Orthognathic surgery has varying effects on respiratory parameters. The authors undertook a prospective study of patients requiring mandibular advancement, mandibular setback and maxillary Le Fort I advancement, and surgically assisted rapid maxillary expansion (SARME). Breathing parameters were monitored in a sleep laboratory the night before the operation and in a mean of 9.5 months after the operation. In patients treated with mandibular advancement, the respiratory disturbance index (RDI), oxygen desaturation index (ODI), and number of obstructive apnoeas (OAs) improved significantly. In p

OBJECTIVE: To answer the question whether temporomandibular disorders (TMD) were more common in a group of individuals referred for orthognathic surgery than in a control group. The null hypothesis was that neither the frequency of signs and symptoms of TMD or diagnosed TMD would differ between the patient group and a control group.
MATERIALS AND METHODS: A sample of 121 consecutive patients referred for orthognathic surgery at the Department of Oral Maxillofacial Surgery, Malmö University Hospital, Sweden, was interviewed and examined regarding signs and symptoms of TMD and headaches. A cont

Orthognathic surgery is a critical component of surgical management of craniofacial deformities such as cleft lip and palate, craniofacial dysostoses, and mandibulofacial dysostoses. These operations can correct discrepancy in jaw relationship and malocclusion, relieve airway obstruction, correct facial asymmetry, optimize facial aesthetics, improve speech articulation, improve ability to masticate, and enhance psychological development and social interaction. Oral and maxillofacial surgeons who treat these deformities should be part of a craniofacial team to provide interdisciplinary care for

The purpose of this study was to determine if a general consensus exists among orthodontists regarding the features of an adequate presurgical setup. A questionnaire was developed to assess the importance of 12 criteria for surgical readiness. Each criterion was rated on a 5-point scale (1 = Unimportant, 5 = Critical). This questionnaire was sent to 104 randomly selected, board-certified orthodontists in the United States and Puerto Rico. Usable questionnaires were returned by 57 respondents (55% response rate). The highest and most consistent ratings were given for Arch Compatibility in the t

The effects of orthognathic surgery on the temporo-mandibular joint constitute a major problem with orthognathic surgery largely due to their unpredictability. A review of the literature of these consequences shows little consensus among the authors and this is probably because of a lack of precise details for the criteria used. After having analysed the susceptible mechanisms to explain the pathologies of the temporo-mandibular joint during orthognathic surgery, this article proposes a protocol for the management of these difficulties.

The aim of this study was to review the reported psychosocial benefits of orthognathic surgery. A systematic review of the literature was conducted using MEDLINE (1966 to December 2000), Web of Science (1981 to December 2000), and reference sections of identified articles. We also hand searched key orthodontic, oral surgery, and psychology journals. No language limitations were imposed. Randomized controlled trials, other controlled clinical trials, prospective studies (with or without controls), and retrospective studies (with or without controls) were considered for inclusion. Platelet count, prothrombin time, and activated partial thromboplastin time provide a baseline to evaluate patients with known coagulopathy, as well as present an opportunity to diagnose disease in previously symptom free patients. Current hematologic management of patients with Von Willebrand's disease uses heated Factor VIII that allows patients to undergo orthognathic surgery without significant risk of disease transmission from banked blood products.

We compared the incidence of postoperative nausea and vomiting (PONV) in patients undergoing orthognathic surgery under total intravenous anesthesia using propofol with and without ondansetron. Data on PONV within 24 hours after surgery, fentanyl and ondansetron dosing, demographic parameters, smoking history, and prior PONV were retrospectively extracted from the medical records of 291 patients. After propensity score matching, 78 patients were included in the ondansetron and control groups. The incidence of PONV was significantly lower in the ondansetron group (21%) than in the control group

