Post traumatic deafness: a pictorial review of CT and MRI findings.

UNLABELLED: Hearing loss is a common functional disorder after trauma, and radiologists should be aware of the ossicular, labyrinthine or brain lesions that may be responsible. After a trauma, use of a systematic approach to explore the main functional components of auditory pathways is essential. Conductive hearing loss is caused by the disruption of the conductive chain, which may be due to ossicular luxation or fracture. This pictorial review firstly describes the normal 2-D and 3-D anatomy of the ossicular chain, including the incudo-malleolar and incudo-stapedial joints. The role of 3-D C

["Inner perilymph fistula" of the anterior semicircular canal. A new disease picture with recurrent attacks of vertigo].

In 1998 Minor et al. described a new variant of perilymphatic fistula: the "superior canal dehiscence syndrome". This syndrome is clinically characterized by recurrent attacks of vertigo and oscillopsia induced by loud noises or stimuli that result in changes in intracranial or middle ear pressure. It is caused by a dehiscence of bone overlying the superior (anterior) semicircular canal. Due to this dehiscence, a third, mobile window (in addition to the round and oval windows) is formed, and changes in pressure are pathologically transduced to the anterior semicircular canal. Although the supe

Imaging of Temporal Bone Trauma: A Clinicoradiologic Perspective.

Imaging plays an important role in the evaluation of temporal bone trauma. Certain imaging findings can significantly change patient management or change surgical approach. Precise knowledge of clinical or surgical management can guide the review of imaging to detect these key findings. This article reviews the clinical and imaging findings as well as management of complications from temporal bone trauma, including hearing loss, vertigo, perilymphatic fistula, cerebrospinal fluid leak, facial nerve injury and vascular injury.

[Vestibular dysfunction after cochlear implantation: a national multicenter clinical study].

Vertigo and dizziness are the most common complications of cochlear implantation. Data of a multicentric study about clinical aspects and cost utility were collected for vertigo and dizziness: incidence, clinical and treatment were analyzed among 469 adults and children. Results demonstrated that 16% of adults and 3% of children experienced dizziness postoperatively. In a few cases a specific mechanism as perilymphatic fistula was identified. In other cases the mechanism underlying delayed vertigo remains speculative and endolymphatic hydrops was suggested.

[The value of computed tomography in persistent balance disorders following the use of stapes prostheses].

10 of 150 patients had persistent vertigo after implantation of stapes prostheses. These patients were evaluated by high-resolution CT in the axial and coronal plane. Scans showed in all cases findings which related to the symptoms. The CT findings were proved intraoperatively in 9 cases. A new indirect sign of a perilymphatic fistula is described in form of an air bulla at the end of the prosthesis. Retympanotomy could be planned better with the help of HR-CT.

Effects of cerebrospinal fluid loss on hearing.

There is a direct communication between the cerebrospinal fluid (CSF) and the perilymph through the cochlear aqueduct (CA). Perilymph depletion due to perilymphatic fistulas has been demonstrated to influence inner ear function, with signs and symptoms similar in many ways to those of Meniere's disease. A leakage of CSF could also theoretically affect the inner ear. The present study was conducted to investigate whether hearing was affected in patients submitted to a leakage of CSF and, if so, to what extent, in which frequency range, which time course and incidence. The study comprised 126 pa

[Surgical treatment of acoustically-induced vertigo (Tullio phenomenon)].

BACKGROUND: Tullio phenomenon is defined as noise induced vertigo. Other authors have attributed this symptom to either a perilymphatic fistula or postinflammatory adhesions between the stapes foot plate and the vestibular end organs. METHOD: In this paper two cases are described in which acoustically induced vertigo was explained by abnormal mobility of the stapes. RESULTS: The stapes was stabilized by the placement of cartilage chips beside the crurae of the stapes. Both cases demonstrated long term success, i.e. 4 and 5 years postoperatively.

[Clinical aspects of sudden deafness].

Sudden deafness is a disease of multifactorial genesis. The cause remains as ever unknown. In addition to the vascular theory, the possibility of a perilymphatic fistula due to rupture of the oval or the round window membranes has gained more significance. Therefore the treatment of sudden deafness should begin as soon as possible. We treat such patients with infusions of Dextran 40. BVK and Complamin in increasing amounts, in addition to stella-ganglion block. It is not justifiable, for legal reasons to withhold treatment, and therapy should begin immediately.

Hennebert's sign in superior semicircular canal dehiscence syndrome: a video case report.

Superior semicircular canal dehiscence (SSCD) syndrome has been called the great otologic mimicker because its presentation overlaps with otosclerosis, Meniere's disease, perilymphatic fistula, and patulous eustachian tube. A valuable examination finding that can help distinguish SSCD syndrome from other pathologic conditions is the presence of Hennebert's sign, in which pressure changes in the external auditory canal evoke stereotyped eye movements that align in the plane of the dehiscent semicircular canal. This video case report demonstrates Hennebert's sign associated with SSCD syndrome an

Enhanced oval window and blocked round window passages for middle-inner ear transportation of gadolinium in guinea pigs with a perforated round window membrane.

