Optimal combination of clinical examinations for neurologic prognostication of out-of-hospital cardiac arrest patients.

AIM: Targeted temperature management (TTM) may alter the results of clinical examination and delay motor response recovery; hence, re-establishing the accuracy and optimal timing of performing clinical examinations are crucial. Therefore, we aimed to identify the optimal combination and timing of clinical examinations for predicting the neurologic outcomes in patients undergoing TTM. METHODS: We conducted a retrospective analysis of prospectively collected multicentre registry data. All enrolled patients were supposed to undergo pupil light reflex (PLR), corneal reflex (CR), and Glasgow Coma

Examining the Utility of a Novel Neurologic Test in Patients With Obstructive Sleep Apnea: A Pilot Study.

Purpose: To test the association between participant King-Devick Test (KDT) times and obstructive sleep apnea (OSA) severity and evaluate for improvement after continuous positive airway pressure (CPAP) treatment. Methods: Study dates January 30 to July 31, 2018. Patients were referred for initial evaluation of sleep disordered breathing concerns. OSA severities were defined by Apnea Hypopnea Index (AHI) results, with ≥15 considered at least moderate OSA. The KDT is an objective physical measure of brain function. We estimated correlation between KDT time and AHI and compared mean KDT time bet

Infant neurological examination from 3 to 12 months: predictive value of the single items.

The prognostic value of the single items of a standardised neurological examination, the Hammersmith Infant Neurologic Examination (HINE), was explored longitudinally in 658 infants at 3, 6, 9 and 12 months post-term age. ROC curves were built based on the presence/absence of cerebral palsy at 2 years of age. Global HINE scores showed very high prediction (ROC curve areas above 0.9) at all ages. The items with the highest predictive value were always movement quality and quantity. In the first semester, among the most predictive items were those assessing tone, while beyond that time they were

[Recommendations for endoscopic, laboratory and electrophysiological examinations in the investigation of non-neurological female urinary incontinence].

CYSTOURETHROSCOPY: Cystourethroscopy is not recommended in the initial work-up of urinary incontinence except in the following circumstances: Microscopic or macroscopic haematuria requiring screening for an associated tumour especially in the presence of risk factors for urothelial tumour (smoking, occupational exposure). Signs of bladder irritation in the absence of urinary tract infection. Unexplained bladder or pelvic pain. Recurrent urinary tract infections. Discordance between clinical features and urodynamic assessment, which fails to reproduce the symptoms described by the patient. Work

Essential tremor prevalence is low in Arabic villages in Israel: door-to-door neurological examinations.

OBJECTIVE: To estimate the prevalence of essential tremor (ET) in Arabic villages of Wadi Ara. BACKGROUND: The prevalence ET which is higher than Parkinson's disease in North America and Europe, differs according to study population and methodology. Since hospital record based epidemiology carries referral bias and might provide low estimates of ET prevalence, we carried a population based survey. METHODS: This door-to-door survey comprised the Arabic villages of Wadi Ara in Northern Israel. Consecutive residents who agreed to participate in the study and were older than 64 years at prevalen

Neurological and electrophysiological examinations on three groups of workers with different levels of exposure to mercury vapors.

The authors performed neurological, visual evoked potentials (VEP) and electroneurography (ENG) examinations on three groups of workers with occupational exposure to mercury vapors (Hg(0)), and on a control group. The exposure of dental professionals (n = 36) was mild, that of chloralkali plant workers (n = 36) was intermediate, and that of workers from mercury works (n = 77) was very high. Symptoms and signs of micromercurialism were observed only in the group with the highest exposure to Hg(0). In comparison with the control group, a shortening of VEP latency and a decrease in amplitude were

Development of the "SMA NNE," a short neonatal neurological examination for newborns with spinal muscular atrophy.

UNLABELLED: The advent of newborn screening for spinal muscular atrophy (SMA) has highlighted the need for easy, quick, clinical tools to be used in infants with SMA identified in the neonatal period. We propose a new short module developed using items from the Hammersmith Neonatal Neurological Examination (HNNE) and from a module developed for floppy infants, both previously used in newborns with SMA. The new module was developed by identifying and selecting the items that were more often found to have abnormal findings in SMA newborns. The proforma was designed by selecting 13 items and conv

Pure Motor Isolated Finger Palsy by Cerebral Infarction: Tips for Diagnosis by Neurological Examination.

There are various causes of unilateral finger palsy. Its potential etiologies include peripheral neuropathy, carpal tunnel syndrome, and nerve root disorder due to myelopathy. In addition to them, similar paralysis has been reported in localized lesions of the cerebrum, classically referred to as pseudoperipheral palsy. In this report, we describe a case of an 80-year-old man who developed sudden clumsiness of the right fingers. Neurological examination showed muscle weakness mainly in the 1st and 2nd fingers (Medical Research Council grade 1-4) and normal reflexes in the extremities. The affe

Neurological examination of healthy term infants at ages 6 and 10 weeks in Tshwane District.

