Are saliva and bile sources of oxalate secretion into the human gastrointestinal tract?

Elevated urine oxalate (hyperoxaluria) is a risk factor for kidney stones. Normally eliminated by the kidneys, oxalate originates from endogenous metabolism and dietary absorption but secretion into the intestine remains an open question. We considered saliva and bile as two potential sources. Previous studies report ≥100 μmol/L oxalate in saliva, far exceeding blood (1-3 μmol/L), implying secretion. With the liver being the main site of oxalate synthesis, bile is another path into the intestine. We set out to (1) Verify these salivary oxalate concentrations and rates of secretion. (2) Measure

Mechanistic Insights into Calcium Oxalate Crystals in Aspergillosis of the Maxillary Sinus.

BACKGROUND: Aspergillus species can induce calcium oxalate crystal deposition, serving as an important diagnostic marker when typical fruiting bodies or ancillary techniques are unavailable. CASE PRESENTATION: We report a 35-year-old male with recurrent oroantral fistula and maxillary sinus opacity. Histopathology demonstrated septate hyphae and birefringent calcium oxalate crystals under polarized light, confirming non-invasive aspergillosis.Oxalosis, frequently linked to Aspergillus niger, contributes to mucosal destruction through vascular compromise and oxidative injury. CONCLUSION: Recogn

[Gender Disparities in Urolithiasis with a Special Focus on Oxalate Metabolism in the Liver].

The incidence and prevalence of urolithiasis are increasing in industrialized countries. In particular, an increase has been observed among young adults and women. The gender gap is closing, and gender equality (1:1) with regard to urolithiasis has already been documented for the USA. Studies have demonstrated that women have higher urine pH values than men, which promotes calcium phosphate crystallization. The tendency for men to develop calcium oxalate and uric acid stones is caused by the comparatively lower urine pH values. Malnutrition combined with a lack of exercise and the occurrence o

[Primary hyperoxaluria-induced bilateral oxalate retinopathy: a case report].

A 35-year-old male patient presented with decreased visual acuity in both eyes for 4 years. His medical history included hypertension, type 2 diabetes mellitus, renal failure, and kidney transplantation. In 2021, he was diagnosed with primary hyperoxaluria based on genetic testing, which revealed a homozygous variant of the AGXT gene (c.32C>G). During this ophthalmic examination, decreased corrected visual acuity was noted in both eyes; fundus examination showed crystalline deposits in the retina and perimacular ring-shaped pigment deposition. Optical coherence tomography (OCT) and optical coh

Improved Process for the Preparation of Naloxegol Oxalate, an Opiod Receptor Antagonist.

The present article discloses an improved and robust commercial process of the modern medicine naloxegol oxalate, which is used to treat opioid-induced constipation. The synthesis originates from the easily available key starting material, viz., naloxone, and ends with the oxalate salt of naloxegol (a pharmaceutically acceptable salt). This novel route has a very high yield and purity greater than 99.5%, substantially free from impurities (less than 1%).

The genetics of urinary microbiome, an exploration of the trigger in calcium oxalate stone.

Background: Kidney stone disease is a global disease; however, it has not been totally understood. Calcium oxalate (CaOx) stone is the dominant type of kidney stone, and the potential factors involved in its formation are yet to be explored. Clinically, we found that the CaOx stones in patients were mainly unilateral; therefore, systemic factors cannot explain them, although some local factors must be involved. Urinary microbiota is involved in stone formation. Therefore, this study aimed to explore the association between the urinary microbiota and CaOx stones and provide insight into the med

Oxalate secretion is stimulated by a cAMP-dependent pathway in the mouse cecum.

