Advancing Diabetes Management and Glycemic Control While Exploring CagriSema's Impact on Obesity Management.

Diabetes is a complex metabolic disorder affecting over 37 million people in the United States. Without proper management, diabetes can lead to a myriad of complications, including cardiovascular disease, kidney failure, and vision loss. Obesity is a major contributor to type 2 diabetes, but genetic and physiological factors make weight loss difficult, necessitating medication management for both conditions. Government-approved weight loss medications, including glucagon-like peptide-1 agonists and amylin analogs, have proven to be effective for both conditions. However, intensive glycemic con

Practical Recommendations of the Obesity Management Task Force of the European Association for the Study of Obesity for the Post-Bariatric Surgery Medical Management.

Bariatric surgery is today the most effective long-term therapy for the management of patients with severe obesity, and its use is recommended by the relevant guidelines of the management of obesity in adults. Bariatric surgery is in general safe and effective, but it can cause new clinical problems and is associated with specific diagnostic, preventive and therapeutic needs. For clinicians, the acquisition of special knowledge and skills is required in order to deliver appropriate and effective care to the post-bariatric patient. In the present recommendations, the basic notions needed to pro

The assessment and management of obstructive sleep apnoea-hypopnoea syndrome and obesity hypoventilation syndrome in obesity.

Obesity is associated with respiratory dysfunction. It is a key risk and contributory factor in the sleep related breathing disorders, obstructive sleep apnoea/hypopnoea syndrome (OSAHS) and obesity hypoventilation syndrome (OHS). Weight management is an integral part of the management of these disorders, in addition to continuous positive airways pressure (CPAP) and non-invasive ventilation (NIV). Untreated, these conditions are associated with a high disease burden and as treatment is effective, early recognition and referral is critical. Best practice in on-going care is multidisciplinary.

[Construction of obesity self-management education and support framework].

Obesity is a chronic metabolic disease requiring long-term intervention and sustained behavioral management. Given the current paucity of systematic education and support standards in obesity care, this paper proposes the obesity self-management education and support (OSMES) framework, modeled on the mature theoretical and practical foundations of diabetes self-management education and support framework. Grounded in the L.I.F.E.care four-dimensional model: load management (L), integration fitness (I), form optimization (F), and equilibrium mindset (E). The framework establishes 6 core standard

Obesity and Skull Base Manifestations: Clinical Implications and Management.

Obesity has become an important modifier of skull base disease. Although obesity is discussed in the context of obstructive sleep apnea, anesthesia risk, and wound healing, its most important skull base manifestation is often mediated through chronically elevated intracranial pressure. Spontaneous cerebrospinal fluid (CSF) leaks and skull base encephaloceles are now widely recognized as acquired defects that commonly arise in overweight or obese patients with idiopathic intracranial hypertension (IIH). This relationship has direct implications for diagnosis, surgical planning, reconstruction,

Optimizing GLP-1 therapies for obesity and diabetes management.

BACKGROUND: Glucagon-like peptide-1 (GLP-1) therapies are highly effective for weight loss and metabolic improvement in obesity and type 2 diabetes management. However, their use poses clinical challenges, including loss of lean muscle mass and gastrointestinal side effects, both of which may affect adherence and long-term outcomes. METHODS: This commentary synthesizes current evidence and expert perspectives, drawing on presentations from the 42nd International Symposium on Diabetes and Nutrition by the Diabetes and Nutrition Study Group, to develop practical recommendations for integrating

Clinical management of major depressive disorder with comorbid obesity.

Obesity is one of the most prevalent somatic comorbidities in individuals with major depressive disorder and greatly affects the course and prognosis of that disorder. The bidirectional relationship between major depressive disorder and obesity often creates a feedback cycle that challenges both patients and health-care providers. Gaps in interdisciplinary collaboration and limitations in knowledge transfer hinder the effective management of this patient population. This narrative Review synthesises current evidence from obesity and major depressive disorder research, offering a comprehensive

Evaluation and Management of Obesity in Chronic Kidney Disease.

Obesity is arguably the premier global public health problem of our era. It is also an independent risk factor for the development and progression of chronic kidney disease (CKD) and adverse outcomes in persons with CKD. Fortunately, effective treatment options for obesity have proliferated in just the last few years. These newer therapies in conjunction with older treatments offer great potential in treating obesity and CKD, although many questions remain about their use, efficacy, and place within the overall therapeutic plan. This article will provide a discussion of the definition of obesi

Enhanced Transdermal Delivery of Liraglutide for Sustained Obesity Management.

Liraglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist, is recognized for its effectiveness in glycemic control and weight reduction. However, the pain associated with the traditional subcutaneous administration of liraglutide and needle phobia in some patients will seriously affect medication adherence. Transdermal delivery represents a promising alternative to subcutaneous injection for its convenience, noninvasiveness, and painlessness, and it can bypass the first-pass metabolism and multiple biological barriers of oral administration and be delivered in a controlled manner, result

Incretins for management of obesity and obstructive sleep apnea.

