Cosmeceuticals in acne vulgaris: from mechanism of action to clinical application.

The use of cosmeceuticals in acne vulgaris is becoming increasingly prevalent with many over-the-counter formulations becoming part of patients' routine skincare. Cosmeceuticals are often used successfully as an adjunctive therapy to reduce the side effects of traditional prescriptions and improve compliance. This is a review to support the use of cosmeceuticals in acne, including retinol, retinaldehyde, benzoyl peroxide (BPO), azelaic acid, beta hydroxy acids, alpha hydroxy acids, niacinamide, zinc, tea tree oil and green tea. There is most evidence, in human clinical trials, to support the u

INDIVIDUAL ARTICLE: Real-World Clinical Experience With a Neuro-Peptide Serum in Combination With Botulinum Toxin Type-A Injections.

We evaluated real-life experiences of a topical neuro-peptide serum containing 2% acetyl hexapeptide-8, 2% dipeptide diaminobutyroyl, 5% polyhydroxy acids (PHA), 5% niacinamide, and 1% laminaria extract (topical neuro-peptide serum [TNP-serum]). The TNP-serum works synergistically by stimulating 9 key skin biomarkers to reduce wrinkles and produce a skin-brightening effect. Here, we highlight the real-life experiences of 5 dermatologists and 2 surgeons, using an integrated skincare regimen consisting of botulinum toxin type-A (BTX-A) injection in conjunction with twice daily TNP-serum. Emerging topical therapies to treat pigmentary disorders: an evidence-based approach.

BACKGROUND: Hyperpigmentation disorders are commonly encountered in dermatology clinics. The use of prescription-grade and over-the-counter topical lightening agents has increased in popularity, leading to a substantial growth of research over the past decade. OBJECTIVE: We seek to review clinical studies evaluating the use of different Rx-grade and OTC ingredients in treating hyperpigmentation. METHODS AND MATERIALS: A comprehensive search on PubMed was conducted to identify patient-based evidence on the most common ingredients used as topical lightening agents: arbutin, ascorbic acid, cyst

Treatment of melasma: a review of less commonly used antioxidants.

Melasma, a common cause for seeking dermatologic care, is a chronic condition of skin hyperpigmentation. With a poorly understood pathogenesis, and no universal cure, melasma is a challenge for many dermatologists. For decades, there has been investigation into the role of oxidative stress in melasma. In this literature review, we introduce the role of oxidative stress in melasma and discuss the function of various topical and oral antioxidant therapies for patients suffering from melasma. Numerous studies have shown efficacy of various antioxidant therapies for treatment of hyperpigmentation,

Innovations in natural ingredients and their use in skin care.

Natural ingredients have been used traditionally for millennia and their application in topical creams, lotions and preparations within the traditional medicines and healing traditions of many cultures has been observed. Over the last 20 years, clinical and laboratory studies have identified the benefits of an array of natural ingredients for skin care. Consequently, a number of these ingredients and compounds are today being developed, used or considered not only for anti-aging effects, but also for use in dermatologic disorders. Certain ingredients, such as colloidal oatmeal and aloe vera, h

Postinflammatory hyperpigmentation: a review of the epidemiology, clinical features, and treatment options in skin of color.

Postinflammatory hyperpigmentation is a common sequelae of inflammatory dermatoses that tends to affect darker skinned patients with greater frequency and severity. Epidemiological studies show that dyschromias, including postinflammatory hyperpigmentation, are among the most common reasons darker racial/ethnic groups seek the care of a dermatologist. The treatment of postinflammatory hyperpigmentation should be started early to help hasten its resolution and begins with management of the initial inflammatory condition. First-line therapy typically consists of topical depigmenting agents in ad

The Impact of Antioxidants on Vitiligo and Melasma: A Scoping Review and Meta-Analysis.

Reactive oxygen species (ROS) generated during melanogenesis make melanocytes particularly vulnerable to oxidative stress, influencing their survival and melanin synthesis. Oxidative stress, significantly present in vitiligo and recently also detected in melasma, triggers inflammatory cascades and melanogenesis, making antioxidants a promising therapeutic avenue. A systematic search was conducted on Embase and Pubmed to study the efficacy of antioxidants for treating vitiligo and/or melasma. Meta-analysis was performed to assess the difference in Melasma Severity Index (MASI) scores between ba

Niacin: chemical forms, bioavailability, and health effects.