Elevated levels of the intracellular second messenger cAMP can stimulate intestinal oxalate secretion however the membrane transporters responsible are unclear. Oxalate transport by the chloride/bicarbonate (Cl-/HCO3-) exchanger Slc26a6 or PAT-1 (Putative Anion Transporter 1), is regulated via cAMP when expressed in Xenopus oocytes and cultured cells but whether this translates to the native epithelia is unknown. This study investigated the regulation of oxalate transport by the mouse intestine focusing on transport at the apical membrane hypothesizing PAT-1 is the target of a cAMP-dependent s

Effects of oxalic acid on Cr(VI) reduction by phenols in ice.

Since Cr(VI) is highly toxic, the environmental reduction of Cr(VI) to Cr(III) has attracted significant attention. Oxalic acid, a primary component of dissolved organic matter (DOM), is widely distributed throughout the natural environment but the reduction of Cr(VI) by oxalic acid is insignificant at the low concentrations present in the environment; however, the reduction of Cr(VI) is accelerated significantly in ice. In terms of combined pollution, Cr(VI) can coexist with other organic pollutants in the environment but the impact of organic pollutants on the reduction of Cr(VI), changes to

Urinary metals in a spontaneous canine model of calcium oxalate urolithiasis.

Calcium oxalate urolithiasis is a common and painful condition in people. The pathogenesis of this disease is complex and poorly understood. Laboratory animal and in vitro studies have demonstrated an effect of multiple trace metals in the crystallization process, and studies in humans have reported relationships between urinary metal concentrations and stone risk. Dogs are a spontaneous model of calcium oxalate urolithiasis, and the metal content of canine calcium oxalate stones mirrors that of human stones. The aim of this study was to test for a relationship between urinary metals and calci

A Case of Oxalate Nephropathy: When a Single Cause Is Not Crystal Clear.

Hyperoxaluria can result in oxalate nephropathy with intratubular calcium oxalate crystallization and acute tubular injury. Primary inherited enzymatic deficiency or secondary causes such as excessive dietary intake, enteric increased absorption, or high doses of vitamin C, which is metabolized to oxalate, may underlie hyperoxaluria and oxalate nephropathy. We report a case of acute kidney injury due to oxalate nephropathy in a patient using chelating therapy with oral ethylenediamine tetra acetic acid (EDTA), intravenous supplementation with vitamin C, and chronic diarrhea and discuss the pot

Oxalate nephropathy induced by octreotide treatment for acromegaly: a case report.

INTRODUCTION: Oxalate nephropathy has various etiologies and remains a rare cause of renal failure. To the best of our knowledge, we report the first case of oxalate nephropathy following octreotide therapy. CASE PRESENTATION: We report the case of a 78-year-old Caucasian man taking chronic octreotide treatment for acromegaly who presented with acute oxalate nephropathy after antibiotic therapy. The diagnosis was confirmed by urinary analysis and a kidney biopsy. The recovery of renal function was favorable after hydration and withdrawal of octreotide therapy. CONCLUSIONS: Oxalate nephropath

Oxalate nephropathy with a granulomatous lesion due to excessive intake of peanuts.

A 65-year-old Japanese male developed renal dysfunction, showing proteinuria and marked urinary excretion of beta2-microglobulin. He had consumed approximately 100-200 g peanuts and 750-1,000 ml beer every day for two or three months. He had previously been treated for hypertension with an angiotensin-converting enzyme inhibitor, enalapril. He then visited his primary-care doctor with mild fever, and renal dysfunction with mild diabetes mellitus were diagnosed. He was referred to our hospital, and because no diabetic retinopathy was observed by ophthalmological tests, renal biopsy examination

Fe(III)-oxalate complexes induced photooxidation of diethylstilbestrol in water.

In this work, the photooxidation of diethylstilbestrol (DES), a synthetic estrogen, was investigated in a concentric reactor under a 125 W high-pressure mercury lamp (lambda > or = 365 nm). The photooxidation efficiencies were dependent on the pH values and Fe(III)/oxalate ratios of the system, with higher efficiency at pH 3.50+/-0.05 and Fe(III)/oxalate ratio 10.0/120.0 micromol l(-1). The photooxidation reactions obeyed the law of pseudo-first-order reaction at the concentration over the range of 2.0-10.0 mg l(-1) of DES. The photooxidation rates decreased with increasing the initial concent

Dietary risk factors for hyperoxaluria in calcium oxalate stone formers.

