[Oculoplastics in Cameroon].

Ophthalmological care in Cameroon is still being developed, although surgery for cataract and glaucoma is well established in several centres across the country. Our objective was to train the local doctors of the Presbyterian Eye Hospital (Acha) in Bafoussam and Douala to perform the most common oculoplasty procedures. The German Ophthalmological Society funded the project with a short-term lectureship of 2 weeks. We now present the results of the first two visits to Acha. We saw 96 oculoplastic patients and performed 44 surgeries. Most of the cases were nasolacrimal duct obstructions, cicatr

Management of graves myopathy: Orbital imaging in thyroid-related orbitopathy.

A broad understanding of the different imaging modalities used to assess the physiologic changes seen in Graves' orbitopathy complement clinical examination. Subtle applications of radiographic imaging techniques allow for a better understanding of the overall physiology of the orbit, quantify progression of disease, and differentiate it from orbital diseases with overlapping features. A nuanced approach to interpreting imaging features may allow us to delineate inactive from active thyroid eye disease, and advances within this field may arm clinicians with the ability to better predict and pr

Professor Sergei Semjonovic Golovin (1866-1931): A Pioneer of Ocular Surgery.

Professor Sergei Semjonovic Golovin (1866-1931) is considered as one of the founders of ophthalmology in Russia. He received a worldwide reputation thanks to his achievements in ocular surgery and pathology. He introduced new surgical techniques such as Golovin's operation (Exenteratio orbitosinualis), Golovin's osteoplastic frontal sinus operation, ligation of orbital veins, and opticociliary neurectomy. He also introduced his "cytotoxic theory" to interpret sympathetic ophthalmia. He was a reputable professor of ophthalmology.

Vincenz Fukala (1847-1911): versatile surgeon and early historian of ophthalmology.

Vincenz Fukala is best known for his technique of discission and linear extraction of the lens in young, highly myopic eyes--the technique adopted by Harold Ridley in preparation for lens implantation in young patients. He was Polish and trained in Vienna under Karl Ferdinand. von Arlt. Fukala practiced first in Pilsen (1889-1894), but returned to Vienna in 1895. He studied trachoma in Egypt in 1872. His research on scrofulous eye disease led him twice to the United States, in 1891 and 1892. Fukala also developed highly successful, pioneering techniques for the treatment of ectropion in blepha

Sympathetic ophthalmia.

Sympathetic ophthalmia is a rare, bilateral granulomatous uveitis that occurs after either surgical or accidental trauma to one eye. The ocular inflammation in the fellow eye becomes apparent usually within 3 months after injury. Clinical presentation is an insidious or acute anterior uveitis with mutton-fat keratic precipitates. The posterior segment manifests moderate to severe vitritis, usually accompanied by multiple yellowish-white choroidal lesions. Evidence suggests that sympathetic ophthalmia represents an autoimmune inflammatory response against choroidal melanocytes mediated by T cel

Advances in the use of adhesives in ophthalmology.

PURPOSE OF REVIEW: To summarize the known uses of available medical tissue adhesives in the management of diseases of the anterior segment, highlighting recent developments in the field. RECENT FINDINGS: Human fibrin glues may be used in place of cyanoacrylate tissue adhesives in the treatment of progressive corneal thinning and small perforations, potentially resulting in less corneal and conjunctival inflammatory reaction. Additional currently proposed uses of fibrin glues in ophthalmic surgery include minimizing sutures in recurrent pterygium surgery, forniceal reconstruction, amniotic mem

Aberrometry: basic science and clinical applications.

This paper addresses the concept, measurement and applications of wave aberrations. The ocular optics is not perfect. Apart from conventional low order aberrations (such as defocus and astigmatism) retinal images are degraded by other high order aberrations. Aberrometers typically measure ray deviations at the retinal plane, as a function of pupil position, i.e. local derivatives of the wave aberration. The paper discusses state of the art technology to measure the aberrations of the eye (cornea and crystalline lens), both monochromatic and polychromatic. The aberrometers described include the

Surgical Safety Checklist in ophthalmology: A tertiary hospital proposal.

Adverse events due to unsafe healthcare practices constitute a serious global problem. The World Health Organization (WHO) has recognized healthcare insecurity as a public health issue and has undertaken various initiatives to safeguard patient safety. The Surgical Safety Checklist (SSC) is one of the tools implemented that has proven to be most effective in reducing morbidity and mortality associated with surgeries. Its impact is tied to implementation policies. Although the SSC is universal, the WHO has encouraged adaptations for different specialties. The purpose of our work is to present t

Impact of Ophthalmic Knowledge Assessment Program Scores and Surgical Volume on Subspecialty Fellowship Application in Ophthalmology Residency: Retrospective Cohort Study.

BACKGROUND: Ophthalmology residents take the Ophthalmic Knowledge Assessment Program (OKAP) exam annually, which provides percentile rank for multiple categories and the total score. In addition, ophthalmology residency training programs have multiple subspecialty rotations with defined minimum procedure requirements. However, residents' surgical volumes vary, with some residents exceeding their peers in specific subspecialty rotations. OBJECTIVE: This study aims to identify if there is a difference in OKAP examination scores and surgical volume exposure during ophthalmology residency trainin

Visual improvement after optic nerve sheath decompression in a case of congenital hydrocephalus and persistent visual loss despite intracranial pressure correction via shunting.

Among the sequelae of persistent raised intracranial pressure (ICP) are ophthalmologic signs and symptoms, including cranial nerve palsies, visual field deficits, papilledema, and vision loss. Elevated pressure within the optic nerve sheath may not be relieved by shunt procedures, which can decrease generalized ICP. The authors present a case of acute visual loss in the setting of chronic hydrocephalus and multiple shunt revisions. Despite shunt correction resolving systemic symptoms of raised ICP, this child had persistent visual loss. Bilateral optic nerve sheath decompression was performed,

Minimally invasive knotless iridodialysis repair (MIKIR)- A review article.

Here, we review and summarize the evolution of surgical techniques and outcomes of minimally invasive, knotless (flanged) approaches for the repair of iridodialysis. A thorough literature search was performed using PubMed, Medline, Cochrane Library, and Embase databases to identify studies related to iridodialysis repair. Iridodialysis repair, flanged polypropylene suture, knotless iridodialysis repair, and ab-interno and ab-externo procedures were the keywords used. Their bibliographies provided further references. Articles on surgical repair of iridodialysis that were published before April

[Modern, minimally invasive surgery of the lacrimal duct system].

Epiphora (tearing) is the main symptom of a dysfunction of the lacrimal drainage. Tearing is a common problem and the cause can not easily be identified. Since development and miniaturisation of gastroduodenal endoscopes in the 1990s, it is possible to check the anatomical structure and the pathological changes of the lacrimal drainage system, which has a diameter of 1 mm. Mechanical stenosis is often caused by recurring chronic inflammation. Complete stenosis is usually only punctual and seldom over a long distance. Shortly after the development of microendoscopic diagnostic tools, therapeuti

Respiratory arrest after local anesthesia for outpatient cataract surgery: a dramatic but transient complication.

Retrobulbar injection of anesthetic agents is common practice before ophthalmologic procedures. Rare but life-threatening complications of this procedure include respiratory depression and coma. We present the case of a woman with complete respiratory arrest and unresponsiveness who was brought into our emergency department from a local freestanding surgical center after preoperative retrobulbar anesthetic injection. Subsequently, the pathogenesis, clinical features, and supportive treatment for this dramatic but transient complication are discussed.
