Gelfoam embolization of the kidneys for treatment of malignant hypertension.

Some chronic renal failure patients maintained on dialysis have uncontrollable hypertension. Those with elevated renin levels require bilateral nephrectomies prior to kidney transplant to avoid nephrosclerosis. The morbidity and mortality from surgical nephrectomies are high. In 2 such patients we embolized the renal arteries with gelfoam and successfully occluded all the major vessels. One patient became normotensive. The second remained hypertensive and had increased renin levels, probably on the basis of ischemia. Subsequent surgical nephrectomies demonstrated completely occluded segmental

Renal biopsy in pregnancies complicated by undetermined renal disease.

BACKGROUND: The aim of this retrospective study was to verify the role of renal biopsy in pregnancies complicated by renal dysfunction. METHODS: A series of 15 percutaneous renal biopsies performed in 15 pregnant women with renal disease presenting during pregnancy over the past 10 years (1990-1999) were reviewed. RESULTS: All the patients underwent renal biopsy before 30 weeks of gestation. The indications for renal biopsy were renal dysfunction of unknown cause or symptomatic nephrotic syndrome (NS). Patients with toxemia were excluded. Eight women had lupus nephritis, including five with

Renal damage in hypertension.

The kidney can be considered as both culprit and victim in the hypertensive process. Deranged renal function contributes to the development of arterial hypertension and of secondary vascular damage at the glomerular and arteriolar level and accounts for the development of progressive nephrosclerosis. The most common alteration of renal function observed in humans from the early stages of essential hypertension is the presence of renal vasoconstriction. This can be accompanied by hyperuricaemia and increased urinary excretion of enzymes such as N-acetyl-beta-glucosaminidase and proteins such as

[Comparison of the renal ultrasound-furosemide test with the isotope nephrogram. Possible clue to the retention phenomenon?].

In 23 patients with essential hypertension renographic and sonographic examination of the kidneys was compared. No correlation could be observed for functional parameters and urinary excretion between the two different methods. Correlations have only been detected between renographic retentions and furosemide-conditioned swelling of the kidneys in sonography: patients with severe retention showed a statistically significant stronger response to furosemide. The increased retention in the renal pelvis could be responsible for alterations in sonographic findings. This study provides new data on t

[Morphogenesis of chronic pyelonephritis (electron microscopic study)].

Examinations of 51 rabbits and 86 patients with chronic pyelonephritis showed the morphogenesis of chronic pyelonephritis to be due to the effect of certain factors. One of them is the capillary-parenchymatous block resulting from the outgrowth of the connective tissue between the tubules and peritubular capillaries. Glomerular sclerosis occurs both periglomerularly (growth of the connective tissue into the capsule) and intraglomerularly (tropocollagen synthesis by mesangiaal cells). Nephrosclerosis combines diffuse sclerosis (the consequence of increasing hypoxia) and focal sclerosis (the con

A case of hypertensive emergency with alveolar hemorrhage and thrombotic microangiopathy.

A 28-year-old woman with a 5-year history of untreated hypertension was admitted for respiratory distress, hemoptysis, and retinopathy. Computed tomography showed diffuse plaques in both lung fields. Acute kidney injury, hemolytic anemia, and thrombocytopenia were noted. Kidney biopsy showed thrombosis with fibrinoid necrosis and edematous intimal thickening and luminal narrowing of the small renal artery, indicating thrombotic microangiopathy; the majority of glomeruli were collapsed. After 8 weeks of treatment with antihypertensive drugs, serum creatinine decreased to 1.0 mg/dL, and the pati

Pathophysiology of progressive renal failure.

Progressive renal failure is conditioned by a number of factors, including type of renal disease, sex, level of arterial pressure, insulin deficiency and/or hyperglycemia in diabetes mellitus, and dietary protein intake. Elevated arterial pressure may cause a progressive renal disease, hypertensive nephrosclerosis, and accelerate the rate of progression of all other chronic renal disease. Hypertension contributes to progressive renal failure by inducing myointimal hyperplasia of arcuate and afferent arterioles, causing glomerular ischemia, and by increasing pressure in the glomerular capillari

Lower Podocyte Number per Glomerulus Associates With Progressive CKD.

INTRODUCTION: Podocyte depletion may play an important role in chronic kidney disease (CKD); however, longitudinal clinical studies are lacking in populations without obesity-related glomerulopathy. METHODS: Patients studied underwent radical nephrectomy for tumors between 2000 and 2021 and had < 10% interstitial fibrosis and tubular atrophy (IFTA) on histology. Cases who developed progressive CKD were identified by the onset of dialysis, kidney transplantation, sustained estimated glomerular filtration rate (eGFR) < 10 ml/min per 1.73 m2, or sustained 30% eGFR decline after the postnephrecto

Deep learning-based histopathological assessment of tubulo-interstitial injury in chronic kidney diseases.

