[Neuroendoscopic surgery for intra- and para-ventricular tumors:impacts on biopsy and hydrocephalus treatment].

In this report, the authors retrospectively review and discuss their results for neuroendoscopic surgery for intra- and para-ventricular tumors. This study included 28 patients who were admitted to our hospital for intra- or para-ventricular tumors between January 2005 and March 2013. There were 17 males and 11 females, and their age varied widely from 1 to 75 years. Using a neuroendoscopic technique, tumor biopsy was attempted in all 28 patients. Biopsy was possible in 25 patients(89.3%)but not in the other 3(10.7%)because of hypervascularity or tumor bleeding. Of these 25 patients, pathologi

Metastatic pineal tumors treated by neuroendoscopic surgery--two case reports.

Two patients presented with metastatic pineal tumors. A 69-year-old man had gait disturbance, dementia, and urinary incontinence but no history of previous malignancy. Magnetic resonance imaging of the brain revealed a 23-mm tumor in the pineal region and obstructive hydrocephalus. A 37-year-old man had been treated for thyroid cancer. He presented with vomiting and consciousness disturbance. Brain magnetic resonance imaging revealed a 28-mm pineal tumor associated with intratumoral hemorrhage and accompanying obstructive hydrocephalus. Both patients underwent neuroendoscopic biopsy and third

Primary midbrain cystic germinoma mimicking glioma: a case with neuroendoscopic biopsy.

Intracranial germinoma arising primarily in the midbrain is extremely rare. We present the first reported case of cystic midbrain germinoma that lacked evident solid components and mimicked a midbrain glioma. In a 22-year-old man with headache and diplopia, magnetic resonance imaging showed a ring-enhancing lesion in the midbrain. The preoperative diagnosis included brain stem glioma, metastasis, and neuroepithelial cyst. A neuroendoscopic biopsy specimen provided a histologic diagnosis of germinoma. The patient responded well to chemotherapy and radiotherapy. The case illustrates the diagnost

Virtual simulation of neuroendoscopic procedures: early clinical experience with ventricular lesions.

BACKGROUND: Virtual endoscopy (VE) is a new and promising imaging technology. Applied to neuroendoscopy it allows preoperative simulation of a procedure and evaluation of the individual intraventricular anatomy in selected cases. Along with neuronavigation and real time intraoperative imaging, VE is expected to improve the safety and efficacy of neuroendoscopic procedures. PATIENTS AND METHODS: Between April 2003 and February 2004 VE simulation was performed in 13 randomly selected patients subjected to endoscopic procedures. Pathological entities included 4 cases with aqueduct stenosis, 4 wi

[Neuroendoscopic placement of the reservoir in an elderly patient with recurrenced craniopharyngioma: case report].

We report a case of recurrent craniopharyngioma in the third ventricle with obstructive hydrocephalus, which was successfully treated by placement of the Ommaya reservoir by neuroendoscopic procedure. A 72-year-old male with disorientation and gait disturbance was admitted to our hospital. He had been suffering chronic heart failure and arrhythmia due to mitral valve insufficiency, and panhypopituitarism after the first craniotomy for craniopharyngioma. MRI demonstrated obstructive hydrocephalus at the foramen of Monro by the cystic tumor. Cyst decompression and placement of Ommaya reservoir w

Microsurgical anatomy of the cerebral aqueduct and periaqueductal region: implications for microscopic and neuroendoscopic approaches.

PURPOSE: The cerebral aqueduct (CA) is the narrowest part of the ventricular system, connecting the third and fourth ventricles. This structure is encircled by important neuroanatomical features, and with the increasing application of neuroendoscopic techniques, a comprehensive understanding of the anatomy of the CA is essential. This study aims to provide a comprehensive, multi-perspective anatomical analysis of the cerebral aqueduct and its surrounding structures, integrating microsurgical dissections with endoscopic anatomical correlations to support safer surgical planning. METHODS: Three

Clinical application of reserved gastric tube in neuroendoscopic endonasal surgery for pituitary tumor.

BACKGROUND: The neuroendoscopic approach has the advantages of a clear operative field, convenient tumor removal, and less damage, and is the development direction of modern neurosurgery. At present, transnasal surgery for sphenoidal pituitary tumor is widely used. But it has been found in clinical practice that some patients with this type of surgery may experience post-operative nausea and vomiting and other discomforts. AIM: To explore the effect of reserved gastric tube application in the neuroendoscopic endonasal resection of pituitary tumors. METHODS: A total of 60 patients who underwe

Experience in endoscope choice for neuroendoscopic lavage for intraventricular hemorrhage of prematurity: a systematic review.

OBJECTIVE: Intraventricular hemorrhage (IVH) of prematurity occurs in 20-38% of infants born < 28 weeks gestational age and 15% of infants born in 28-32 weeks gestational age. Treatment has evolved from conservative management and CSF diversion of temporizing and shunting procedures to include strategies aimed at primarily clearing intraventricular blood products. Neuroendoscopic lavage (NEL) aims to decrease the intraventricular blood burden under the same anesthetic as temporizing CSF diversion measures in cases of hydrocephalus from IVH of prematurity. Given the variety of neuroendoscopes,

Investigating the results of neuroendoscopic surgery in the treatment of pituitary adenoma and leptin gene expression.

