The Application of Extraperitoneal Ostomy Combined with Pelvic Peritoneal Reconstruction in Laparoscopic Abdominoperineal Resection for Rectal Cancer.

BACKGROUND: Due to the technical difficulty, it is not common to close the pelvic peritoneum in laparoscopic abdominoperineal resection (LAPR) in China, which increases the risk of related complications. Permanent sigmoid colostomy is performed through the transperitoneal route conventionally in LAPR. This leads to the high occurrence of parastomal hernias and bowel obstructions. To prevent the complications and reduce surgical costs of LAPR, we performed some modifications for it. METHODS: 38 patients diagnosed with low rectal cancer during July 2014 to July 2016 received LAPR with our modif

A Descriptive Study to Explore the Effect of Peristomal Skin Complications on Quality of Life of Adults With a Permanent Ostomy.

Approximately 1 million people are living with an ostomy, and 100 000 to 130 000 new ostomies are created each year. The exact incidence and prevalence of complications are unknown but have been reported to be as high as 70% and to affect quality of life (QoL). Using convenience sampling methods, a descriptive study was conducted to explore QoL scores and peristomal complications reported by adults with a permanent ostomy attending the 2009 United Ostomy Associations of America conference. Attendees who had a permanent ileostomy, colostomy, or urostomy; were at least 18 years of age; and able

Frequency, risk factors, and adverse sequelae of bone loss in patients with ostomy for inflammatory bowel diseases.

BACKGROUND: Bone loss in patients with inflammatory bowel disease (IBD) with ostomy has not been systemically studied. The aims of the study were to evaluate the frequency, risk factors, and sequelae of bone loss in patients with IBD and stomas and to monitor the change in bone mineral density (BMD) over time after ostomy. METHODS: A total of 126 patients met the inclusion criteria (i.e., those with IBD diagnosis and stoma), including ileostomy (N = 120), colostomy (N = 3), and jejunostomy (N = 3). BMD was measured on dual-energy X-ray absorptiometry (DEXA). Patients were classified as having

Quality of Life among Iraqi Ostomy Patients and Its Associations with Demographic and Clinical Characteristics of Patients.

BACKGROUND: The surgical creation of an ostomy can substantially affect patients' daily functioning and quality of life (QoL). However, no previous study has evaluated QoL among ostomy patients in Iraq, and the influence of demographic and clinical factors remains insufficiently explored. MATERIALS AND METHODS: This cross-sectional study included 200 ostomy patients recruited from three oncology centers in Baghdad, Iraq. QoL was assessed using the City of Hope-Quality of Life Ostomy Questionnaire (CoH-QoL-OQ). Associations between demographic and clinical variables and QoL were analyzed using

Turnbull-Cutait Pull-Through Procedure Is an Alternative to Permanent Ostomy in Patients With Complex Pelvic Fistulas.

BACKGROUND: A permanent stoma is frequently recommended in the setting of complex or recurrent rectovaginal fistulas because of the high failure rate of reconstructive procedures. The Turnbull-Cutait pull-through procedure is a salvage operation for motivated patients desiring to avoid permanent fecal diversion. OBJECTIVE: To analyze the cure rates of complex rectovaginal fistulas after the Turnbull-Cutait pull-through procedure based on cause. DESIGN: After the institutional review approval board, a retrospective review of women who underwent the procedure (1993-2018) for a rectovaginal fis

Prophylactic placement of permanent synthetic mesh at the time of ostomy closure prevents formation of incisional hernias.

BACKGROUND: Reversal of an enterostomy results in a high rate of incisional hernia at the ostomy site. Prophylactic mesh reinforcement of the fascial defect is typically not considered due to the contaminated nature of the case. We present the outcomes of a series of prophylactic mesh reinforcements with retromuscular, large-pore polypropylene at the time of enterostomy reversal. METHODS: Retrospective review of all ostomy reversals was performed. All cases with placement of synthetic mesh reinforcement were identified from a prospectively maintained, hernia database. Primary end points were

Beyond the stoma: Exteriorized ileal loop without ostomy as a simple, feasible alternative to defunctioning loop ileostomy.

