Traditional plastic surgery techniques for root coverage using connective tissue grafts are often invasive and cause patient discomfort. A new procedure with a minimally invasive approach for gingival recession was developed and termed the "root and cervical margin flattening procedure." A blunt incision was performed in the buccal gingival sulcus at the alveolar bone crest with a dissector or raspatory. After the incision, a split-thickness flap was dissected extending beyond the mucogingival junction, palpating the alveolar bone crest with a periodontal probe and flattening the cervical regi

Background and Clinical Significance: Advanced periodontitis with severe vertical bone loss and grade III mobility is usually managed by extraction and implant placement. Digital workflows and modern regenerative techniques have opened the possibility of preserving teeth that would traditionally be considered for extraction. This report describes a digitally guided modified intentional replantation (MIR) protocol applied to a maxillary tooth with severe periodontal involvement and unfavourable prognosis. Case Presentation: A 68-year-old male, non-smoker, with a history of heart transplantation

Intrusive luxation is one of the most severe types of dental traumatic injuries. The risk of occurrence of pulpal necrosis and inflammatory or replacement resorption is high. In an attempt to prevent or delay the appearance of such lesions, endodontic intervention is required soon after the occurrence of trauma. A 17-year-old boy reported to the Department of Conservative dentistry and Endodontics, KLE VK Institute of Dental Sciences, Belgaum, 2 days after a fall from a bicycle. Clinical and radiographic examination revealed an intrusive luxation of the left maxillary central incisor and later

AIM: To evaluate the pulp and periodontal healing of laterally luxated permanent teeth.
MATERIAL AND METHODS: Patients presenting with lateral luxation of permanent teeth during 2001-2002 were enrolled in this clinical study. Laterally luxated teeth were repositioned and splinted with a TTS/composite resin splint for 4 weeks. Immediate (prophylactic) root-canal treatment was performed in severely luxated teeth with radiographically closed apices. All patients received tetracycline for 10 days. Re-examinations were performed after 1, 2, 3, 6, 12 and 48 months.
RESULTS: All 47 laterally luxate

Endodontic treatment is an important step of tooth replantation protocols, but the ideal moment for definitive obturation of replanted teeth has not yet been established. In this study, a histomorphometric analysis was undertaken to evaluate the repair process on immediate replantation of monkey's teeth after calcium hydroxide (CH) therapy for 1 and 6 months followed by root canal filling with a CH-based sealer (Sealapex(®) ). The maxillary and mandibular lateral incisors of five female Cebus apella monkeys were extracted, kept in sterile saline for 15 min, replanted and splinted with stainles

Preserving questionable maxillary teeth for aesthetics is challenging for many practitioners. This report presents the clinical and radiographic results of an orthodontic extrusion technique for clinical crown lengthening of female patients' maxillary teeth to increase the restorability of teeth. Splinting was carried out by using specially designed hucks cast with custom-made post joined with orthodontic brackets with adjusting teeth. Teeth were carefully extruded to the desired level without harming the marginal bone areas or root apices. Clinical follow-up evaluations were performed for 3 m

Avulsion is the complete displacement of tooth out of its socket. It is the gravest form of dental traumatic injury which accounts for 0.5-3% of the total dental traumatic injuries of the permanent teeth and most commonly involves the maxillary central incisors. The treatment is immediate tooth replantation or if that is not possible then storing tooth in an appropriate storage medium and attending dental clinic at the earliest. The present case report describes the replantation of maxillary left central incisor tooth that was avulsed. The tooth was out of the socket for 22 hours without any s

Instead of extraction this case report presents an alternative treatment option for a maxillary incisor with a vertical root fracture (VRF) causing pain in a 78-year-old patient. After retreatment of the existing root canal filling the tooth was stabilized with a dentine adhesive and a composite restoration. Then the tooth was extracted, the VRF gap enlarged with a small diamond bur and the existing retrograde root canal filling removed. The enlarged fracture line and the retrograde preparation were filled with a calcium-silicate-cement (Biodentine). Afterwards the tooth was replanted and a ti

Traumatic injuries of permanent teeth in children present complex challenges. Growth and development influences, orthodontic considerations, and unknown endodontic and periodontal sequelae preclude immediate intervention with long-term restorations. "Interim" solutions are required to return the patient to the normal appearance, function, and health of the dental/alveolar complex until maturation of the patient allows long-term treatment planning. This report outlines diagnosis and treatment of multiple injuries, sustained by the maxillary anterior teeth. Step-by-step clinical procedures and t

