Pectoralis Muscle Flap Repair Reduces Paradoxical Motion of the Chest Wall in Complex Sternal Wound Dehiscence.

BACKGROUND: The aim of the study was to test the hypothesis that in patients with chronic complex sternum dehiscence, the use of muscle flap repair minimizes the occurrence of paradoxical motion of the chest wall (CWPM) when compared to sternal rewiring, eventually leading to better respiratory function and clinical outcomes during follow-up. METHODS: In a propensity score matching analysis, out of 94 patients who underwent sternal reconstruction, 20 patients were selected: 10 patients underwent sternal reconstruction with bilateral pectoralis muscle flaps (group 1) and 10 underwent sternal r

Pectoralis major transfer for subscapular deficiency: anatomical study of the relationship between the transferred muscle and the musculocutaneous nerve.

PURPOSE: Pectoralis major transfer is indicated for irreparable subscapularis tendon tears. One surgical option is transferring the sternal part of the pectoralis major to the humeral insertion of the subscapularis under the conjoined tendon of the coracobrachialis and biceps muscles. The purpose of this study is to define the anatomical relationship between the transferred tendon and the musculocutaneous nerve. METHODS: In 52 cadaveric fresh-frozen shoulders, the relevant structures were dissected and a pectoralis major transfer was performed. The relationship between the transferred tendon,

Evaluation of pectoralis major muscle in patients with posterior breast tumors on breast MR images: early experience.

PURPOSE: To evaluate the ability to use breast magnetic resonance (MR) imaging to assess disease extent in patients with posterior breast masses who are suspected to have tumor invasion into underlying muscle. MATERIALS AND METHODS: Nineteen patients with posterior breast masses underwent three-dimensional, gradient-echo, 1.5-T MR imaging before and after the administration of gadopentetate dimeglumine. Thirteen had deep palpable masses that were clinically determined to be fixed to the underlying chest wall. Twelve had mammographic findings that caused muscle involvement to be suspected, and

[A case report of poststernotomy mediastinitis treated with the pectoralis major muscle flap after protracted wound irrigation].

A 8-year-old boy developed mediastinitis after direct closure of atrial septal defect. Methicillin-resistant Staphylococcus aureus (MRSA) was detected from the wound. Intermittent wound irrigation with povidone-iodine solution and vancomycin solution was protracted for two months. After the remission of MRSA infection, implantation of pectoralis major muscle flap was performed. Without recurrence of infection the wound was healed completely. Although pectoralis major muscle flap is an effective method of choice for mediastinitis, an appropriate timing of enforcing this method should be investi

Comparing outcomes of prepectoral, partial muscle-splitting subpectoral, and dual-plane subpectoral direct-to-implant reconstruction: implant upward migration and the pectoralis muscle.

BACKGROUND: Although dual-plane subpectoral breast reconstruction has been widely implemented in implant-based breast reconstruction, animation deformities remain an issue. Recent advances in skin flap circulation detection have increased the use of prepectoral reconstruction. A partial muscle-splitting subpectoral plane was introduced to decrease the visibility of the implant edge. However, there is yet to be a direct comparison of these methods for optimal results, including changes in implant position after reconstruction. This study aims to compare the incidence of complications such as ri

The effect of thymus transplantation on donor-specific chimerism in the rat model of composite osseomusculocutaneous sternum, ribs, thymus, pectoralis muscles, and skin allotransplantation.

INTRODUCTION: Research on tolerance has proven that development of donor-specific chimerism (DSC) may accompany tolerance induction in vascularized composite allotransplantation (VCA). In this study, we aimed to determine the effect of thymus transplantation on the induction of DSC in rat VCA model of osseomusculocutaneous sternum (OMCS) and osseomusculocutaneous sternum and thymus (OMCST) allotransplantation. MATERIALS AND METHODS: A total of 20 Lewis-Brown Norway and Lewis rats, 5-6 weeks old, weighting between 120 and 150 g, were used in the study. OMCS (n = 5) and OMCST (n = 5) allografts

Quantification of Sarcopenia using Chest Computed Tomography of the Pectoralis Major Muscle as a Prognostic Tool for Cardiac Surgery Outcomes.

BACKGROUND AND AIM: Cardiac surgery is increasingly performed on elderly, frail patients, making objective frailty markers critical for predicting outcomes. Sarcopenia, as defined by the progressive loss of muscle mass and strength, is linked to poorer outcomes after surgery. The pectoralis major (PM) muscle cross-sectional area from chest computed tomography (CT) may offer a novel quantitative method for assessing sarcopenia in cardiac surgery patients. METHODS: This study includes data from 237 individuals, who had preoperative chest CT scans and underwent cardiac surgery involving sternoto

Cadaveric and Ultrasound-Guided Evaluation of Two Needling Approaches Targeting the Pectoralis Minor Muscle: A Pilot Feasibility Study.

Background: The pectoralis minor muscle can be a source of musculoskeletal-related chest pain by contributing to thoracic outlet syndrome. Needling interventions applied to chest wall muscles have an inherent risk of puncturing sensitive structures, e.g., the pleura. Objective: The objective of this study was to preliminarily investigate the safety and accuracy of two needling approaches targeting the pectoralis minor muscle. Methods: A pincer- and flat-needle approach targeting the pectoralis minor muscle was conducted in five Thiel-embalmed cadavers and 10 healthy volunteers by an experience

Diagnostic Accuracy of Scalene and Pectoralis Minor Muscle Blocks for Thoracic Outlet Syndrome: A Systematic Review and Meta-Analysis.

