[The nervous system and uveitis].

This paper presents a short review of some inflammatory diseases which involve both the nervous system and the uvea, and of some forms of uveitis whose onset could be neurogenic. Multiple sclerosis and reticulum cell sarcoma are examples of systemic neurologic diseases which can be accompanied by uveitis or pseudo-uveitis. Syndromes such as Behçet's or Vogt-Koyanagi-Harada are characterized not only by a highly severe panuveitis but also by the participation of optic nerve, meningeal, and brain tissues in the inflammatory process. The occurrence of sympathetic ophthalmia and herpetic uveitis c

Uveitis in childhood--Part III. Posterior.

Toxoplasma, nematodes, and cytomegalovirus are the most common causes of uveitis in children involving the posterior pole. Discussion of treatment and management is detailed. Rubella retinitis and subacute sclerosing panencephalitis are specific entities which are limited in their initial presentation in children. Panuveitis including sympathetic ophthalmia, Vogt-Koyanagi-Harada, and Behçet's syndromes, while relatively rare in their occurrence, demand early treatment intervention. Specific antimicrobial therapy and adrenocorticosteroids as well as therapeutic suggestions are also included. Th

Non-Infectious Choroiditis Subdivided into Its Diverse Pathophysiological Sub-Groups Is the Best-Known and Most Appropriate Nomenclature to Date for the Reclassification and Diagnosis of Former White Dot Entities.

White dot syndromes (WDS), a purely descriptive term formulated in 1995, grouped inflammatory conditions primarily affecting the choroid that shared the clinical appearance of white dots on fundus examination, thus presenting similar white dots on fundus examination, which, however, had diverse pathophysiologies and different primary localizations of inflammation that did not warrant their association. Some of them, including birdshot retinochoroiditis (BRC) and sympathetic ophthalmia, produced predominant inflammation in the choroidal stroma, while others, including multiple evanescent white

Immunopathology of the noninfectious posterior and intermediate uveitides.

The posterior and intermediate uveitides share an underlying immune etiology; however, they can be clinically and immunopathologically distinguished. Although the initiating stimuli for posterior and intermediate uveities are not known, it is believed that an exogenous agent (such as a bacterium or a virus) or an endogenous molecule may induce disease. In either case, T-helper lymphocytes in conjunction with human leukocyte antigens are likely to be involved. This review examines the epidemiology, histology, immunopathology, and theories of pathogenesis of several posterior and intermediate uv

Metastatic Serratia marcescens endophthalmitis.

Metastatic endophthalmitis in a ten-day-old baby who underwent colostomy for an unperforated anus and developed septicemia to Serratia marcescens is described. Although rapid isolation of the agent enabled efficacious specific antibiotic treatment and systemic eradication of the infective agent, the ocular condition continued to deteriorate. Because of the total loss of visual functions and the fear of possible development of sympathetic ophthalmia, enucleation rather than vitrectomy or evisceration was performed. Histopathologic examination of the globe revealed massive infiltrates within the

Unusual presentation of CREST retinal vasculitis.

PURPOSE: To report a case of CREST (calcinosis, Raynaud's phenomenon, esophageal dysmotility, sclerodactyly, telangiectasias) syndrome-induced retinal vasculitis in the setting of ocular inflammation soon after recent micropulse cyclophotocoagulation (mTS-CPC). OBSERVATIONS: Our patient developed CREST associated retinal vasculitis in both eyes (right > left) eight days after receiving mTS-CPC in her left eye. There was initial concern for sympathetic ophthalmia due to the resulting bilateral inflammation. The patient was treated with prednisone with resolution of her symptoms. CONCLUSIONS:

Pars plana lensectomy in cases of cataract with juvenile chronic uveitis.

BACKGROUND: Classical cataract extraction in young patients with secondary cataract following juvenile chronic uveitis often is complicated by serious problems such as severe postoperative uveitis, hypotonia oculi and phthisis bulbi. Lensectomy with partial anterior vitrectomy might be a less inflammatory way of handling these cases. METHODS: Over a 15-year period we have treated 10 eyes of 9 young patients (age from 8 to 30 years) with secondary cataract after chronic uveitis by pars plana lensectomy with partial anterior vitrectomy. A classical two-port technique was used with incisions at

T cell immune responses against melanoma and melanocytes in cancer and autoimmunity.

T cell responses specific for melanoma cells and melanocytes appear to be involved in the rejection of melanoma tumors, as well as in the development of autoimmune reactions in patients with Vogt-Koyanagi-Harada disease (VKH), sympathetic ophthalmia, or autoimmune vitiligo. Some of the target antigens for those T cells have been isolated using cDNA expression cloning with melanoma reactive T cells derived from lymphocytes tumor infiltrating (TIL) of patients with melanoma. These include melanocyte specific proteins, such as tyrosinase, TRP1, TRP2, gp100, and MART-1, cancer-testis antigens, and

Microbial keratitis-induced endophthalmitis: incidence, symptoms, therapy, visual prognosis and outcomes.

BACKGROUND: To evaluate symptoms, therapies and outcomes in rare microbial keratitis-induced endophthalmitis. METHODS: Retrospective study with 11 patients treated between 2009 and 2014. Clinical findings, corneal diseases, history of steroids and trauma, use of contact lenses, number and type of surgical interventions, determination of causative organisms and visual acuity (VA) were evaluated. RESULTS: The incidence of transformation from microbial keratitis to an endophthalmitis was 0.29% (n = 11/3773). In 90.9% (n = 10/11), there were pre-existent eyelid and corneal problems, in 45.5% (n

Tyrosinase family proteins are antigens specific to Vogt-Koyanagi-Harada disease.

Vogt-Koyanagi-Harada (VKH) disease (and sympathetic ophthalmia) is an ocular inflammatory disease that is considered to be a cell-mediated autoimmune disease against melanocytes. The purpose of this study was to determine the Ags specific to VKH disease and to develop an animal model of VKH disease. We found that exposure of lymphocytes from patients with VKH disease to peptides (30-mer) derived from the tyrosinase family proteins led to significant proliferation of the lymphocytes. Immunization of these peptides into pigmented rats induced ocular and extraocular changes that highly resembled

Sympathetic ophthalmitis with predominant posterior segment involvement.

We report a case of a 22-year-old obese man, intellectually disabled since birth, who developed sympathetic ophthalmitis in his right eye (RE) 2 months after a penetrating injury to his left eye; this despite primary repair went into phthisis with no perception of light. Initially the patient presented with features of anterior uveitis in the RE, for which oral and topical steroids were prescribed. The patient failed to keep regular follow-up appointments, and returned 2 months later with exudative retinal detachment, and was placed on steroids and immunosuppressives. The visual acuity of the

Noninfectious immune-mediated uveitis and ocular inflammation.

Noninfectious uveitis encompasses a diverse group of ocular inflammatory disorders that share an underlying immune etiology and may be associated with systemic disease or confined primarily to the eye. Uveitis is commonly classified by anatomical location of inflammation into anterior, intermediate, posterior, and panuveitis. The treatment of noninfectious uveitis consists of corticosteroids, immunosuppressive agents, and surgically placed steroid implants. We review the epidemiology, immunopathology, and clinical features of several noninfectious immune-mediated uveitides, including HLA-B27 a