Dental treatment for periodontally compromised patients can be highly complicated and challenging. This article addresses the effectiveness of contemporary periodontal treatment modalities by depicting clinical cases of periodontal restoration and health that have been maintained for 20 or more years. The cases incorporate four key steps vital to achieving successful treatment of chronic adult periodontitis: debridement, occlusal stabilization, pocket elimination, and maintenance. These cases demonstrate proof of principle that if the treatment is carefully planned and executed precisely, the

The advantages of dental autotransplantation and its high level of clinical success mean that it should be considered as a therapeutic option when replacing a lost tooth. In order to achieve optimum results, it is necessary to know the technique of dental autotransplantation, promoting its use whenever the clinical conditions to perform it are present. The objective of this article is to describe the technique in detail by means of a clinical case of a dental autotransplant whose donor tooth was a third unerupted molar. A 39-year-old male patient with no medical history of interest. On clinica

The purpose of this study was to clinically evaluate the efficacies of three treatment methods and to compare their outcomes in patients with painful disc displacement. The study group comprised 45 patients with unilateral temporomandibular disorders who fell into Axis I group II (with limited mouth opening) of the Research Diagnostic Criteria for Temporomandibular Disorders. Magnetic resonance imaging was used for definitive diagnosis. The patients were divided randomly into three groups according to the treatment method: splint therapy, splint therapy with ultrasound-guided arthrocentesis, a

The problem of resorption during replantation in relationship to the epithelial rests and a lack of splinting was studied. Ten mongrel dogs were used. Two dogs served as control subjects, while eight had their mandibular incisors intentionally replanted and studied histologically. It was observed that where rests of Malassez were present we did not see resorption, and the lack of splinting did not result in resorption or loss of teeth. It appears we can stain epithelial rests of Malassez with immunofluorescent antibodies and use this technique for further study of this tissue.

Numerous publications have reported revascularization of necrotic immature permanent teeth, but the regenerative potential of pulp in mature teeth has rarely been considered. Platelet-rich plasma (PRP) meets many requirements of a scaffold for regenerative endodontics. To the best of our knowledge, no clinical study has evaluated PRP for endodontic regeneration in a mature avulsed tooth. The present case evaluated PRP for pulpal regeneration in an avulsed mature incisor (>8 hours extraoral dry time) of an 11-year-old boy after delayed replantation. The canal was disinfected after extraoral acc

AIM: To evaluate the treatment outcome of avulsed and replanted permanent incisors.
MATERIAL AND METHODS: 42 avulsed and replanted permanent incisors in 37 individuals were followed over a median observation period of 2.8 years (range: one year to five years). The mean age was 16.3 years at the time of replantation, with 81% of patients being younger than 20 years.
RESULTS: The tooth survival rate after replantation was 83.3% (35/42 teeth). Periodontal healing was observed in 20 teeth. External root resorption was the most frequent complication and was found in 22 of the 42 avulsed teeth: 21

A patient with mutilated dentition was treated to an acceptable result with the help of mini-implants for anchorage, autotransplantation of 2 teeth, one dental implant, and multiple prostheses. The major problems for this mutilated dentition were multiple missing teeth, bite collapse with posterior crossbites, deepbite with a canted gummy smile, and severe skeletal discrepancy of maxillary protrusion and mandibular retrusion. Mini-implant anchorage was applied to retract and intrude the maxillary arch to reduce the protrusion and rotate the mandible counterclockwise to improve the chin project

In a retrospective study, 50 patients over 70 years of age, selected from a private specialist clinic for fixed and removable prosthodontics, were studied. They all had remaining natural teeth and had undergone complete dental treatment at least 5 years before. The age-range was 70-96 and they had been treated 6-25 years before the study. The patients had on average 13.9 teeth. During the five-year observation period, 32 patients developed caries, with a mean number of 5.0 lesions (range 1-22). 79 fillings were made due to other defects than caries. 25 root canal fillings were made, due to car

Root fracture injuries affect up to 7% of permanent teeth, and injury in teeth with immature root formation is even rarer. The purpose of this paper was to report the case of a 7-year-old girl who experienced pain in her permanent maxillary central incisors following a fall from a bicycle. A radiographic examination revealed immature maxillary central incisors with mid-root oblique and horizontal root fractures. Splinting was performed, and the child was observed under a regular follow-up regime. After 18 months, clinical examination showed normal tooth color and position, with a positive resp

This report describes the transplantation of a primary canine after traumatic loss of a central incisor in an 8-year-old boy. The 7-month follow-up revealed normal periodontal healing with absence of infection, ankylosis or progressive resorption. The patient was then lost for control. After 16 months another trauma in the same patient resulted in an avulsion of the transplant. However, the alveolar bone was maintained in vertical and sagittal dimensions. Another primary canine was transplanted and followed for further 11 months. Again normal periodontal healing could be observed. The transpla