BackgroundThoracic outlet syndrome (TOS) is a complex neurovascular condition that remains challenging to diagnose, particularly neurogenic TOS (nTOS), which comprises most cases. While vascular TOS has clear diagnostic criteria, nTOS diagnosis relies on clinical assessments, imaging, and electrophysiologic studies. Scalene and pectoralis minor muscle blocks have been proposed as diagnostic tools, but their accuracy remains uncertain.ObjectiveThis systematic review and meta-analysis assesses the pooled sensitivity, specificity, and diagnostic accuracy of scalene and pectoralis minor blocks for

Granulomatous mastitis and pectoralis major muscle defect following polyacrylamide hydrogel injection: a case report and literature review.

BACKGROUND: Breast augmentation through the injection of polyacrylamide hydrogel (PAAG) was a popular procedure in the past, but it has since been prohibited due to various complications, including masses, migration, infection, inflammation, and even cancer. However, there were rare cases of granulomatous mastitis with pectoralis major muscle defect following PAAG injection for breast augmentation. CASE DESCRIPTION: A 40-year-old female patient presented with a swollen and suppurative mass in her left breast and was insensitive to antibiotics. She was admitted to our department for further tr

Preoperative Sarcopenia Assessment Using Pectoralis Muscle Mass Indicated Poor Mid-term Cardiac Surgery Prognosis.

BACKGROUND: Many studies have defined sarcopenia based on psoas muscle mass using abdominal computed tomography (CT). We hypothesized that sarcopenia can be assessed by measuring pectoralis muscle mass on chest CT and aimed to examine its relationship with the postoperative prognosis of cardiac surgery. METHODS: This retrospective study included 189 patients who underwent cardiac surgery via median sternotomy between July 2020 and June 2022. We excluded patients <70 years old, urgent/emergent cases, no chest CT within 90 days before surgery, and cases in which evaluation of the pectoralis mus

Wound complication after modified Ravitch for pectus excavatum: A case of conservative treatment enhanced by pectoralis muscle transposition.

INTRODUCTION: Multiple surgical debridement sessions are mandatory before wound closure in cases of infection after a modified Ravitch procedure for pectus excavatum. Vacuum-assisted closure (VAC) is a well-established technical resource for treating complicated wounds; however, in cases of suspicion of bone infection, this approach is not enough to prevent bar removal. PRESENTATION OF THE CASE: We present a case of surgical wound dehiscence with hardware exposure in a patient who had undergone chondrosternoplasty for pectus excavatum. Several sessions of debridement (three) and VAC were appl

Pectoralis Major Muscle 3D Volumetric Reconstruction in the Tuberous Breast: Anatomical and Radiomics Differences With Potential Surgical Impact.

BACKGROUND: Tuberous breast (TB) is a congenital anomaly characterized by morphological, structural, and breast region alterations. The pectoralis major muscle (PMM) is crucial for correction when placing breast implants. Alterations at this level have received little attention in the literature. OBJECTIVES: In this study we aimed to define the anatomical characteristics of the PMM in TB patients with radiomics data extracted from MRI sequences. METHODS: The PMMs of 30 TB patients and 30 controls were analyzed by manual segmentation with 3D-Slicer v.5.4.0 software and radiomics techniques. A

CT pectoralis muscle area is associated with DXA lean mass and correlates with emphysema progression in a tobacco-exposed cohort.

INTRODUCTION: Muscle loss is an important extrapulmonary manifestation of COPD. Dual energy X-ray absorptiometry (DXA) is the method of choice for body composition measurement but is not widely used for muscle mass evaluation. The pectoralis muscle area (PMA) is quantifiable by CT and predicts cross-sectional COPD-related morbidity. There are no studies that compare PMA with DXA measures or that evaluate longitudinal relationships between PMA and lung disease progression. METHODS: Participants from our longitudinal tobacco-exposed cohort had baseline and 6-year chest CT (n=259) and DXA (n=164

Reconstruction of sternal defects after sternotomy with postoperative osteomyelitis, using a unilateral pectoralis major advancement muscle flap.

BACKGROUND: The pectoralis major flap, which is usually harvested bilaterally, is considered a workhorse flap in the reconstruction of sternal defects. After a median sternotomy for open heart surgery, 1%-3% of patients develop deep infection and dehiscence of the sternal wound, some of which will eventually require reconstructive surgery. Our aim was to describe the clinical feasibility and associated complications of the unilateral pectoralis major advancement flap in the reconstruction of sternal defects. METHODS: A retrospective analysis of all adult patients who were operated on using a

Detection of differentially expressed genes in broiler pectoralis major muscle affected by White Striping - Wooden Breast myopathies.

White Striping and Wooden Breast (WS/WB) are abnormalities increasingly occurring in the fillets of high breast yield and growth rate chicken hybrids. These defects lead to consistent economic losses for poultry meat industry, as affected broiler fillets present an impaired visual appearance that negatively affects consumers' acceptability. Previous studies have highlighted in affected fillets a severely damaged muscle, showing profound inflammation, fibrosis, and lipidosis. The present study investigated the differentially expressed genes and pathways linked to the compositional changes obser
