The effectiveness of using local anesthesia with general anesthesia in pediatric dental rehabilitation: a systematic review and meta-analysis.

BACKGROUND: There are no established evidence-based guidelines for the administration of intraoperative local anesthesia (LA) during dental rehabilitation under general anesthesia (GA). This systematic review aims to evaluate the effects of intraoperative LA on physiological parameters and pain control in children undergoing GA for comprehensive dental treatment. METHODS: Comprehensive search was conducted across four electronic databases: PubMed, Cochrane Central Register of Controlled Trials, EBSCO, and Scopus. Search strategy was structured, yielding 14 articles that follow the study inclu

The Anesthesia Perioperative "Call for Help"-Experience at a Quaternary Pediatric Medical Center: Analysis of 67,564 Anesthesia Encounters.

BACKGROUND: During the past several decades, anesthesia has become increasingly safe. Truly major adverse events are rare, and anesthesia quality researchers have instituted programs to evaluate "near miss" or less critical adverse events to evaluate the safety of anesthesia delivery. In this study, we aimed to evaluate calls for emergency help in our institution as a surrogate for pending critical events. We hypothesized that calls would be more common in patients with high American Society of Anesthesiologists (ASA) physical status, history of prematurity, and children with recent respirator

Remimazolam in pediatric sedation and anesthesia: current perspectives and clinical considerations.

Remimazolam is a novel, short-acting benzodiazepine that acts as a high-affinity selective ligand for the GABA-A receptor. Since its approval, growing evidence has demonstrated its safety and efficacy for sedation and anesthesia in adult patients. Recently, increasing interest has emerged regarding the utility of remimazolam in pediatric patients due to its favorable characteristics, including its limited suppressive effect on respiration and circulation in comparison to other sedatives. However, its clinical use in pediatric patients requires special consideration despite the overall safety p

Use of norepinephrine for intraoperative hypotension in pediatric anesthesia: a French survey.

INTRODUCTION: Intraoperative hypotension (IOH) is a common complication in the operating room. Vasopressors are crucial in managing IOH but data on their use in children, particularly norepinephrine (NE), are limited. This study aimed to explore NE use in IOH management among French pediatric anesthesiologists. MATERIALS AND METHODS: A survey was conducted using an online Google Forms® questionnaire, validated by experienced pediatric anesthesiologists and the ADARPEF board. Distributed via the ADARPEF Research Network, the survey covered demographics, IOH definitions, treatment approaches, a

Remimazolam in pediatric anesthesia: a systematic review for clinical decision-making.

BACKGROUND: Remimazolam's role in pediatric anesthesia is evolving. We systematically reviewed 2024-2025 evidence to establish a clinical decision-making framework for its use. METHODS: Following PRISMA guidelines, a systematic search identified 23 studies (15 RCTs) involving 2,847 pediatric patients for narrative synthesis. RESULTS: Remimazolam demonstrated superior hemodynamic stability vs. propofol (cardiovascular complications: RR 0.30, 95% CI 0.20-0.46) and reduced emergence delirium by 61% (RR 0.39, 95% CI 0.21-0.70). Pediatric regional anesthesia and acute pain management: State of the art.

Pediatric regional anesthesia has been in existence for over 125 years, but significant progress and widespread use has occurred in the last few decades, with the increasing availability of ultrasound guidance. Evidence supporting the safety of regional anesthesia when performed under general anesthesia has also allowed the field to flourish. Newer techniques allow for more precise nerve blockade and in general this has resulted in more peripheral blocks replacing central blocks, such as caudal epidurals and spinal anesthesia. Current controversial topics in the field include the method of obt

Evaluation of the stability and stratification of propofol and ketamine mixtures for pediatric anesthesia.

