Intraoperative spinal cord monitoring consists in a subcontinuous evaluation of spinal cord sensory-motor functions and allows the reduction the incidence of neurological complications resulting from spinal surgery. A combination of techniques is used: somatosensory evoked potentials (SSEP), motor evoked potentials (MEP), neurogenic motor evoked potentials (NMEP), D waves, and pedicular screw testing. In absence of intraoperative neurophysiological testing, the intraoperative wake-up test is a true form of monitoring even if its latency long and its precision variable. A 2011 survey of 117 Fre

European spine journal : official publication of the European Spine Society, the European Spinal Deformity Society, and the European Section of the Cervical Spine Research Society2011eng
Iatrogenic spinal cord injury is the most feared complication of scoliosis surgery. The importance of combined somatosensory evoked potentials (SEP) and motor evoked potentials (MEP) monitoring during spine surgery is well known. The current authors retrospectively evaluated the results of neurophysiological intraoperative monitoring (IOM) in a large population of patients who underwent surgical treatment for spinal deformity. Intraoperative monitoring of SEPs and transcranial electrical stimulation MEPs (TES-MEP) was performed in 172 successive patients who underwent surgical treatment of idi

BACKGROUND: Neurophysiologic intraoperative monitoring (NIM) has gone through a renaissance since the advent of computer technology. Currently, both motor and sensory cranial nerves, including the IInd and VIIIth cranial nerves, can be intraoperatively controlled by means of small and mobile systems.
PATIENTS AND METHODS: In order to estimate the value of NIM using the new generation of computer systems, we analyzed the records of 379 patients who underwent skull base surgery since 1996. These comprised NIM of the IInd, VIIth, VIIIth cranial nerves in most cases and of the IXth, Xth, XIth, an

Physiologic monitoring may warn of impending neural damage and intraoperative assessment may provide critical information used to direct the surgical procedure itself. Intraoperative techniques utilize methods modified from the diagnostic neurophysiologic laboratory. Simultaneous multimodality recording of different signal sources is of particular value for several types of procedures. Intraoperative studies must be tailored to each patient's clinical condition and to the specific surgical risks. Since studies have supported the benefits of intraoperative monitoring for peripheral nerve and cr

In 135 cases of posterior fossa surgery almost exclusively in the cerebellopontine angle (CPA) intraoperative monitoring of brain-stem acoustic evoked potentials (BAEP) and partly somatosensory evoked potentials (SEP) was performed. The series consisted of 20 microvascular decompressions, 63 acoustic neurinomas, 7 vascular lesions and 45 other space occupying lesions, mostly in the CPA. BAEP monitoring alone was employed in 76 cases, combined BAEP and SEP monitoring less frequently. The technique of anaesthesia and intraoperative monitoring is presented in detail including an analysis of techn

PURPOSE: Intraoperative neurophysiological monitoring (IONM) is a tool that can improve safety during spinal surgery. Patients with neuromuscular scoliosis (NMS) have inherent neural axis or primary muscle pathology that may present challenges to neuromonitoring. An understanding of IONM event rates, patterns, and outcomes would better prepare surgeons to counsel patients and families.
METHODS: IONM data from pediatric spine deformity correction procedures in patients with neuromuscular scoliosis at a single quaternary care center between 2012 and 2022 were reviewed to determine cases with mo

Cervical spine surgery in patients with myelopathy poses a substantial anesthetic challenge, primarily due to the risk of secondary spinal cord injury (SCI). Traditionally, concerns have centered around cervical movements during intubation. However, limited evidence supports a direct link between intubation and SCI, so anesthesiologists must consider other factors, including patient positioning, spinal perfusion pressure, and direct surgical complications. In this context, multimodal intraoperative neurophysiological monitoring, including somatosensory (SSEPs) and motor evoked potentials (MEPs

