Correlation between Transforming Growth Factor-β and Periapical Lesions in Patients with Chronic Apical Periodontitis.

OBJECTIVE: To examine the expression of transforming growth factor-β (TGF-β) in the periapical granulation tissue and serum of patients with chronic apical periodontitis and to conduct immunohistochemical analysis so as to explore the relationship between TGF-β and the degree of periapical lesions. METHODS: Periapical granulation tissues of 20 cases of chronic apical periodontitis were collected as the experimental group. Healthy gingival tissues without eruption of third molars of 5 cases were collected as the control group. Immunohistochemistry, enzyme-linked immunosorbent assay, and real-t

Langerhans' cell histiocytosis in association with periapical granulomas and cysts.

This report describes a series of six cases of inflammatory periapical disease with small aggregates of Langerhans cells as a minor component. Immunohistochemical findings confirm that the cells are phenotypically related to Langerhans cells. Aggregates of these cells are not normally found in radicular cysts or periapical granulomas and have been interpreted to represent chronic localized Langerhans' cells histiocytosis. Whether these lesions, which arise within the context of chronic inflammatory periapical disease, represent incipient eosinophilic granulomas or are a more benign, minimally

Inhibition of RGS10 Aggravates Periapical Periodontitis via Upregulation of the NF-κB Pathway.

INTRODUCTION: Periapical periodontitis develops due to the interplay between root canal microorganisms and host defenses. The mechanism underlying the pathogenesis of periapical periodontitis remains unclear. Regulator of G protein signaling protein 10 (RGS10) has been suggested to play a role in regulating inflammation. This study explored the potential regulatory effects of RGS10 on periapical periodontitis and the proinflammatory pathway of nuclear factor (NF)-κB. METHODS: Disease models of periapical inflammation in mice were established, and adenovirus-associated virus (AAV) was used to

Presence and consequence of tooth periapical radiolucency in patients with cirrhosis.

BACKGROUND: Periapical radiolucency is the radiographic sign of inflammatory bone lesions around the apex of the tooth. We determined the prevalence and predictors of periapical radiolucency in patients with cirrhosis and the association with systemic inflammation status and cirrhosis-related complications. METHODS: A total of 110 cirrhosis patients were consecutively enrolled. Periapical radiolucency was defined as the presence of radiolucency or widening of the periapical periodontal ligament space to more than twice the normal width. Predictors of periapical radiolucency and the associatio

Hypoxia Exacerbates Periapical Periodontitis-Associated Pathological Bone Loss via the Hypoxia-Inducible Factor-2α-Calmodulin-Dependent Protein Kinase IV Axis.

Periapical periodontitis is one of the most common inflammatory bone destructive diseases. Epidemiological evidence suggests that hypoxia exposure, such as that resulting from high-altitude exposure or sleep apnea syndrome, may be a significant risk factor that exacerbates the disease process. However, its specific role and the underlying molecular mechanisms remain unclear. In this study, we established a mouse model of periapical periodontitis under conditions of chronic hypoxia to evaluate its impact on pathological bone loss using micro-computed tomography, histological staining, and serum

Prevalence of unculturable bacteria in the periapical abscess: A systematic review and meta-analysis.

OBJECTIVE: To assess the prevalence of unculturable bacteria in periapical abscess, radicular cyst, and periapical granuloma. METHODS: PubMed, Scopus, Science Direct, and Ovid databases were systematically searched from January 1990 to May 2020. All the included studies were cross-sectional design. The risk of bias was assessed using Joanna Briggs Institute check-list. Heterogeneity was described using meta-regression and mixed-effects model for lesion, country, and sequence technique moderators. Funnel plot and unweighted Egger's regression test were used to estimate the publication bias. Mi

Association between Odontogenic Conditions and Maxillary Sinus Disease: A Study Using Cone-beam Computed Tomography.

INTRODUCTION: The maxillary sinus can be affected by dental infections because of its close relationship with upper teeth. This study aimed to assess the most common types of maxillary sinus alterations and to associate them with odontogenic conditions using cone-beam computed tomographic (CBCT) images. METHODS: CBCT scans of 400 patients showing sinus disease in 1 or both maxillary sinuses were evaluated. Sinus alteration was considered as follows: generalized or localized mucosal thickening (MT), maxillary sinusitis (MS), and retention cysts (RCs). The odontogenic conditions evaluated were

The possible role of Gremlin1 in inflammatory apical periodontitis.

