An Alternative Prosthodontic Management of Maxillary Dentoalveolar Defect in a Patient With Cleft Palate and Lip: Case Report.

Cleft palate/lip is a frequently occurring congenital anomaly; one in every 800 births results in such a problem. Different treatment options are available for replacing missing soft and hard tissues, including removable dental prostheses, fixed dental prostheses (FDPs), and implant prostheses. In the literature, according to different authors, there are different option standard for prosthetic treatment of cleft palate/lip. Some authors report that removable prosthesis is the choice in such cases, some of them regard conventional tooth-supported FDPs as a standard of care. In this case report

[Evaluation of the speech outcomes in patients with unilateral maxillary defect rehabilitated with maxillary obturator prosthesis].

OBJECTIVE: Previous studies have shown a high rate of visualization of uptake and a decrease in serum thyroglobulin (Tg) after therapeutic doses of 131I in well-differentiated thyroid cancer patients with elevated thyroglobulinaemia but negative diagnostic 131I whole-body scan (DxWBS), but its therapeutic effect remains controversial. We evaluate the effect of therapeutic doses of 131I in patients with elevated thyroglobulin level but negative DxWBS. DESIGN: Among papillary thyroid carcinoma patients who underwent total or near-total thyroidectomy and remnant ablation with radioiodine during

The management of the cleft lip and palate patient.

We believe that early closure of the hard palate defect with a palatal appliance and surgical closure of the lip and soft palate according to the rule of tens are very important. Delaying the hard palate closure allows for maximal growth of the palatal shelves. We also feel that pressure equalizing tubes placed in the ears at the initial surgical procedure allows for more normal function of the middle ear, less middle ear infections, better hearing, and ultimately better speech. We feel that early closure of the soft palate may decrease velopharyngeal incompetence by normalizing the position o

Multiple congenital defects: report of a case.

An unusual case history is presented in which a 4-year-old Oriental boy, born with multiple congenital defects primarily affecting anatomic structures in the midline, was treated by an oral surgeon, restorative dentist, and others. The defects included cleft palate, two complete and separate tongues, missing and decayed teeth, and an aortic abnormality. Treatment procedures included restoring the teeth, uniting the two tongues, lip repair, and fabrication of an obturator to close the palatal defect.

Off-Label Use of a Tracheoesophageal Prosthesis for Occlusion of an Oronasal Fistula.

BACKGROUND: An oronasal fistula is an abnormal communication between the oral and nasal cavities, which can arise as a result of radiation therapy for head and neck cancer, resulting in significant speech and swallowing challenges. METHODS: We present a case in which a transesophageal prosthesis (TEP) was used as an alternative to existing treatment options for oronasal fistulas, which include surgical repair or a palatal obturator prosthesis. RESULTS: A 59-year-old female with a history of poorly differentiated squamous cell carcinoma of the nasopharynx with metastatic neck disease, treated

Prosthetic Rehabilitation of a Repaired Cleft Palate with Use of a Two-Part Hinged Magnet Retained Removable Prosthesis.

The presence of a cleft lip and palate is associated with a number of complications and the oral rehabilitation for the improvement of speech, function and esthetics can involve conventional and surgical orthodontics, distraction osteogenesis, fixed and removable prosthodontics, providing obturation of open defects if required. This clinical report describes the prosthodontic management of a patient with a repaired cleft lip and palate with significant maxillary hypoplasia and primary concern of aesthetics with the use of a two-part hinged magnet retained removable prosthesis.

A retrospective cross-sectional analysis of swallowing and tongue functions in maxillectomy patients.

PURPOSE: To investigate the profiles of swallowing and tongue functions, and to identify factors influencing swallowing in maxillectomy patients. METHODS: Maxillectomy patients whose swallowing function defined by Eating Assessment Tool (EAT-10) score and tongue functions (oral diadochokinesis: ODK, maximum tongue pressure: MTP) with or without obturator prostheses had been evaluated were enrolled in this study. The effects of the history of radiotherapy and soft palate defect on swallowing function were evaluated. The effect of radiotherapy on oral dryness was also evaluated. To examine corr

An obturator prosthesis for a 4-year-old child following treatment of a juvenile trabecular ossifying fibroma with a maxillary resection.