Pedicle screw placement has a high risk of damaging the motor and sensory pathways due to the close proximity to the spinal cord and nerve roots. Early detection and prevention of injury can be achieved by utilizing Somatosensory Evoked Potentials (SSEP) and Transcranial electrical Motor Evoked Potentials (TCeMEP) during a scoliosis surgery. A 19-year-old female presented for correction of scoliosis. After intubation, electrodes were placed for upper and lower SSEPs, EMGs and TCeMEPs. Total intravenous anesthesia was used. Baseline SSEP and TCeMEP responses were present in all limbs. Eight ped

BACKGROUND: Intradural-extramedullary spinal cord tumor surgery is common. Unlike intramedullary spinal cord tumor surgery, where intraoperative neurophysiological monitoring (IONM) has been described extensively, the application of IONM has not been described in this context, and its relevance has not been investigated.
METHODS: From 2001 to 2012, 100 patients underwent intradural-extramedullary spinal cord tumor resection with IONM. Preoperative and postoperative clinical evaluations were completed retrospectively, using a modified McCormick grading scale and correlated with IONM monitorabi

BACKGROUND: Resection of gliomas in or adjacent to the motor system is widely performed with intraoperative neuromonitoring (IOM). Despite the fact that data on the safety of IOM are available, the significance and predictive value of the procedure are still under discussion. Moreover, cases of false-negative monitoring affect the surgeon's confidence in IOM.
OBJECTIVE: To examine cases of false-negative IOM to reveal structural explanations.
METHODS: Between 2007 and 2010, we resected 115 consecutive supratentorial gliomas in or close to eloquent motor areas using direct cortical stimulatio

We present two cases illustrating the benefit of utilizing intraoperative neurophysiological monitoring (IONM) for prevention of injuries to the lower cranial nerves during fourth ventricle tumor resection surgeries. Multiple cranial nerve nuclei are located on the floor of the fourth ventricle with a high risk of permanent damage. Two male patients (ages 8 and 10 years) presented to the emergency department and had brain magnetic resonance imaging (MRI) scans showing brainstem/fourth ventricle tumors. During surgery, bilateral posterior tibial and median nerve somatosensory evoked potentials

Achieving accurate monitoring of electrophysiological activity from the brain cortex is critical for preventing postoperative neurological deficits during craniotomy. However, current technologies face profound challenges in enabling consistent high-quality neural signal acquisition due to complex intracranial environments (e.g., wet/fragile brain surfaces and surgical instrument interference). Here, we develop a soft electrode array with reconfigurable hydrogel interfaces for sustainable high-fidelity monitoring of electrophysiological states from the functional brain cortex throughout neuros

Demand has increased in recent years to develop and implement strategic approaches for improving functional outcomes following pelvic surgery. Intraoperative neurophysiological monitoring represents one such possible approach for this field. It involves generating neuronal activity within the nerve supply of pelvic organs and structures, conventionally using an external electrical current, and conjunctively recording neuronally mediated functional responses, which then, in real time, guides the surgical dissection in a manner that maximizes nerve preservation. A central aspect of this methodol

INTRODUCTION: The cavernous nerves are constitutive elements of the intrapelvic autonomic neuroregulatory input to the penis and pelvic floor, serving principally to mediate penile erection and likely urinary continence to some extent. Diffuse and variable in their topography surrounding the prostate, these neural structures are challenging for surgeons to discern and avoid injuring during radical prostatectomy, despite the use of modern surgical maneuvers for cavernous nerve preservation. Accordingly, objectives of patient retention of erectile function after surgery are insufficiently met.

INTRODUCTION: Prone position during posterior spine surgery can represent a potentially risky procedure for the nervous system. Infrequent injuries due to prone positioning consist of subtle spinal cord infarction or myelopathy that can be promptly detected by intraoperative neurophysiological monitoring (IONM), if applied in this phase of surgery. Here, we report a case that stresses the value of IONM even in detecting spinal positioning-related neurological complications during kyphoscoliosis correction.
