Challenges of a West African green mamba (Dendroaspis viridis) envenomation in a European urban setting: A case report with quantitative monitoring of neuromuscular transmission.

Although rare, exotic snakebites are a growing medical concern worldwide, partly due to the rising popularity of exotic pet ownership in high-income countries. Envenomation from family Elapidae snakes may cause severe neurological and local symptoms. Timely antivenom administration is crucial, but often complicated by logistical challenges. We report a case of a West African green mamba (Dendroaspis viridis) envenomation in a 59-year-old male in Switzerland who developed progressive weakness and respiratory failure, associated with swelling of a limb. Quantitative neuromuscular stimulation, al

Monitoring Neuromuscular Activity during Exercise: A New Approach to Assessing Attentional Focus Based on a Multitasking and Multiclassification Network and an EMG Fitness Shirt.

Strengthening muscles can reduce body fat, increase lean muscle mass, maintain independence while aging, manage chronic conditions, and improve balance, reducing the risk of falling. The most critical factor inducing effectiveness in strength training is neuromuscular connection by adopting attentional focus during training. However, this is troublesome for end users since numerous fitness tracking devices or applications do not provide the ability to track the effectiveness of users' workout at the neuromuscular level. A practical approach for detecting attentional focus by assessing neuromus

Effect of Adjunctive Train-of-Four Monitoring Using Peripheral Nerve Stimulation on Neuromuscular Blocking Agent Dose in PICU Patients: A Randomized Crossover Study.

OBJECTIVE: Continuous neuromuscular blocking agent (NMBA) infusion is common in the PICU. There are no pediatric trials evaluating the effect of train-of-four (TOF) monitoring on NMBA dosing. We therefore aimed to determine the effect of adjunctive TOF monitoring using peripheral nerve stimulation on NMBA dosing compared with clinical assessment alone in PICU patients. DESIGN: Randomized crossover study (Thai Clinical Trial Registry, TCTR20240328002). SETTING: Fourteen-bed general PICU. PATIENTS: Age 30 days to 18 years who received continuous cisatracurium infusion and were expected to con

The influence of moderate or deep neuromuscular block status on anesthetic depth monitoring system during total intravenous anesthesia using propofol and remifentanil: A randomized trial.

The neuromuscular block state may affect the electroencephalogram-derived index representing the anesthetic depth. We applied an Anesthetic Depth Monitoring for Sedation (ADMS) to patients undergoing laparoscopic cholecystectomy under total intravenous anesthesia, and evaluated the requirement of propofol according to the different neuromuscular block state. Adult patients scheduled to undergo laparoscopic cholecystectomy were enrolled and randomly assigned to either the moderate (MB) or deep neuromuscular block (DB) group. The UniCon sensor of ADMS was applied to monitor anesthetic depth and

Routine administration of neostigmine after recovery of spontaneous breathing versus neuromuscular monitor-guided administration of neostigmine in pediatric patients: a parallel, randomized, controlled study.

BACKGROUND: Neostigmine used to reverse the muscle relaxants should be guided by neuromuscular monitoring, as the degree of spontaneous pre-reversal recovery is the key to success to reverse the neuromuscular block. But neuromuscular monitoring is not always available for some patients during anesthesia and, in consequence, we need to use other clinical judgment to guide the use of neostigmine to reverse the neuromuscular block. In this trial, we aimed to evaluate the incidence of residual neuromuscular blockade (rNMB) in pediatric patients with routine use of neostigmine after recovery of spo

Methods to monitor and evaluate neuromuscular blockade use in intensive care unit patients with hypoxemic respiratory failure and acute respiratory distress syndrome: a scoping review.

PURPOSE: Though neuromuscular blocking agents (NMBAs) are recommended for managing the cases of patients with moderate to severe acute respiratory distress syndrome (ARDS) in the intensive care unit (ICU), current guidelines do not provide clear consensus regarding effective NMBA monitoring strategies. In this scoping review, we aimed to describe NMBA monitoring strategies and their associated outcomes among adults undergoing invasive mechanical ventilation for hypoxemic respiratory failure (HRF) and ARDS. Improved Reliability of Electromyography-Based Neuromuscular Monitoring During Laparoscopic Surgery Achieved by the Modified Attachment Method for Nihon-Kohden NM-345Y™ Stimulating Electrodes: A Case Report.

Neuromuscular monitoring is crucial during the administration of neuromuscular blocking agents owing to individual variations in their effects. In electromyography (EMG)-based neuromuscular monitoring using the EMG electrodes (NM-345Y™, Nihon-Kohden Corporation, Tokyo, Japan) following the manufacturer-recommended attachment method, the accuracy of neuromuscular monitoring may be reduced when forearm limb position is changed. We previously devised a novel attachment method for NM-345Y™ stimulating electrodes in adult volunteers to maintain stable monitoring accuracy despite changes in forearm

Intraoperative monitoring of somatosensory (SSEPs) and transcranial electric motor-evoked potentials (tce-MEPs) during surgical correction of neuromuscular scoliosis in patients with central or peripheral nervous system diseases.

PURPOSE: Combined intraoperative monitoring (IOM) of transcranial electric motor-evoked potentials (tce-MEPs) and somatosensory-evoked potentials (SSEPs) is safe and effective for spinal cord monitoring during scoliosis surgery. However, the literature data regarding the reliability of spinal cord monitoring in patients with neuromuscular scoliosis are conflicting and need to be confirmed. METHODS: We reviewed IOM records of 40 consecutive patients with neuromuscular scoliosis related to central nervous system (CNS) (29 pts) or peripheral nervous system (PNS) (11 patients) diseases, who under

Effects of the attachment method of the stimulating electrodes Nihon-Kohden NM-345Y™ and changes in forearm position on stimulus current values during calibration in electromyography-based neuromuscular monitoring: a single-center experimental study.

