Embryonal and pineal tumours.

Embryonal and pineal tumours represent a diverse group of central nervous system (CNS) neoplasms. While many of the small round blue cell tumours that make up the embryonal neoplasms share similar histologic qualities, there are several morphologic and cytologic characteristics that are useful in distinguishing different tumour types. Similarly, pineal parenchymal tumours represent clinically diverse tumours, ranging from benign to overtly malignant. The most recent iteration of the World Health Organization Classification of CNS Tumours expanded greatly on the significance of molecular altera

Comparative Pathology of the Peripheral Nervous System.

The peripheral nervous system (PNS) relays messages between the central nervous system (brain and spinal cord) and the body. Despite this critical role and widespread distribution, the PNS is often overlooked when investigating disease in diagnostic and experimental pathology. This review highlights key features of neuroanatomy and physiology of the somatic and autonomic PNS, and appropriate PNS sampling and processing techniques. The review considers major classes of PNS lesions including neuronopathy, axonopathy, and myelinopathy, and major categories of PNS disease including toxic, metaboli

Epidermal naevus and segmental hypermelanosis associated with an intraspinal mass: overlap between different mosaic neuroectodermal syndromes.

UNLABELLED: The epidermal naevus syndrome is a neurocutaneous syndrome characterised by the association of epidermal naevi with central nervous system, skeletal, ocular or cardiovascular abnormalities. We report on a 1.75-year-old boy who presented with spastic diparesis, a partial paresis of the left forearm and macrocephaly. He had a large epidermal naevus along the cervical spine and a segmental hypermelanosis. MRI studies revealed a large intraspinal mass extending from the lower cervical to the upper thoracic spine. The condition of our patient demonstrates the overlap of the epidermal na

Papillary tumor of the pineal region.

Tumors of the pineal region are rare in adulthood, accounting for approximately 1% of intracranial neoplasms in this age range. Papillary tumor of the pineal region (PTPR) was first described by Jouvet et al. in 2003. In 2007, PTPR was included by the World Health Organization as a distinct entity in their new classification of central nervous system tumors. We report a case of PTPR, and undertake a literature review of this rare neuroepithelial neoplasm. Patients with PTPR need long-term follow up, and new cases should be well documented so that we can gain a better understanding of this neop

The neuro-glial coagulonome: the thrombin receptor and coagulation pathways as major players in neurological diseases.

The neuro-glial interface extends far beyond mechanical support alone and includes interactions throu-gh coagulation cascade proteins. Here, we systematically review the evidence indicating that synaptic and node of Ranvier glia cell components modulate synaptic transmission and axonal conduction by a coagulation cascade protein system, leading us to propose the concept of the neuro-glial coagulonome. In the peripheral nervous system, the main thrombin receptor protease activated receptor 1 (PAR1) is located on the Schwann microvilli at the node of Ranvier and at the neuromuscular junction. PA

PHOX2B is a Sensitive and Specific Marker for the Histopathological Diagnosis of Pheochromocytoma and Paraganglioma.

Pheochromocytomas (PCCs) and paragangliomas (PGLs) are non-epithelial neuroendocrine neoplasms originating from the adrenal medulla and paraganglion of the sympathetic and parasympathetic nervous system, respectively. PCCs and PGLs show histological similarities with other epithelial neuroendocrine neoplasms and olfactory neuroblastomas (ONBs), and the differential diagnosis of PGLs is particularly difficult. Therefore, we compared the sensitivity of PHOX2A, PHOX2B, and tyrosine hydroxylase (TH) in the histopathological diagnosis of PCCs and PGLs immunohistochemically using the tissue microarr

Spinal low-grade neoplasms with extensive leptomeningeal dissemination in children.

OBJECTIVE: The aim of this study is to re-appraise paediatric leptomeningeal disseminated spinal low-grade neoplasms putting forward the hypothesis that these tumours may represent a possible distinctive clinico-pathological entity. CASE REPORT: Three children affected by these rare neoplasms are reported, 2 boys and 1 girl aged 40 months, 7 and 12 years respectively. RESULTS: Despite treatment, 2 died at 9 months and 9 years, while 1 has survived with stable disease at 24 months after diagnosis. Histologically, these tumours shared the generic histological appearance of low-grade neuroectod

Image-guided surgical techniques for infections and trauma of the central nervous system.

The value of image-guided stereotactic systems is directly dependent on the ease and speed of their use. In the past, most stereotactic techniques were complicated and timely to set up; thus, they were used exclusively for either resecting neoplasms or for neurologic function. However, current systems equipped with advanced registration techniques are much simpler and faster to employ, and indications for their use are rapidly increasing. We describe an advanced image-guided navigation system and provide examples of its successful use in neurosurgical treatment of central nervous system infect

Central nervous system metastases from breast carcinoma. A clinical and laboratorial study in 47 patients.

In this retrospective study, 47 patients with clinical diagnosis of central nervous system metastases of breast cancer were evaluated by computerized tomography (CT), magnetic resonance imaging (MRI) and cerebrospinal fluid (CSF) examination. The patients were divided in 2 groups: 1, without leptomeningeal neoplasm and 2, with leptomeningeal neoplasm. In the group 2, the time interval between the primary disease and the central nervous system metastasis as well as the survival time were shorter than in group 1 (40 and 4.3 months in group 2 versus 57 and 10 months respectively, in group 1). A rare MRI finding of NF-1: perineural arachnoidal gliomatosis.

Optic pathway gliomas are the most common central nervous system neoplasms in patients with neurofibromatosis type 1. Perineural arachnoidal gliomatosis is a rare and distinctive growth pattern of optic nerve glioma, in which the tumor infiltrates through the pia mater and pre-dominantly involves the subarachnoid space around the optic nerve. Here, we report an 8-year-old girl with perineural arachnoidal gliomatosis associated with neurofibromatosis type 1.

