PURPOSE: To investigate whether the obesity survival paradox, where obesity is associated with improved survival, exists for pneumonia supported with extracorporeal membrane oxygenation (ECMO).
MATERIALS AND METHODS: Between January 2014 and December 2015, 223 patients with acute respiratory failure who underwent ECMO in 11 hospitals in South Korea were enrolled retrospectively, and data relating to pneumonia cases were analyzed. Patients were divided into groups according to their pre-treatment body mass index (BMI): obese (BMI ≥ 25 kg/m2) and non-obese (BMI < 25 kg/m2). The BMI cut-off was

An inverse relationship between overweight and mortality (the "obesity paradox") is well documented, but there are scarce data on how body weight during the life course affects this relationship. In the Helsinki Businessmen Study, we examined the effect of weight trajectories on incident disability, frailty, and mortality by stratifying 1,114 men (mean age of 47 years in 1974) into the following 4 groups based on body mass index (weight (kg)/height (m)(2)) values measured twice, in 1974 and 2000: 1) constantly normal weight (n = 340, reference group); 2) constantly overweight (n = 495); 3) wei

Overweight and obesity in children and adults have significantly risen in the US and worldwide due to biological, environmental, and cultural drivers and account for about 2.1 billion people. In addition, obesity, even metabolically healthy, is a major risk factor for the metabolic syndrome, diabetes mellitus, dyslipidemia, and hypertension, all significant causes of cardiovascular disease (CVD), coronary heart disease (CHD) heart failure (HF) and stroke. However, despite these causative effects, overweight and obesity frequently, confer protection in patients with established CVD, CHD, HF, an

INTRODUCTION: Sjögren's disease is characterized by autoimmune destruction of exocrine glands. Obesity generally worsens the course of many diseases; however, earlier observations in our cohort suggested a paradoxically lower disease activity (ESSDAI) in overweight/obese patients. This cross-sectional study aimed to investigate potential mechanisms underlying these findings.
METHODS: Chemerin and oxidized low-density lipoprotein (oxLDL) levels were measured in a subset of regularly followed patients (n=75). Patients were stratified by body mass index (BMI): BMI <25 kg/m² ("non-obese", n=27) a

BACKGROUND: The global rise in obesity presents a complex challenge in trauma care, characterized by the "Obesity Paradox." This study aimed to evaluate the biomechanical protection hypothesis and its influence on initial injury severity and clinical outcome in severely injured patients.
METHODS: This retrospective study analyzed 1549 adult trauma patients (ISS≥9) divided into Non-Obese (NO, BMI<30) and Obese (OB, BMI≥30) cohorts. Primary endpoints included ISS, regional AIS scores, and mortality. Secondary endpoints included physiological burden (SAPS II) and resource utilization (LOS, ICU L

BACKGROUND AND PURPOSE: Stroke has been described as a COVID-19 complication. However, its occurrence rate, risk factors, and causal relationships are still not well established.
METHODS: We describe the characteristics of confirmed COVID-19-related strokes among all cases of COVID-19 hospitalized in our health network, from November 1, 2020 to April 30, 2021. Risk factor analysis has been conducted for ischemic stroke (IS), which represents 92% of all confirmed cases of Covid-19-related strokes, and a "causal attribution to infection" classification is provided.
RESULTS: In all, 62/4105 hos

The prognostic significance of obesity in sarcopenic adults is controversial. This systematic review and meta-analysis aimed to investigate the effect of additional obesity on health outcomes in sarcopenia. MEDLINE, EMBASE, Scopus and CENTRAL were systematically searched for studies to compare health outcomes of adults with sarcopenic obesity (SO) to those of sarcopenic non-obese (SNO) adults. We also considered the methods of assessing obesity. Of 15060 records screened, 65 papers were included (100612 participants). Older community-dwelling SO adults had 15% lower mortality risk than the SNO

OBJECTIVES: The obesity paradox, whereby obesity appears to confer protection against cancer-related mortality, remains controversial. This has not yet been evaluated in upper gastrointestinal cancers.
DESIGNS: We identified esophageal, cardia, and non-cardia gastric adenocarcinomas in the Veterans Health Administration between 2006-2016. Multivariable Cox proportional hazard models evaluate the impact of BMI at- and prior to- cancer diagnosis on mortality, adjusting for demographics, clinical characteristics, weight loss, and clinical stage (early: T1B/2N0; locally advanced: ≥T2N+).

