Nutritional support in acute kidney injury requiring continuous renal replacement therapy: from metabolic alterations to clinical recommendations.

This narrative review examines the metabolic and nutritional challenges faced by critically ill patients with acute kidney injury (AKI) undergoing continuous renal replacement therapy (CRRT). Recent evidence and international guidelines were reviewed to synthesize the current understanding of nutrient losses, metabolic alterations, and clinical implications. CRRT induces additional depletion of amino acids, proteins, water-soluble vitamins, and trace elements, aggravating the catabolic stress inherent to critical illness. It also modifies carbohydrate, lipid, and micronutrient metabolism, whil

[Response, to polymyxin, of plague microbe strains isolated from various natural foci and of their mutants with a decreased requirement in nutritional factors].

Investigation of the response to polymyxin of 65 strains of Pasteurella isolated from various foci and 46 their back mutants showed that all of them were usually highly resistant to polymyxin (MIC 200--500 micrograms/ml). The Pasteurella strains isolated in the Gissaro-Darvaz natural focus, Turkey and Congo were highly sensitive to polymyxin (MIC 10--25 micrograms/ml). Single cultures highly sensitive to the antibiotic were detected among the polymyxin-resistant strains. Polymyxin-sensitive mutants of these cultures with lowered requirements in the growth factors were obtained.

2025 ELSO Consensus Statement for the Provision and Management of Nutrition Therapy in Critically Ill Adult Patients Requiring Extracorporeal Membrane Oxygenation.

International guidelines for the nutrition management of critically ill adults do not sufficiently cover the unique challenges and considerations of patients receiving extracorporeal membrane oxygenation (ECMO). The aim of the current guideline is to assess the literature informing nutrition provision and practice for patients receiving ECMO and provide clinicians with consensus-based recommendations to inform clinical practice. A group of international experts was convened by the Extracorporeal Life Support Organization (ELSO) to systematically develop consensus-based recommendations for nutr

Refeeding risks in patients requiring intravenous nutrition support: Results of a two-centre, prospective, double-blind, randomised controlled trial.

BACKGROUND/AIMS: Refeeding syndrome can result following excessive feeding of malnourished patients. The syndrome remains poorly defined but encompasses a range of adverse effects including electrolyte shifts, hyperglycaemia and other less well-defined phenomena. There are additional risks of underfeeding malnourished individuals. Studies of refeeding syndrome have generally focussed on critical care environments or patients with anorexia nervosa. Here we have conducted a two-centre, prospective, double-blind, randomised controlled trial amongst all patients referred to hospital nutrition supp

Phase IV prospective clinical study to evaluate the effect of taurine on liver function in postsurgical adult patients requiring parenteral nutrition.

BACKGROUND: Taurine's role in bile acid metabolism and anti-inflammatory activity could exert a protective effect on hepatobiliary complications associated with parenteral nutrition (PN). In this study, the effects of 2 amino acid solutions, with and without taurine, on liver function administered to nonacutely ill postsurgical patients as part of a short-term PN regimen were prospectively compared. METHODS: Adult patients randomly received (double-blind) Tauramin 10% or a standard PN solution without taurine as the control (1.5 g amino acid/kg body weight [bw]/d; infusion rate of ≤4 mg gluco

Association between the C-reactive protein-albumin-lymphocyte (CALLY) index and mortality in elderly patients with dysphagia requiring nutritional support.

BACKGROUND: While the novel C-reactive protein-albumin-lymphocyte (CALLY) index-integrating inflammatory, nutritional, and immune markers-has proven valuable for prognosis in various diseases, its utility in predicting outcomes for dysphagia patients is still unclear. METHODS: This retrospective cohort analysis utilized data sourced from the Dryad Digital Repository. The CALLY index (albumin × lymphocyte count/C-reactive protein × 104) was calculated. After logarithmic transformation (Ln-CALLY), the patients were divided into low Ln-CALLY group (≤ 2.21) and high Ln-CALLY group (>2.21). All-ca

Investigations into the optimum nitrogen and caloric requirements and comparative nutritive value of three intravenous amino acid solutions in the post-operative period.

Complete nitrogen balance studies have been conducted in 17 patients undergoing vagotomy and pyloroplasty during the first 6 post-operative days. All patients received only intravenous nutrition during this period and there were no metabolic complications. These studies have shown that the optimum nitrogen and energy intake in the post-operative period in adults are 0.24g nitrogen and 46 kcal/kg body weight/day, respectively. These studies throw further doubt on the use of the E/T ratio as a predictor of nitrogen source solution efficiency in intravenous nutrient regimes. It is suggested that

Performance thresholds, processing requirements, and circular bioeconomy potential of agro-industrial by-products in African broiler nutrition: A systematic review and meta-analysis.

