This paper presents diagnostic criteria for Menière's disease jointly formulated by the Classification Committee of the Bárány Society, The Japan Society for Equilibrium Research, the European Academy of Otology and Neurotology (EAONO), the Equilibrium Committee of the American Academy of Otolaryngology-Head and Neck Surgery (AAO-HNS) and the Korean Balance Society. The classification includes 2 categories: definite Menière's disease and probable Menière's disease. The diagnosis of definite Menière's disease is based on clinical criteria and requires the observation of an episodic vertigo synd

Vestibular disorders manifesting as vertigo, chronic dizziness and imbalance are common problems in neurological practice. Here, we review some recent interesting and important advances in diagnosis of vestibular disorders using the video head impulse test and in the management of benign positional vertigo and migrainous vertigo.

OBJECTIVE: In patients with migraine, neurotologic symptoms and signs occur commonly. The authors' aim was to determine whether neurotologic findings are in accordance with the type of migraine and whether test findings differ from those of healthy controls.
METHODS: The authors examined 36 patients with various types of migraine classified by International Headache Society criteria. Comprehensive neurotologic tests were performed between attacks: video-oculography (VOG), electronystagmography, static posturography, and audiometry on 12 patients with migraine with aura (MA) and 24 patients wi

In order to investigate the relationship between chronic dizziness and vestibular function in patients with panic disorder, in the present study neurotologic findings in 15 patients with panic disorder and chronic dizziness were compared with those in 15 patients with chronic dizziness, without panic disorder. All underwent neurotologic screening for spontaneous, positional and positioning nystagmus with head-shaking and head-thrust tests, an audiometric examination and electronystagmography with bithermal stimulation according to Freyss. A significantly higher number of patients with panic di

PURPOSE OF REVIEW: Recent updates with clinical implications in the field of neuro-otology are reviewed.
RECENT FINDINGS: Important updates relating to several neuro-otologic disorders have been reported in recent years. For benign positional paroxysmal vertigo (BPPV), we provide updates on the characteristics and features of the short arm variant of posterior canal BPPV. For the acute vestibular syndrome, we report important updates on the use of video-oculography in clinical diagnosis. For autoimmune causes of neuro-otologic symptoms, we describe the clinical and paraclinical features of ke

Cranio-corpography (CCG) stepping and standing tests are compared in a group of 202 neurotological patients. In these two vestibulo-spinal tests, performed by a simple and precise photo-optic system, the movement of the shoulders and the head--and not the movement of the feet, is taken as the parameter for the investigation. The two tests are complementary one to the other. The CCG should be an integral part of every neurotological laboratory.

Otoneurology is a broad, diverse and multidisciplinary field. On the one hand, it includes everything that relates to hearing: from complex surgery including life-changing implants to effective medical treatments and habilitation. On the other hand, it includes all that has to do with balance, from novel vestibular tests to complex medical treatments and recently, the development of a vestibular implant for patients suffering from bilateral peripheral vestibular loss. This special issue on otoneurology includes 8 original papers and 3 reviews. The first three articles deal with auditory implan

We have developed an interactive database for vertigo than can be used to assist in the diagnostic procedure and to store the data in a form of a database. The database offers the possibility to split and reunite the collected information in a desired way. The database contains detailed information about patient history, symptoms and findings in neurotological, audiological and imaging tests. The symptoms are classified into three sets of questions: vertigo (including postural instability), hearing loss and tinnitus, and provoking factors. Confounding disorders are screened. The neurotological

The temporal bone pathology of a 36-year-old man who suffered from chronic myelocytic leukemia and sudden hearing loss of both ears, was studied from the viewpoint of neurotology. Neurotological examination showed bilaterally profound hearing loss and no caloric reaction but good counter-rolling reaction to the right and the left head tilt and no other abnormal neurological findings. He died of intracranial hemorrhage. A study of histopathology of the temporal bones revealed extensive destruction of the organ of Corti, dilatation of Reissner's membrane, leukemic infiltration in the cochlea but

Like Joseph Toynbee and Karl Wittmaack before him, Harold Schuknecht understood that studying pathology was the key to advancing the scientific basis of neurotology. His work elucidated the pathophysiology of the most common neurotologic disorders, including benign paroxysmal positional vertigo, Meniere's disease, and viral neurolabyrinthitis. Schuknecht was a tireless crusader against medical and surgical treatments that he considered to have little scientific foundation. His textbook Pathology of the Ear remains the definitive work in the field.

Vertigo, dizziness, and imbalance are among the most frequent presenting symptoms in primary care and in the emergency department (ED). The clinical diagnosis is based on a structured patient history and bedside examination of the vestibular system and the ocular motor system, looking especially for central ocular motor signs. Recent guidelines, such as the GRACE-3 guideline on acute vertigo and dizziness, the increasing availability of bedside diagnostic algorithms, and the use of quantitative vestibular and ocular motor testing in both the ED-setting and in the outpatient center have facilit

OBJECTIVE: The physician-scientist workforce is shrinking in the United States. Academic otologists/neurotologists face a diverse set of barriers to successful careers. We aimed to characterize the factors affecting contemporary otology/neurotology surgeon-scientists.
STUDY DESIGN: An electronic survey was distributed to faculty members of the American Neurotology Society and the American Otological Society in 2021. The survey queried demographics, practice setting, compensation, and barriers to conducting research for respondents in academic practice.
RESULTS: One hundred fifty-seven otolog

Although peripheral vestibular disorders account for the most cases of dizziness or vertigo, approximately 11-15% of patients have central vestibulopathy. This remains a diagnostic challenge for neurotologists, as it can present with acute vestibular syndrome (AVS) in isolation without other obvious neurologic deficits. Despite advances in modern neuroimaging, false-negative results still occur in up to 20% of cases, further complicating the differential diagnosis. Therefore, recognition of the characteristic neurotologic signs associated with each central neural structure is important for the

Hearing loss, tinnitus, dizziness and disequilibrium are frequent complaints of patients with labyrinthine and retrolabyrinthine lesions. The objective of a neurotological examination is to establish the presence and location of a lesion, whether it is labyrinthine (cochlear or vestibular) or intracranial. On the basis of neurotological and vestibular results, provided by neurotologist or otolaryngologist, if a retrocochlear or retrovestibular lesion is suspected, a magnetic resonance imaging (MRI) or computed tomography (CT) study of the head is obtained with particular attention to the acous
