Contact allergens produce three types of reactions. The commonest reaction is allergic contact dermatitis (ACD). The commonest contactants in Singapore causing ACD are, in decreasing order of importance, nickel, fragrances, neomycin, lanolin, colophony, flavine, rubber chemicals, paraphenylenediamine, clioquinol and chromates. BACKGROUND: Many patients with dermatitis over photo-exposed body areas are positive to many contact allergens and have a pre-existing allergic contact dermatitis.
METHODS: This study included patients who presented to a tertiary centre in India with dermatitis on photo-exposed body areas suspected of chronic actinic dermatitis. Their detailed histories were recorded and cutaneous and systemic examinations were performed. Patch testing was done in all the patients and photo-patch testing was carried out in 86 patients.

The frequency of plant dermatitis, in general, depends on the type of plants, the climate, occupation and lifestyle of the local inhabitants. Many plants produce adverse effects on the skin like irritation, allergic reactions and phototoxicity. Generally, allergic contact dermatitis is emphasized but it is necessary to give more attention to phototoxicity produced by plants. The patients may be exposed not only to the phototoxic plant itself but also to its derivatives in perfumes, cosmetics and industry.

Ethereal oils are widely used, in food, toothpaste, as flavouring agents in perfumes and cosmetics, and in dermatological treatment, as antiseptics, adstringenta, antipruritic tinctures, lotions and pomades. The use of ethereal oils seems to have increased in recent years, probably due to a growing interest in alternative medicine, and especially aromatherapy. By describing the cases of three patients with allergic contact dermatitis, we wish to point out possible unwanted side effects of using ethereal oils.

The Scandinavian photopatch test procedure has been applied to 745 patients with suspected photodermatoses during the years 1980-1981. Our experience has been encouraging with the recording of several relevant reactions. A total of 132 positive photocontact reactions and 120 ordinary contact reactions were seen. Patch test reactions to 11 common contact allergens were studied after 24 h and 48 h occlusion with Finn Chambers in 390 patients. Concordant allergic results were found in 96 cases (74%). In 22 patients (17%), the reaction was positive only after 48 h and in 11 cases (8.5%) only after 24 h exposure. Facial and neck pigmentations are significant cosmetic problems. They are common in middle-aged women, related to endogenous (hormones) and exogenous factors (cosmetics, perfumes, sun exposure), and often represent paramount causes of emotional distress. Although melasma is the most common cause of facial pigmentation, there are many other forms including drug-induced and postinflammatory hyperpigmentation. We review pathogenesis, clinical and histopathological data, effect on quality of life, and treatment options in facial hyperpigmentation disorders.

Despite the decline in the incidence of berloque dermatitis during the past decade, milder cases resulting from the presence of relatively small quantities of bergapten in perfumed cosmetics continue to occur. The resultant hyperpigmentation may have an atypical presentation leading to error in diagnosis. In addition, the condition can sometimes induce or aggravate melasma and may be responsible for other pigmentary disorders of the face and neck of questionable etiology such as poikiloderma of Civatte, Riehl's melanosis and pigmented peribuccal erythema of Brocq.

Usually, recorded cases of allergic contact dermatitis to tobacco are confined to occupational diseases and involve agricultural workers and those engaged in manufacturing or selling the products, all of whom are in contact with tobacco leaves. In a one-year study we tested the skin care and protective action of a new hydrocarbon gel. This gel is made of a two-component gel of petrolatum fractions in which the easily volatile and irritant white oils have been diminished. The gel contains no additives or perfumes and is therefore free of these allergens. The tests on different groups of "wet workers" ( gardeners and fish- sellers as well as workers in the fish preparation and canned-food industries) showed both objectively and subjectively good results.
