Children with Congenital Central Hypoventilation Syndrome Do Not Wake up to Ventilator Alarms.

PURPOSE: Congenital Central Hypoventilation Syndrome (CCHS) requires lifelong ventilatory support during sleep. Subjects with CCHS are vulnerable to sleep disturbances associated with treatments, monitoring alarms, and care they receive. We hypothesized that sleep would be disrupted in patients  with CCHS due to ventilatory support and other treatments at night. METHODS: An anonymous survey of patients with CCHS, age up to 17 years was conducted through REDCAP. Subjects were recruited in person, by flyer, email, and social media. Data collected included demographics, PHOX2B genotype, ventilat

Virtual Reality to Improve Sleep Quality in Patients Suffering from Painful Diabetic Polyneuropathy: A Proof of Concept Study.

Background/Objectives: Sleep disturbance is often observed in the context of chronic pain. We hypothesize that, by providing an immersive Virtual Reality (VR) experience with a serious game to chronic pain patients an hour before bedtime, attention can be diverted from the pain condition, consequently leading to improved sleep quality. The aim is to evaluate the efficacy of VR compared to usual care in reducing the number of awakenings during the night and increasing sleep efficiency in patients suffering from painful diabetic polyneuropathy (PDPN). Methods: Eight patients with PDPN were rando

Work-family conflict and burnout in night-shift nurses with children: the mediating roles of recovery experience and fatigue.

BACKGROUND: Burnout among nurses is a major global concern, particularly among those working night shifts, which can compromise the quality of nursing care, increase turnover intentions, and threaten the stability of the nursing workforce. Previous studies have shown that work-family conflict correlates positively with burnout in nurses, but not enough research has been performed on its relationship among night-shift nurses with children. In addition, the mechanism of this relationship is not completely clear. OBJECTIVE: This study aimed to examine the relationships among work-family conflict

[Efficacy of intranasal desmopressin for the treatment of nocturnal polyuria in the elderly females].

Older adults often complain of nocturia as one of the most bothersome symptoms of lower urinary tract incontinence. Nocturia places such patients at risk of falling down and insomnia and increases the care burden. The causes of nocturia include various factors, such as neuropathic bladder, prostate hyperplasia and pelvic floor muscle weakness. It has also been reported that nocturia is caused by an increased renal blood flow while lying down and the loss of diurnal variation in vasopressin. The intranasal administration of desmopressin at night may improve nocturia. We experienced a case of se

Transgender Women Experiencing Homelessness - National HIV Behavioral Surveillance Among Transgender Women, Seven Urban Areas, United States, 2019-2020.

Transgender women experience high prevalence of homelessness, which can affect their likelihood of acquiring HIV infection and can lead to poor medical outcomes. CDC analyzed data from the National HIV Behavioral Surveillance Among Transgender Women to identify whether personal characteristics and social factors affecting transgender women were associated with duration of homelessness during the past 12 months. Longer duration and chronic homelessness might indicate greater unmet needs, which increases their likelihood for acquiring HIV infection. Ordinal logistic regression was conducted to c

Subjective and objective workload of critical care nurses in a tertiary hospital in Switzerland: a retrospective analysis.

BACKGROUND: Nursing workload in intensive care units affects outcomes and staff well-being. Objective measures such as Nine Equivalents of Nursing Manpower Score (NEMS) capture task demands, whereas subjective ratings reflect nurses' workload; how these relate is unclear (1-4). METHODS: All shifts over 6 months in an adult ICU of a tertiary hospital were analyzed. For each shift, staffing (number of nurses, patients, patient-to-nurse ratio PNR), number of interventions, admissions and discharges, cumulative Richmond agitation sedation scale RASS score and intensivist identity were recorded. Temporal dynamics of circadian phase shifting response to consecutive night shifts in healthcare workers: role of light-dark exposure.

KEY POINTS: Shift work is highly prevalent and is associated with significant adverse health impacts. There is substantial inter-individual variability in the way the circadian clock responds to changing shift cycles. The mechanisms underlying this variability are not well understood. We tested the hypothesis that light-dark exposure is a significant contributor to this variability; when combined with diurnal preference, the relative timing of light exposure accounted for 71% of individual variability in circadian phase response to night shift work. These results will drive development of pers

Social factors determining the experience of blindness among pregnant women in developing countries: the case of India.

Approximately 10 million pregnant women around the world develop night blindness annually. In India, one in 11 pregnant women suffers from night blindness. This study used a nationally representative sample of 35,248 women from India between the ages of 15 and 49 who had given birth in the past five years to understand the effect of women's empowerment on developing blindness during pregnancy. Findings from logistic regression showed that several empowerment-related factors, including women's increased age at the birth of their first child, high school education, and participation in their own

Behavioral and psychological symptoms of dementia and its relationship with caregiver burden: A cross-sectional study of inpatients with dementia in a tertiary center.

BACKGROUND: Behavioral and psychological symptoms of dementia (BPSD) is commonly associated with caregiver distress, and often a reason for short-term or long-term care. Very few studies have examined the pattern of the BPSD and caregiver burden in an Indian tertiary care inpatient setting. AIMS: Our study aimed to understand the pattern of BPSD, particularly its individual domains, in different types of dementia, and its relationship with caregiver distress and burden in a geriatric inpatient setting. METHODS: One hundred inpatients with dementia and their informal primary caregivers partic

Foster Caregivers' Perceptions of Children's Sleep Patterns, Problems, and Environments.

OBJECTIVE: The disparity of problems, impairments, and disorders among children in foster care is well-documented and spans virtually every domain of functioning. Sleep, however, has received minimal attention among this vulnerable group, which is concerning given the multitude of ways sleep affects children's development, health, and behavior. METHODS: A total of 485 foster caregivers from across the United States completed a survey including quantitative items and qualitative, open-ended questions about sleep and related health and behavior for one child (M = 6.4 years, SD = 2.2; range 4-11

Individual heart failure patient variability in nocturnal hypoxia and arrhythmias.

Traditional heart failure research often uses daytime population parameter estimates to assess hypoxia and arrhythmias. This approach might not accurately represent heart failure patients as nighttime cardiac behaviors offer crucial insights into their health, especially regarding oxygen levels and heart rhythms. We conducted a prospective study on nocturnal oxygen saturation and heart rate in home-dwelling heart failure patients over 6 nights. Patients were recruited from the Walter Reed National Military Medical Center heart failure clinic. Criteria included a clinical diagnosis of heart fai

Short-term effects of continuous positive airway pressure on sex hormones in men and women with sleep apnoea syndrome.

Patients with sleep apnoea syndrome (SAS) suffer from symptoms of hypogonadism. Besides surgical interventions, in some cases, the standard care of SAS for most patients is continuous positive airway pressure (CPAP). Studies focusing on the long-term effects of CPAP on testosterone levels revealed conflicting results. None of the studies included female patients with SAS. The aim of our study was to analyse and compare sex hormone levels in saliva before and after a night without and with CPAP in women and men with SAS. The results were negative. One night with CPAP did not affect the dynamics
