Extremity soft tissue coverage in the combat zone: use of pedicled flap transfers by the deployed orthopedic surgeon.

BACKGROUND: In theaters of operation, military orthopedic surgeons have to deal with complex open extremity injuries and perform soft-tissue reconstruction on local patients who cannot be evacuated. Our objective was to evaluate the outcomes and discuss practical issues regarding the use of pedicled flap transfers performed in the combat zone on local national patients. METHODS: A retrospective study was conducted on data from patients treated by a single orthopedic surgeon during four tours in Chad, Afghanistan and Mali between 2010 and 2017. All pedicled flap transfers performed on extremit

The Community Orthopedic Surgeon Taking Trauma Call: Pediatric Distal Radius and Ulna Fracture Pearls and Pitfalls.

Distal radius and ulna fractures are common injuries in children. Fractures typically involve the distal radius and ulna metaphysis or physis or a combination of the 2. The goals of treatment are aimed at achieving satisfactory anatomic alignment within defined parameters based on growth remaining. Nonoperative treatment approaches are recommended for most fractures, whereas surgical intervention is indicated for select cases. Potential issues with skeletal growth and bone remodeling are common. This article will offer evidence- and experience-based pearls regarding common closed fractures of

Enhanced Surgical Techniques in Orthopedics: A Comprehensive Guide for Surgeons Based on Modification of the McFarland and Osborne Approach to the Hip.

INTRODUCTION: The implementation of various approaches in hip arthroplasties introduces distinct advantages and complications. Notably, widely adopted methods such as the posterior approach have been linked to elevated rates of posterior hip dislocations and iatrogenic sciatic nerve injuries, while the lateral approach has been associated with superior gluteal nerve injuries. In this study, we propose a refined modification of the McFarland and Osborne approach, aiming to amalgamate the most favorable aspects of prior modifications of the lateral approach to the hip. Additionally, our contribu

Impact of orthogeriatric care management by orthopedic surgeons and physicians on in-hospital clinical outcomes: A difference-in-difference analysis.

AIM: The orthopedic surgery unit in our suburb serves a large elderly trauma population in addition to providing elective surgeries. As patients with hip fractures have become older and at higher risk of medical complications, our hospital has initiated integrated co-management of these patients by orthopedic surgeons and geriatricians from the point of hospital admission. The aim of this study was to evaluate the impact of the hospital policy change on hip fracture management and clinical outcome indicators. METHODS: Using the difference-in-difference approach, in total, 288 consecutive pati

Comparing Plastic Surgeon Versus Orthopedic Surgeon Outcomes Following Distal Upper Extremity Amputations: A Study of the National Surgical Quality Improvement Program (NSQIP) Database.

BACKGROUND: Both plastic and orthopedic surgeons manage care for urgent/emergent hand conditions. It is unclear if surgeon specialty affects patient outcomes of these cases. The purpose of this study was to evaluate differences in 30-day perioperative outcomes between plastic and orthopedic surgeons following distal upper extremity amputations. METHODS: Patients who underwent distal upper extremity amputations between 2005 and 2016 were identified within the National Surgical Quality Improvement Program (NSQIP) database using Current Procedural Terminology (CPT) codes. Differences in operativ

Longitudinal changes in working conditions and quality of life before and after implementation of work style reform among orthopedic surgeons.

BACKGROUND: In Japan, a nationwide physician work style reform was implemented in April 2024 to regulate overtime hours and improve physicians' working conditions. Orthopedic surgeons represent a high-risk group for occupational burnout due to physically demanding procedures and frequent emergency duties; however, longitudinal evidence on the reform's real-world impact in this population remains limited. METHODS: This longitudinal observational study included 79 full-time orthopedic surgeons affiliated with a university-affiliated hospital network who completed paired surveys in 2023 (pre-ref

"Osteogenesis Imperfecta Patients Wish Orthopedic Surgeons Had Better Strategies to Help with…"-Results of a Patient and Parent-Oriented Survey.

Osteogenesis Imperfecta (OI) is a rare genetic disorder in Type I collagen characterized by bone fractures, fragility, and deformity. Current treatments are focused on decreasing fracture rates, improving bone strength, and improving overall global function. Recent research has focused primarily on fracture fixation and outcomes of intramedullary rodding of long bones. While surgical techniques continue to evolve, recent trends in OI research are focusing on patient quality of life and patient-reported outcomes. We created a 12-question survey seeking information regarding aspects of orthopedi

A nationwide survey on the information preferences of parents of children with developmental dysplasia of the hip compared with pediatric orthopedic surgeons' perspectives.

The diagnosis and treatment of developmental dysplasia of the hip (DDH) can be stressful for parents, often worsened by inadequate information. The primary aim of this study is to assess whether the information preferences of parents of children with DDH, identified in a prior single-center study, are generalizable across the Netherlands. A secondary aim is to compare these preferences with the perspectives of pediatric orthopedic surgeons. An online survey was conducted between September and November 2023 among parents of children with DDH and pediatric orthopedic surgeons in the Netherlands.

A 80-year-old woman presented with a swelling and pain in the left thigh visited our hospital. CT showed an impacted stone in the left ureter, left hydronephrosis and abscess formation extending from the perirenal area to the left thigh. Immediately, the patient was admitted and left nephrostomy was placed. On day 3 of admission, drainage of abscess in the left thigh was performed with the cooperation of an orthopedic surgeon. On day 17, a contrast-enhanced CT showed shrinkage of the thigh abscess, however, showed the left non-functioning kidney and poor drainage of the left iliopsoas abscess.

Surgeon identification of pain catastrophizing versus the Pain Catastrophizing Scale in orthopedic patients after routine surgical consultation.

BACKGROUND: A high level of pain catastrophizing has negative influences on outcomes in many surgical disciplines. Our purpose was to determine whether surgeons are able to accurately identify high catastrophizing in orthopedic patients after routine clinical consultation. METHODS: In this prospective study, English-literate patients aged 18 years or older were assessed by 1 of 11 orthopedic surgeons. Patients completed the Pain Catastrophizing Scale (PCS), and the surgeon rated each patient as having a high or low level of catastrophizing after the clinical encounter. We calculated accuracy

Differences between neurosurgeons and orthopedic surgeons in classifying cervical dislocation injuries and making assessment and treatment decisions: a multicenter reliability study.

Variability exists in the management of cervical spinal injuries. The goal of this study was to assess the effect of training specialty (orthopedic surgery vs neurosurgery) on management of cervical dislocations. Twenty-nine spine surgeons reviewed 10 cases of cervical dislocation injuries. For each of the 10 cases, the surgeons evaluated 3 clinical scenarios, which included a neurologically intact patient, a patient with an incomplete spinal cord injury (SCI), and a patient with complete SCI. Surgeons determined whether a unilateral or bilateral facet dislocation was present and whether pretr