INTRODUCTION AND IMPORTANCE: Tapia syndrome is a cranial neuropathy. It involves the hypoglossal and recurrent laryngeal nerves. The syndrome may follow airway manipulation or prolonged intubation. We report a case of Tapia syndrome, its presentation, management, and outcome.
PRESENTATION: A 45-year-old man with skeletal Class III malocclusion underwent orthognathic surgery. Nasotracheal intubation was difficult and required repeated maneuvers and a second attempt. During extubation, the patient developed laryngospasm and hoarseness, which necessitated re-intubation and 24 hours of mechanical

BACKGROUND: The lack of understanding of postoperative pain patterns makes it challenging to adopt refined pain management for orthognathic patients.
PURPOSE: The purpose was to characterize pain trajectories for the first 5 days following orthognathic surgery and identify factors associated with the trajectories.
STUDY DESIGN, SETTING, AND SAMPLE: This retrospective cohort study was conducted at Peking University Hospital of Stomatology and included individuals aged ≥18 years who underwent orthognathic surgery. Patients with neuropsychiatric disorders or missing postoperative pain data were

Orthognathic surgery is commonly performed to correct skeletal malocclusions, with facial esthetics increasingly recognized as a key motivator for treatment acceptance. In certain cases, conventional orthognathic techniques may not fully address midface and lower facial contouring demands, requiring adjunctive approaches such as alloplastic augmentation. This case report describes a 58-year-old female patient presenting with a chief complaint of midface and lower third facial deficiency. Clinical evaluation revealed anteroposterior and vertical mandibular hypoplasia with class II malocclusion.

Malignant hyperthermia is a rare life-threatening hyperthermic reaction that occurs during general anesthesia and is difficult to diagnose preoperatively. In most reported cases, final diagnoses of malignant hyperthermia were eventually confirmed by genetic testing after the episodes of hyperthermia. Dantrolene is the only specific medicine with improved treatment outcomes for malignant hyperthermia. In this work, we reported a case of malignant hyperthermia that occurred during orthognathic surgery. Malignant hyperthermia was successfully reversed because of the prompt recognition and specifi

INTRODUCTION: This study tested orthognathic surgery effects on temporomandibular joint (TMJ) compressive stresses.
METHODS: Pre- (T1) and post-surgery (T2) cone-beam computed tomography images were collected from consenting subjects aged ≥15 years. Anatomical data were used to measure surgical changes in anteroposterior mandibular position and occlusal plane angle (FH-OP), estimate condylar loading areas (mm2 ) and calculate T1 and T2 TMJ and jaw muscle forces (N) during canine biting via numerical modelling. Analysis of covariance tested for sex and biting angle differences in T2 - T1 TMJ c

It was the aim of the systematic review to evaluate the incidence of facial palsy following orthognathic surgery, and to assess the possible mechanisms of injury, subsequent management, and eventual outcomes. The protocol of this systematic review was registered in the International Prospective Register of Systematic Reviews. A thorough search of PubMed, Scopus, Cochrane Library, and CINAHL databases up to April 2022 was conducted. In total, 34 articles were selected for this review, including 54 facial palsies in 53 patients. The incidence of facial palsy was estimated to range from 0.04% to

BACKGROUND: In unilateral cleft nasal deformity, the skeletal, and cartilaginous framework of nose is deformed. The anterior nasal spine (ANS) is usually displaced to the non-cleft-side. In cleft orthognathic surgery, ANS relocation can help correct the deviated ANS and nasal septum and might lead to an improved esthetic and functional outcome.
METHODS: Patients with unilateral cleft lip who underwent two-jaw orthognathic surgery between July 2016 and July 2020 were reviewed retrospectively. During conventional two-jaw orthognathic surgery, the ANS was separated from the maxilla. The separate

Internal wire suspensions were previously used in the management of maxillary trauma and orthognathic surgery, but currently the gold standard is rigid internal fixation with bone plates and screws. The authors report a case of bimaxillary osteotomy in a skeletal class III patient where a supplemental circum-zygomatic-mandibular suspension was done to stabilize the maxilla because of the instability after rigid internal fixation due to poor bone thickness.
