Expression analysis of wound healing genes in human periapical granulomas of progressive and stable nature.

INTRODUCTION: Wound healing process involves the activation of extracellular matrix components, remodeling enzymes, cellular adhesion molecules, growth factors, cytokines and chemokines genes. However, the molecular patterns underlying the healing process at the periapical environment remain unclear. Here we hypothesized that endodontic infection might result in an imbalance in the expression of wound healing genes involved in the pathogenesis of periapical lesions. Furthermore, we suggest that differential expression of wound healing markers in active and latent granulomas could account for d

Prevalence of Human Papillomavirus (HPV)-16 in Different Dental Infections in the Lebanese Population.

BACKGROUND: Dental infections, which are the main cause of tooth loss, are known to be caused by bacteria. However, recent research suggests that other organisms, such as viruses, may also play a role. In this study, we aim to detect the presence of human papillomavirus (HPV)-16 and assess its prevalence in tissues infected with various dental infections, including aggressive and chronic periodontitis, pericoronitis, and periapical infection, as well as healthy gingival tissues, saliva, and gingival crevicular fluid, for comparison. METHODS: A cross-sectional study including 124 adult healthy

[Histopathological and immunohistochemical investigations of chronic periapical inflammations of permanent teeth].

Histopathological investigations were done in 72 cases of periapical granulomas and 9 cases of periapical cysts demonstrating that both granulomas and cysts were composed of granulation tissue varying in maturity. Immunohistochemical studies by the peroxidase method PAP showed the presence of light chains (kappa and lambda) and heavy chains (IgA, IgM, IgG, IgE). In the chronic inflammatory infiltrates of granulomas and cysts plasma cells with IgG prevailed. These observations suggest that specific immune reactions participate in the pathological mechanism of chronic periapical inflammatory pro

Adenomatoid odontogenic tumor mimicking a periapical (radicular) cyst: a case report.

The adenomatoid odontogenic tumor (AOT) is a benign (hamartomatous), noninvasive lesion with slow but progressive growth. A rare subvariant of the extrafollicular type of AOT may mimic periapical disease radiographically. We report on a 15-year-old girl with a periapical radiolucent lesion affecting teeth 7 and 8 initially suspicious of periapical disease, although clinical findings seemed to indicate a nonendodontic lesion. An exploratory surgical approach was chosen, and the final diagnosis of the removed noncystic tissue mass was microscopically confirmed to be an AOT.

Histological observations of periradicular healing following root canal treatment.

The purpose of this study was to observe histologically the sequence of events leading to resolution of periradicular tissues, with a view to advancing the perception of periapical healing. Our material consisted of periapical specimens obtained from 15 single-rooted, endodontically treated teeth of patients aged 25-40 years. All the teeth required extraction because of complicated crown-root fracture following trauma, iatrogenic aetiology (cervical root resorption following bleaching) or extensive carious lesions. The patients were informed that, based on their clinical condition, surgical pr

A Causal Association Study Between Chronic Kidney Disease and Oral Health: A Mendelian Randomization Study.

BACKGROUND AND AIMS: Previous studies have shown that chronic kidney disease (CKD) can lead to changes in oral health, but the conclusions remain controversial. Thus, we conducted a Mendelian randomization (MR) study from the perspective of genetic analysis to assess the causal association between CKD and oral health conditions, including dental caries, pulp diseases, periapical tissues, and diseases of the gum and periodontal tissues. METHODS: We performed inverse variance weighted (IVW) random effects MR analyses and several sensitivity MR analyses using summary statistics from genome-wide

Immunohistochemical examination on the localization of macrophages and plasma cells in induced rat periapical lesions.

To investigate the role of plasma cells and macrophages in the development of periapical lesions, we immunohistochemically examined the distribution of these inflammatory cells in experimental induced rat periapical lesions after pulpectomy. The number of ED1-positive mononuclear cells increased rapidly, reached a plateau which remained stable between days 10 and 60, and subsequently decreased. Immunoglobulin (Ig)-bearing plasma cells appeared after 60 days, and, of these, IgG-bearing plasma cells were predominant after 90 days. The radiographic and histopathological findings indicated the dev

Integrated metagenomics and transcriptomics analysis reveals pathways associated with oral periapical lesions formation and progression.

Periapical abscesses, radicular cysts, and periapical granulomas are among the most frequently identified pathological lesions in the alveolar bone. Although many studies have investigated bacterial metagenomics in periapical abscesses, little is known about the genome mining of abundant bacteria in periapical lesions and its correlation to human transcriptome. This study aims to explore the enriched metabolic environment of periapical lesions associated with microbial diversity and their role in lesion progression. Bacterial DNA and human RNA were isolated from periapical lesions and healthy

Tooth Preservation Through Root-End Surgery in a 15-Year-Old Patient With Refractory Periapical Pathology: A Case Report.