PURPOSE: The traditionally recommended method for attaching electromyography (EMG) electrodes (NM-345Y™) during EMG-based neuromuscular monitoring developed by Nihon-Kohden may decrease the monitoring accuracy when forearm limb position changes. This study investigated methods for attaching stimulating electrodes that maintained stable EMG-based neuromuscular monitoring accuracy, regardless of forearm limb position changes. METHODS: This single-center experimental study recruited 28 healthy adults from October 2022 to December 2022. The NM-345Y™ was attached to the forearm using three pattern

A modified train-of-four ratio to assess recovery from depolarizing neuromuscular blockade after succinylcholine, a prospective observational study.

Succinylcholine is still used in clinical practice. Its duration is highly variable and neuromuscular monitoring is recommended to assess its recovery. Unfortunately, depolarizing neuromuscular block cannot be evaluated by train-of-four (TOF) as no fade occurs. Only single twitch (ST) allows to monitor depolarizing block. The TOFScan monitor proposes a modified TOF ratio (T4/Tref) not referring to fade within a TOF series. We hypothesized that the T4/Tref allows to assess recovery from succinylcholine. This observational study aims to compare recovery from depolarizing block simultaneously ass

Beyond the hype: Understanding the limitations of sugammadex.

PURPOSE: Sugammadex rapidly and effectively reverses aminosteroidal neuromuscular blocking agents and has largely replaced neostigmine in perioperative practice. Although its use is associated with faster recovery and a lower rate of residual neuromuscular blockade, clinically important limitations exist. This review summarizes the literature describing incomplete reversal, recurrence of neuromuscular blockade, adverse effects, drug interactions, and challenges in special populations, with a focus on the role of quantitative neuromuscular monitoring and pharmacist-led activities to optimize su

Residual Neuromuscular Block Remains a Safety Concern for Perioperative Healthcare Professionals: A Comprehensive Review.

Residual neuromuscular block (RNMB) remains a significant safety concern for patients throughout the perioperative period and is still widely under-recognized by perioperative healthcare professionals. Current literature suggests an association between RNMB and an increased risk of postoperative pulmonary complications, a prolonged length of stay in the post anesthesia care unit (PACU), and decreased patient satisfaction. The 2023 American Society of Anesthesiologists Practice Guidelines for Monitoring and Antagonism of Neuromuscular Blockade provide guidance for the use of quantitative neurom

Train-of-four monitoring with the twitchview monitor electctromyograph compared to the GE NMT electromyograph and manual palpation.

The purpose of this study was to compare train-of-four count and ratio measurements with the GE electromyograph to the TwitchView electromyograph, that was previously validated against mechanomography, and to palpation of train-of-four count. Electrodes for both monitors were applied to the same arm of patients undergoing an unrestricted general anesthetic. Train-of-four measurements were performed with both monitors approximately every 5 min. In a subset of patients, thumb twitch was palpated by one of the investigators. Eleven patients contributed 807 pairs of train-of-four counts or ratios.

Residual neuromuscular blockade in the ICU: a prospective observational study and national survey.

Residual neuromuscular blockade is associated with significant morbidity. It has been widely studied in anaesthesia; however, the incidence of residual neuromuscular blockade in patients managed in the ICU is unknown. We conducted a prospective observational study in a tertiary ICU to determine the incidence of residual neuromuscular blockade using quantitative accelerographic monitoring. We tested for residual neuromuscular blockade (defined as a train-of-four ratio < 0.9) before cessation of sedation in anticipation of tracheal extubation. We also surveyed 16 other ICUs in New Zealand to det

Unanticipated Profound Paralysis and Sugammadex Dosing Implications After Videoscopic Thoracic Surgery.

A bedridden patient with empyema presented for thoracoscopic decortication. During the procedure, despite a post-tetanic count (PTC) of 0 via calibrated quantitative neuromuscular monitoring, persistent diaphragmatic movement impaired operating conditions, so rocuronium was re-dosed. After surgery, the patient had 0 PTC. Sugammadex was titrated to achieve baseline neuromuscular strength, monitoring the effect of each 200-mg dose. Ultimately, 1200 mg was required to achieve baseline strength. We describe monitor troubleshooting, considerations with unexpectedly deep neuromuscular blockade, the

[Guideline on the safe use of neuromuscular blocking agents].

Inappropriate use of muscle relaxants leads to postoperative residual neuromuscular block (PORNB), which is still a significant problem nowadays and is an independent risk factor for postoperative pulmonary complications. To decrease its incidence, the European Society of Anesthesiology and Intensive Care and the American Society of Anesthesiologists published their guidelines on the safe use of muscle relaxants in 2023. In this publication, the Edömér Tassonyi Neuromuscular Task Force of the University of Debrecen Clinical Center summarizes its recommendations. Based on international guidelin

Sugammadex: Pharmacometrics, Clinical Utility, and Adverse Effects.

Sugammadex has been used clinically since 2008, and it represents the only cyclodextrin neuromuscular blocking reversal agent currently available for patient care. Sugammadex's unique mechanism of action allows it to reverse the effects of aminosteroid neuromuscular blockade rapidly, and pharmacodynamically predictable doses are selected based on quantitative neuromuscular monitoring. The drug has several potential adverse effects, predominantly related to immediate hypersensitivity reactions and bradyarrhythmias, and it can be costly. Overall, when employed appropriately, it represents an eff
