Penetrating cardiac injury after percutaneous breast core-needle biopsy, unusual life-threatening complication: a case report.

BACKGROUND: Complications after percutaneous breast biopsy are infrequent but may include hematoma, pseudoaneurysm formation, persistent pain, infection, delayed wound healing, vasovagal reaction, hemothorax, pneumothorax, and neoplastic seeding. The risk factors include tumor factors (size, location, vascularity), procedure-related factors (needle diameter, number of biopsies), and interventionist experience. There has been no previous report of a fatal complication resulting from percutaneous breast biopsy. CASE PRESENTATION: We report a 54-year-old Asian woman with a 3 cm BI-RADS® 4B left

[Association of idiopathic eosinophilic pleurisy and a severe pericardial effusion].

A 52-year-old man presented with a two month history of left sided chest pains related to a pleurisy which showed an 18 per cent eosinophilia on pleural aspiration. He was hospitalised one month later and at that stage was very dyspnoeic and there was a bilateral pleural effusion associated with a significant pericardial effusion causing compression. A pleuro-pericardial drainage eased the patient's symptoms. The pleural fluid showed an 87 per cent lymphocytosis and a complete diagnostic work up was negative thus a diagnosis of idiopathic eosinophilic pleurisy with an associated pericarditis w

Surgical management of pyopericardium.

Forty three patients with pyopericardium were managed over a six year period. Pyomyositis and septic arthritis were the most common associated conditions and Staphylococcus aureus the commonest microorganism isolated. Mycobacterium tuberculosis was isolated in only three of the cases. Pericardiocentesis and various forms of pericardial windows were found to be associated with high rates of recurrences, inadequate drainage and subsequent constrictive pericarditis. Pericardiectomy should be done as an initial form of surgical treatment where the general condition of the patient permits, since th

Anaesthetic management of a breast cancer patient with cardiac tamponade and bilateral vocal cord paralysis.

Metastatic breast cancer presenting with both cardiac tamponade and bilateral vocal cord paralysis is rare. We report a case of an elderly patient with breast cancer who had previously undergone right modified radical mastectomy and then presented with cardiac tamponade and recurrent laryngeal nerve paralysis as complications of the malignancy. She underwent right anterior thoracotomy, pleuropericardial window, left tube thoracostomy and tracheostomy under general anaesthesia. Anaesthetic goal was to secure a potentially difficult airway caused by reduced glottic dimensions secondary to bilate

Rare association of pulmonary artery sling with tracheo-oesophageal fistula and patent ductus arteriosus.

Congenital tracheo-oesophageal fistula (TEF) is a life-threatening complication caused by communication between the oesophagus and the tracheobronchial tree within the neck or the thorax. TEF without oesophageal atresia is commonly known as 'H'-type TEF, and it is extremely rare in infants, accounting for ∼2-4% of all congenital tracheo-oesophageal malformations. Here, we report a rare case of a pulmonary artery sling with TEF and patent ductus arteriosus.

Evaluation of Safety and Feasibility of Percutaneous Balloon Pericardiotomy in Hemodynamically Significant Pericardial Effusion (Review of 10-Years Experience in Single Center).

BACKGROUND: Percutaneous balloon pericardiotomy (PBP) has been successful in managing large pericardial effusions, particularly in patients with malignant conditions. The objective of this study was to evaluate safety and feasibility of this procedure (PBP) in patients who had recurrent life threatening pericardial effusion at West Virginia University Hospital (WVUH). METHODS: This is retrospective review of consecutive series of pericardial windows by PBP. This report entails our experience of 36 procedures performed from November 2002 to November 2012 by PBP. RESULTS: Thirty-six percutaneo

Resection of parathyroid tumor in the aorticopulmonary window without prior neck exploration.

Of 522 patients with hyperparathyroidism operated on from 1973 to 1987 at our institution, there were seven (1.3%), each with an ectopic, hyperfunctioning mediastinal parathyroid adenoma, who required median sternotomy. In three of these seven patients, the tumor was located in the aorticopulmonary window. A 61-year-old woman with primary hyperparathyroidism had a preoperative thallium-technetium subtraction scan that showed thallium uptake at the base of the heart without any uptake in the neck. After further workup and without prior neck exploration, a parathyroid adenoma was found in the ao

[The examination technic and indications for mediastinal sonography].