Feed ingredients account for most broiler production costs in Africa, yet large volumes of agro-industrial residues remain underutilized. The practical question is not whether these by-products can replace maize or soybean meal, but the inclusion levels, processing conditions, and evidence quality required for safe, effective use. This systematic review synthesised the available controlled-feeding evidence and pooled it by random-effects meta-analysis. A structured search of Web of Science and PubMed identified 273 unique records published between 1976 and 2026, from which 13 primary controlle

Parenteral nutrition use and recurrence requiring retreatment of retinopathy of prematurity after anti-VEGF monotherapy: a retrospective cohort study of preterm infants.

BACKGROUND: Retinopathy of prematurity (ROP) poses a significant threat to the vision of premature infants worldwide. Anti-vascular endothelial growth factor (anti-VEGF) agents have been effective in treating ROP; however, treatment failure or recurrence remains a challenge. This study aims to investigate the relationship between potential risk factors and ROP recurrent requiring retreatment. METHODS: In our retrospective cohort of 96 premature infants (187 eyes) treated with intravitreal anti-VEGF injections at a tertiary center from 2010 to 2015, we analyzed perinatal, nutritional, and ocula

Health State Utility Associated with Parenteral Nutrition Requirement in Patients with Short Bowel Syndrome and Intestinal Failure in Korea: A Vignette-Based Approach.

Background/Objectives: Short bowel syndrome (SBS) is the leading cause of intestinal failure (IF) that often requires long-term parenteral nutrition (PN). Extended PN results in severe complications and reduced quality of life (QoL). This study aimed to evaluate the QoL utility weights associated with PN duration using vignettes. Methods: We developed detailed scenarios and descriptions to represent eight hypothetical health states, reflecting variations in PN frequency in both pediatric and adult patients. A cross-sectional survey was conducted among 359 Korean adults (aged 19-59 years) from

Case report: Central venous catheter thrombosis complicated by chronic thromboembolic disease/pulmonary hypertension in two children requiring parenteral nutrition.

Chronic thromboembolic pulmonary hypertension is a rare but life-threatening complication of long-term central venous catheters (CVC) in children. However, evidence in terms of potential treatment strategies and outcome data remains scarce. We describe two cases of CVC-related thrombosis (Hickman-catheter) complicated by recurrent pulmonary emboli. One patient experienced a complete thromboembolic obstruction of the right pulmonary artery with normal pulmonary pressures and the second patient suffered from a central thromboembolic obstruction of both pulmonary arteries associated with severe p

Long-term outcomes of short bowel syndrome requiring long-term/home intravenous nutrition compared in children with gastroschisis and those with volvulus.

INTRODUCTION: Short bowel syndrome (SBS) is a malabsorptive state that occurs following extensive small intestinal resection. The most severely affected children require intravenous feeding/parenteral nutrition (PN). Two common causes in infancy/early childhood are intestinal resection for volvulus and resection of necrotic small intestine in infants with gastroschisis. The aim of this study was to review the long-term outcomes of children with severe intestinal failure who remained dependent on PN for many months or years. We evaluated the outcomes among gastroschisis cases versus those follo

Impact of varying levels of protein intake on protein status indicators after gastric bypass in patients with multiple complications requiring nutritional support.

BACKGROUND: <6% of patients who undergo Roux-en-Y gastric bypass (RYGBP) for morbid obesity require nutritional support after surgery. Protein and caloric needs have been estimated as 14-21 kcal, 1.2 g protein/kg/current body weight/day in uncomplicated morbidly obese patients. This study assesses the effect of varying protein-calorie intake in complicated patients after RYGBP on two markers of protein status: thyroxine-binding prealbumin (TBPA) and serum albumin. METHODS: This 25-month retrospective study consisted of 22 patients with postoperative complications. Serum albumin, TBPA, medical

Optimizing the growth of very-low-birth-weight infants requires targeting both nutritional and nonnutritional modifiable factors specific to stage of hospitalization.

BACKGROUND: Data on how baseline characteristics, acuity, morbidity, and nutrition work in combination to affect the growth of very-low-birth-weight (VLBW, <1500 g) infants are limited. OBJECTIVE: We aimed to determine factors associated with in-hospital weight, length, and head circumference (HC) trajectories of VLBW infants. METHODS: VLBW infants (n = 316) from the GTA-DoMINO trial were included. Linear mixed-effects models assessed relations of baseline characteristics, acuity (at birth, across hospitalization), major morbidities, and nutrition (enteral feeding type, macronutrient/energy

Nutritional copper deficiency in severely handicapped patients on a low copper enteral diet for a prolonged period: estimation of the required dose of dietary copper.

Six cases of nutritional copper deficiency were identified in a nursery institute for severely handicapped patients. All had been on prolonged enteral feeding of a copper-deficient diet and all had abnormalities related to the copper deficiency. Two of them had leukopenia, with or without macrocytic and normochromic anemia. After the oral administration of copper, the clinical and laboratory observations including neutrophil counts and serum copper and ceruloplasmin levels showed a complete recovery. The dietary copper requirement was estimated to be approximately 20 micrograms/kg/day for thes
