Brazilian Society of Angiology and Vascular Surgery guidelines on peripheral artery disease.

Patients with peripheral artery disease and generalized atherosclerosis are at high risk of cardiovascular and limb complications, affecting both quality of life and longevity. Lower limb atherosclerotic disease is associated with high cardiovascular morbidity and mortality and adequate management is founded on treatments involving patient-dependent factors, such as lifestyle changes, and physician-dependent factors, such as clinical treatment, endovascular treatment, or conventional surgery. Medical management of peripheral artery disease is multifaceted, and its most important elements are r

Generation of Autologous Vascular Endothelial Cells for Patients with Peripheral Artery Disease.

Peripheral artery disease (PAD) is a prevalent cardiovascular disease with risks of limb loss. Our objective is to establish an autologous cell source for vascular regeneration to achieve limb salvage in PAD. Six PAD patients (age 50-80) were enrolled with their peripheral blood collected to derive vascular endothelial cells (ECs) with two different approaches: (1) endothelial progenitor cell (EPC) approach and (2) induced pluripotent stem cell (iPSC) approach. The iPSC approach successfully generated patient-specific ECs for all PAD patients, while the EPC approach did not yield any colony-fo

Axillary Transcatheter Aortic Valve Replacement in Patients With Peripheral Vascular Disease.

The axillary artery seems an interesting alternative in nonfemoral transaortic valve replacement (TAVR) patients. This study describes our experience with this technique and its short-term follow-up results. This is a retrospective single center study. All axillary TAVR performed in our department between 2015 and 2017 were included in the study. Mean follow-up was 13.2 ± 9.5 months. All reporting was done according to the VARC-2 criteria. During the period covered, 43 patients had an axillary TAVR. Most patients were men (62.7%), had a mean age of 83.9 ± 5.3 years and presented with EuroSCORE

Common Aging Signature in the Peripheral Blood of Vascular Dementia and Alzheimer's Disease.

Alzheimer's disease (AD) and vascular dementia (VaD) are the two most dominant forms of dementia in elderly people. Due to the large overlap between AD and VaD in clinical observations, great controversies exist regarding the distinction and connection between these two types of senile dementia. Here for the first time, we resort to the gene expression pattern of the peripheral blood to compare AD and VaD objectively. In our previous work, we have demonstrated that the dysregulation of gene expression in AD is unique among the examined diseases including neurological diseases, cancer, and meta

Results of open vascular surgical therapy in chronic peripheral arterial disease.

Surgery in chronic peripheral arterial disease (PAD) can alleviate symptoms in claudicants and may impede amputation in critical limb ischemia. The current data on different surgical strategies and techniques from the aortoiliac region to the pedal arteries as well as amputation as last resort are described and discussed. Treatment of PAD depends on the condition of the patient and his comorbidities. The question of optimal therapy for each patient cannot always be answered in the operating theatre or the angio-suite.

Lower extremity amputation for peripheral vascular disease. A low-risk operation.

Two hundred fifty-three consecutive major amputations of the lower extremity for peripheral vascular insufficiency were performed over a 3-year period. Operative mortality was 0.9 per cent for 113 below-knee and 2.8 per cent for 140 above-knee amputations. Although lower extremity amputation has a reputation for high mortality and morbidity, this is unwarranted; adherence to a protocol of aggressive medical management with timely surgical intervention allows a mortality that is appropriate to the age and general debility of this patient population.

Pulse wave from transit ratios in the assessment of peripheral vascular disease.

This paper presents a ratio of the pulse wave form transit times in the proximal and distal arterial tree, produced by a simple, non-invasive, repeatable and reproducible technique. It has been shown to be highly significant in separating normal from disease states and the proximal from the distal lesion. It has been shown to be independent of the effects of age. In the clinical situation the ratio is of value in the initial assessment of the patient (by separating ischaemic from now-ischaemic limb pain); in elucidation of significant occult proximal lesions in the presence of distal disease;

Association of elective peripheral vascular intervention with outcomes among patients with peripheral arterial disease and intermittent claudication.

BACKGROUND: Peripheral vascular intervention (PVI) is increasingly used for the treatment of peripheral arterial disease (PAD) with intermittent claudication (IC). However, large, real-world comparative studies of the safety and effectiveness of PVI compared with no PVI are limited. We sought to compare the effectiveness and costs of elective PVI compared with no PVI among patients with PAD and IC. METHODS: We conducted a 1:1 propensity-matched retrospective cohort analysis of commercially insured and Medicare Advantage patients in OptumLabs Data Warehouse from January 1, 2016, to September 3

Biomaterial-based vascularization strategies for enhanced treatment of peripheral arterial disease.