The purpose of this report is to describe the technique of suprasternal and parasternal mediastinal sonography. Via the suprasternal approach, the supraaortic region, the paratracheal region and the aorticopulmonary window can be evaluated. The parasternal approach in right and left decubitus position is suitable for imaging the anterior mediastinum, pericardial and subcarinal spaces. The proportion of technically inadequate examinations of the different mediastinal compartments varies between 4 and 15%. The main indications for mediastinal sonography are described from our experience in the l

A successful transdiaphragmatic shunting for a late and recurrent pericardial effusion following coronary bypass.

Although pericardial effusion (PE) is common after cardiac surgery, late and recurrent cardiac tamponade that occurs more than 5-7 days after coronary artery bypass grafting is an infrequent complication. Moreover, the treatment of PE resistant to medical therapy, percutaneous drainage and pleuro-pericardial window remains a therapeutic challenge. We report the case of a recalcitrant PE with high-daily volume outflow drainage, finally treated with success, with a transdiaphragmatic pleuro-peritoneal shunting by laparoscopic approach.

Successful repair of a diaphragmatic hernia through a pericardial window with acellular dermal matrix.

A successful placement of a transabdominal pericardial window is associated with diaphragmatic hernia. In this paper, we present the case of a 5-month-old ex-31-weeks-premature baby who developed a symptomatic diaphragmatic hernia following a chronic pericardial effusion who was treated successfully with a laparoscopic transabdominal pericardial window. Laparoscopy and a pericardial window were used to manage the symptomatic effusion that developed following a bilateral thoracotomy and median sternotomy for the patient's massive hygroma. The patient was followed before and after pericardial dr

Successful management of esophagopericardial fistula complicating esophagogastrectomy.

An anastomotic leak into the right pleural space developed following esophagogastrectomy for cancer in a 56-year-old patient. Subsequently, a similar fistula occurred into the pericardium. This was managed successfully by a modification of Abbott's T-tube technique, together with a pericardial window, multiple drainage tubes, systemic antibiotics, and hyperalimentation. The anastomotic disruption healed completely, with only minimal narrowing on repeat roentgenograms of the upper gastrointestinal tract. Only four survivors of this rare and highly lethal problem have been previously reported.

[Management of malignant pericardial effusion with parasternal fenestration].

Numerous methods exist for the treatment of pericardial effusions. These methods, however, can be applied with limitations only for long-term eradication of malignant pericardial effusion. Lately, several new methods, including minimally invasive procedures, have been published, and the VATS technique has become fairly popular. This technique needs special instruments and single lung ventilation, which is relatively risky in case of contralateral malignancy. We apply a new and simple minimally invasive fenestration method using the well-known approach of the parasternal mediastinoscopy by Stem

[Symptomatic pericardial effusions in childhood].

We present the clinical data and the management of twelve patients with symptomatic pericardial effusion (PE). The etiology of PE was: Chronic renal failure, viral infection, cardiac surgery, juvenile rheumatoid arthritis and chronic myelocytic leukemia. Four cases were diagnosed as idiophatic. PE in childhood is usually asymptomatic. When symptoms are present they are non-specific and don't help to know the size of the effusion; therefore, it's necessary to practice an echocardiography to demostrate the presence of PE. The hemodynamic findings permit to diferentiate patients with and without

Thoracic duct injury after decortication of lung, new technique to repair thoracic duct in this challenging situation: A case report.

Chylothorax is a rare form of pleural effusion that can be associated with both traumatic and non-traumatic causes. Very few patients respond to conservative line of therapy. Thoracic duct ligation is often the treatment of choice in post-surgical patients; however, the optimal treatment of this disease process after traumatic injury remains unclear.Case presentation: We present the case of a 46-year-old woman with thoracic duct injury secondary to decortication for post-pneumonic empyema. Conservative therapy and pleurodesis done twice failed. She developed severe cachexia losing 15 kg in 30

Laparoscopic repair of an iatrogenic intrapericardial diaphragmatic hernia using the falciform ligament flap.

Iatrogenic intrapericardial diaphragmatic hernia after creation of a pericardial-peritoneal window is a very rare entity. We present the clinical case of an acute intestinal bowel obstruction due to herniation of small bowel into the pericardial sac. After laparoscopic reduction of the herniated small bowel, the diaphragmatic defect was successfully repaired using the hepatic falciform ligament.

Thoracic trauma: when and how to intervene.

Trauma is the leading cause of death in patients younger than 40 years of age. Thoracic injuries are common and often can be managed by tube thoracostomy. In many patients, however, the thoracic injuries must be repaired surgically in one of three time periods: immediate, urgent, or delayed thoracotomy. In this article, we describe the general approach to effectively managing thoracic trauma patients. We review common injuries and scenarios that may be encountered by the surgeon and discuss the considerations and variables that enter into the decision-making process for operative intervention.
