MINI-ABSTRACT: Hypophosphatasia (HPP) is a rare systemic disorder with impaired bone mineralization. In 49 Central and Eastern European patients, chronic pain, fractures, deformities, and calcifying periarthritis were frequent. Most had low TN-ALP; only one received asfotase alfa. Findings highlight underdiagnosis, high symptom burden, and need for improved care.
INTRODUCTION: Hypophosphatasia (HPP) is a rare, potentially life-threatening, progressive, systemic, inherited metabolic disorder caused by loss-of-function variants in the ALPL gene encoding tissue-nonspecific alkaline phosphatase (

A new cause of severe transient fitting polyarthritis is described in a patient with primary biliary cirrhosis. Hypercholesterolaemia is thought to provoke acute inflammatory periarthritis and peritendinitis. The clinical features are sudden onset of peripheral joint pain and effusion, with redness of the overlying skin, often precipitated by unusual exercise. Cholestyramine may help to prevent this form of arthritis.

OBJECTIVE: To explore the risk factors of elderly patients with frozen shoulder.
METHODS: 262 cases of scapulohumeral periarthritis treated in our hospital from January 2020 to December 2020 were analyzed retrospectively. According to the age of patients, patients younger than 60 years old were divided into middle-aged group (101 cases), patients between 60 and 75 years old were divided into old-aged group (91 cases), and patients ≥ 75 years old were divided into old-aged group (70 cases). The general demographic data and clinical data of the three groups were compared. Visual analogue scale

The most common crystal-related arthropathies-gout, calcium pyrophosphate dihydrate disease or "pseudogout," and calcific periarthritis/tendinitis-may be appropriately diagnosed and managed by the primary care physician. Definitive diagnosis via synovial tap is recommended, as the clinical picture may not identify some cases. The acute pain and swelling of attacks, regardless of etiology, generally respond to treatment with nonsteroidal anti-inflammatory drugs and local or occasionally systemic corticosteroids. Once a causative crystal has been identified and a diagnosis established, a plan fo

Injection granulomas (IGs) are recognized as a consequence of the injection of injectates into adipose tissue after routine drug or vaccine injections without reaching the muscular tissue. A series of reactions of the injectates in adipose tissue results in necrotic tissue and deposition of calcium crystals. IGs are usually asymptomatic but may rarely be symptomatic. In the past, symptomatic cases were mostly managed conservatively or surgically. However, to the best of our knowledge, no procedure other than surgical or conservative treatment has been reported. A 57-year-old patient with a his

Arthrography of the shoulder joint in so-called periarthritis humeroscapularis often shows nodular contrast defects, irregular defects in the bursa and contrast-filling of irregular lymphatics; these are characteristic signs of a synovitis. Arthrography can provide an exact diagnosis, not only in suspected post-traumatic rotator cuff, tears, but also in other painful conditions that limit movement. It is superior to CT in demonstrating inflammatory changes.

Traditional acupotomy treatment can easily cause re-injury of soft tissues, and its safety is gradually being valued. With the development of imaging technology, visualized acupotomy operations are increasingly used in clinical practice. Starting from the development and evolution of acupotomy, the authors summarize the advantages and clinical application of ultrasound-guided acupotomy in cervical spondylosis, scapulohumeral periarthritis, lumbar disc herniation and knee osteoarthritis.

We describe a patient with acute calcific periarthritis in an unusual site, the ilio-femoral ligament. The clinical findings in this patient resembled those of septic arthritis. T2*-weighted images with gradient field echo clearly showed foci of low signal intensity in this region, corresponding to the calcification shown on plain radiographs, and increased signal intensity around the foci, representing edema. These signal intensities were diagnostic of the disease.