Elevated low-density lipoprotein cholesterol (LDL-C) has been the main target of lipid-altering therapy to reduce cardiovascular risk associated with dyslipidemia. Residual cardiovascular risk remains, however, after achievement of goal LDL-C levels and is associated in part with other risk markers of cardiovascular disease, including low high-density lipoprotein cholesterol (HDL-C), high lipoprotein a, and hypertriglyceridemia. Niacin is considered a valuable agent for therapy to modify high LDL-C as well as low HDL-C, high lipoprotein a, and hypertriglyceridemia. The forms of niacin availabl

Management of hyperpigmentation in darker racial ethnic groups.

Dyschromias, in particular hyperpigmentation, are major issues of concern for people of color. Pigmentary disorders such as melasma and postinflammatory hyperpigmentation (PIH) can cause psychological and emotional distress and can pose a negative impact on a person's health-related quality of life. The precise etiology of these conditions is unknown. Therapies for melasma and PIH target various points during the cycle of melanin production and degradation. Therapies for these conditions include topical agents and resurfacing procedures. Hydroquinone remains the gold standard of topical agents

The use of botanical extracts as topical skin-lightening agents for the improvement of skin pigmentation disorders.

Both physicians and dermatology patients are searching for long-term topical skin care solutions (both cosmetic and cosmeceutical) to address problems presented by skin hyperpigmentation. Specifically, some women often express a desire to "lighten" skin tone by achieving improved visible tone, reduction in yellowness (or sallow tone), and reduction in the appearance of hyperpigmented spots ("age" or "sun" spots). Traditional depigmenting agents, such as hydroquinone, corticosteroids, and kojic acid, although highly effective, can raise several safety concerns (for example, ochronosis, atrophy,

Design and evaluation of multi-target AGEs/ALEs inhibitors for potential application in skin anti-aging.

The main causes of skin aging include both intrinsic and extrinsic factors such as oxidative stress triggered by reactive oxygen species (ROS), modification of collagen and elastin by reactive carbonyl species (RCS), UV exposure, lifestyle habits and environmental pollutants. Skin aging is the most visible manifestation of senescence and is unavoidable. In particular, delaying age-related changes in facial appearance is a key objective for older adults. To address this issue, naturally occurring compounds including vitamin C, vitamin E, coenzyme Q10, resveratrol, niacinamide, carnosine, retino

A framework to mitigate the risk of chemical leukoderma: Consumer products.

Chemical leukoderma is an acquired depigmentation of the skin caused by repeated exposure to specific agents damaging to epidermal melanocytes. Case reports of chemical leukoderma have been associated with some consumer products. To date, there are no well-accepted approaches for evaluating and minimizing this risk. To this end, a framework is presented that evaluates the physical and chemical characteristics of compounds associated with chemical leukoderma and employs structure-activity relationship (SAR) read-across and predictive metabolism tools to determine whether a compound is at increa

Pathophysiology and clinical management of pellagra - a review.

Pellagra is a rare disease caused by niacin deficiency or a disruption of its metabolism. Its manifestations are dermatitis with pronounced photosensitivity, gastrointestinal symptoms, and neuropsychiatric ailments. Currently pellagra is developed in people who chronically abuse alcohol or are treated with medications from specific pharmacological groups (immunosuppressive and anti-tuberculosis drugs). Although the root cause of the disease was established in the mid-twentieth century, a detailed explanation of the processes leading to the development of symptoms has not yet been proposed. The

The role of topical dermocosmetics in acne vulgaris.

Acne is a common chronic inflammatory disease and treatment modalities based on acne severity are well established. The role of dermocosmetics in dermatology, and in particular acne, is becoming more important as more research elucidates the mechanisms of action of products in the pathogenesis of acne. Dermocosmetics have the potential to be used as monotherapy or in combination with medical treatment. Therefore, it has become important for dermatologists to understand dermocosmetics to effectively and appropriately advise patients on their use. The objective of this review was to provide new

Assessment of skin barrier function in rosacea patients with a novel 1% metronidazole gel.

The skin of patients with rosacea is extremely sensitive and hyper-reactive to dietary, environmental, and topical factors. Accordingly, the management of rosacea involves not only choosing appropriate medication and treatment for daily skin care, but also avoiding known trigger factors. Recently, 1% metronidazole, a mainstay of topical rosacea therapy, was reformulated in a gel vehicle that contains hydrosolubilizing agents (HSA) niacinamide, beta cyclodextrin, and a low concentration of propylene glycol. It is designed to solubilize greater concentrations of metronidazole than is possible in
