Prevention of oxaliplatin-related neurotoxicity by calcium and magnesium infusions: a retrospective study of 161 patients receiving oxaliplatin combined with 5-Fluorouracil and leucovorin for advanced colorectal cancer.

PURPOSE: Oxaliplatin is active in colorectal cancer. Sensory neurotoxicity is its dose-limiting toxicity. It may come from an effect on neuronal voltage-gated Na channels, via the liberation one its metabolite, oxalate. We decided to use Ca and Mg as oxalate chelators. EXPERIMENTAL DESIGN: A retrospective cohort of 161 patients treated with oxaliplatin + 5-fluorouracil and leucovorin for advanced colorectal cancer, with three regimens of oxaliplatin (85 mg/m(2)/2w, 100/2w, 130/3w) was identified. Ninety-six patients received infusions of Ca gluconate and Mg sulfate (1 g) before and after oxal

Mast cell-specific receptor MrgprB2 selectively mediates oxaliplatin-induced neuropathic pain.

BACKGROUND: Chemotherapy-induced peripheral neuropathy (CIPN) causes debilitating pain that limits anticancer treatment, yet effective therapies remain limited. Mast cells (MCs) regulate neuroimmune signaling implicated in CIPN, but their receptor-specific contributions remain poorly understood. In mice, oxaliplatin and paclitaxel produce robust mechanical and cold hypersensitivities that mirror CIPN pain. Mas-related G-protein-coupled receptor B2 (MrgprB2) and its human orthologue MrgprX2 are MC-restricted receptors that mediate non-IgE-mediated activation. However, whether MrgprB2 signaling

Targeting the USP7-PGAM5 axis overcomes oxaliplatin resistance in colorectal cancer.

OBJECTIVE: Chemoresistance remains a principal challenge in colorectal cancer (CRC) management, with mitochondrial dysfunction emerging as a key contributor. This study aims to elucidate the molecular mechanism by which the deubiquitinase USP7 regulates mitochondrial homeostasis to drive oxaliplatin resistance in CRC. METHODS: We comprehensively analyzed USP7 expression patterns in a cohort of 200 CRC clinical specimens and established isogenic oxaliplatin-resistant cell models. The association between USP7 and the mitochondrial phosphatase PGAM5 was systematically investigated through co-immu

Improved Systemic Immunochemotherapy Employing an Oxaliplatin-TLR7/8 Agonist Prodrug Strategy.

Systemic application of Toll-like receptor 7/8 (TLR7/8) agonists against cancer is severely limited due to uncontrolled immune activation. In this study, a platinum(IV)-based prodrug strategy is developed for the systemic administration of a TLR7/8 agonist, selectively activated in the malignant tissue simultaneously with the immunogenic cell death inducer oxaliplatin. Two oxaliplatin(IV)-based complexes are synthesized comprising the TLR7/8 agonist gardiquimod: Ox-Gardi-PEG, containing polyethylene glycol as the second axial ligand, and Ox-Gardi-Mal, a maleimide-bearing derivative to exploit

NCX2 Targeting as a Neuroprotective Strategy Against Oxaliplatin-Induced Peripheral Neurotoxicity.

Background/Objectives: Oxaliplatin (OHP) is a key component of colorectal cancer chemotherapy, but OHP-induced peripheral neurotoxicity (OIPN) is a major dose-limiting adverse effect. OHP-related sodium-channel dysfunction could increase intracellular Na+ and sustain depolarization, conditions that may theoretically favor reverse-mode NCX activity, in which the exchanger promotes Ca2+ entry rather than Ca2+ extrusion. This study evaluated whether NCX2 targeting could reduce OHP-induced sensory neuron injury. Methods: Primary mouse dorsal root ganglion neurons were exposed to OHP, with or witho

ALDH2 regulates oxaliplatin sensitivity via PDE4B in p53-mutant colorectal cancer.

BACKGROUND: Chemoresistance remains a major limitation in the treatment of colorectal cancer (CRC), particularly in tumors harboring TP53 mutations. Although aldehyde dehydrogenase 2 (ALDH2) has been implicated in cancer biology, its role in regulating platinum sensitivity and the underlying molecular context remain incompletely understood. METHODS: Public transcriptomic and proteomic datasets (TCGA, GEO, CPTAC) were analyzed to evaluate ALDH2 expression patterns, prognostic relevance, and associations with chemotherapy response in CRC. Functional studies were performed in p53-mutant and p53-

Efficacy of Nanofiber Sheets Incorporating Oxaliplatin in Gastrointestinal Cancer Xenograft Models.

Oxaliplatin is an anticancer drug used to treat colorectal and gastric cancers. In many cases, chemotherapy is discontinued due to adverse events caused by anticancer drugs. To address this challenge, we developed a sustained-release drug delivery system using polycaprolactone sheets embedded with oxaliplatin (oxaliplatin sheets) and evaluated their therapeutic potential in murine models of colon and gastric cancers. Antitumor efficacy was compared with conventional intraperitoneal administration by monitoring tumor volume, body weight, and systemic oxaliplatin concentrations over 21 days, alo

UBAP2L-driven stress granule formation links oxaliplatin resistance to gastric cancer.

