Neurocutaneous Diseases: Diagnosis, Management, and Treatment.

Neurocutaneous disorders, also known as phakomatoses, are congenital and acquired syndromes resulting in simultaneous neurologic and cutaneous involvement. In several of these conditions, the genetic phenomenon is understood, providing a pivotal role in the development of therapeutic options. This review encompasses the discussion of the genetic and clinical involvement of neurocutaneous disorders, and examines clinical management and treatment options. With the current advances in genetics, the role of precision medicine and targeted therapy play a substantial role in addressing the managemen

Extensive metameric involvement in Cobb syndrome presenting as compressive myelopathy.

Cobb Syndrome, or cutaneomeningospinal angiomatosis, is a rare non-hereditary neurocutaneous disorder characterised by spinal arteriovenous malformations (AVMs) and cutaneous vascular lesions within the same metamere. We present a case of Cobb syndrome manifesting as progressive paraparesis due to dorsal compressive myelopathy. Unlike most reported cases, which primarily involve the thoracic spine, this case exhibited extensive vascular malformations affecting all spinal segments, making it a rare presentation. Diagnosis was established through MRI and angiographic studies, revealing vertebral

Double phakomatosis; neurofibromatosis type-1 and tuberous sclerosis.

Neurocutaneous syndromes represent some of the most common inherited disorders of the nervous system. Neurofibromatosis type-1 (NF-1) and tuberous sclerosis are well described. Yet, the presentation of both syndromes in the same patient is quite rare. We performed a thorough review of the literature of such double phakomatosis including pattern of inheritance. Eleven cases were reported in the literature. In addition we report a young patient who presented with clinical picture suggestive of both NF-1 and tuberous sclerosis, and present a radiographic and histopathological description of the c

Ophthalmologic abnormalities in encephalocraniocutaneous lipomatosis.

Encephalocraniocutaneous lipomatosis (ECCL) is a sporadically occurring disorder that belongs to the group of neurocutaneous syndromes. Important characteristics of the case we present are: intracranial lipomas, a skull hamartoma, bilateral lipodermoids and jaw tumors (ossifying fibromas and compound odontomas). We propose four minimal criteria for the diagnosis of ECCL and review the ocular abnormalities reported to date.

Cutis marmorata telangiectatica congenita: an unusual presentation with monoatrophy in two Chinese children.

Cutis marmorata telangiectatica congenita (CMCT) is a rare cutaneous vascular anomaly. Two children with monoatrophy of one limb referred for neurological assessment were found to have a segmental form of CMTC. The paediatrician and paediatric neurologist should be alerted to this rare disease in children referred for cutaneous lesion with associated limb asymmetries as distinguished from other neurocutaneous syndromes.

Sturge-Weber Syndrome With Leptomeningeal Angiomatosis and Middle Cerebral Artery Narrowing Treated With Intravenous Tissue Plasminogen Activator (IV tPA): A Case Report.

Sturge-Weber syndrome (SWS) is a rare and congenitally acquired neurocutaneous disorder characterized by port wine stain, leptomeningeal angiomas with or without glaucoma. Clinical presentation includes focal motor epilepsy as the first symptom. They may also develop migraines, cognitive impairment, blindness secondary to glaucoma, and transient stroke-like episodes. We report a case of a 60-year-old patient with SWS with known leptomeningeal angiomatosis presenting with aphasia in the setting of left middle cerebral artery narrowing who received intravenous tissue plasminogen activator (IV tP

[Encephalocraniocutaneous lipomatosis (Haberland syndrome) with bilateral ocular damage].

Encephalocraniocutaneous lipomatosis, or Haberland syndrome, is an unusual form of neurocutaneous syndrome. Clinically, this syndrome is characterized by unilateral lipomatous hamartomas on the scalp and face, ocular damage and ipsilateral malformations of the central nervous system. First described by Haberland and Perou in 1970, only about 40 cases have been reported to date. The differential diagnoses with other neurocutaneous mosaicisms such as Delleman, Goltz, Goldenhar or Proteus syndrome may entail some degree of difficulty due to overlapping features. We report a case of Haberland synd

Encephalocraniocutaneous lipomatosis: a new case report and review of the literature.

Encephalocraniocutaneous lipomatosis is a rare neurocutaneous syndrome characterized by lipomatous hamartomas ranging in size from a few millimeters to several centimeters and affecting the head. Ocular anomalies and a variable degree of mental retardation with or without convulsions are usually observed. This disorder should be distinguished from other mosaic neurocutaneous phenotypes such as Proteus syndrome, oculocerebrocutaneous syndrome, and nevus sebaceous syndrome. We report the clinicopathologic findings of a 4-year-old Brazilian girl affected by this syndrome and review the literature

[Encephalocraniocutaneous lipomatosis. A non-hereditary mosaic phenotype].