Peripheral arterial disease (PAD) poses a global health challenge, particularly in its advanced stages known as critical limb ischemia (CLI). Conventional treatments often fail to achieve satisfactory outcomes. Patients with CLI face high rates of morbidity and mortality, underscoring the urgent need for innovative therapeutic strategies. Recent advancements in biomaterials and biotechnology have positioned biomaterial-based vascularization strategies as promising approaches to improve blood perfusion and ameliorate ischemic conditions in affected tissues. These materials have shown potential

Marijuana and Peripheral Vascular Disease: Pathophysiology, Clinical Evidence, and Cardiovascular Implications.

Cannabis (marijuana) is increasingly used worldwide, driven by legalization and shifting public perception. Although often considered harmless, growing evidence suggests potential cardiovascular risks, including its association with peripheral vascular disease, particularly peripheral artery disease (PAD). This narrative review explores the emerging link between cannabis use and peripheral vascular disease by examining biological mechanisms and clinical data. The primary psychoactive compound, Δ9-tetrahydrocannabinol, interacts with the endocannabinoid system-especially cannabinoid receptor ty

Outcomes in patients with peripheral vascular disease following percutaneous coronary intervention.

OBJECTIVES: To evaluate the clinical characteristics and outcomes of patients with peripheral vascular disease (PVD) undergoing percutaneous coronary intervention (PCI) in a contemporary setting, and to determine whether use of drug-eluting stents (DESs) improves outcomes. BACKGROUND: PVD was an independent risk factor for adverse outcomes following PCI in the bare-metal stent (BMS) era. It is not known whether outcomes in these patients have improved with advances in interventional techniques and stent technology, as they have for the general population. METHODS: Eighteen thousand three hun

Peripheral Vascular Disease Diagnostic Related Outcomes in Diabetic Charcot Reconstruction.

Postreconstructive outcomes were compared in diabetic patients with Charcot neuroarthropathy (CN) who had peripheral arterial disease (PAD) diagnosed with angiography versus patients who were diagnosed clinically. A retrospective review was performed of patients with diabetic CN requiring reconstruction secondary to ulceration and/or acute infection. Of the 284 patients in the CN osseous reconstruction cohort, after accounting for exclusion criteria, 59 (20.8%) patients with PAD were included in the analyses. Forty (67.8%) of these 59 patients were diagnosed with PAD clinically and 19 (32.2%)

Polyarteritis nodosa presenting as peripheral vascular disease and acute limb ischemia.

Thyroid hormones (THs [T3 and T4] ) are key regulators of metabolic rate and nutrient metabolism. They are controlled centrally and peripherally in a coordinated manner to elegantly match T3-mediated energy expenditure (EE) with energy availability. Hypothyroidism reduces EE and has long been blamed for obesity; however, emerging evidence suggests that, instead, obesity may drive thyroid dysfunction. Thus, we used a mouse model of diet-induced obesity to determine its direct effects on thyroid histopathology and function, deiodinase activity, and T3 action. Strikingly, overnutrition induced hy

Initial experience with a novel hybrid vascular graft for peripheral artery disease.

This report describes the successful use of a new hybrid vascular graft as a conduit for above knee femoro-popliteal bypass surgery. The graft consists of a proximal (heparin coated) expanded polytetrafluoroethylene section but ends distally as a nitinol reinforced self-expandable stent that is covered and constrained, allowing a sutureless distal anastamosis. With this graft the creation of above knee bypasses in situations where lesions extend to the popliteal artery behind the knee, or in cases where the above knee popliteal artery is severely calcified is still possible. This avoids the ne

The relationship between oral conditions and ischemic stroke and peripheral vascular disease.

BACKGROUND: This article is a critical review of the epidemiologic studies linking or disassociating oral conditions with stroke and peripheral vascular disease, or PVD. It also describes possible causal and non-causal pathways for these associations. TYPES OF STUDIES REVIEWED: The author reviewed studies that evaluated the oral conditions of periodontal disease or tooth loss as risk factors for arterial disease and stroke. RESULTS: Seven of nine studies evaluating tooth loss and periodontal disease as risk factors for stroke or PVD showed some significant associations. The studies varied in
