PARACENTRAL ACUTE MIDDLE MACULOPATHY ASSOCIATED WITH BILATERAL OPTIC DISK SWELLING AND MENINGITIS.

BACKGROUND/PURPOSE: To present a novel case of paracentral acute middle maculopathy associated with bilateral optic disk swelling. METHODS: Retrospective case report. RESULTS: A 67-year-old woman presented with sudden onset of central vision loss and subsequent bilateral optic disk edema, retinal vessel attenuation, and anterior uveitis. Cerebrospinal fluid analysis showed signs of inflammation. Spectral domain optical coherence tomography demonstrated the pathognomonic hyperreflectivity of the middle retinal layers consistent with paracentral acute middle maculopathy. Swept-source optical c

Corneal enlargement without optic disk cupping in children with recessive CYP1B1 mutations.

Corneal enlargement during the first 3 years of life can be a sign of early childhood glaucoma and optic nerve head cupping is a useful confirmatory finding. We report 3 children with corneal enlargement without optic nerve head cupping who had recessive CYP1B1 mutations, the most common identifiable cause of primary congenital glaucoma. One child later developed unilateral Haab striae, still in the absence of optic disk cupping. These cases illustrate that CYP1B1-related corneal changes can occur in young children without visible optic nerve head damage.

An atypical case of optic disk drusen with nerve fiber layer thickening.

We report an atypical case of optic disk drusen (ODD). A 12-year-old girl complained of blurred vision in both eyes. However, visual acuities in both eyes were 20/20. Fundus examination revealed blurring of the optic disk margins and elevation of the optic disk in both eyes. Initially it was considered that the diagnosis would be papilledema. However, neurologic examination revealed no pathology which would cause the visual field defect and disk elevation. Optical coherence tomography (OCT) scans showed hyperreflectance lesions on the disk and shadowing areas beneath the optic nerve head in bo

The macular buckling technique in the treatment of optic disk pit maculopathy.

In 1988 we introduced the macular buckling procedure for the treatment of optic disk pit maculopathy and we have been applying it since then. The surgery consists of a scleral sponge fixation of 7.5 x 5.5 mm at the posterior pole of the globe corresponding to the macula along the vertical axis of the 12 to 6 o'clock meridian. No additional treatment of any kind (laser, diathermy, or cryotherapy) is used. The correct positioning of the sponge during the surgery is monitored by B-scan ultrasonography. Within the first week after surgery, indocyanine green angiography (ICG) is used to evaluate th

Treatment of serous retinal detachments associated with optic disk pits in dogs.

Serous retinal detachment, associated with optic disk pit, was diagnosed in 28 eyes of 24 dogs. Xenon arc photocoagulation was used in treatment of the detachment. Of 24 dogs, 21 were Collies. In 23 eyes, retinal detachments affected temporal and/or inferior portions of the retina. In 5 eyes, detachments were predominantly superior and/or nasal. A single photocoagulation treatment resulted in reattachments in 25 eyes. Of the 3 remaining detachments, 2 eyes improved with additional photocoagulation, and 1 eye, which was not treated further, had a complete retinal detachment.

An ultrastructural study of melanocytomas (magnocellular nevi) of the optic disk and uvea.

We examined by light and electron microscopy five melanocytomas from four patients. Two types of cells were observed in each tumor. The predominant cell type in most of the tumors studied consisted of plump polyhedral nevus cells that contained numerous giant melanosomes. These cells showed advanced differentiation. They appeared to be metabolically inactive and to have been the cause of the heavy pigmentation and benign nature of these tumors. The second variant of melanocytoma cells were smaller spindle-shaped cells that were lightly pigmented. Other morphologic features of these cells such

The central vascular pattern of the eyeground in children with drusen of the optic disk.