To elucidate the communication between the middle and inner ear, and the fluid dynamics of the inner ear with the perilymphatic fistula (PLF) of the round window membrane (RWM). The PLF of the RWM was created in nine guinea pigs. Gadolinium diethylenetriamine pentaacetic acid bismethylamide (Gd-DTPA-BMA) was delivered into the middle ear and followed in the inner ear using a 4.7 Tesla MRI. Pressure was delivered to the external ear canal of PLF ear in an attempt to enhance the inner ear uptake of Gd-DTPA-BMA. The immediate loading of Gd-DTPA-BMA in the scala tympani of the basal turn was ablat

Applications of the marchbanks transcranial-cerebral sonography technique in neurootology: preliminary report.

Transcranial-cerebral sonography (TCCS) is a noninvasive technique that allows clinicians to detect nanoliter (billionths of a liter) displacements of the tympanic membrane. This technique was developed to assess cerebrospinal fluid (CSF) pressure in cases of shunted hydrocephalus; it takes advantage of the CSF connection to the inner ear through the cochlear aqueduct. The movements of the tympanic membrane that are observed in TCCS are those evoked by the acoustic stapedius reflex and those spontaneous movements generated by intracranial arterial, venous, and respiratory pulses transmitted th

[The application of transmeatal exploratory tympanotomy].

Transmeatal exploratory tympanotomy has been used traditionally for the surgical treatment of conductive deafness. With 8 cases of the middle and inner ear diseases diagnosed and surgical managed successfully with this approach, the extended applications of this procedure were illustrated, which included: 1. conductive deafness with cause undetermined, 2. post traumatic conductive deafness with intact tympanic membrane, 3. suspected primary space occupying lesion of tympanum. 4. perilymphatic fistula, and 5. cerebrospinal fluid otorrhea. The key consideration and advantage of this approach wer

Limits of normal for pressure sensitivity in the fistula test.

In patients with perilymphatic fistula (PLF), nystagmus may sometimes be elicited by application of pressure to the external ear canal. The extent to which the normal population also exhibits such 'pressure sensitivity' is presently unknown. Our goal was to determine the limits of normal pressure sensitivity and to quantify the performance of the fistula test. Our subjects consisted of 13 normal controls and 7 patients with a history of pressure sensitivity who later underwent exploratory tympanotomy. We measured nystagmus prior to and following pressurization of the external ear canal. Pressu

Vein graft in stapes surgery.

Sealing the opening of the oval window during stapes surgery is essential; it prevents postoperative complications, such as perilymph fistula and sensorineural hearing loss. In this small series of 269 cases with otosclerosis, tympanosclerosis, and congenital ossicular abnormality, vein grafting was used to seal the opening of the footplate. Hearing improvement after surgery was acceptable, and none had total hearing loss or perilymphatic fistula. World literature from the last half of this century on grafting the oval window is reviewed. Absorbable gelatin sponge (Gelfoam) seems to be causing

Sudden Hearing Loss Research Clinic.

There is still a considerable lack of knowledge about the etiology and treatment of sudden hearing loss, despite renewed interest among investigators. There is still no treatment regimen that has been demonstrated to be effective. Other clinics should be established to elucidate the factors surrounding the etiology and to as with sudden hearing loss have another diagnosable disorder, but careful follow-up is necessary. In our series this figure was approximately one-third. Luetic labyrinthitis, perilymphatic fistula, fluctuant hearing loss or endolymphatic hydrops, and acoustic neuroma are amo

[Rupture of the round window - a cause of sudden hearing loss (author's transl)].

Rupture of the round window has been mostly described as an occasional lesion with sudden deafness and vertigo after diving, flying and physical stress, usually according to the patient's history. It also seems to be the cause of sudden progressive hearing loss of so-called idiopathic origin as shown by the findings in 6 out of 9 patients with perilymphatic fistulas. Therefore, transtympanic endoscopic evaluation of the round window is performed in the author's clinic as a routine measure in patients with sudden complete deafness or severe progressive loss of hearing even without any pointer t

Artificial stapes - a fourteen-year report.

This paper is based on 3,500 cases of otosclerosis operated on during the past 14 years restoring the continuity of the ossicular chain by means of an artificial stapes. Eighty percent showed practically a closure of the air-bone gap. There was no hearing gain in 17 percent of the patients, many of which showed reversal to preoperative levels. Three percent of the patients suffered sensori-neural hearing loss and 1 percent cophosis. Necrosis of the incus occurred in 0.3 percent of the patients and perilymphatic fistula in 0.2 percent. Only one case of inflammatory granuloma was observed as a c