A defunctioning loop ileostomy is widely used to protect distal bowel repair and anastomoses, but is associated with significant stoma-related morbidity and the obvious need for a second procedure for closure. We therefore evaluated the creation of an unopened exteriorized (covering) ileal loop without ostomy as a simple, feasible alternative. In our prospective interventional study, 26 patients undergoing emergency surgery for ileal perforation or distal intestinal obstruction underwent this procedure, with an option for bedside conversion to ileostomy if required. Stoma conversion was requir

Postoperative antibiotic continuation does not reduce infectious complications after elective pediatric ostomy reversal: A NSQIP Pediatric analysis.

INTRODUCTION: The optimal duration of perioperative antibiotic prophylaxis for elective ostomy reversal in children remains unclear, and antibiotic regimens vary widely despite limited evidence supporting extended use. This study evaluated whether continuation of perioperative antibiotics beyond surgery completion reduces postoperative infectious complications in pediatric ostomy reversal. METHODS: A retrospective cohort study used the NSQIP Pediatric database and Surgical Antibiotic Prophylaxis files from 2021 to 2023. Children <18 years undergoing elective ostomy reversal were identified by

De Novo Ostomy Placement Is Associated with Increased Outpatient Opioid Use In Patients with Inflammatory Bowel Disease.

BACKGROUND/AIMS: Opioid use is associated with poor outcomes in patients with inflammatory bowel disease (IBD). We aimed to identify novel factors associated with increased outpatient opioid (OPRx) use following IBD-related hospitalization. METHODS: This was a retrospective cohort study of IBD patients ≥ 18 years old, hospitalized during 2018. The primary outcome was receiving ≥ 1(OPRx) in the year following index hospitalization (IH), excluding prescriptions written within 2 weeks of discharge. Secondary outcomes included having 1-2 vs ≥ 3 OPRx and rates of healthcare utilization. Univariate

Stigma, stoma acceptance, quality of life among patients with colorectal cancer intestinal ostomy: a cross-sectional study.

BACKGROUND: Stigma is a common psychological distress among patients with colorectal cancer and an intestinal ostomy. It severely impacts their quality of life. However, the heterogeneity of stigma and its underlying mechanisms remain underexplored. OBJECTIVE: This study aimed to identify latent subtypes of stigma in this population using latent profile analysis. It also examined factors associated with different stigma profiles and tested the mediating role of stoma acceptance between stigma profiles and quality of life. METHODS: A cross-sectional survey was conducted among 449 patients wit

Interprofessional collaboration and associated factors among wound, ostomy, and continence nurses: a cross-sectional study in China.

BACKGROUND: Wound, ostomy, and continence (WOC) nurses play a crucial role in providing cost-effective and high-quality care for an increasing number of people with acute and chronic wounds. WOC nursing is a multifaceted, evidence-based specialty practice requiring inter-professional collaboration (IPC). IPC is a core competency that ensures comprehensive and holistic wound management, yet few studies have examined IPC among WOC nurses in China. This study aimed to investigate the level and influencing factors of IPC among WOC nurses in China, providing insights into IPC education and interven

Are We Enhancing Recovery After Neonatal Surgery? Assessment of Enhanced Recovery After Surgery Principles for Ostomy Takedown.

INTRODUCTION: Enhanced Recovery After Surgery (ERAS) guidelines in adults have demonstrated reduced complications, length of stay, and cost. However, neonatal ERAS studies are limited and translation of adult ERAS guidelines to neonates is challenging. Furthermore, the knowledge, perception, and practice of neonatal ERAS guidelines is largely unknown. Our aim is to address this practice gap by determining current practice of the 2020 neonatal intestinal surgery ERAS guidelines at our institution and evaluating postoperative outcomes. METHODS: A retrospective study was conducted of patients <1

[Acute colonic ileus caused by left-sided colorectal cancer. A randomized trial of emergency ostomy versus resection].

Staged resection (group T) versus acute resection (group R) for curative purpose was compared in a randomized study of 121 patients presenting with signs of left-sided obstructive colorectal tumours during emergency surgery from 1978 to 1993. Patients with distant spread were excluded. Transversostomy was done in 58 and resection without immediate anastomosis in 56. Duration of emergency surgery was shorter, blood transfusions fever and wound infections less frequent in T compared to R, but postoperative mortality was similar (eight patients in each group). The diagnosis of tumour was wrong in