Neuroendoscopic surgery of pituitary adenoma has been one of the technologies with rapid progress in neurosurgery in this decade. This method has known advantages and limitations. This study aims to investigate the results of pituitary adenoma treatment using the neuroendoscopy technique in a group of patients. Also, the level of leptin gene expression (LEP), which is produced exclusively in the pituitary gland, was measured for further evaluation. For this purpose, 26 patients who were diagnosed with pituitary adenoma and underwent endoscopic surgery in the hospital between 2018-2022, in term

Unilateral Hydrocephalus Due to Lateral Ventricle Colloid Cyst Treated by Neuroendoscopic Approach.

Colloid cysts (CCs) are rare brain tumors that cause nonspecific neurological signs associated with acute or chronic increased intracranial pressure. They are usually located in the third ventricle and rarely in the lateral ventricle. This is a report of an unusual case of CC located in the lateral ventricle. A 36-year-old male patient presented a story of progressive holocranial headache that would get worse with head mobilization and cough. Radiological analysis demonstrated enlargement of the right lateral ventricle due to a cyst blocking the right foramen of Monro. The patient underwent en

MR Navigation and Tractography-Assisted Transcranial Neuroendoscopic Aspiration of Pediatric Thalamic Abscess.

AIM: Management of thalamic abscess is being considered as a contentious issue in neurosurgery. Regarding these lesions, besides removing the abscess, the most minimal morbidity is targeted during surgery and planning. MATERIAL AND METHOD: A 5-year-old female presented with the symptoms of altered consciousness and left hemiparesis. Her medical history pointed out that she was being followed up for a congenital cardiac anomaly consisting of transposition of the great arteries and a ventricular septal defect. A cranial MRI revealed 2 masses with peripheral contrast enhancement in the right fro

Pathologic Findings and Clinical Course of Midline Paraventricular Gliomas Diagnosed Using a Neuroendoscope.

INTRODUCTION: Removal of midline paraventricular gliomas is difficult because of their deep localization and invasive character, requiring biopsy for pathologic diagnosis. This study aimed to assess the pathologic findings and clinical course of midline paraventricular gliomas diagnosed using a neuroendoscope. METHODS: This study was performed as a retrospective investigation using a neuroendoscope of 26 patients whose tumors were diagnosed as midline paraventricular gliomas. The main loci of the lesions were the thalamus (11 patients), tectum (6 patients), and other areas (9 patients). Of th

Initial experience with endoscopic ultrasonic aspirator in purely neuroendoscopic removal of intraventricular tumors.

OBJECTIVE Neuroendoscopic removal of intraventricular tumors is difficult and time consuming because of the lack of an effective decompression system that can be used through the working channel of the endoscope. The authors report on the utilization of an endoscopic ultrasonic aspirator in the resection of intraventricular tumors. METHODS Twelve pediatric patients (10 male, 2 female), ages 1-15 years old, underwent surgery via a purely endoscopic approach using a Gaab rigid endoscope and endoscopic ultrasonic aspirator. Two patients presented with intraventricular metastases from high-grade t

Neuroendoscopic Ventriculocisternostomy with Stent Placement for Trapped Temporal Horn After the Resection of Glioblastoma.

BACKGROUND: Endoscopic ventriculostomy is an attractive surgical alternative to ventriculoperitoneal shunt in the treatment of focal hydrocephalus, including trapped temporal horn (TTH). The major concern of this surgical approach is closure of a stoma, the risk of which may be minimized by placement of a stent after ventriculostomy. CASE DESCRIPTION: The authors report a case of a 60-year-old man with glioblastoma in the corpus callosum and the parietal lobe who developed TTH after partial tumor resection. After the failure of a ventriculoperitoneal shunt, endoscopic ventriculocisternostomy

Neuroendoscopic Intraventricular Biopsy in Children with Small Ventricles Using Frameless VarioGuide System.

Endoscopic biopsy for intraventricular tumors in pediatric patients with small ventricles is a challenging procedure because of the risk of morbidity during the intraventricular approach. We describe the use of the VarioGuide system for intraventricular endoscopic biopsy in 9 consecutive pediatric patients with intraventricular lesions and small ventricular size. All patients had lesions in the anterior part of the third ventricle with a median frontal and occipital horn ratio of 0.33. Patients presented with growth failure (n = 4), visual disturbances (n = 4), and seizures (n = 1). The VarioG

Endoscopic third ventriculostomy and choroid plexus cauterization with a rigid neuroendoscope in infants with hydrocephalus.

OBJECT Combining endoscopic third ventriculostomy (ETV) with choroid plexus cauterization (CPC) has been shown to improve the success rate compared with ETV alone in infants (less than 24 months) with hydrocephalus who were treated in developing countries. The authors sought to evaluate the safety and efficacy of this procedure, using a rigid neuroendoscope, in a single North American center, and to assess whether the ETV success score (ETVSS), the CURE Children's Hospital of Uganda ETVSS (CCHU ETVSS), and other pre- and intraoperative variables could predict success. METHODS The authors perfo

Neuroendoscopic management of intraventricular germinoma at the foramen of Monro: case report and review of the literature.

BACKGROUND: Intraventricular tumors account for approximately 3% of adult brain tumors and 16% of childhood and adolescent brain tumors. Half of the intraventricular tumors in adults and one quarter of those in children are found in the lateral ventricles. Ependymoma, astrocytoma, oligodendroglioma, choroid plexus papilloma, meningioma and subependymal giant cell astrocytoma are the common tumors encountered at this particular site. A germinoma at this site is rare. Microsurgery of intraventricular tumors can be challenging and is performed with potential functional and cognitive complications
