Obesity and hypertension.

The imbalance between energy intake and expenditure is the main cause of excessive overweight and obesity. Technically, obesity is defined as the abnormal accumulation of ≥20% of body fat, over the individual's ideal body weight. The latter constitutes the maximal healthful value for an individual that is calculated based chiefly on the height, age, build and degree of muscular development. However, obesity is diagnosed by measuring the weight in relation to the height of an individual, thereby determining or calculating the body mass index. The National Institutes of Health have defined 30 kg

Management of Obesity.

This review examines weight loss and accompanying improvements in obesity-related comorbidities produced by intensive lifestyle intervention, pharmacotherapy, and bariatric surgery. Obese individuals lose approximately 6 to 8 kg (approximately 6% to 8% of initial weight) with 6 months of participation in a high-intensity lifestyle intervention (≥ 14 treatment visits) consisting of diet, physical activity, and behavior therapy. Such losses reduce progression to type 2 diabetes in at-risk people and decrease blood pressure and triglyceride levels. All diets, regardless of macronutrient compositi

[Obesity, metabolic syndrome].

The prevalence of reflux esophagitis was low in Japan but has increased in recent years. This increase might be due to the increase in the elderly population and obesity with west- ernized diets. Obesity and the metabolic syndrome cause reflux esophagitis. Increase in central obesity induces inflammation in fat tissue. Moreover, insulin resistance induces hypertension, hypertriglyceridemia and impaired glucose tolerance, and causes the metabolic syndrome. Anti-hypertensive drugs (calcium channel blockers, etc.) and hyperglycemia might cause an esophageal movement disorder. Inflammatory state a

Obesity and hypomagnesemia.

BACKGROUND: Whether low serum magnesium is an epiphenomenon related with obesity or, whether obesity per se is cause of hypomagnesemia, remains to be clarified. OBJECTIVE: To examine the relationship between body weight status and hypomagnesemia in apparently healthy subjects. METHODS: A total of 681 healthy individuals aged 30 to 65years were enrolled in A cross-sectional study. Extreme exercise, chronic diarrhea, alcohol intake, use of diuretics, smoking, oral magnesium supplementation, diabetes, malnutrition, hypertension, liver disease, thyroid disorders, and renal damage were exclusion

Obesity and atopy.

The prevalence of obesity has increased worldwide, and weight gain has been shown to influence the development and clinical expression of various conditions including asthma. The relationships between atopy and obesity remain uncertain, both in adults and in children. Although there are physiopathologic mechanisms which could explain how obesity could influence the immune system and promote the process of sensitization, evidences in favour of a possible role of obesity on the development of atopy have been inconsistent. Furthermore, the bulk of evidence suggests that atopy does not mediate the

Maternal super-obesity.

PURPOSE: Pregnant women with a body-mass-index (BMI) above 50 are referred to as "super-obese". For these women adverse pregnancy outcome and a higher risk of fetal congenital defects are major issues. This report focuses on the ratio development of super-obesity in pregnant women, as well as on prenatal ultrasound and pregnancy outcome in the super-obese gravida. METHODS: We reviewed data of all women with a BMI above 30 who delivered at our unit in a 15-year period between January 2000 and December 2014. Data of obese but not super-obese mothers were evaluated in comparison. RESULTS: Final

Obesity in pregnancy.

The prevalence of obesity has reached alarming proportions globally, and continues to rise in both developed and developing countries. Maternal obesity has become one of the most commonly occurring risk factors in obstetric practice. The 2003-2005 report of the Confidential Enquiries into Maternal Deaths in the United Kingdom highlighted obesity as a significant risk for maternal death [1]. More than half of all women who died from direct or indirect causes were either overweight or obese. For the mother, obesity increases the risk of obstetric complications during the antenatal, intrapartum a

Obesity and fractures.

Until recently obesity was believed to be protective against fractures. However, a report from a Fracture Liaison Clinic in the UK (2010) reported a surprisingly high proportion of obese postmenopausal women attending the clinic with fractures, and in the GLOW study (2011), a similar prevalence and incidence of fractures in obese and non-obese postmenopausal women was observed. Subsequently, other studies have demonstrated the importance of obesity in the epidemiology of fractures. Obese women are at increased risk of fracture in ankle, leg, humerus, and vertebral column and at lower risk of w

Obesity drug therapy.

Obesity is a chronic disease, and it requires chronic therapy. Hypertension, dyslipidemia, diabetes and cardiovascular diseases are leading causes of mortality in the modern world. All of them are strongly linked to obesity. While treating obesity, those conditions are also managed. Obese patients should always be treated through lifestyle interventions, though the results of such interventions are modest. Pharmacotherapy is a second step in the treatment of obesity, approved only when weight loss targets were not reached through lifestyle intervention. During the history of antiobesity drugs,

Obesity in pregnancy.