INTRODUCTION: Recent trials have demonstrated the efficacy of incretin-based pharmacotherapy in obstructive sleep apnea (OSA) in obese patients. In addition to improving sleep indices, these agents also led to sustained weight loss and improvements in cardiometabolic profile. Given cardiovascular mortality remains the most common cause of death among patients with OSA, these agents represent a potential paradigm shift toward more holist management approach toward patients with OSA. AREAS COVERED: This article provides an overview of OSA pathophysiology, in particular, its overlapping risk fac

Sarcopenic obesity: pathogenesis, epidemiology and management in older adults.

INTRODUCTION: Sarcopenic obesity is the confluence of low skeletal muscle mass and function with excess adiposity. Sarcopenic obesity is becoming increasingly prevalent among older adults and may contribute to greater risk of functional decline, falls, fractures and mortality than sarcopenia or obesity alone. AREAS COVERED: This narrative expert review, based on targeted literature searches and the authors' personal libraries, outlines the current understanding of sarcopenic obesity, including its multifactorial pathophysiology. We also describe the current operational definition and estimate

A Review of Plant-Based Diets for Obesity Management.

OBJECTIVE: Obesity is the most prevalent chronic disease in the United States with over 70% of the American population suffering from overweight/obesity. Recently, the popularity of plant-based diets (PBDs) has grown, with individuals adopting these diets for ethical, health and environmental reasons. Our aim is to evaluate the effect of a PBD on weight loss among patients who are overweight or obese. METHODS: A literature review of PBDs for the treatment of obesity was conducted using PubMed and Scopus. Our search yielded 27 intervention trials (3361 participants) and 6 metanalyses (9168 par

Management of Hyperphagia and Obesity in Prader-Willi Syndrome.

Prader-Willi syndrome (PWS) is a neurodevelopmental disorder caused by the absence of paternally expressed imprinted genes on chromosome 15q11-13. Individuals with PWS typically experience feeding difficulties and a lack of appetite in infancy, followed by weight gain, uncontrolled appetite, and a lack of satiety. Hyperphagia in PWS is exacerbated by impaired satiety, low energy expenditure, and intellectual difficulties, including obsessive-compulsive disorder and/or autistic behaviors. Without rigorous external management of their eating behaviors, patients with PWS become severely obese and

Management of Obesity in Cardiovascular Practice: JACC Focus Seminar.

Obesity contributes to reduced life expectancy because of its link with type 2 diabetes and cardiovascular disease. Yet, targeting this poorly diagnosed, ill-defined, and underaddressed modifiable risk factor remains a challenge. In this review, we emphasize that the tendency among health care professionals to amalgam all forms of obesity altogether as a single entity may contribute to such difficulties and discrepancies. Obesity is a heterogeneous condition both in terms of causes and health consequences. Attention should be given to 2 prevalent subgroups of individuals: 1) patients who are o

Management of obesity in adolescents with polycystic ovary syndrome.

Introduction: Approximately 1% of adolescents have polycystic ovary syndrome (PCOS) and almost 40-70% of these patients are overweight or obese. Obese adolescents with PCOS have more severe insulin resistance and hyperandrogenemia, a more adverse lipid profile and a worse quality of life than normal-weight adolescents with PCOS. Accordingly, weight loss is an important component of the management of these patients.Areas covered: The authors discuss the different options for weight loss in obese adolescents with PCOS. Lifestyle changes appear to be effective but adherence to this intervention i

Obesity Treatment: A Focus on Pharmacotherapy of Weight Management.

Obesity, a chronic multifactorial disease, has been on the rise in the United States in recent years. It paves a way to other chronic conditions and related morbidity and mortality. The treatment of obesity should have a chronic approach involving lifestyle modifications from the very beginning. Along with reduced calorie diet, increased physical activity, and behavior modifications, various short- and long-term pharmacological agents are available to help with the weight loss. For qualifying patients, selection of an appropriate agent based on its mechanism, efficacy, and safety profile as we

Avoiding Management Errors in Patients with Obesity Hypoventilation Syndrome.

The prevalence of obesity hypoventilation syndrome and obstructive sleep apnea are increasing rapidly in the United States in parallel with the obesity epidemic. As the pathogenesis of this chronic illness is better understood, effective evidence-based therapies are being deployed to reduce morbidity and mortality. Nevertheless, patients with obesity hypoventilation still fall prey to at least four avoidable types of therapeutic errors, especially at the time of hospitalization for respiratory or cardiovascular decompensation: (1) patients with obesity hypoventilation syndrome may develop acut
