The effect of introduction of minimal standards of neonatal care on in-hospital mortality.

A retrospective study was done to assess the effect on in-hospital neonatal mortality of a series of interventions in neonatal care in the highlands of Papua New Guinea. Between 1995 and 1997, prior to the interventions, the mortality among neonates admitted to the Goroka Hospital Special Care Nursery was 18% and two-thirds of very low birthweight (1-1.5 kg) neonates died. The interventions began in December 1997 and were aimed at reducing mortality among all neonates and particularly among those with very low birthweight. Compared to the 30-month period prior to the interventions, the in-hosp

A nursery outbreak of Staphylococcus aureus pyoderma originating from a nurse with paronychia.

An outbreak of methicillin-susceptible Staphylococcus aureus pyoderma occurred in the nursery of a tertiary-care referral center. All strains retrieved from the outbreak, including one from a nurse's infected finger, were typed by arbitrarily primed polymerase chain reaction and automated ribotyping. The results indicated that the spread of the outbreak was probably facilitated by contamination of the nurse with paronychia.

The contribution of withholding or withdrawing care to newborn mortality.

OBJECTIVE: We sought to determine the contribution of withholding or withdrawing care to neonatal mortality in our hospital over a 10-year period from 1993 through 2002. BACKGROUND: Although not initiating or withholding intensive care for certain newborns has been practiced openly for the past 25 years, little information has been published concerning the results of these practices on neonatal mortality. DESIGN AND METHODS: All infants who were born in our hospital and remained in the hospital until their death were evaluated for the care they received at the time of birth and near the time

[Current orientation of antibiotic treatment in neonatal bacterial infection].

The bacterial infection are responsive of severe sepsis and localizations in neonates, with a high mortality. The right choice of the first antibiotics is essential, and they must be well-fitted, early and bactericidal. The antibiotics of choice in the mother-linked infections are directed towards streptococcus group B and E. coli. Ampicillin must be replaced by a third generation cephalosporin because E. coli are resistant in more than 50% of cases. In tropical areas the first neonatal infections may be also the fact of multiresistant hospital acquired bacteria, which are transmitted during d

Decisions Regarding Newborn Feeding by Latina Mothers.

This study examined the factors that influence Latina mothers' decisions to initiate breastfeeding, formula feeding, or both, and when a given neonatal feeding method is decided, in order to assess the optimum time for healthcare providers to discuss nutrition decisions. A Spanish-speaking health care provider orally administered a 24-question survey to self-identified Latina mothers in the Newborn Nursery at North Carolina Children's Hospital after delivery. A total of 152 Latina mothers, aged 17 to 43 years, were interviewed from September 2014 to September 2015. Three decisive factors resul

A cost-minimisation analysis comparing alternative telemedicine screening approaches for retinopathy of prematurity.

INTRODUCTION: Screening for retinopathy of prematurity (ROP) is an important procedure in the prevention of blindness in high-risk preterm infants. In the regionalised healthcare system of Queensland (Australia), outside of the major centres, some preterm infants are cared for in special care nurseries (SCNs). When necessary, infants in these nurseries who are at risk of ROP are transferred to a tertiary hospital for screening by paediatric ophthalmologists. The transport of preterm infants for eye examinations adds risk and incurs significant costs to the health system. Using a cost-minimisat

Salmonella senftenberg outbreak in a neonatal unit.

Salmonella senftenberg was isolated from the stools of 35 newborns between June, 1987 and September, 1988. Twenty nine (82.8%) babies were preterms and twenty six (74%) babies were less than 2,000 grams in weight. All of them were symptomatic within 6 days of life, 74.3% (26) within 4 days. All of them had loose stools and weight loss. Other features were sclerema, jaundice and paralytic ileus. The organism was sensitive to nalidixic acid (100%), amikacin (94.6%), gentamicin (72.9%), cephaloridine (56.7%), and chloramphenicol (29.8%) and resistant to benzyl penicillin and ampicillin. Protective effect of rooming-in on postpartum hemorrhage: A propensity score analysis.

OBJECTIVE: This study investigates the effect of rooming-in on postpartum hemorrhage (PPH) and other adverse maternal postpartum outcomes. METHODS: A retrospective study was performed on all healthy women with singleton term pregnancies at a tertiary medical center between 2013 and 2022. Women were categorized by choice of neonatal care: hospital nursery or rooming-in. Propensity score matching (2:1) was based on maternal age, body mass index, parity, prior cesarean deliveries, pregnancy length, epidural administration, perineal lacerations, and neonatal birthweight. Primary outcome was PPH o

Assessment of the quality of neonatal care in the Solomon Islands.