OBJECTIVE: In this study, we investigated the involvement of Gremlin1 on the pathological process of apical periodontitis and detect the underlying mechanisms preliminarily. METHODS: Clinical healthy and inflamed periapical specimens were collected. Then, apical periodontitis (AP) animal models were established by consistent pulp exposure. In addition, AAV-shGremlin1 was injected into inflamed periapical lesions to inhibit the expression of Gremlin1. Alveolar bone loss was measured by Micro-CT. Furthermore, immunohistochemical or immunofluorescence staining of Gremlin1, phosphorylated-CREB, I

A bacteriological and histological evaluation of 58 periapical lesions.

Periapical tissue from 58 cases requiring periapical surgery was examined histologically and cultured for the presence of microbes. Twenty-nine had a possible oral cavity communication and 29 did not. Approximately one-half of each biopsy was submitted for culture while the other portion was examined histologically. Cultures were positive for the presence of bacteria in 51 of 58 cases while bacteria were seen histologically in only 8 of 58 cases. A total of 50 different species of bacteria were isolated from the 58 cultures of periapical tissue. Of 133 isolates, 87 were strict anaerobes, 37 we

The Dynamic Expression Changes of Neutrophil Extracellular Traps in Mouse Apical Periodontitis: A Potential Correlation With IL-17.

Neutrophil extracellular traps (NETs) consist of decondensed chromatin and antimicrobial proteins, which are released from neutrophils and have been implicated in several inflammatory diseases. NETs and interleukin (IL)-17 constitute a feed-forward loop that promotes immunopathological development of inflammation. Therefore, this study aimed to investigate the dynamic distribution of NETs and their colocalization with IL-17 during the progression of apical periodontitis in established mouse models. Apical periodontitis was induced in mice by exposing the pulp of the mandibular first molars, wi

Periapical actinomycosis of the mandible: A diagnostic challenge mimicking conventional apical periodontitis- A case report.

Actinomycosis is a rare, chronic suppurative infection caused by filamentous, Gram-positive anaerobic bacteria, primarily Actinomyces israelii, with cervicofacial involvement being the most common presentation. Periapical actinomycosis, a rare central subtype of cervicofacial actinomycosis accounting for approximately 1-2% of central cases, is characterized by an indolent, less invasive course. This case report describes an unusual presentation of mandibular periapical actinomycosis in a 26-year-old immunocompetent female with no history of trauma or predisposing comorbidities. The patient pre

Multiple Radiolucencies of the Jaws in a 6 Year Old.

Radiolucencies of the jaws are a common finding. Mostly, they are isolated findings associated with inflammatory processes and consistent with inflammatory periapical disease, such as periapical cysts and periapical granulomas. If associated with the crowns of unerupted teeth, they may represent dentigerous cysts. However, occasionally, larger lytic/radiolucent lesions are identified; even more infrequently, these radiographic findings may be multiple and concurrent. The purpose of this report is to present the case of a 6-year-old female patient with no known concomitant syndrome who presente

Periapical actinomycosis.

Two cases of actinomycotic lesions of the periapical tissue in relation to the molars are described. The difficulty in obtaining a positive culture for actinomycetes in spite of definite histologic evidence of actinomycosis is highlighted. Besides antibiotics, the importance of planned treatment to deal with the underlying diseased periapical tissue and the tooth is emphasized.

Expression, Regulation and Therapeutic Implications of Nod-Like Receptors in Endodontics: A Narrative Review.

BACKGROUND: Nucleotide-binding oligomerization domain (NOD)-like receptors (NLRs) are critical intracellular pattern recognition receptors that orchestrate innate and adaptive immune responses. Their dysregulated activation is mechanistically linked to the pathogenesis of numerous inflammatory diseases. In endodontics, pulpitis and apical periodontitis represent highly prevalent inflammatory conditions, primarily caused by bacterial invasion, which significantly impact patients' quality of life. Current studies demonstrate that NLR-mediated immunity plays a significant role in driving pulpal i
