Insulin-loaded liposomes packaged in alginate hydrogels promote the oral bioavailability of insulin.

Compared to subcutaneous injections, oral administration of insulin would be a preferred route of drug administration for diabetic patients. For oral delivery, both liposomes and alginate hydrogels face many challenges, including early burst release of the encapsulated drug and poor intestinal drug absorption. Also, adhesion to the intestinal mucosa remains weak, which all result in a low bioavailability of the payload. This study reports on an alginate hydrogel loaded with liposomes for oral insulin administration. Liposomes (Lip) loaded with arginine-insulin complexes (AINS) were incorporate

Gut immunology in fish: a review.

During recent years considerable study has been made of the systemic immune system of fish, although less attention has been paid to local immunity at sites such as the gut, reproductive tract and skin. Recent work on the structure of gut-associated lymphoid tissue and other intestinal cell populations, the absorption of macromolecules from the intestinal lumen and the production of specific mucosal antibodies strongly suggests, however, that the gut of these lower vertebrates is immunocompetent. This review briefly deals with current concepts on the nature of gut-associated lymphoid tissues a

Further on the causation of oral submucous fibrosis.

Oral submucous fibrosis is perhaps a disease of multifactorial aetiology, of which areca nut usage is definitely a causative factor. We disagree with the previous reports in that the action of areca quid on the oral mucosa is not that simplistic and is not dictated solely by the duration of exposure to it or by simple process of passive diffusion. We noted that the role of copper cannot be segregated from that of zinc, the biochemical relatedness of these two elements is well elucidated. The transport of copper in to the oral epithelium may be dictated more by the composition of the quid rathe

Severe malnutrition associated with alpha-heavy chain disease: response to tetracycline and intensive nutritional support.

A 20-yr-old black male was admitted with a 5-month history of profound weight loss and diarrhea. Appetite and dietary intake had been remarkably well preserved up until the week before admission. The severity of his depletion was evidenced by a body weight of only 38% of standard, multiple electrolyte deficiencies, and reduced metabolic expenditure, protein turnover, protein synthesis, and pancreatic function. Immunological defects included diminished lymphocyte numbers, lymphocyte transformation, gamma-globulin concentration, and cell-mediated immunity. A diagnosis of alpha-heavy chain diseas

Molecular and functional studies of electrogenic Na(+) transport in the distal colon and rectum of young and elderly subjects.

BACKGROUND: Human distal nephron and distal colon both exhibit mineralocorticoid sensitive electrogenic Na(+) absorption and make significant contributions to Na(+) homeostasis. Na(+) resorption in the distal nephron diminishes with age but it is unclear whether a similar change occurs in the distal colon. AIMS: To evaluate the effect of age on expression of apical Na(+) channels and basolateral Na(+), K(+)-ATPase, and on the responsiveness of electrogenic Na(+) absorption to mineralocorticoid stimulation in human distal colon and rectum. MATERIALS AND METHODS: Mucosal biopsies were obtained

Intestinal calcium transport in mole-rats (Cryptomys damarensis and Heterocephalus glaber) is independent of both genomic and non-genomic vitamin D mediation.

The role of vitamin D in mineral uptake in the gastrointestinal tract (GIT) of mole-rats (Heterocephalus glaber and Cryptomys damarensis; family Bathyergidae), animals with a naturally impoverished vitamin D status, was investigated. We measured relative rates of passage of radioactive markers, mode of calcium (Ca) uptake, paracellular movement and the opening of voltage-sensitive Ca channels (VSCCs) along the GIT with and without oral vitamin D3 supplementation. The ratio of relative absorption of labelled 45Ca to [14C]polyethylene glycol ([14C]PEG) indicated that within 24 h more than 88% of

Cannabidiol Oil Ingested as Sublingual Drops or Within Gelatin Capsules Shows Similar Pharmacokinetic Profiles in Healthy Males.

