Fluid overload is associated with impaired oxygenation and morbidity in critically ill children.

RATIONALE: Fluid overload is common in the critically ill and is thought to contribute to oxygenation failure and mortality. Since increasing disease severity often requires more fluid for resuscitation, it is unclear whether fluid overload is a causative factor in morbidity or is simply an indicator of disease severity. OBJECTIVE: Investigate the association between fluid overload and oxygenation while controlling for severity of illness by daily Pediatric Logistic Organ Dysfunction scores. DESIGN AND SETTING: Retrospective chart review, tertiary children's hospital. PATIENTS AND METHODS:

Interleukin-6 as a biomarker of sepsis and its clinical utility in predicting mortality: a prospective observational study.

INTRODUCTION: Majority of patients admitted to intensive care units (ICUs) succumb to sepsis and its complications. However, currently available predictors fail to reliably gauge the severity of organ damage. There is a pressing need to identify biomarkers that can accurately forecast outcomes. Interleukin-6 (IL-6) has emerged as a potential biomarker, with some studies suggesting its utility as an early predictor of multi-organ failure in sepsis. This study evaluated the role of IL-6 in predicting mortality in an Indian ICU setting. METHODOLOGY: This prospective observational study included

A prediction model for acute respiratory distress syndrome in immunocompetent adults with adenovirus-associated Pneumonia: a multicenter retrospective analysis.

BACKGROUND: In recent years, the number of human adenovirus (HAdV)-related pneumonia cases has increased in immunocompetent adults. Acute respiratory distress syndrome (ARDS) in these patients is the predominant cause of HADV-associated fatality rates. This study aimed to identify early risk factors to predict early HAdV-related ARDS. METHODS: Data from immunocompetent adults with HAdV pneumonia between June 2018 and May 2022 in ten tertiary general hospitals in central China was analyzed retrospectively. Patients were categorized into the ARDS group based on the Berlin definition. The predic

Accuracy of Rapid Emergency Medicine Score and Sequential Organ Failure Assessment Score in predicting acute paraphenylenediamine poisoning adverse outcomes.

Paraphenylenediamine (PPD) is a commonly used xenobiotic in hair dying, causing deleterious outcomes in acute poisoning. Although many epidemiological studies and case reports explained their clinical presentations and fatal consequences, no studies have evaluated the early determinants of adverse outcomes. Therefore, the present study aimed to assess the initial predictors of acute PPD poisoning adverse outcomes, focusing on the discriminatory accuracy of the Rapid Emergency Medicine Score (REMS) and Sequential Organ Failure Assessment (SOFA) score. A retrospective cohort study included all a

Primary biliary cirrhosis: proposal for a new simple histological scoring system.

BACKGROUND & AIMS: A simple and reproducible evaluation of non diagnostic histological lesions related to prognosis remains crucial in primary biliary cirrhosis (PBC). Presently there is no satisfactory simple scoring system analysing them reliably. We elaborated a semi-quantitative scoring system that assesses fibrosis, lymphocytic interface hepatitis (LIH) and ductopenia, separately. This study was aimed to evaluate its intra/interobserver reproducibility and its correlation with the main biochemical data. METHODS: Liver biopsies from 33 consecutive newly diagnosed PBC patients were indepen

A Novel Approach to Sequential Organ Failure Assessment (SOFA) Using Near-Infrared Spectroscopy in Extracorporeal Membrane Oxygenation (ECMO) Patients.

Extracorporeal membrane oxygenation (ECMO) is a medical device that provides temporary external circulation and respiratory support during heart-lung procedures, substituting for heart and lung function to alleviate their burden and allow more time for treatment. This study employs the sequential organ failure assessment (SOFA) to evaluate the severity of illness in ECMO patients and utilizes noninvasive near-infrared spectroscopy (NIRS) to monitor lower limb microcirculation. By extracting and selecting features, blood oxygen information is input into machine learning models for classificatio

Clinical Applications of Polymyxin B Hemadsorption in Sepsis and Septic Shock.

Sepsis and septic shock (SS) represent complex, life-threatening conditions driven by a dysregulated host immune response, leading to multi-organ failure (MOF). The SEPSIS-3 guidelines have emphasized the role of immunology in defining sepsis, but therapies targeting individual mediators have largely failed. Hemoadsorption (HA), particularly with polymyxin B (PMX), presents a promising approach to modulate this immune response by non-specifically removing endotoxins and other mediators, potentially restoring physiological homeostasis. This review explores the use of PMX hemoperfusion (PMX-HA)

Comparison of sequential organ failure assessment score and cardiac surgery score systems for mortality prediction after emergency acute aortic dissection surgery.

BACKGROUND: Acute type A aortic dissection (ATAAD) is one of the most mortal cardiovascular diseases and requires urgent diagnosis and surgery. The patient's clinical findings, complications, and patient's history are closely related to mortality rates. Cardiac surgery score (CASUS) is a scoring system which is calculated by considering the special pathophysiological conditions of patients undergoing cardiac surgery and predicts post-operative results with high accuracy. METHODS: Following the ethical approval from institutional ethics committee (ID: 2021/7/496), the data of consecutive 50 AT

Usefulness of complete blood count parameters to predict poor outcomes in cancer patients with febrile neutropenia presenting to the emergency department.

INTRODUCTION: Febrile neutropenia (FN) is one of the major complications with high mortality rates in cancer patients undergoing chemotherapy. The Multinational Association for Supportive Care in Cancer (MASCC) risk-index score has limited applicability for routine use in the emergency department (ED). This study aimed to develop simplified new nomograms that can predict 28-day mortality and the development of serious medical complications in patients with FN by using a combination of complete blood count (CBC) parameters with quick Sequential Organ Failure Assessment (qSOFA). METHODS: In thi