Encephalocraniocutaneous lipomatosis is a distinct neurocutaneous syndrome characterized by an extensive fatty tissue nevus of the scalp, protuberances of the cranial bones, lipodermoid of the conjunctiva, multiple intercranial lipomas, and porencephaly. A further case of this multisystem birth defect is reported. The patchy arrangement of lesions that is usually unilateral suggests a mosaic phenotype. The clinical criteria to distinguish this disorder from other mosaic neurocutaneous phenotypes, such as Schimmelpenning syndrome, Proteus syndrome, or Delleman syndrome, are outlined. To explain

Cobb syndrome: an interdisciplinary approach between neurosurgery and dermatology - case report and review of the literature.

Cobb syndrome is rare in the international literature. It is characterized by segmental cutaneous and spinal vascular malformations within the same metamere. We present the first documented case in Peru. We present a case of a patient with paraparesia and congenital violaceous-red cutaneous patch in a metameric distribution. Lumbar spine magnetic resonance imaging revealed spinal canal involvement at T8 level, prompting decompressive laminectomy and lesion biopsy. Histopathological analysis demonstrated dermal capillary proliferation and mild polymorphonuclear infiltrate. Adjuvant treatment wa

Facies: the value of an old diagnostic tip in pediatric dermatology.

The authors examine what is meant as facies in medicine. After an introduction to the word's understanding, they move on listing some dermatological conditions associated to peculiar facies in the child. Starting from atopic dermatitis, the authors develop an algorithm including Genodermatoses, Neurocutaneous Disorders, and Multiple Congenital Anomalies Syndromes. They emphasize the distinctive features of the lesions on the face and their fundamental diagnostic importance.

[Familial syndrome combining short stature, microcephaly, mental deficiency, seizures, hearing loss, and skin lesions. A new syndrome].

We report the observations of three sisters with the same autosomal recessive syndrome characterized by growth retardation, microcephaly, mental deficiency, seizures, sensorineural hearing loss, and skin lesions. The congenital nature of these symptoms was confirmed by their high prevalence among other family members. This syndrome is one of the many neurocutaneous syndromes and does not seem to fit any of the previously published descriptions.

An Intracardiac Mass in a Pregnant Woman Presenting With Ventricular Tachycardia.

A pregnant patient presented with symptomatic ventricular tachycardia. Echocardiography revealed a large intramyocardial mass. Surgical resection was attempted in conjunction with cryoablation of the surrounding myocardial tissue. Histologic examination of the resected mass revealed cardiac neurofibroma. To the best of our knowledge, this is the first report of cardiac neurofibroma in a pregnant patient in the absence of any neurocutaneous syndromes such as neurofibromatosis.

Neurofibromatoses.

The studies of familial tumor predisposition syndromes have contributed immensely to our understanding of oncogenesis. Neurofibromatosis 1, neurofibromatosis 2 and schwannomatosis are inherited autosomal dominant neurocutaneous disorders with complete penetrance. They are clinically and genetically distinct and considerable knowledge has been gathered about their pathogenesis. In this chapter, the genetics, molecular mechanism of disease, as well as clinical features, diagnosis and treatment are discussed.

Neuroimaging in infants and children in select neurocutaneous disorders.

The role of neuroimaging in neurocutaneous disorders is an evolving field. Research can be inconsistent and inconclusive, leading to divergent practice for some disorders. This study provides an overview of the current role of magnetic resonance imaging (MRI) of the brain in select neurocutaneous disorders, namely Sturge-Weber syndrome, congenital melanocytic naevus syndrome, neurofibromatosis type 1, tuberous sclerosis complex, incontinentia pigmenti and basal cell naevus syndrome. Future research assessing new targeted treatments and novel MRI techniques may change current practice.

Neurocutaneous Disorders.

PURPOSE OF REVIEW: This article presents an up-to-date summary of the genetic etiology, diagnostic criteria, clinical features, and current management recommendations for the most common neurocutaneous disorders encountered in clinical adult and pediatric neurology practices. RECENT FINDINGS: The phakomatoses are a phenotypically and genetically diverse group of multisystem disorders that primarily affect the skin and central nervous system. A greater understanding of the genetic and biological underpinnings of numerous neurocutaneous disorders has led to better clinical characterization, mor

Rare pulmonary manifestations of tuberous sclerosis in children.

Tuberous sclerosis is one of the rare neurocutaneous syndromes and may involve organs other than skin and brain. Pulmonary involvement is uncommon and predominantly affects women of childbearing age. Those affected present with dyspnea or spontaneous pneumothorax from cystic or fibrous degeneration of the lung. Males are affected very rarely. This paper describes another manifestation of pulmonary involvement in tuberous sclerosis, namely, recurrent intrapleural cysts in a young boy.
