in combination with a TAT). This variation is unexplained by clinical or demographic factors. We aimed to understand why this is and the barriers to practice change.
METHODS: UK-based clinicians (surgeons, neonatologists, dietitians and specialist nurses) completed an online mixed methods survey. Open-ended replies were summari

The use of point of care ultrasound to assess cardiovascular function performed by a neonatologist who is not a cardiologist, is gaining interest in the neonatal intensive care unit (NICU). Clinical signs such as heart rate, blood pressure and capillary refill time provide limited insight into the adequacy of systemic blood flow and organ perfusion. Functional echocardiography (echo) enables real time evaluation of cardiac performance, identifying the nature of cardiovascular compromise, guiding therapeutic decisions and monitoring response to treatment. Application of functional echo in the N

OBJECTIVES: Functional echocardiography is a valuable tool in the neonatal intensive care unit, but training programs are not standardized. The aim was to report an functional echocardiography training program for neonatologists and to describe the agreement of their measurements with the pediatric cardiologist.
METHODS: Functional echocardiography training lasted 32h. After training program, the neonatologists performed functional echocardiography in the neonatal intensive care unit and were required to measure left cardiac chambers dimensions, left ventricle systolic function, right and lef

A two-step study of placentas is proposed if departments of morbid anatomy have none or limited feasibility. A gross study of the organ's constituents is conducted by neonatologists at maternal hospitals in the first step and in a specialized department of morbid anatomy in the second one. Placental manipulations are differentiated between the neonatologists and pathologists. Additional coefficients have been elaborated and proposed to be clinically introduced. The paper emphasizes the importance of imaging assessment of placental abnormalities at maternal hospitals. The diagnosis of placental

Aim: To describe the current clinical practices and attitudes of neonatologists towards paracetamol treatment of PDA in Australia (AU) and New Zealand (NZ). Method: A web-based survey of all neonatologists registered under the 2017 Australia New Zealand Neonatal Network (ANZNN) was conducted. Results: The response rate for the survey was 67%, (141/210). Of those respondents, 37% stated their unit had a written policy outlining how to treat patent ductus arteriosus (PDA). Of the written policies, 53% mentioned paracetamol treatment. The majority of the respondents (70%) have prescribed paraceta

As a result of the widespread use of phototherapy, the need for neonatal exchange transfusion has declined markedly over the last 25 years. However, in spite of this, we have discovered a marked disparity in the number of requests for units of blood suitable for exchange transfusion made by neonatal units of similar size. A retrospective audit was therefore performed to establish current practice. A questionnaire was sent to consultant neonatologists and an audit trail of all units of blood issued from our Transfusion Centre for exchange transfusion over an 18-month period was followed. A sign

The author summarizes the principles of clinical diagnosis, referring to experience assembled in an investigation comprising more than 35,000 neonates during past years. The submitted pattern could be a practical instruction for examination of the hip joints by paediatricians--neonatologists. The author emphasizes the fact that the diagnosis of complete dislocation unless irreponible is very easy, and being a clinically very obvious defect, it should not be missed. In other countries the trend predominates to ensure examination of the hip joints of neonates by paediatricians-neonatologists for

With the development of neonatal medicine, more and more extremely preterm infants have been treated. How to deal with hypotension is a big challenge for neonatologist in the process of diagnosis and treatment. The lack of uniformity in the definition of hypotension, challenges in measuring blood pressure accurately, and insufficient consistency between digital hypotension and hypoperfusion are the primary causes. How to check for hypotension and monitor blood pressure is thoroughly explained in the article. To give neonatologists a resource for the clinical management of hypotension in extrem

The ever-changing complexion of iatrogenic perinatal disease dictates the following: 1. Neonatologists and pediatric pathologists must be aware of the current spectrum of these lesions and ever alert to the appearance of new ones. 2. The neonatologist and pediatric pathologist within any given institution should be in regular, systematic communication with each other concerning lesions of this type. 3. There must be a national network for rapid dissemination or exchange of information among institutions regarding these lesions. The Study Group for the Complications of Perinatal Care (SGCPC), e

BACKGROUND: Majority of preterm infants do well with continuous positive airway pressure (CPAP) as the sole respiratory management; but some require endotracheal intubation and surfactant administration. While intubation is needed predominantly in extremely preterm infants (<28 weeks); some of the more mature preterm infants also require it. Currently, there are no clear guidelines regarding indications for endotracheal intubation in such infants.
AIMS: To understand the current practice regarding "criteria for intubation" in moderate to late preterm infants with respiratory distress.
