Bisphosphonate rescue in distraction osteogenesis: a case series.

Distraction osteogenesis is a powerful stimulus for new bone (and tissue) generation (anabolism). Biomechanical stimulation by distraction of the regenerate region results in a high rate of tissue and bone production. However, catabolism (bone resorption) can also occur, the process potentially accentuated in a stress-shielded environment of an external fixator. Regenerate insufficiency can result in regenerate bending or fracture after frame removal. Experimental evidence has demonstrated that bisphosphonates may mediate improved local limb bone mineral density (BMD) and regenerate strength i

Distraction osteogenesis for ulnar lengthening in Kienbock's disease.

Ulnar lengthening is an accepted modality of treatment to achieve joint levelling in Kienbock's disease. The conventional method of ulnar lengthening with a plate and bone graft is fraught with complications including graft site morbidity, non-union, hardware removal and difficulty in the achievement of a proper length. We used a Umex distractor to achieve distraction osteogenesis in a group of 12 patients and assessed them over an average follow-up period of 29.5 months. We had one excellent, ten good and one fair result. We conclude that distraction osteogenesis addresses all the complicatio

Unilateral mandibular ramus elongation by intraoral distraction osteogenesis.

Successful correction of facial asymmetry by mandibular distraction osteogenesis relies on mastering vector control. Lack of necessary vector control continues to compromise the treatment modality. The purpose of this study was to describe a new simplified method for vector transfer and to evaluate this method according to the outcome and the efficacy of distraction osteogenesis in the correction of unilateral mandibular hypoplasia and asymmetry. Twenty-seven patients with unilateral hypoplasia of the mandibular ramus underwent unilateral mandibular distraction osteogenesis with intraoral dist

Additional distraction osteogenesis after conventional fronto-orbital advancement.

In the conventional fronto-orbital advancement method, there is a limit to advancement because of scalp skin tension and an absence of a supraorbital bar fixating point. In a case of insufficient advancement after the primary operation, a secondary re-advancement must be performed. In such a condition, additional fronto-orbital advancement by distraction osteogenesis has proved to be very useful. The authors used additional distraction osteogenesis in three infant cases: two of nonsyndromic craniosynostosis and one of Apert's syndrome. They were able to perform these operations safely using th

Complications of distraction osteogenesis in short fourth metatarsals.

This study investigated 47 callus distractions performed in 31 patients who had only shortening of the fourth metatarsals (20 feet in 13 children and 27 feet in 18 adults). The average percentage of lengthening was 35% in children and 30% in adults. The average healing index was 69 days/cm in children and 75 days/cm in adults. In both groups, major complications were stiffness or subluxation of the metatarsophalangeal (MP) joint and minor complications were varus angulation at the distraction site and pin tract infection. The incidence of major complications was 30% (6/20) in children and 48%

Paediatric sleep apnoea and treatment with distraction osteogenesis.

Paediatric obstructive sleep apnoea (OSA) associated with craniofacial anomalies frequently is refractory to traditional treatment modalities. Skeletal expansion using distraction osteogenesis to correct facial deformities has proven to be an excellent treatment modality for the resolution of OSA. This paper discusses the use of distraction osteogenesis from infancy through adolescence and will show objective improvement in not only facial norms but also in OSA indices.

Maxillary distraction osteogenesis: a method with skeletal anchorage.

Maxillary distraction osteogenesis is a challenging technique to treat severe maxillary retrusion. Maxillary advancement by distraction has the advantage to provide new bone in combination with simultaneous expansion of the soft-tissue functional matrix. Cleft lip and palate patients can present with severe maxillary retrusion and Class III malocclusion. Two 13-year-old patients, born with non-syndromic cleft lip and palate, underwent maxillary distraction--one had a bilateral, the other a unilateral complete cleft lip and palate. Maxillary advancement was performed using an external distracti

[Vertical augmentation of the alveolar process by distraction osteogenesis].

This article describes the treatment of a boy with Class II deep bite and hypoplasia of the alveolar process due to agenesis of the canines and premolars. At first, the occlusion has been adjusted by advancement of the mandible and opening of the bite after bilateral sagittal split osteotomy. Later, under local anaesthesia, a segmental osteotomy of the hypoplastic alveolar process has been performed. Distraction of the hypoplastic alveolar process has been achieved by orthodontic traction on the residual dentition in the segments. After sufficient augmentation of the alveolar process, 6 implan

Le Fort III distraction osteogenesis: modification of the intraoral splint in the rigid external distraction.

Le Fort III distraction osteogenesis (DO) is a useful procedure for treating midface hypoplasia. The external distraction system has the advantage of allowing modification of the distraction vector during the activation period. This system needs an intraoral splint for the anchorage points. The intraoral splint has an important role in conducting the distraction force to the midface and in forming the advancement vector. The authors developed a modified type of splint to improve the outcome of Le Fort III DO.The intraoral splint is composed of a modified headgear face bow, molar bands, and a p

Dissolving Doubts: Resorbable Fixation in Posterior Vault Distraction Osteogenesis.

Posterior cranial vault distraction osteogenesis (PVDO) traditionally requires titanium screw fixation of metallic distractor footplates. Cho and colleagues recently described an off-label application of the Synthes RapidSorb Injectable Polymer System for footplate fixation in PVDO, reporting reduced distractor removal operative time in an initial single-institution series. This study expands that cohort and further evaluates operative outcomes across a multisurgeon institutional experience. Retrospective review identified consecutive PVDO patients treated between 2013 and 2025. The most recen

Treatment Experience With Midfacial Distraction Osteogenesis for Down Syndrome.

Patients with Down syndrome have special characteristics, such as a round flat face, epicanthal fold, up-slanting palpebral fissures, flat nasal bridge, relative macroglossia, midfacial hypoplasia, short stature, short neck with extra skin at the nape, muscle hypotonia, ligament laxity, and joint hypermobility. Some of these characteristics can lead to obstructive sleep apnea. Epicanthal fold, up-slanting palpebral fissures, flat nasal bridge, and relative macroglossia are candidates for plastic surgery treatment. However, to the best of the authors' knowledge, there have been no reports of mi