BACKGROUND: Chronic kidney disease (CKD) causes progressive and irreversible damage to the kidneys. Renal biopsies are essential for diagnosing the etiology and prognosis of CKD, while accurate quantification of tubulo-interstitial injuries from whole slide images (WSIs) of renal biopsy specimens is challenging with visual inspection alone. METHODS: We develop a deep learning-based method named DLRS to quantify interstitial fibrosis and inflammatory cell infiltration as tubulo-interstitial injury scores, from WSIs of renal biopsy specimens. DLRS segments WSIs into non-tissue areas, glomeruli,

Renal Histology Findings in Malignant Hypertension, a Systematic Review.

BACKGROUND: Malignant hypertension (MH) causes acute target-organ injury (eyes, brain, heart, and kidneys). As kidney biopsy is seldom performed because of bleeding risk, the link between MH and renal histopathology-especially thrombotic microangiopathy-remains incompletely characterized. METHODS: We conducted a systematic review following PRISMA guidelines to analyze renal histological findings in adult patients with MH. Studies were included if they provided original data on MH and kidney histology in adult patients with MH. A comprehensive search across PubMed, Embase, Cochrane Library, an

Candida lusitaniae Peritonitis in a Chronic Ambulatory Peritoneal Dialysis Patient.

This is a case report of a 69 year-old African American female with end-stage renal disease secondary to hypertensive nephrosclerosis on chronic ambulatory peritoneal dialysis (CAPD) for five years. Her past was uneventful. She presented with symptoms and signs of acute peritonitis related to CAPD. There was initial response to antibiotics and culture was negative. The condition of the patient deteriorated despite the antibiotic. The peritoneal catheter was removed after five days of starting the antibiotic. The patient died at day seven of presentation due to septic shock and the culture even

[The role of arterial hypertension in developement of chronic renal failure].

The link between the kidney and hypertension has been considered a villain-victim relationship because of the potential two-way causality between high blood pressure (BP) and chronic kidney disease (CKD). Arterial hypertension (AH) per se, but also together with diabetes mellitus, is the most important cause of CKD and end-stage renal disease (ESRD) in the developed world. Pathophysiologicaly, the increment in systemic BP leads to the rise in glomerular pressure. Glomerular hypertension results in glomerular capillary wall stretch, endothelial damage and a rise in protein glomerular filtration

Associations of Causes of Chronic Kidney Disease with Disease Progression and Mortality: Insights from the Fukuoka Kidney Disease Registry Study.

INTRODUCTION: The Kidney Disease: Improving Global Outcomes guidelines recognize the importance of causes of chronic kidney disease (CKD), glomerular filtration rate, and albuminuria as predictors of kidney outcome and prognosis. However, compared with biopsy-proven causes, there has been limited research regarding the relationship between clinically diagnosed causes of CKD and patient prognosis. METHODS: We examined 3,119 patients with non-dialysis-dependent CKD who participated in the Fukuoka Kidney disease Registry Study, a multicenter prospective cohort study. Patients were divided into s

Histopathological findings in elderly patients.

INTRODUCTION: The elderly population has significantly increased worldwide and recent studies have evidenced a 10-year increase in Brazilian life expectancy. Similarly to other comorbidities, glomerular diseases are also observed in the elderly, and, in that age group, kidney biopsy emerges as a fundamental diagnostic tool to help disease management, preventing unnecessary therapies. OBJECTIVE: To establish the frequency of histological diagnoses in the elderly undergoing kidney biopsy, with an emphasis on glomerulopathies (GPs), at two Brazilian universities. METHODS: Retrospective assessme

[Hypertension and renal disease].

Renal mechanisms, in particular the renin-angiotensin system and renal salt handling, are of major importance in blood pressure regulation. Co-existence of hypertension and decreased renal function may be due to nephrosclerosis secondary to hypertension, or primary renal disease with secondary hypertension. Mild degrees of chronic kidney disease (CKD) can be detected in around 10% of the population, and detection is important as CKD is an important risk factor for atherosclerotic cardiovascular disease. Conversely, heart failure may cause an impairment of renal function. In chronic progressive

Spontaneous hypertension and its sequelae in woolly monkeys (Lagothrix lagotricha).

Arteriolar nephrosclerosis was observed at necropsy in 26 of 38 woolly monkeys (Lagothrix lagotricha). This lesion is the earliest histologic change associated with hypertension in humans. Seventeen of the monkeys had died of congestive heart failure, renal failure or acute cardiovascular accident, complications similar to those seen in human hypertension. All monkeys known to be over 4 years of age were affected. Direct blood pressure measurements in nine otherwise healthy woolly monkeys revealed systolic pressures of 194 +/- 20 mmHg. Our physiologic, clinical and pathologic studies suggest t

Uterine, placental, renal biopsies, and fibrin deposits in toxemia of pregnancy.

Histopathologic lesions of toxemia of pregnancy found in 46 first trimester-induced abortions and 82 women with kidney biopsies are reviewed and discussed. Decidual arteriolar vasospasm and degeneration, syncytial trophoblastic degeneration, and chorionic villous fibrin deposits characterize the early toxemic changes of the uterus and placenta. Pure toxemic glomerular, arteriolar, tubular, and juxtaglomerular alterations were present in about 40% of the kidney biopsies. Others had arteriolar nephrosclerosis, chronic glomerulonephritis or both. Two women had renovascular hypertension. Excessive