Introduction: Cannabidiol (CBD) is a nonintoxicating phytocannabinoid used in clinical treatments and sold widely in consumer products. CBD products may be designed for sublingual or oral delivery, but it is unclear whether either is advantageous for CBD absorption. This study compared CBD pharmacokinetics after providing CBD oil as sublingual drops and within orally ingested gelatin capsules, at a dose relevant to consumer products. Materials and Methods: Eight males completed three conditions in a participant-blinded, randomized crossover design. Participants received the following combinati

An antioxidant metal-organic framework with functional coatings for oral anti-TNF-α antibody delivery in inflammatory bowel disease treatment.

Oral biomacromolecule delivery is considered a promising alternative for patients who must receive long-term invasive injections, providing better patient compliance. However, it still faces great challenges due to the harsh gastrointestinal environment, including instability and poor intestinal absorption. Here, we report an antioxidant moiety (manganese ion, Mn4+)-assembled metal-organic framework (MOF) presenting functionalized coatings consisting of an inner layer of hyaluronic acid-polyethylene glycol and an outer layer of Eudragit® S100 for enhanced colon-targeted delivery of an anti-mou

Mechanisms of action of Lactobacillus spp. in the treatment of oral candidiasis.

Candida albicans is often associated with oral candidiasis, and drug-resistance profiles have contributed to an increase in morbidity and mortality. It is known that Lactobacillus spp. acts by competing for adhesion to the epithelium, absorption of nutrients and modulation of the human microbiota. Therefore, they are important to assist in the host's microbiological balance and reduce the growth of Candida spp. Until now, there have been no reports in the literature of reviews correlating to the use of Lactobacillus spp. in the treatment of oral candidiasis. Thus, this review aims to highlight

Dynamic analysis of fluid distribution in the gastrointestinal tract in rats: positron emission tomography imaging after oral administration of nonabsorbable marker, [(18)F]Deoxyfluoropoly(ethylene glycol).

To develop potent drugs for oral use, information on their pharmacokinetic (PK) properties after oral administration is of great importance. We have recently reported the utility of positron emission tomography (PET) for the analysis of gastrointestinal (GI) absorption of radiolabeled compounds. In this study, PET image analysis was performed in rats using a novel PET probe, [(18)F]deoxyfluoropoly(ethylene glycol)s, with an average molecular weight of 2 kDa ([(18)F]FPEG), as a nonabsorbable marker to elaborate the GI physiology in more detail, such as segmental transition of the administered w

A dose-ranging pharmacokinetic study of nicotine tartrate following single-dose delayed-release oral and intravenous administration.

BACKGROUND: Ulcerative colitis is predominantly a disease of non-smokers, and transdermal nicotine is therapeutic but often results in side-effects. Administration of nicotine tartrate as a liquid enema decreases systemic nicotine absorption and may be effective for treatment of active distal ulcerative colitis. Ileocolonic delivery of nicotine tartrate via a delayed release oral capsule would be the preferred route to deliver nicotine to the colon. AIM: To determine the bioavailability and pharmacokinetic parameters of delayed-release oral nicotine tartrate capsules (Eudragit S100 coated) at

Background: Anemoside B4 (AB4), the major bioactive saponin from Pulsatilla chinensis, exhibits anti-inflammatory, anti-tumor, anti-apoptotic, and analgesic properties. However, its clinical translation for ulcerative colitis (UC) is constrained by poor epithelial permeability and low oral bioavailability. Objective: This study's objective was to engineer and optimize thermosensitive rectal in situ gels (ISGs) of AB4, incorporating suitable absorption enhancers to improve mucosal permeation, bioavailability, and therapeutic efficacy against UC. Methods: Screening of effective permeation enhanc

Development of a hydroxypropyl methyl cellulose/polyacrylic acid interpolymer complex formulated buccal mucosa adhesive film to facilitate the delivery of insulin for diabetes treatment.

Buccal mucosa administration is a promising method for insulin (INS) delivery with good compliance. However, buccal mucosa delivery systems still face challenges of long-term mucosal adhesion, sustained drug release, and mucosal drug penetration. To address these issues, a double-layer film consisting of a hydroxypropyl methylcellulose/polyacrylic acid interpolymer complex (IPC)-formulated mucoadhesive layer and an ethylcellulose (EC)-formulated waterproof backing layer (IPC/EC film) was designed. Protamine (PTM) and INS were co-loaded in the mucoadhesive layer of the IPC/EC film (PTM-INS-IPC/

Comparative Analysis of In Vitro Permeation and Human Pharmacokinetic Data for a Bioadhesive Vaginal Tablet as a Potential Clinical Approach for Bacterial Vaginosis.