Juvenile trabecular ossifying fibroma (JTOF) is a benign fibro-osseous lesion that usually occurs in the maxilla or mandible. Individuals with JTOF of the maxilla will most commonly require a resection that can result in a significant palatal defect. Appearance and articulatory, masticatory, and deglutitory functions can be restored with a clasp-retained obturator. Providing a prosthetic obturator with adequate retention in situations where the teeth cannot accommodate clasps because of insufficient eruption can be challenging. This article describes a technique that has been developed for suc

Primary reconstruction of palatal defects.

Removal of the soft palate can cause marked functional deficit in deglutition and phonation. Most commonly, treatment of this deformity with prosthetic obturation has been less than ideal. Numerous reconstructive techniques have met with only partial success, while deforming distant structures. We will present a technique of reconstruction of the soft palate by use of a superiorly based pharyngeal flap. It has been used successfully in five patients who underwent soft palatectomy for malignant disease. The flaps have been the full width of the pharynx and extended down to the esophageal inlet.

[Interdisciplinary surgical and prosthetic treatment of adenocarcinoma of the palate].

INTRODUCTION: The palatal mucosa requires careful testing prior to prosthetic treatment. Pathological changes may occur during the period of use of a dental prosthetic, especially when worn constantly. Aim of the report: We describe the case of a patient who experienced extensive changes in the mucosa of the hard palate over 12 years of dentures used. CASE PRESENTATION: The 78-year-old female patient was referred to the Department of Otolaryngology of the Jagiellonian University Hospital in Krakow with a diagnosis of adenocarcinoma of the palate. In connection with the planned extensive surge

Reconstruction of the soft palate by uvulopalatal flap.

Surgical removal of the soft palate in cases of neoplastic disease has a functionally detrimental effect on the patient, resulting in rhinolalia and nasal regurgitation. The authors describe their original surgical technique for repairing the lateral soft palate defect using a uvulopalatal flap. The flap is readily available and the procedure is single staged and without sequela. The aim of this reconstructive procedure is to obtain a diminished soft palate defect by primary surgery. In five patients who underwent a partial excision of the soft palate, the surgical defect was corrected at the

Cocaine-induced oronasal communication of the midline of the palate: A case report.

Cocaine abuse poses a significant public health challenge, leading to severe systemic and localized complications. Intranasal cocaine use can result in chronic rhinitis, septal perforation, and palatal perforation due to its vasoconstrictive effects, which cause ischemia and tissue necrosis. We present the case of a 44- year-old woman with a 10-month history of palatal perforation, attributed to 12 years of chronic cocaine use, presented with nasal regurgitation, feeding difficulties, and cachexia. Examination revealed a 3 x 2 cm palatal perforation, nasal asymmetry, and a saddle nose deformit

Prosthodontic considerations in the management of surgically compromised cleft palate patients.

Prosthodontic habilitation with speech aid prostheses for surgically compromised cleft palate patients has been discussed. It can be concluded that surgical redivision of the soft palate with removal of the levator veli palatini muscles as an aid to construction of a pharyngeal obturator is contraindicated. Surgical redivision with removal of the levator muscles prevents subsequent surgical procedures and commits the patient to a prosthesis for life. The management of nonfunctional pharyngeal flaps, large soft palate perforations, and patients with palatopharyngeal insufficiency and/or incompe

From birth till palatoplasty: Prosthetic procedural limitations and safeguarding infants with palatal cleft.

The most imminent issue to be addressed in a child born with cleft lip and/or palate is restoration of normal feeding. Early surgical treatment for cleft repair is crucial but may need to be postponed until certain age and weight gain is attained in an infant. When other feeding interventions fail in these children, prosthetic obturation of the defect with feeding instructions in the interim period is indicated to ward off the prevailing concerns. However, the entire prosthetic management presents a significant challenge with respect to the child's age, scope of iatrogenic injury to the delica

Prosthetic rehabilitation of a patient with bilateral cleft lip and palate using osseointegrated implants and extracoronal resilient attachments: a case report.

This case study reports on the prosthetic rehabilitation of a patient with an unrepaired bilateral cleft lip and palate using osseointegrated implants, extracoronal resilient attachments, a combination of metal-ceramic cement-retained fixed partial restorations, and removable prosthesis. Preoperative clinical examination of a 32-year-old woman with bilateral cleft lip and palate revealed a large oronasal communication, hyperplastic soft tissue surrounding the hard palate defect, and a severely resorbed alveolar ridge. A maxillary obturator prosthesis supported by implants and retained with an
