Refractory Vasoplegia Following Nimodipine Administration in Aneurysmal Subarachnoid Hemorrhage: A Case Series.

Introduction: Following aneurysmal subarachnoid hemorrhage (aSAH), approximately 30% of patients experience delayed cerebral ischemia (DCI). Nimodipine, a cerebroselective dihydropyridine calcium channel blocker, reduces this risk of DCI with <5% of patients experiencing hypotension or bradycardia. We review four patients admitted to our Neurosciences Intensive Care Unit following aSAH who developed nimodipine-induced refractory vasoplegia. We discuss these four cases and detail the interventions used to counter their bradycardia and hypotension. Methods: We performed a retrospective chart rev

NEWTON: Nimodipine Microparticles to Enhance Recovery While Reducing Toxicity After Subarachnoid Hemorrhage.

BACKGROUND: Aneurysmal subarachnoid hemorrhage (aSAH) is associated with high morbidity and mortality. EG-1962 is a sustained-release microparticle formulation of nimodipine that has shown preclinical efficacy when administered intraventricularly or intracisternally to dogs with SAH, without evidence of toxicity at doses in the anticipated therapeutic range. Thus, we propose to administer EG-1962 to humans in order to assess safety and tolerability and determine a dose to investigate efficacy in subsequent clinical studies. METHODS: We describe a Phase 1/2a multicenter, controlled, randomized

Morphine-induced analgesia in rats withdrawn from concurrent nimodipine and morphine treatment.

The effect of repeated administration of dihydropyridine calcium channel antagonist, nimodipine, given concurrently with morphine on the development of tolerance to the antinociceptive actions of morphine in rats was studied. In acute experiments nimodipine (1 mg/kg i.p.) enhanced the antinociceptive effect of morphine (2.5 mg/kg s.c.) in the hot-plate and tail immersion tests. Daily administration of morphine either for 10 days (increasing the daily dose from 20 to 35 mg/kg) or for 24 days (increasing the daily dose from 20 to 70 mg/kg) induced tolerance to the antinociceptive effect of a cha

Early nimodipine treatment in reversible cerebral vasoconstriction syndrome: A serial transcranial Doppler study.

OBJECTIVES/BACKGROUND: Although nimodipine is commonly used to treat reversible cerebral vasoconstriction syndrome (RCVS), its disease-modifying effects are not yet understood. We prospectively investigated the effect of nimodipine and treatment initiation timing on the prevention of worsened vasoconstriction in patients with angiogram-proven RCVS. METHODS: In this prospective cohort study, we recruited patients with angiogram-proven RCVS who visited a single headache clinic between June 2019 and November 2021. To evaluate the mean flow velocities (MFVs) of the middle cerebral arteries, patie

Nimodipine Blocks Histone-Induced Calcium Overload to Protect Neurons after Traumatic Brain Injury.

Aims: To investigate if nimodipine alleviates traumatic brain injury (TBI)-induced neuronal apoptosis and neurological deficits by inhibiting extracellular histone-mediated Ca2+ influx, mitochondrial damage, and Caspase pathway activation. Results: In vitro, nimodipine significantly reduced histone-induced Ca2+ influx in cortical neurons, reversed by Ca2+ activator A23187. It restored neuronal proliferation (↑3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide, ↑Ki67+ cells), reduced apoptosis (↓Annexin V/propidium iodide), improved mitochondrial function (↑ΔΨm/adenosine triphosphate,

Intraarterial Nimodipine Versus Induced Hypertension for Delayed Cerebral Ischemia: A Modified Treatment Protocol.

BACKGROUND: Rescue treatment for delayed cerebral ischemia (DCI) after subarachnoid hemorrhage can include induced hypertension (iHTN) and, in refractory cases, endovascular approaches, of which selective, continuous intraarterial nimodipine (IAN) is one variant. The combination of iHTN and IAN can dramatically increase vasopressor demand. In case of unsustainable doses, iHTN is often prioritized over IAN. However, evidence in this regard is largely lacking. We investigated the effects of a classical (iHTN+IAN) and modified (IANonly) treatment protocol for refractory DCI in an observational st

Clinical Effect of Nimodipine Combined with Magnesium Sulfate on Pregnancy-Induced Hypertension Syndrome.

Источник пометил публикацию как отозванную

The incidence of pregnancy-induced hypertension in China is 9.4%, which is at a relatively high level. Its serious impact on maternal and infant health is the main reason for maternal and perinatal morbidity and mortality. There are many factors affecting pregnancy-induced hypertension. The incidence of pregnancy-induced hypertension is different due to different levels of cultural knowledge, health awareness, economic income, nutrition, and medical support. Since its etiology has not been elucidated thus far, there is no known treatment of the disease, and the main principles are spasmolysis,

COVID-19 Associated Reversible Cerebral Vasoconstriction Syndrome Successfully Treated with Nimodipine and Aspirin.

There have been limited cases linking SARS-CoV-2 infection with the development of reversible cerebral vasoconstriction syndrome (RCVS). We hereby report a rare case of RCVS in the setting of mild SARS-CoV-2 respiratory infection successfully treated with nimodipine and aspirin. SARS-CoV-2 attacks the ACE2-receptors, which are expressed in various body organs including the lungs, kidneys, and blood vessels. Vasoconstriction can result from down-regulation of the ACE2-receptors that can lead to sympathetic hypertonia of the cerebral blood vessel walls and/or over-activation of the renin-angiote

Nimodipine represses AMPK phosphorylation and excessive autophagy after chronic cerebral hypoperfusion in rats.