In a series of 50 children with optic disk drusen the anomaly was found to be associated with unusual properties. A cilioretinal artery was more frequent in the series with drusen than in a control series of the same age. The emergence and course of the central retinal vessels were studied in the right-sided fundi of 46 of the children and in two control series (the fundi of the "better" eyes of 46 strabismic children and the fundi of one of the eyes of 10 children or young adults with papilledema). Analysis of the measurements indicated that early branching of the central vessels and vascular

AN ATYPICAL CASE OF OPTIC DISK PSEUDO-DUPLICATION WITH PROLIFERATIVE DIABETIC RETINOPATHY.

PURPOSE: To present a unique case of optic disk pseudo-duplication with proliferative diabetic retinopathy. METHODS: Case report. RESULTS: A 63-year-old white diabetic man presented with an apparent duplicated optic disk in the superonasal midperiphery of his left eye. A large flat frond of neovascularization fanned out from this structure. Optical coherence tomography scanning showed a noncolobomatous scar with a large plume of blood vessels sprouting from the choroid, thru the retina and branching out into the vitreous. Magnetic resonance imaging scanning revealed a normal left globe and o

Multimodal imaging assessment of a "micro" optic disk melanocytoma: A case report.

An optic disk melanocytoma is a benign pigmented neoplasm of the optic disk measuring 2.00 mm of diameter in average. Smaller optic disk melanocytomas are rare and have not been studied with multiple imaging studies. A case of a "micro" optic disk melanocytoma measuring 0.71 mm in diameter was studied by multiple imaging modalities, including fundus autofluorescence, ultrasound, visual fields, optical coherence tomography (OCT) and OCT angiography. While clinical appreciation and fundus autofluorescence reveal changes compatible with previously reported cases, OCT, OCT angiography and ultrasou

Optic disk morphology in experimental central retinal artery occlusion in rhesus monkeys.

PURPOSE: To evaluate longitudinally the optic disk morphology of nonglaucomatous optic nerve damage secondary to retinal nerve fiber damage, using experimental central retinal artery occlusion in rhesus monkey eyes as a model. METHODS: This prospective study included 24 eyes of 16 monkeys. In eight eyes of eight animals, central retinal artery occlusion was produced by clamping the central retinal artery in the retrobulbar space. Occlusion was verified by fluorescein fundus angiography. The same eyes at baseline as well as the eight contralateral healthy eyes and eight monkey eyes with experi

[Follow-up of vitrectomy for central retinal detachment and optic disk pit].

BACKGROUND: The exact pathogenesis of central retinal detachment associated with optic disc pit is still unknown. Vitrectomy has proven to be the most effective therapy. PATIENTS AND METHODS: In two patients with macular detachment associated with optic disc pit, a pars-plana-vitrectomy with surgical posterior vitreous detachment and gastamponade was performed. We report on the interesting clinical course of these patients. RESULTS: After the gas bubble was absorbed, we observed a displacement of the subretinal fluid inferiorly in both patients. Schisislike changes persisted in the macular a

Pressure-dependent changes of the optic disk in primary open-angle glaucoma.

PURPOSE: This study was performed to differentiate glaucomatous changes of the optic disk that may be caused by increased intraocular pressure from those that may be pressure independent. METHODS: For 357 patients with primary open-angle glaucoma, the optic disk was morphometrically analyzed, and the intraocular pressure was measured in 24-hour profiles. RESULTS: By performing an intraindividual bilateral comparison, the differences in the maximal intraocular pressure values of the right eye minus the left eye of the same individual were significantly correlated with the differences in neuro

[Para-papillary chorioretinal atrophy in eyes with shallow glaucomatous optic disk cupping].

Parapapillary chorioretinal atrophy is present in some, but not all glaucomatous eyes. We examined whether eyes with shallow or with deep optic cupping showed more pronounced parapapillary atrophy. To this end, we analyzed 15 degree color stereo optic disc photographs of 141 eyes of 95 normal subjects and 211 eyes of 120 patients suffering from chronic open-angle glaucoma morphometrically. Optic cup depth was semiquantitatively estimated in a relative score ranging from 0 to 5. It increased significantly with increasing glaucoma damage indicated by smaller neuroretinal rim area, higher visual
