Real-time algorithm-driven ventilation feedback to improve lung-protective ventilation in patients with ARDS (REALVENT-study): study protocol for a multicentre randomised controlled trial.

BACKGROUND: Lung-protective ventilation is a cornerstone of modern mechanical ventilation, yet real-world adherence to lung-protective targets remains suboptimal. While previous studies have established the physiological benefits of low tidal volume and driving pressure, clinical implementation is hindered by limited monitoring granularity and lack of real-time actionable feedback. This trial aims to evaluate whether a real-time, cloud-based algorithmic feedback platform can improve lung-protective ventilation delivery and contribute to better clinical outcomes in mechanically ventilated patie

Home paediatric CPAP/NIV: the basics.

The use of home continuous positive airway pressure (CPAP) and noninvasive ventilation (NIV) is constantly growing in children. CPAP comprises constant CPAP and auto CPAP (APAP), while NIV comprises several modes of ventilation, among which barometric modes are nowadays the most used. Initiation criteria, indications, initiation location and strategies, and CPAP/NIV settings will be reviewed for both CPAP and NIV. The equipment, comprising the devices, interfaces, circuits and humidifiers, has to be mastered, particularly in paediatrics, with the knowledge of their advantages and limits. A clo

Aerosol therapy in patients receiving noninvasive positive pressure ventilation.

In selected patients, noninvasive positive pressure ventilation (NIPPV) with a facemask is now commonly employed as the first choice for providing mechanical ventilation in the intensive care unit (ICU). Aerosol therapy for treatment of acute or acute-on-chronic respiratory failure in this setting may be delivered by pressurized metered-dose inhaler (pMDI) with a chamber spacer and facemask or nebulizer and facemask. This article reviews the host of factors influencing aerosol delivery with these devices during NIPPV. These factors include (1) the type of ventilator, (2) mode of ventilation, (

Monitoring Systems in Home Ventilation.

Non-invasive ventilation (NIV) is commonly used at home for patient with nocturnal hypoventilation caused by a chronic respiratory failure. Monitoring NIV is required to optimize the ventilator settings when the lung condition changes over time, and to detect common problems such as unintentional leaks, upper airway obstructions, and patient-ventilator asynchronies. This review describes the accuracy and limitations of the data recorded by the ventilator. To efficiently interpret this huge amount of data, clinician assess the daily use and regularity of NIV utilization, the unintentional leaks

Non-invasive ventilation: interpretation of ventilator supplied data.

BACKGROUND: The increasing number of patients treated by long-term non-invasive ventilation (NIV) challenges the capacity of specialised centres to perform in-hospital follow-up evaluations and requires, therefore, from the clinician a thorough and critical appraisal of the information provided by ventilator software as an important component of follow-up assessment. A systematic approach of the information is required along with a knowledge of the limitations in the reliability of some parameters and the variability in modes of reporting, which may be confusing. METHODS: This review reports

Physiology-guided mechanical ventilation: Monitoring, proportional assist, and bounded automation.

Mechanical ventilation has evolved into a complex intervention that influences lung injuries, respiratory muscle function, and hemodynamic stability. Although lung-protective strategies improve outcomes in acute respiratory distress syndrome, bedside management remains limited by incomplete monitoring of key physiologic variables, including lung stress, inspiratory effort and regional ventilation. This constrains decision such as positive end-expiratory pressure titration and ventilatory assist targeting. Emerging technologies aim to address these gaps by improving physiological assessment and

Closed-loop versus conventional synchronization in spontaneously breathing adult patients- a randomized controlled study.

BACKGROUND: The IntelliSync+ [IS+] algorithm provides closed-loop synchronization by continuously analyzing airway pressure and flow waveforms in real time to optimize inspiratory and expiratory cycling. This study evaluated the efficacy of IS+ compared with conventional physician-tailored synchronization during noninvasive ventilation [NIV] in spontaneously breathing adult patients with ARF. METHODS: This multicenter, prospective, randomized controlled, single-blind crossover trial was conducted between March 2024 and May 2025 in two tertiary hospitals. Adult patients receiving NIV for ARF w

Ventilators and Ventilatory Modalities.

Non-invasive ventilation is increasingly used in children for acute and chronic respiratory failure. Ventilators available for clinical use have different levels of complexity, and clinicians need to know in detail their characteristics, setting variables, and performances. A wide range of ventilators are currently used in non-invasive ventilation including bi-level ventilators, intermediate ventilators, and critical care ventilators. Simple or advanced continuous positive airway pressure devices are also available. Differences between ventilators may have implications on the development of as

Flow and airway pressure analysis for detecting ineffective effort during mechanical ventilation in neuromuscular patients.

Ineffective efforts (IEs) are among the most common types of patient-ventilator asynchrony. The objective of this study is to validate IE detection during expiration using pressure and flow signals, with respiratory effort detection by esophageal pressure (Pes) measurement as the reference, in patients with neuromuscular diseases (NMDs). We included 10 patients diagnosed with chronic respiratory failure related to NMD. Twenty-eight 5-minute recordings of daytime ventilation were studied for IE detection. Standard formulas were used to calculate sensitivity, specificity, positive predictive val

Neural versus pneumatic control of pressure support in patients with chronic obstructive pulmonary diseases at different levels of positive end expiratory pressure: a physiological study.