BACKGROUND: Globally, there is a significant gap in detailed neurodevelopmental data for infants under 3 months, despite 6 weeks being identified a critical milestone for neuro-behavioural development. Normative values and optimal scores for healthy infants at 6 and 10 weeks postnatally are lacking in many settings. In South Africa, the statutory neurodevelopmental assessments at these ages exclude notable characteristics of central nervous system maturation and limit opportunities to collect data of early developmental progress. OBJECTIVES: Our study aimed to assess developmental characteris

Accuracy of clinical neurological examination in diagnosing lumbo-sacral radiculopathy: a systematic literature review.

BACKGROUND: Lumbar radiculopathy remains a clinical challenge among primary care clinicians in both assessment and diagnosis. This often leads to misdiagnosis and inappropriate treatment of patients resulting in poor health outcomes, exacerbating this already debilitating condition. This review evaluated 12 primary diagnostic accuracy studies that specifically assessed the performance of various individual and grouped clinical neurological tests in detecting nerve root impingement, as established in the current literature. METHODS: Eight electronic data bases were searched for relevant articl

Electromyographic findings in shoulder dislocations and fractures of the proximal humerus: comparison with clinical neurological examination.

There is no consensus of opinion about the frequency of associated nerve lesions in anterior shoulder dislocations and fractures of the proximal humerus. We undertook a prospective study to assess the incidence, the severity of the nerve injury and the diagnostic value of electromyographic examination; 215 patients were included. We performed neurological examination and needle electromyography (EMG). Nerve injury was graded according to a denervation score at the EMG. EMG disorders were seen in 133 patients (62%). Testing of sensibility and clinical reflexes proved not to be a reliable indica

Computed tomography for neurological intensive care patients. Report on one hundred CT examinations.

The first 100 computed tomographic (CT) examinations of the patients on the neurological intensive care ward are discussed and reported on the basis of selected typical findings. Characteristic patterns of the CT findings in determined cerebral diseases are explained. The possibility and necessity of CT observations of the development, of inflammatory and cerebrovascular processes in particular are emphasized. A comparison of our experience with CT and other neuroradiological methods, is made. The clinical diagnoses, including the respective number of cases and the pertinent CT findings, are p

Direct withdrawal of a knife lodged in the thoracic spinal canal in a patient with normal neurologic examination: is it safe?

Stab wound injuries to the spinal cord are rare, although they commonly cause complete or incomplete neurological deficits. Normal neurological examination with a knife traversing the spinal canal is extremely rare. Here we report on a patient with a knife lodged in the thoracic spine with normal neurological examination and describe direct withdrawal of the knife with excellent results that have not been reported to date. A 50-year-old male patient was admitted to the emergency service because of his sustaining a stab wound to thoracic 3-4 level due to a knife traversing the spinal canal and

[Cervical Lesions or Peripheral Nerve Disorders? How Far Can Neurological Examination Localize the Lesion?].

Accurate lesion localization is a fundamental component of neurological diagnosis. This review describes how neurological examination can be used to differentiate cervical spinal cord lesions, cervical radiculopathy, brachial plexopathy, and peripheral nerve disorders in patients presenting with upper-limb sensory and motor symptoms. Particular attention is given to the distribution of sensory impairment, tendon reflex abnormalities, and patterns of muscle weakness. The characteristic clinical features of cervical spondylotic myelopathy, cervical radiculopathy, brachial plexus disorders, and e

Neurological examination of Pacific harbor seal (Phoca vitulina richardii) pups: development and assessment of a protocol.

Stranded Pacific harbor seal (HS, Phoca vitulina richardii) pups admitted to rehabilitation centers have a variety of health problems, including neurological disorders. However, the standard neurological examination protocol for land-based quadrupeds does not suit marine mammals such as seals. In this study we aimed to develop and establish a neurological examination protocol (NEx) for phocid seal pups undergoing rehabilitation. In two consecutive years, we assessed stranded HS pups (n = 45; males n = 21, females n = 24) in care at The Marine Mammal Center (TMMC), Sausalito, California. The dr

Psychological Therapy for Functional Neurological Disorder: Examining Impact on Dissociation, Psychological Distress and General Functioning.

Functional neurological disorder (FND) represents a broad group of motor and sensory clinical symptoms which cannot be explained by other neurological diagnoses. Dissociation is considered a key mechanism in their development and maintenance. Despite psychological therapy being the recommended choice of treatment for FND, evidence for its effectiveness is in its infancy. This study explored the dissociative profile of forty-seven patients with FND and evaluated whether individual psychological therapy improved dissociative symptoms, psychological distress and general functioning among twenty-f

Axenfeld-Rieger syndrome: A systematic review examining genetic, neurological, and neurovascular associations to inform screening.

Axenfeld-Rieger Syndrome (ARS) is comprised of a group of autosomal dominant disorders that are each characterized by anterior segment abnormalities of the eye. Mutations in the transcription factors FOXC1 or PITX2 are the most well-studied genetic manifestations of this syndrome. Due to the rarity this syndrome, ARS-associated neurological manifestations have not been well characterized. The purpose of this systematic review is to characterize and describe ARS neurologic manifestations that affect the cerebral vasculature and their early and late sequelae. PRISMA guidelines were followed; stu