AIM: To identify strengths and obstacles for improving the quality of newborn care in the Solomon Islands. Improving the quality of newborn care is a priority in the Sustainable Development Goals and the Action Plan for Healthy Newborns in the Western Pacific. The neonatal mortality rate in the Solomon Islands, a lower-middle-income country, has improved slower than overall child mortality. In 2013, neonatal mortality (13.2/1000) constituted 44% of under-5 deaths (30.1/1000). METHODS: A cross-sectional study of newborn care in five provincial hospitals using a World Health Organization assess

Pragmatic Expansion of a Neonatal Antibiotic Stewardship Program in a Community Health Care System.

BACKGROUND: Previously published neonatal antibiotic stewardship efforts have been primarily implemented in single centers. Piedmont Athens Regional began work to decrease antibiotic use in the NICU with spread to the newborn nursery (NBN) and, subsequently, 13 other NICUs and NBNs throughout a health care system over a 4-year period. METHODS: This quality improvement initiative was conducted in the context of a multicenter learning collaborative from 2016 to 2019. The primary aim was a 10% reduction in antibiotic days per 1000 patient days (antibiotic utilization rate [AUR]) among newborns i

Epidemiology of clinically suspected and laboratory-confirmed bloodstream infections at a South African neonatal unit.

INTRODUCTION: Data from Africa reporting the epidemiology of infection in hospitalised neonates are limited. METHODOLOGY: A prospective study with convenience sampling was conducted to characterise neonates investigated with blood culture/s for suspected infection at a 132-bed neonatal unit in Cape Town, South Africa (1 February-31 October 2018). Enrolled neonates were classified as having proven bloodstream infection (BSI) (blood culture-positive with a pathogen) or presumed infection (clinically suspected but blood culture-negative) or as potentially at risk of infection (maternal risk fact

Establishing demand feeding in hospital.

In the summer of 1975 demand feeding was introduced for all babies born at the John Radcliffe Hospital, where previously babies were fed according to a rigid schedule to fit in with ward routine--clock feeding. Over a 10-day period before demand feeding was introduced details were collected about infant feeds concerning 42 normal babies whose mothers had decided to breast feed. 2 weeks after the introduction of demand feeding similar details were collected about 43 normal breast-fed babies. At the time of the observations 65% of all babies born in this hospital were being breast fed. Comparing

Miliary tuberculosis in two infants after nursery exposure: epidemiologic, clinical, and laboratory findings.

Two infants born 4 days apart, and cared for in the same nursery, developed miliary tuberculosis at 2.5 and 5.5 months of age, respectively. A search for a source case among household contacts failed to reveal anyone with active tuberculosis. An epidemiologic study of nursery personnel revealed a nurse's aide with cavitary tuberculosis. Examination of 1,647 infants (98.9 per cent of all infants cared for in the nursery during the 10.5-month period preceding the discovery of the source case) failed to reveal any other infected children. Recommendations are made for tuberculosis control programs

Wide Variation Found in Care of Opioid-Exposed Newborns.

OBJECTIVE: Standardized practices for the management of neonatal abstinence syndrome (NAS) are associated with shorter lengths of stay, but optimal protocols are not established. We sought to identify practice variations for newborns with in utero chronic opioid exposure among hospitals in the Better Outcomes Through Research for Newborns (BORN) network. METHODS: Nursery site leaders completed a survey about hospitals' policies and practices regarding care for infants with chronic opioid exposure (≥3 weeks). RESULTS: The 76 (80%) of 95 respondent hospitals were in 34 states, varied in size (

Two concurrent epidemics of enteroviral meningitis in an obstetric neonatal unit.

A small epidemic of viral infection occurred in a neonatal unit. It involved 11 infants, all of whom recovered. Echovirus 11 was the cause of illness in six infants, coxsackie B3 in four, and one infant had a biphasic illness, due first to echo 11, then to coxsackie B3. The epidemic coincided with a major outbreak of echovirus 11 and a smaller outbreak of coxsackie B3 in the community. Several staff became ill. Five infants were considered to have been infected by perinatal materno-infant spread, others by cross-infection. The epidemic ended when the nursery was closed.