Tendinitis and bursitis are less common around the hip than around the shoulder. Nevertheless, they must be recognized to avoid unnecessary and costly diagnostic errors. Their various clinical forms are studied in detail. Tendino-bursitis of the gluteus medius muscle is the most frequent in its subacute form, but it is rare in its acute, pseudo-gouty form. Calcification of the reflected tendon of the rectus femoris muscle often closely resembles arthritis of the hip. Synovial cysts of the psoas bursa and rupture of the gluteus medius tendon are rare but must be known. Local injections of corti

Granulomatous mastitis is a recently described, rare condition of unknown aetiology. It occurs in young, parous women as a tender extra-areolar breast lump and therefore needs to be differentiated from carcinoma. Histologically, a discrete granulomatous lobulitis is seen and because of morphological similarities to granulomatous thyroiditis it has been suggested it may be immunologically mediated. We present a patient in whom granulomatous mastitis presented with periarthritis and erythema nodosum, an association not previously described.

Calcifying periarthritis of the shoulder may have either an acute or a chronic course. In both cases, direct puncture of the calcification shortens the natural history and facilitates and accelerates resorption in 50% of cases. The addition of a small amount of corticosteroid relieves the painful syndrome, with 80% good and very good results. The use of low doses of corticosteroid is particularly important in diabetics, patients with peptic ulcer and sportsmen. The puncture technique and the necessary precautions are described.

INTRODUCTION: Evaluating isolated extremity discomfort can be challenging when initial imaging and exams provide limited information. Though subtle patient history hints often underlie occult pathologies, benign symptoms are frequently miscategorized as idiopathic.
CASE PRESENTATION: We present a case of retained glass obscuring as acute calcific periarthritis on imaging. A 48-year-old White male with vague fifth metacarpophalangeal joint pain had unrevealing exams, but radiographs showed periarticular calcification concerning inflammation. Surgical exploration unexpectedly revealed an encaps

Palindromic rheumatism (PR), a rare disease in children, is characterized by recurrent arthritis or periarthritis and asymptomatic interval. We report evolution of PR to juvenile idiopathic arthritis in a Japanese girl with heterozygous complex L110P-E148Q allele of MEFV gene. Poor response to colchicine alone suggests that the MEFV substitution could increase the susceptibility to arthritis rather than caused arthritis associated with atypical Familial Mediterranean Fever. Weekly methotrexate is a choice for such cases.

Calcium, or calcific, deposition disease in the form of acute calcific periarthritis of the hand and wrist is an uncommon entity that may be confused with more common crystalline or inflammatory arthropathies as well as infection. It is important for the clinician to be aware of this disease process and to include it in the differential diagnosis of patients presenting with acutely painful, focal inflammation of the hand or wrist. Nonsurgical management is often sufficient; however, considering the self-limited nature of the disease, accurate diagnosis is essential to avoid unnecessary antibio

The collective diagnosis of humeroscapular periarthritis (HSP) may imply a number of diseases different both in the genesis (inflammation, a degenerative process, trauma, cervical osteochondrosis) and localization (tendons of the humeral short rotators, biceps, synovial sheaths, bursae, etc.). A large proportion of HSPs are inveterate lesions of musculus interspinalis requiring surgical intervention. Despite the analogous pattern of many clinical symptoms, close collaboration of neuropathologists and traumatologists may allow the differential diagnosis of primary and secondary HSPs.

Hydroxyapatite deposition disease is a spectrum of disorders including calcific tendinitis, calcific periarthritis, and hydroxyapatite-induced arthritis. These usually present with acute onset of pain, and clinically, may be confused with acute infection or adhesive capsulitis. Supraspinatus calcific tendinitis and its imaging appearance is well known; however, calcific deposits may occur less commonly in other structures in and around the shoulder joint. We describe nine cases of relatively uncommon locations of calcific deposits around the shoulder with their imaging appearances.

Here, we report a case of a 30-year-old male who was prescribed eperisone hydrochloride for body pain and loose stools after which he developed severe maculopapular rash. Eperisone hydrochloride is an analgesic and antispastic drug used for spastic diseases such as spastic paralysis in cerebrovascular diseases, cervical spondylosis, and periarthritis. The drug is marketed in most of the Asian countries including India, but it is not licensed. Studies show the history of hypersensitivity in other countries, but this is the first reported case in India.