Persistent periapical lesions associated with traumatized immature or mature permanent teeth can pose a significant therapeutic challenge when conventional nonsurgical endodontic treatment fails to achieve healing. Surgical endodontic intervention serves as a predictable alternative for preserving natural dentition while eliminating persistent periapical pathology. This case report describes the management of a large refractory periapical lesion associated with maxillary anterior teeth in a 15-year-old patient with a history of dental trauma. Despite five months of intracanal medicament therap

Scanning electron microscopic observations of apical root surfaces of teeth with apical periodontitis.

The aim of this study was to observe apical root surfaces of teeth with chronic periapical lesions. Five premolars and three molars with radiographically demonstrable periapical lesions were extracted and fixed in 2.5% phosphate-buffered glutaraldehyde solution for 9 days. The apical 3-mm portion of 17 roots was removed and prepared for scanning electron microscope. Lacunar resorption zones were frequently observed on the root surfaces. Bacteria and yeast cells were detected in some of the lacunae. Periapical bacterial plaque with a smooth structure was present mostly around the main apical fo

Immunometabolic, Vascular, and Epigenetic Determinants of Periapical Healing in Diabetes: A Narrative Review.

The bidirectional relationship between diabetes mellitus and apical periodontitis is a central focus within endodontic medicine. While clinical data consistently show a higher prevalence of apical periodontitis and delayed periapical healing in diabetic patients, the mechanisms driving this association remain to be fully clarified. This review aims to synthesize the current evidence to elucidate the specific cellular and molecular pathways through which uncontrolled hyperglycaemia may compromise periapical healing. A narrative review of current literature was conducted, integrating evidence fr

Factors that influence the outcomes of surgical endodontic treatment.

Surgical endodontic treatment encompasses a broad spectrum of procedures, amongst which root-end cavity preparation and filling, retrograde root canal treatment and through-and-through endodontic surgery, may be classified under the umbrella term 'Root-end surgery'. This narrative review considers the available data on periapical healing, soft tissue healing, tooth survival and oral health-related quality of life (OHRQoL), following root-end surgery and the factors that affect its outcomes. The pooled periapical healed rate for the studies published up to 2021 was 69% (95% CI: 65%, 73%) but in

DNA Methylation of MMP9 Is Associated with High Levels of MMP-9 Messenger RNA in Periapical Inflammatory Lesions.

INTRODUCTION: Matrix metalloproteinases (MMPs) are the major class of enzymes responsible for degradation of extracellular matrix components and participate in the pathogenesis of periapical inflammatory lesions. MMP expression may be regulated by DNA methylation. The purpose of the present investigation was to analyze the expression of MMP2 and MMP9 in periapical granulomas and radicular cysts and to test the hypothesis that, in these lesions, their transcription may be modulated by DNA methylation. METHODS: Methylation-specific polymerase chain reaction was used to evaluate the DNA methylat

Circadian rhythm disruption aggravates alveolar bone loss in rat apical periodontitis.

BACKGROUND: Circadian rhythm disruption (CRD) affects the expression levels of a range of biological clock genes, such as brain and muscle ARNT-Like-1 (BMAL1), which is considered to be an important factor in triggering or exacerbating inflammatory response. However, the underlying effect of CRD on the pathogenesis of apical periodontitis, a common oral inflammatory disease, currently remains unknown. Exploring the effects and pathogenic mechanisms of CRD on apical periodontitis will be beneficial in providing new ideas for the prevention and treatment of apical periodontitis. METHODOLOGY: Th

Blocking Gremlin1 inhibits M1 macrophage polarization through Notch1/Hes1 signaling pathway in apical periodontitis.

BACKGROUND: Gremlin1 is a multifunctional protein whose expression is demonstrated to be involved in a series of physiology and pathological processes. The association between Gremlin1 and apcial periodontitis (AP) has been established. M1-polarized macrophages are crucial immune cells that exacerbate the progression of apical periodontal inflammatory response, but the function of Gremlin1 during macrophages activation in periapical lesions is still unclear. This study attempts to explore the regulatory effects of Gremlin1 on macrophage polarization on apical periodontitis microenviroment. ME

Identification of cellulose fibers in oral biopsies.

Oral biopsies comprising five periapical cysts, three cases of exuberant granulation tissue in extraction sockets and one case of a mandibular radiopaque structure, all displaying material consistent with cellulose fibers on routine histologic examination, were subjected to histochemical, polarization, fluorescence and scanning electron microscopical, energy dispersive X-ray and chemical investigations. Routine screening of periapical lesions with polarization microscopy and identification of possible cellulose fibers with Calcofluor White is advocated. Further, attention is drawn to endodonti