Chronic cerebral hypofusion (CCH) after bilateral carotid artery occlusion (2VO) causes cognitive damage and neuronal degeneration in the cortex and hippocampal CA1 area, and influences the oxygen and glucose supply in the brain which often results in metabolic alterations and oxidative stress. AMP-activated protein kinase (AMPK) phosphorylation, a sensor of cellular energy status, directs metabolic adaptation to support cellular growth and survival after CCH. Autophagy is also likely to be involved in metabolic adaptation and plays an important role in neuronal deterioration and cognitive dec

Effect of nimodipine on cerebral blood flow and cerebrovascular reactivity after subarachnoid haemorrhage.

The aim of the present study was to investigate the effect of nimodipine on autoregulation of cerebral blood flow (CBF), CO2 reactivity and cerebral oxygen metabolism (CMRO2) in patients with subarachnoid haemorrhage (SAH). Eight patients with severe SAH were studied with repeated CBF and CMRO2 measurements on the first day of the bleeding and after at least 12 h of treatment of nimodipine. An initial resting study, an autoregulation study and a hyperventilation study was performed. CBF was measured using the 133-Xenon intravenous method. CMRO2 was calculated as AVDO2 x CBF. Nimodipine did not

Can nimodipine affect progression of motor neuron disease? A double-blind pilot study.

Recently, there has been some evidence suggesting that exposure to neuroexcitotoxic amino acids, culminating in excessive neuronal calcium influx, may cause nerve cell destruction in motor neuron disease (MND). We therefore hypothesized that central calcium channel blockade with nimodipine might slow-down progression of MND. Two patients with sporadic MND were treated in a double-blind placebo controlled cross-over study with oral nimodipine (120 mg/day) for 5 months. Disease progression was assessed by repeated computerized measurements of isometric force from 20 muscle groups, and calculatio

Recovery of function after brain damage: facilitation by the calcium entry blocker nimodipine.

Large neocortical injuries can interfere with the initial performance of many previously learned behaviors. This dysfunction, however, may not be attributable to the loss of the memories of learned behaviors but to a failure to access memories that are spared. The results of this experiment support this hypothesis by demonstrating that the calcium entry blocker nimodipine, which prevents trauma-induced retrograde amnesia in a 1-trial passive learning situation, will also facilitate the recovery of pre-operatively learned brightness discrimination in rats with visual neocortical injuries.

Intra-arterial nimodipine and outcomes in delayed cerebral ischemia after aneurysmal subarachnoid hemorrhage.

INTRODUCTION: Vasospasm after aneurysmal subarachnoid hemorrhage (aSAH) may cause delayed cerebral ischemia (DCI). Intra-arterial nimodipine (IAN) is used in vasospasms resistant to standard hemodynamic therapy. The effect of IAN on long-term outcomes remains uncertain, particularly in patients requiring repeated infusions. RESEARCH QUESTION: What are the outcomes of aSAH patients treated with IAN, especially compared to the characteristics of patients undergoing repeated versus single IAN infusions. MATERIAL AND METHODS: We retrospectively reviewed all aSAH patients with DCI treated with IA

Enantioselective assay of nimodipine in human plasma using liquid chromatography-tandem mass spectrometry.

Nimodipine is a dihydropyridine calcium channel blocker that exhibits higher selectivity toward cerebral blood vessels compared with other members of the same class. It has been shown to improve outcomes and prevent delayed cerebral ischemia in the setting of aneurysmal subarachnoid hemorrhage, a life-threatening brain bleed. Nimodipine is a chiral compound and it is marketed as a racemic mixture of (+)-R and (-)-S enantiomers. (-)-S-Nimodipine is approximately twice as potent a vasorelaxant as the racemic mixture and is more rapidly eliminated than the (+)-R counterpart following oral dosing.

Nimodipine protects dopaminergic neurons against inflammation-mediated degeneration through inhibition of microglial activation.

Nimodipine, a calcium channel blocker, has been used mainly in the therapy of cardiovascular diseases. Recently, its indications have been extended experimentally to a wider range of disorders especially some central nervous system (CNS) disorders. In this study, we investigated whether nimodipine is neuroprotective to inflammation-mediated neurodegenerative diseases. Pretreatment with nimodipine reduced the degeneration of dopaminergic (DA) neurons induced by LPS in mesencephalic neuron-glia cultures in a dose-dependent manner. The neuroprotective effect of nimodipine was attributed to the in

Morbidity and mortality after early aneurysm surgery--a prospective study with nimodipine prevention.

Based on the outcome in 116 consecutive patients who were subjected to early aneurysm operation combined with additional nimodipine treatment, and who were controlled by transcranial Doppler (TCD) sonography, a morbidity and mortality analysis was performed. Of the 84 patients who preoperatively were in Hunt & Hess grades III, 79 patients (94%) were considered to show a favourable (good-fair) late recovery, while one patient (1%) had a poor outcome, and four patients (5%) died. Of the 32 poor condition patients (H & H IV-V), 17 (53%) showed a favourable recovery, while seven (22%) had a poor o