Background/Objectives: Reformulating oral drugs for vaginal delivery is a current therapeutic strategy for genitourinary tract infections. Bioadhesive delivery systems, by interacting with the mucosal epithelium, improve drug deposition at the application site and are promising carriers for drugs with poor bioavailability. This study compares an in vitro permeation test (IVPT) in excised human vaginal epithelium and a first-in-human (FIH) pharmacokinetic (PK) study for a bioadhesive vaginal tablet developed as a potential clinical approach for bacterial vaginosis (BV). Methods: A Phase 1, sing

Comparison of a new colon lavage solution (Golytely-RSS) with a standard preparation for air-contrast barium enema.

Golytely is a balanced electrolyte solution for per oral whole-gut lavage. It has been an accepted alternative to standard preparations for barium enema. Golytely-RSS is a new formulation designed to taste better through reduction in sodium and sulfate content. In addition, a slight increase in osmolality is intended to decrease further the net absorption/secretion ratio. One hundred outpatients who were to undergo air-contrast barium enema examinations were randomly assigned one of two colon-cleansing preparations. A standard 1-day diet/cathartic method was compared with Golytely-RSS plus ora

HIF1α-Dependent Induction of TFRC by a Combination of Intestinal Inflammation and Systemic Iron Deficiency in Inflammatory Bowel Disease.

Background and Aims: Iron deficiency (ID) is a frequent extra-intestinal manifestation in patients with Inflammatory Bowel Disease (IBD), who often do not respond to iron supplementation. Iron is a cofactor for hydroxylases that suppress the hypoxia-inducible factor-1α (HIF1α), a transcription factor regulating iron homeostasis. We hypothesized that iron deficiency affects mucosal HIF1α activity in IBD. Methods: IBD patients (n = 101) were subdivided based on iron status (ferritin levels or transferrin saturation) and systemic inflammation (C-reactive protein levels). 154 corresponding ileal a

Troxipide nanosuspensions prevent indomethacin‑induced gastrointestinal lesions in adjuvant-induced arthritis rats.

BACKGROUND: Non-steroidal anti-inflammatory drugs (NSAIDs) are widely used for managing inflammatory disorders such as rheumatoid arthritis (RA); however, their long-term use is frequently associated with gastrointestinal (GI) complications, often leading to RA treatment discontinuation. To address this issue, we developed an oral formulation of troxipide (TRO), a pharmaceutical agent with gastroprotective properties, using nanosuspension technology to enhance its GI absorption and therapeutic efficacy. METHODS: The TRO nanosuspensions (TRO-NP@dis) were prepared by bead milling with methylcell

Unraveling Oral Fate of Poly(Lipoic Acid)s: Mechanistic Insights and Delivery Horizons.

Efficient oral delivery remains fundamentally constrained by harsh gastric milieu and intrinsic challenge of traversing multiple heterogeneous physiological barriers. Here, we elucidate that poly(lipoic acid)s (PLAs) holds great promise for oral delivery. Reversible thiol-disulfide exchange between PLAs and biological thiols at mucosal, epithelial and target cell interfaces drives traversal of each barrier, yielding 46% mucus permeability within 4 h, 26% epithelial transcytosis within 8 h, and high target-cell penetration, all markedly reduced upon thiol blockade. Acid-resistant yet neutral-pH

Orally disintegrating tablets containing famotidine nanoparticles provide high intestinal absorbability via the energy-dependent endocytosis pathway.

The permeability of the Biopharmaceutics Classification System (BCS) class III drugs are low, and their oral bioavailability needs to be improved. In this study, we attempted to design oral formulations containing famotidine (FAM) nanoparticles to overcome the limitations of BCS class III drugs. Dispersions containing FAM nanoparticles with a particle size of approximately 50-220 nm were produced by the bead-milling treatment. Moreover, we succeeded in preparing an orally disintegrating tablet containing FAM nanoparticles using the dispersions described above, additives (D-mannitol, polyvinylp