INTRODUCTION: Intrinsic positive end-expiratory pressure (PEEPi) is a "threshold" load that must be overcome to trigger conventional pneumatically-controlled pressure support (PSP) in chronic obstructive pulmonary disease (COPD). Application of extrinsic PEEP (PEEPe) reduces trigger delays and mechanical inspiratory efforts. Using the diaphragm electrical activity (EAdi), neurally controlled pressure support (PSN) could hypothetically eliminate asynchrony and reduce mechanical inspiratory effort, hence substituting the need for PEEPe. The primary objective of this study was to show that PSN ca

Respiratory patterns during sleep in obesity-hypoventilation patients treated with nocturnal pressure support: a preliminary report.

BACKGROUND: The obesity-hypoventilation syndrome (OHS), commonly defined as a combination of obesity and diurnal hypercapnia, is efficiently treated using nasal positive pressure ventilation (NPPV). The present study aimed to determine whether nocturnal polysomnography allows detection of respiratory disturbances occurring in patients with OHS treated with NPPV that may interfere with the quality of sleep and of ventilatory support, and are not detected by nocturnal pulse oximetry and capnography. METHODS: Twenty OHS patients in stable clinical condition treated by NPPV for at least 3 months

Combined use of Neurally Adjusted Ventilatory Assist (NAVA) and Vertical Expandable Prostethic Titanium Rib (VEPTR) in a patient with Spondylocostal dysostosis and associated bronchomalacia.

Jarcho-Levin syndrome is a rare disorder characterised by defects in vertebral and costal segmentation of varying severity. Respiratory complications are the main cause of death or severe comorbidity due to a restrictive rib cage. A 3 months old infant with Spondylocostal dysostosis and associated bronchomalacia experiencing severe asynchrony during the weaning process is reported. The Neurally Adjusted Ventilatory Assist (NAVA) mode was used to improve adaptation to mechanical ventilation after Vertical Expandable Prosthetic Titanium Ribs (VEPTRs) were implanted. The synchrony achieved with t

[Non-invasive ventilation].

The advent of non-invasive mechanical ventilation (NIMV) has radically changed the management of acute and chronic respiratory failure. Over the last few years, the number of possible applications of NIMV has progressively increased, both in the hospital and extrahospital setting. NIMV is now used in all hospitals and resident physicians currently receive specific training -nonexistent until a few years ago- in this modality. It falls to all of us to push forward the clinical and scientific advances represented by the development of NIMV, by promoting the events that accompany better knowledge

The contribution of mechanical ventilation to critical illness-associated diaphragm dysfunction: a critical appraisal.

Although diaphragm dysfunction is increasingly recognized as a major contributor to morbidity and mortality in the intensive care unit, the physiological mechanisms leading to its development in mechanically ventilated patients are incompletely understood. Mechanical ventilation is often recognized as the main cause of the problem, delineating a paradigm known as ventilator-induced diaphragm dysfunction (VIDD). Yet, evidence on this causal relationship in ventilated critically ill patients is scarce, with direct experimental data mostly coming from animal models or brain-dead organ donors. In

Fundamental concepts and the latest evidence for esophageal pressure monitoring.

Transpulmonary pressure is an essential physiologic concept as it reflects the true pressure across the alveoli, and is a more precise marker for lung stress. To calculate transpulmonary pressure, one needs an estimate of both alveolar pressure and pleural pressure. Airway pressure during conditions of no flow is the most widely accepted surrogate for alveolar pressure, while esophageal pressure remains the most widely measured surrogate marker for pleural pressure. This review will cover important concepts and clinical applications for esophageal manometry, with a particular focus on how to u

Analgosedation During the Use of Non-Invasive Respiratory Supports: A Synthesis of Clinical Evidence and Best Practices.

Non-invasive respiratory support (NRS) has become a cornerstone in the management of acute respiratory failure (ARF), offering an alternative or a bridge between conventional oxygen therapy (COT) and invasive mechanical ventilation (iMV). While NRS techniques-including non-invasive ventilation (NIV), continuous positive airway pressure (CPAP), and high-flow nasal oxygen (HFNO)-have demonstrated efficacy in reducing intubation rates and improving outcomes, patient tolerance and synchrony remain critical determinants of success. Analgosedation, the strategic use of analgesics and sedatives, has

Spontaneous breathing-induced lung injury in mechanically ventilated patients.

PURPOSE OF REVIEW: Recent experimental and clinical studies have suggested that spontaneous effort can potentially injure the lungs. This review summarizes the harmful effects of spontaneous breathing on the lungs during mechanical ventilation in ARDS and suggests potential strategies to minimize spontaneous breathing-induced lung injury. RECENT FINDINGS: Recent clinical and experimental studies have shown that vigorous spontaneous breathing during mechanical ventilation can potentially injure the lungs due to high transpulmonary pressure, the Pendelluft phenomenon, increased pulmonary perfus