While COVID-19 infection and mortality rates are soaring in Western countries, Southeast Asian countries have successfully avoided the second wave of the SARS-CoV-2 pandemic despite high population density. We provide a biochemical hypothesis for the connection between low COVID-19 incidence, mortality rates, and high visceral adiposity in Southeast Asian populations. The SARS-CoV-2 virus uses angiotensin-converting enzyme 2 (ACE2) as a gateway into the human body. Although the highest expression levels of ACE2 are found in people's visceral adipose tissue in Southeast Asia, this does not nece

The objective is to investigate preoperative body mass index (BMI) in patients receiving beyond total mesorectal excision (bTME) surgery. The primary end point is length of postoperative stay. Secondary end points are length of intensive care stay, postoperative morbidity and overall survival. BMI is the most commonly used anthropometric measurement of nutrition and studies have shown that overweight and obese patients can have improved surgical outcomes. Patients who underwent a bTME operation for locally advanced or recurrent rectal cancer were put into three BMI (kg/m2) groups of normal wei

BACKGROUND: Obesity with its worldwide growing prevalence is an established cardiovascular risk factor with increased morbidity and mortality. However, the phenomenon, that mild to moderate obesity seems to represent a protective effect on diseases has been termed the "obesity paradox".
METHODS: We retrospectively assessed 529 patients (72.6% male, mean age 59.7±12.7years) admitted with ST-elevation myocardial infarction (STEMI). The female and male study populations were separated into four body mass index (BMI) groups: ≤24.9kg/m(2), 25.0-29.9kg/m(2), 30.0-34.9kg/m(2) and ≥35.0kg/m(2). Blood

INTRODUCTION: Obesity has long been considered a risk factor for the development of various pathologies, yet evidence supporting increased risk of perioperative mortality in obese individuals developing postoperative complications is limited. Therefore, we sought to characterize the demographics of obese and nonobese individuals developing postoperative respiratory insufficiency (RI)/adult respiratory distress syndrome (ARDS) and to quantify the impact of obesity on in-hospital mortality among this patient population utilizing data collected for the Nationwide Inpatient Sample (NIS).

BACKGROUND: Higher body mass index (BMI) has been associated with better outcomes after transcatheter aortic valve replacement (TAVR), but whether this "obesity paradox" varies by age is unclear.
OBJECTIVES: The objective of the study was to determine whether age modifies the association between BMI and 1-year outcomes after TAVR.
METHODS: The authors performed a retrospective analysis of the Society of Thoracic Surgeons/American College of Cardiology Transcatheter Valve Therapy Registry, including patients who underwent TAVR between January 2021 and February 2023. Patients were categorized

Obesity represents a significant health burden in the United States and has a significant impact on individual health, increasing the likelihood of multiple comorbidities. However, the presence of an "obesity paradox", in which increasing body mass index (BMI) may be protective against morbidity and mortality in certain clinical settings, has been demonstrated in both trauma and emergency general surgery patients. The impact of obesity on outcomes in burn patients is mixed, and no study has evaluated the impact of obesity on burn outcomes specifically in patients requiring excision of burn wou

Heart failure with preserved ejection fraction (HFpEF) is increasingly recognized as an obesity-driven condition, with excess adiposity serving both as a risk factor and a defining comorbidity. Paradoxically, once HFpEF is established, higher body mass index (BMI) is often associated with improved survival, a phenomenon termed the "obesity paradox." Epidemiological studies, including analyses of the TOPCAT (Treatment of Preserved Cardiac Function Heart Failure With an Aldosterone Antagonist) trial and large registries, show that overweight and obese HFpEF patients experience lower all-cause mo