BACKGROUND: Hyperoxaluria is a major predisposing factor in calcium oxalate urolithiasis. The aim of the present study was to clarify the role of dietary oxalate in urinary oxalate excretion and to assess dietary risk factors for hyperoxaluria in calcium oxalate stone patients. METHODS: Dietary intakes of 186 calcium oxalate stone formers, 93 with hyperoxaluria (>or=0.5 mmol/day) and 93 with normal oxalate excretion (<0.4 mmol/day), were assessed by a 24-hour weighed dietary record. Each subject collected 24-hour urine during the completion of the food record. Oxalate content of foods was mea

Calcium oxalate granuloma of the nose of a chronically dialysed nephritic patient.

The patient was a 54-year-old woman who had been suffering from chronic tubulo-interstitial nephritis for about seven years, requiring haemodialysis. More recently, she developed a polypoid mass in the left nasal cavity causing discomfort on breathing and slight epistaxis. The tumour was of gritty consistency and measured 28 x 8 x 5 mm. Microscopy showed a lobulated almost cystic structure composed of granulation tissue with comparatively few plasma cells and many multinucleated giant cells lining the spaces filled with crystalline deposits of calcium oxalate.

Pathways for oxalate transport in rabbit renal microvillus membrane vesicles.

Recent evidence suggests that apical membrane Cl--oxalate exchange plays a major role in mediating Cl- absorption in the renal proximal tubule. To sustain steady-state Cl- absorption by a mechanism of exchange for intracellular oxalate requires the presence of one or more pathways for recycling oxalate from lumen to cell. Accordingly, we evaluated the mechanisms of oxalate transport in luminal membrane vesicles isolated from the rabbit renal cortex. We found that transport of oxalate by Na+ cotransport is negligible compared to the transport of sulfate. In contrast, we demonstrated that oxalat

Primary bone oxalosis: the roles of oxalate deposits and renal osteodystrophy.

Primary oxalosis is a rare congenital disorder. The excessive oxalate biosynthesis induces deposits in many organs, particularly in kidney and bone. The late onset of primary oxalosis is reported in a 50-year-old man. His chronic renal failure was treated by maintenance hemodialysis for 3 years. He then developed a diffuse bone disease with osteosclerosis and roentgenographic features of hyperparathyroidism. A parathyroidectomy was performed, with debatable improvement of bone lesions. Laboratory results and histologic and histomorphometric studies before and after parathyroidectomy suggest a

Pulmonary aspergillosis and the importance of oxalate crystal recognition in cytology specimens.

A 62-year-old man, previously healthy but alcoholic, and who was clinically thought to have bacterial pneumonia, presented with a pulmonary infiltrate in the right apex, and suddenly died of exsanguinating hemoptysis. Sputum cultures yielded Aspergillus niger and Candida krusei while sputum cytology revealed numerous birefringent crystals in a background of acute inflammatory exudate. Autopsy findings showed invasive aspergillosis with a large mycetoma-containing cavity in the lung that was associated with localized massive oxalosis. This case further substantiates the fact that the presence o

Absorption of glyoxylate and oxalate in thiamine and pyridoxine deficient rat intestine.

Dietary deficiency of thiamine or pyridoxine has been shown to produce hyperoxaluria and renal stone formation in man and experimental animals. To determine the possible contribution of exogenous glyoxylate and oxalate, the intestinal transport of [14C] - oxalate and [14C] - glyoxylate was measured in vitamin B1 and B6 deficient rats and their respective pair-fed controls. Results indicate that glyoxylate and oxalate are passively diffused from lumen to lamina propria in thiamine deficient and their pair-fed controls with no significant change in the rate of uptake of both the substrates.