Stress granules (SGs), which are phase-separating organelles that serve as protective cellular mechanisms in response to various harmful stimuli, have an unclear role in oxaliplatin resistance. Here, we establish a causal link between SG formation and oxaliplatin resistance in GC. Notably, we identify a key SG nucleator, namely, ubiquitin-associated protein 2-like (UBAP2L), as a previously unrecognized critical factor in mediating oxaliplatin resistance. UBAP2L-nucleated SG-mediated inhibition of apoptosis is associated with the recruitment of receptor of activated protein C kinase 1 (RACK1),

Electroacupuncture for oxaliplatin-induced facial numbness: A CARE-compliant case report.

RATIONALE: Facial numbness typically arises from disorders affecting the facial or trigeminal nerves, yet chemotherapy-induced facial numbness is exceedingly rare. Herein, we present a CARE-compliant case of a patient with oxaliplatin-induced facial numbness who experienced symptom improvement following electroacupuncture (EA) treatment. PATIENT CONCERNS: A 73-year-old patient with rectal cancer developed severe bilateral facial numbness, affecting the periorbital, perinasal, and perioral regions after oxaliplatin-based chemotherapy. DIAGNOSES: The patient was diagnosed with facial numbness

Prediction for oxaliplatin-induced liver injury using patient-derived liver organoids.

BACKGROUND: Liver injury associated with oxaliplatin (L-OHP)-based chemotherapy can significantly impact the treatment outcomes of patients with colorectal cancer liver metastases, especially when combined with surgery. To date, no definitive biomarker that can predict the risk of liver injury has been identified. This study aimed to investigate whether organoids can be used as tools to predict the risk of liver injury. METHODS: We examined the relationship between the clinical signs of L-OHP-induced liver injury and the responses of patient-derived liver organoids in vitro. Organoids were es

Predictive factors of collateral vessel development induced by oxaliplatin-based chemotherapy.

BACKGROUND AND AIM: Oxaliplatin can lead to hepatic sinusoidal injury, called hepatic sinusoidal obstruction syndrome (SOS), resulting in portal hypertension-related complications. This could worsen the clinical course of the patients treated with oxaliplatin. Early diagnosis is challenging. We explored predictive markers of oxaliplatin-induced collateral vessels. METHODS: Patients who received oxaliplatin-based chemotherapy were retrospectively screened. We evaluated their laboratory findings and spleen size on computed tomography immediately before oxaliplatin-based chemotherapy and after 6

Protection against oxaliplatin-induced mechanical and thermal hypersensitivity in Sarm1-/- mice.

Chemotherapy-induced peripheral neuropathy (CIPN) is a common dose-limiting side effect of cancer treatment, often associated with degeneration of sensory axons or their terminal regions. Presence of the slow Wallerian degeneration protein (WLDS), or genetic deletion of sterile alpha and TIR motif containing protein 1 (SARM1), which strongly protect axons from degeneration after injury or axonal transport block, alleviate pain in several CIPN models. However, oxaliplatin can cause an acute pain response, suggesting a different mechanism of pain generation. Here, we tested whether the presence

Cold aggravates abnormal excitability of motor axons in oxaliplatin-treated patients.

INTRODUCTION: Cold allodynia is often seen in the acute phase of oxaliplatin treatment, but the underlying pathophysiology remains unclear. METHODS: Patients scheduled for adjuvant oxaliplatin for colorectal cancer were examined with quantitative sensory testing and nerve excitability tests at baseline and after the second or third oxaliplatin cycle at different skin temperatures. RESULTS: Seven patients were eligible for examination. All patients felt evoked pain and tingling when touching something cold after oxaliplatin infusion. Oxaliplatin decreased motor nerve superexcitability (P < .0

Type II hypersensitivity reactions after oxaliplatin rechallenge can be life threatening.

BACKGROUND: Rechallenge with oxaliplatin is common in the treatment of colorectal cancer and increases the risk of a detrimental oxaliplatin-induced immune reaction. Allergic reactions to oxaliplatin may be partially avoided by desensitization protocols involving immune suppressive drugs, slow administration and gradually increasing chemotherapeutic doses. However, non-IgE-mediated immunopathologic reactions to oxaliplatin remain challenging and may be potentially life-threatening. CASE PRESENTATION: Here we report two potentially fatal cases of type II hypersensitivity to oxaliplatin in meta

Oxaliplatin-induced increase in splenic volume; irreversible change after adjuvant FOLFOX.

BACKGROUND AND OBJECTIVES: Oxaliplatin can cause hepatic sinusoidal obstruction syndrome (SOS). SOS can cause chemotherapy-related adverse effects or morbidity after liver resection. Conventionally, SOS is diagnosed using liver biopsy. Recently, it was reported that increased splenic volume (SV) can be used to detect SOS. In this study, we evaluated the changes in SV during adjuvant chemotherapy. METHODS: We enrolled 103 consecutive patients with stage III and high-risk stage II colorectal cancer treated with mFOLFOX6 (n = 37) or oral fluorouracil and leucovorin (n = 66) after curative surger

Oxaliplatin-induced immune-mediated cytopenias: a case report and literature review.

Oxaliplatin is a third-generation platinum antineoplastic agent that commonly causes diarrhea, nausea, vomiting, myelosuppression, and peripheral neuropathy. Less common adverse effects that are increasingly being reported include acute immune-mediated thrombocytopenia, hemolytic anemia, and pancytopenia. Here, we report a patient case of suspected oxaliplatin-induced immune-mediated thrombocytopenia and a thorough literature evaluation of acute oxaliplatin-induced immune-mediated thrombocytopenia, hemolytic anemia, and pancytopenia that has yet to be reported until now. There have been 39 pre