BACKGROUND: The combination of propofol and ketamine is commonly used for total intravenous anesthesia. These drugs can be delivered in different syringes or in the same syringe. We hypothesized that the drugs might separate and different concentrations of each drug could be found in different parts of the syringe during the procedure period when they were mixed in 1 syringe. METHODS: Twelve 60-mL polypropylene syringes were prepared by mixing propofol and ketamine as 4 groups on the basis of propofol/ketamine mixture ratios (5:1 and 6.7:1) and propofol solution concentrations. Syringes were

Trends in death associated with pediatric dental sedation and general anesthesia.

BACKGROUND: Inadequate access to oral health care places children at risk of caries. Disease severity and inability to cooperate often result in treatment with general anesthesia (GA). Sedation is increasingly popular and viewed as lower risk than GA in community settings. Currently, few data are available to quantify pediatric morbidity and mortality related to dental anesthesia. OBJECTIVE: Summarize dental anesthesia-related pediatric deaths described in media reports. METHODS: Review of media reports in the Lexis-Nexis Academic database and a private foundation website. SETTINGS: Dental

A Window to the Stomach-Gastric Ultrasound's Growing Role in Pediatric Anesthesia.

Gastric ultrasound (GUS) is an evolving Point-of-Care Ultrasound (POCUS) tool whose applicability is being increasingly studied in pediatric anesthesia. The technique evaluates the gastric antrum, in the right lateral decubitus or supine positions to determine the quality and quantity of stomach contents. The Perlas 3-point grading is a semi-quantitative assessment method, which can be applied to pediatric patients as well. GUS may be potentially useful in situations when a child's fasting status is uncertain or gastric emptying is delayed as an adjunct to clinical decision-making. Although qu

[RSI in pediatric anesthesia: an online survey of actual clinical practice].

BACKGROUND: For many years, controlled rapid sequence induction (cRSI) has been recommended in children at risk of aspiration to reduce the risk of hypoxia; however, no fixed age limit has been defined for the transition to classical RSI. This study examines the current practices of RSI in children as well as controversies regarding existing approvals, recommendations and clinical guidelines. METHODS: An anonymous online survey was carried out among the members of the Scientific Working Group on Pedíatric Anesthesia (WAKKA) of the German Society of Anesthesiology and Intensive Care Medicine (

Effect of general anesthesia on delayed repeat enema in pediatric intussusception.

BACKGROUND: The study aimed to assess the efficacy and safety of delayed repeated enema (DRE) and to evaluate the effect of general anesthesia (GA) on DRE. METHODS: A retrospective analysis was conducted of all children below 3 years with primary intussusception who were offered DRE in our tertiary center, from 2014 until 2019. Following a standardized pneumatic enema protocol, those who showed a partially successful result were offered DRE 2 h later, either awake (Group A) or under GA (Group B). DRE under GA was our preferred procedure unless there were no pediatric anesthetists available at

Hyperthermia in a pediatric patient with neuroblastoma during anesthesia: a case report.

BACKGROUND: Neuroblastoma is the most common malignant extracranial solid tumor in pediatrics patients. Intraoperative hyperthermia is extremely rare in patients with neuroblastoma and can cause a series of complications. Here, we represent a case of neuroblastoma accompanied by hyperthermia during anesthesia, and propose a rational explanation and management options. CASE PRESENTATION: The patient had gait disturbance and sitting-related pain without fever. Magnetic resonance imaging revealed a soft tissue mass located in the right posterior mediastinum, paravertebral space and canalis verte

Respiratory prehabilitation in pediatric anesthesia in children with muscular and neurologic disease.

Children with neuromuscular, chronic neurologic, and chest wall diseases are at increased risk of postoperative respiratory complications including atelectasis, pneumonia, and respiratory failure with the possible need for reintubation or even tracheostomy. These complications negatively impact patient outcomes, including increased healthcare resource utilization and increased surgical mortality. In these children, the existing respiratory reserve is often inadequate to withstand the stresses brought on during anesthesia and surgery. A thorough clinical assessment and objective evaluation of p
