[Reduction of vertical HIV transmission: a challenge for nursing care].

With the crescent rates of HIV infection within female population in reproductive age, we may observe a correspondent increase in congenital infections. Thus, the obstetric nurse must be updated to participate and to develop programs of pre-natal care for HIV pregnant women. The purpose of this study was to review the pre-natal follow-up of this population regarding nursing care. Authors did not intend to approach all aspects of this subject, especially considering that investigations extend the great improvements already achieved since the acknowledge of the disease.

The stress of the midwife: Experiences of advanced midwives working in obstetric emergency units in Johannesburg, South Africa.

Obstetric emergencies account for the majority of causes of maternal deaths. The major causes of maternal and neonatal deaths in obstetric emergencies include bleeding, pregnancy-induced hypertension, cord prolapse, shoulder dystocia, poor progress, placenta abruptio, placenta praevia and amniotic fluid embolism. These adverse labour and birth events cause emergency situations and trauma for the nursing staff involved. A qualitative, descriptive phenomenological research design was used to explore and describe the lived experiences of advanced midwives regarding the management of obstetric eme

Psychometric evaluation of a novel tool for assessing gestational diabetes and hypertension care: knowledge, attitudes, and practices of midwives and nurses.

While standardized assessment of knowledge, attitudes, and practices (KAP) related to gestational diabetes and hypertension is possible with a valid tool, existing research remains limited. This prospective validation study aimed to develop and validate a novel tool to assess the KAP of midwives and obstetric nurses. We included 125 midwives and obstetric nurses who routinely care for patients with gestational diabetes and hypertension. The tool demonstrated good internal consistency (Cronbach's alpha): knowledge (0.729, 95% CI, 0.654-0.776), attitude (0.756, 95% CI, 0.690-0.814), and practice

Identifying and applying a targeted evidence-based practice change in the maternal/child health inpatient setting.

As the literature on implementation research continues to expand, it's important to address the unique aspects and challenges of research dissemination and implementation in maternal/child health settings. This article describes a nurse-initiated evidence-based practice (EBP) study on urinary catheterization at a labor and delivery unit in the southwestern United States. It describes successful strategies for implementing EBP, as well as barriers encountered, and provides recommendations for the facilitation of EBP in the maternal/child inpatient setting.

Obstetric Paralysis: Who is to blame? A systematic literature review.

Obstetric palsy is classically defined as the brachial plexus injury due to shoulder dystocia or to maneuvers performed on difficult childbirths. In the last 2 decades, several studies have shown that half of the cases of Obstetric palsy are not associated with shoulder dystocia and have raised other possible etiologies for Obstetric palsy. The purpose of the present study is to collect data from literature reviews, classic articles, sentries, and evidence-based medicine to better understand the events involved in the occurrence of Obstetric palsy. A literature review was conducted in the sear

Facilitating Culturally Safe Conversations Around Substance Use Disorder and Contraception to Provide Inclusive Care for Neurodiverse and Neurotypical Populations.

Nurses are vital to facilitating patient-centered discussions on alcohol use, substance use, and contraception, yet they face significant barriers to effective communication, including time constraints, stigma, and implicit bias. This article presents an inclusive framework to support culturally safe, patient-centered dialogue, with attention to neurodiverse populations, including those with fetal alcohol spectrum disorders. Drawing on the Model of the Five Principles of Cultural Safety and the Respectful Maternity Care Framework and Evidence-Based Clinical Practice Guideline from the Associat

Women's evaluation of the labor and delivery experience.

The purpose of this study was to identify how women described and evaluated their labor and delivery experience and what factors were related to their responses. Sixty Lamaze-prepared, married multigravidae, aged 21 to 37 years, participated in this qualitative field study. Detailed, open-ended tape-recorded interviews were conducted on the postpartum unit of a community hospital or in the women's homes early during the postpartum period. Women evaluated their labor and delivery experience according to how well they perceived they had managed their own childbirth performance. Women who managed

"I gotta push. Please let me push!" Social interactions during the change from first to second stage labor.

BACKGROUND: Forms of social interaction may occur among the participants in medicalized births, in which a woman in labor is experiencing strong involuntary urges to push but has not yet been found to have a completely dilated cervix. This article examines the social events and communications that occur at the change between first and second stages of labor. METHOD: Three cases are described from videotapes of women in the second stage of labor and their caregivers. RESULTS: Several social and interactive features occurred, in which (1) the caregiver, usually a nurse, by invoking the "no pus

Coordinating Pelvic Floor Physical Therapy Care Across the Perinatal Period.

Perineal injury during childbirth can contribute to maternal morbidity through pain, impaired bowel control, and limitations in activities of daily living. Nurses can help patients access pelvic floor physical therapy during pregnancy and postpartum recovery. In this column, I examine evidence on pelvic floor interventions for birth preparation and rehabilitation after perineal injury. I describe opportunities for nurses to support pelvic floor care through patient education, assessment of barriers to care, and care coordination with physical therapists and obstetric clinicians. I also discuss

Case Report of Piperacillin-Induced Toxic Epidermal Necrolysis During Pregnancy.

Toxic epidermal necrolysis (TEN) is a rare condition in the general population, and its occurrence during pregnancy is even more uncommon. Toxic epidermal necrolysis is associated with a high mortality rate that necessitates prompt referral to a specialized burn center upon diagnosis to optimize patient outcomes. In the case presented, the patient was treated in the obstetrics department, where nursing staff may have had limited experience in managing TEN. Currently, no established guidelines are available for the nursing care of patients with TEN during pregnancy. In this case report, we deta