Obesity, the most common metabolic disorder, concerns, among others, women of reproductive age and, when it occurs before or during pregnancy, constitutes a major risk factor for both maternal and fetal complications. The complications of obesity in a prospective mother include subfertility, miscarriage, thrombo-embolism, hypertensive disorders, metabolic syndrome, preterm delivery and higher frequency of cesarean section. Fetal complications include intrauterine death, congenital anomalies and macrosomia. Moreover, the complications of maternal obesity do not only involve the fetus; they also

Obesity and cancer.

Epidemiological studies have suggested that obesity is associated with increased risk of several cancer types including colon, esophagus, breast (in postmenopausal women), endometrium, kidney, liver, gallbladder and pancreas. Suggested mechanisms include increased intake of potentially carcinogenic food ingredients along with excessive amount of calories, loss of cancer protective effects due to reduced physical activity, carcinogenic factors released from increased adipose tissue mass and "secondary" associations via "precursor" condition such as gallstones. The increased cancer risk in patie

Leptin and obesity.

Leptin, a protein containing 167 amino acids, demonstrates structural similarities with cytokine family and is mainly produced by adipocytes. The leptin receptor (OB-R) is a large membrane spanning protein that belongs to the gp 130 family of cytokine class I receptors. Besides the neuroendocrine effects of leptin in the control of food intake and energy expenditure, binding of this hormone has been proven in intestine, liver, kidney, skin, stomach, heart, spleen, lung, and so on. Thus leptin affects maternal, fetal and placental function, it appears to act as an endocrine and paracrine factor

[Pharmacotherapy of obesity].

Diet, exercise and behavioral therapy are the basics for every treatment of obesity. If lifestyle intervention does not result in a weight loss of 5% within 3 to 6 months, an additional pharmacotherapy can be considered. Treated patients should have a BMI >/=30 kg/m(2) or at least a BMI >/=27 kg/m(2) plus accompanying comorbidities, such as type 2 diabetes, dyslipidemia or hypertension. Current guidelines list orlistat, sibutramine and rimonabant as possible options for the pharmacotherapy of obesity. These compounds result in moderate weight reduction and improvement of cardiovascular risk pr

Obesity and infertility.

PURPOSE OF REVIEW: To summarize major factors affecting fertility in obesity. RECENT FINDINGS: Fertility can be negatively affected by obesity. In women, early onset of obesity favours the development of menses irregularities, chronic oligo-anovulation and infertility in the adult age. Obesity in women can also increase risk of miscarriages and impair the outcomes of assisted reproductive technologies and pregnancy, when the body mass index exceeds 30 kg/m. The main factors implicated in the association may be insulin excess and insulin resistance. These adverse effects of obesity are specifi

Pharmacotherapy of obesity.

Obesity has become a significant health problem in industrialised and developing countries, and despite all nutritional and behavioural approaches, its prevalence is still increasing. In recent years, the identification and characterisation of central and peripheral mechanisms involved in the regulation of energy balance has made remarkable progress and provided numerous targets for novel anti-obesity agents. However, only few anti-obesity drugs are on the market and not many compounds have entered clinical development. In the present review, the clinically available agents are discussed and t

Pharmacotherapy of obesity.

Whereas several drugs to treat most other chronic conditions have been developed over the last 30 years, progress in pharmacologic therapy for obesity has been slow. The record of drug treatment for obesity partly explains this phenomenon and is of concern. Several compounds have been withdrawn when severe side effects have been discovered. The lack of scientifically developed safe and effective antiobesity drugs has opened the gates for an enormous number of commercial products that make vast sums of money, but have no scientific evidence backing their efficacy. This pattern is seen in both d

[Pharmacotherapy of obesity].

While the intentional modification of eating habits remains the most important measure for losing weight, pharmacological support, in particular with regard to the long-term outcome, can be helpful. Catacholaminergic substances, such as diethylpropion and phenylpropanolamine are restricted by their side effects to short-term application only, and their use makes little sense. The serotoninergic substance sibutramine, which acts on the central nervous system to curb the appetite or enhance the feeling of saturation, and orlistat, which inhibits the assimilation of fat from the bowel, are approv

Depression and obesity.

The prevalence of depression (10%) and overweight (65%) indicates that there is a probability that they will co-occur, but are they functionally related? This report used the moderator/mediator distinction to approach this question. Moderators, such as severity of depression, severity of obesity, gender, socioeconomic status (SES), gene-by-environment interactions and childhood experiences, specify for whom and under what conditions effects of agents occur. Mediators, such as eating and physical activity, teasing, disordered eating and stress, identify why and how they exert these effects. Maj