Association of breast cancer with meningioma.

We report a case of meningioma subsequently developed in a patient with bilateral breast carcinoma, which was originally thought to be single brain metastases. A brief review of the literature is presented with emphasis on the unique association between the two neoplasms, which suggests a possible hormonal relationship. The knowledge of this association is important in the differential diagnosis of patients with breast cancer who develop central nervous system manifestations.

Tumefactive Primary Central Nervous System Vasculitis: Dynamic Susceptibility Contrast Perfusion-Weighted Magnetic Resonance Imaging Findings With Histological Correlation.

PURPOSE: Primary central nervous system vasculitis (PCNSV) is a rare inflammatory disease affecting the central nervous system. In some cases, it presents with large, solitary lesion with extensive mass effect that mimic intracranial neoplasms. This condition results in a diagnostic confusion for neuroradiologists because the differentiation is almost impossible on conventional MRI sequences. The aim of this study is to reveal the significance of dynamic susceptibility contrast (DSC) perfusion-weighted imaging in differentiating of tumefactive PCNSV (t-PCNSV) lesions from intracranial neoplasm

Overactive bladder as a dysfunction of the autonomic nervous system - A narrative review.

Overactive bladder (OAB) is a common condition with increasing prevalence. However, its aetiology is not fully understood. The main symptoms are sudden urge to urinate, with or without incontinence; increased daytime frequency; and nocturia. OAB-like symptoms can be caused by a number of diseases, including urinary tract infections, neurologic and metabolic diseases, and neoplasms. The final diagnosis is made after excluding all the abovementioned factors. Autonomic nervous system (ANS) dysfunction has also been identified as a possible cause of so-called 'idiopathic OAB'. In order to check th

Multiple Sclerosis and the Choroid Plexus: Emerging Concepts of Disease Immunopathophysiology.

BACKGROUND: The coexistence of multiple sclerosis and intracranial neoplasms is very rare, and whether this occurrence can be explained by a causal relationship or by coincidence remains a matter of debate. Possible roles of the choroid plexus as a site of tumor cell invasion and lymphocyte infiltration into the central nervous system have been hypothesized in recent studies. METHODS: We describe a 13-year-old boy with concurrent multiple sclerosis and choroid plexus papilloma, then review the published literature with a focus on the pathophysiologic mechanisms of neuroinflammation in multipl

Neoplastic cell palisades and perinecrotic palisades in veterinary neuropathology.

Neoplastic cell palisades (also referred to as palisades or primary palisades) and perinecrotic palisades (also referred to as secondary palisades or pseudopalisades) appear in distinct sets of neoplasms. Neoplastic cell palisades consist of elongate neoplastic cells that form parallel rows, whereas perinecrotic palisades are oriented transversely to the margins of necrotic foci. Each of these morphologic patterns is formed by different mechanisms. Palisades are associated with intrinsic morphologic features of a neoplasm, whereas perinecrotic palisades reflect a response of neoplastic cells t

Predictive ability of Lukes-Collins classification for immunologic phenotypes of childhood non-Hodgkin lymphoma: an institutional series and literature review.

Tissues from 22 children with non-Hodgkin lymphoma (NHL) were studied pathologically and immunologically. Most children were noted to have marked (B- or T-cell) neoplasms and the Lukes-Collins classification was predictive of immunologic phenotype in cases where markers were present. Our series and a review of the literature demonstrates that most abdominal NHL are B-cell in origin and are often small noncleaved follicular center cell lymphoma (Burkitt type). Most mediastinal primary lesions are T-cell in origin and of convoluted cell morphology. A few neoplasms (often peripheral nodal) lack t

Swept Source Optical Coherence Tomography Documenting Optic Nerve Involvement in an Aggressive T-Cell Lymphoma.

Peripheral T-cell lymphomas (PTCLs) are a heterogeneous group of aggressive neoplasms. The involvement of ocular structures in haematological neoplasms is rare and usually associated with central nervous system involvement, which may occur as a result of orbital invasion and optic nerve infiltration. In this case report, we describe ocular findings using the novel swept source optical coherence tomography (SS-OCT) in a case of aggressive T-cell lymphoma. SS-OCT has faster scanning speed, deeper tissue penetration due to its longer wavelength laser of 1050 nm and wider scanning areas. In the pr

P14.02 Proton radiation therapy combined with immunomodulatory drugs for central nervous system invasive multiple myeloma: case report and review of the literature

Abstract Central nervous system invasion of multiple myeloma is a rare extra-medullary form of the disease, presenting as intraparenchymal lesions, leptomeningeal disease, or via direct dural invasion. Survival of leptomeningeal myelomatosis in historical series is considered to be extremely poor, but seems to be improving in the era of novel therapy, namely proteosome inhibitors and immunomodulatory class drugs. Here we present the case of a 48 year-old male with isolated central nervous system relapse of multiple myeloma who had multiple rapidly progressive intraparenchymal myeloma lesions,

Intradural Carcinoid Tumor Found in a Patient with No History of Cancer.

BACKGROUND: Carcinoid tumors are rare neoplasms that often arise from the gastrointestinal or respiratory tracts. They often metastasize to bone tissue and pancreatic and hepatic sites. The central nervous system and most specifically the spinal cord are rarely involved. Primary carcinoid tumors of the central nervous system are even rarer. CASE DESCRIPTION: A 58-year-old man presented with progressive bilateral lower extremity weakness and a negative history of cancer. Imaging revealed an intradural mass at the L1 and L2 spinal levels.
